To treat hallux valgus, minimally invasive osteotomy on 1st metatarsal neck,immobilization with "8-shape" bandagev was performed, and the effect was definited. The principles of osteotomy stability were analyzed from the osteotomy position, methods, direction and angle. The grinding drill was used to increase friction coefficient between the ends of osteotomy. Correct direction of osteotomy and suitable angle were the key point of stability. The immobilization with "8-shape" bandage complied with the principle of elastic fixation created the conditions for the slight movement of the osteotomy ends. Compared with internal fixation ,it was better on osteotomy healing,and the osteotomy ends were stable and healed with cartilage

Objective To investigate the effect of preconditioning electrostimulation of fastigial nucleus (FNS) on the neuronal mitochondrion of rats early after cerebral ischemia. Methods Forty Wistar rats were used and divided into 5 groups: a normal control group, a sham FNS group and 3 FNS groups. The FNS of the rats in the FNS groups was conducted for 1 hr, 1 day or 7 days, respectively, before the models of middle cerebral artery occlusion(MCAO)were made with them. Three hours after the model was established, the animals were sacrificed and the water content of the brain of the ischemic side was measured. The neuronal mitochondrion was observed by electron microscope, and its Vv, specific volume (Sv) and specific surface (Ss) were analyzed. The rats in the normal control group were not given any special treatment. For the rats in the sham-FNS group, intrinsic neurons of FN were destroyed with ibotenic acid (IBO), and 5 days later, the FN was electrically stimulated, and 1 day later, MCAO models were made. Results Three hours after MCAO, the water content of the brain increased and neurological function score decreased in the sham-FNS group, while the Vv and Sv of neuronal mitochondrion increased and the Ss decreased, which were significantly different from those of the normal control group(P

Objective To observe the changes of hospitalization rates of patients with goiter in Lianyungang City in a coastal area with mild iodine deficiency in 2002-2010.Methods Medical reports of patients with goiter from county hospitals were studied at county level with a retrospective method.In accordance with the National Iodized Salt Monitoring Program,nine townships(towns) were taken from each county (district) [total of five counties(districts)]; four villages were selected from each township (town); salt samples were taken from eight households in each village,and salt iodine was determined using direct titration of sodium thiosulfate.Results The hospitalization rate of patient with goiter increased from 1.87/10 million in 2002 to 7.05/10 million in 2009,and then fluctuated to 6.06/10 million in 2010(x2 =281.91,P＜ 0.01).Among them,female's hospitalization rates were significantly higher than that of male 's(The variation range of x2 values were 35.23-116.04,all P ＜ 0.01),and the hospitalization rates of people over the age of 40 years were significantly higher than that of people less than 40 years of age(The variation range of x2 values were 33.04-263.04,all P ＜ 0.01),and the hospitalization rates of people in urban areas were significantly higher than that of people in rural areas (The variation range of x2 values were 18.35-140.00,all P ＜ 0.01).Average salt iodine was 27.61-30.13 mg/kg in 2002-2010.The coverage rate of iodized salt and the consumption rate of qualified iodized salt both increased year by year (x2 =183.75,211.99,P ＜ 0.01).There was a positive correlation between hospitalization rate of patient with goiter and coverage rate of iodized salt(spearman correlation coefficient was 0.83,P ＜ 0.05).There was a positive correlation between hospitalization rate of patient with goiter and the consumption rate of qualified iodized salt too (spearman correlation coefficient was 0.93,P ＜ 0.05).Conclusions In Lianyungang City,the hospitalization rate of patient with goiter,the coverage rate of iodized salt and the consumption rate of qualified iodized salt all show upward trend in 2002-2010.The relationship among them is worth further study.

Objective To analyze postoperative effect of a new rectopexy technique with Douglas pouch elevation for complete rectal prolapse (CRP) in adults.Methods From January 2010 to May 2011, 52 CRP cases were treated by rectopexy with Douglas pouch elevation.In terms of different surgical techniques, patients were divided into two groups : Group A (n =28) received laparoscopic rectopexy with Douglas pouch elevation;and Group B (n =24) received laparoscopic rectopexy with Douglas pouch elevation combined with procedure for prolapse and hemorrhoids (PPH).Rectal prolapse, constipation, and fecal incontinence in the two groups were assessed respectively before surgery, and on the 6th, 12th, and 24th month after surgery, postoperative complications were evaluated with severe grading of surgical complications.Results Symptoms of rectal prolapse disappeared at half a year after surgery in all patients, and relapsed to different extent afterwards.However, two years after operation, the status of rectal prolapse in Group B tended to be stable.The trend of constipation after surgery was consistent with that of rectal prolapse.In addition, fecal continence improved gradually on half a year after surgery in the two groups and recovered to the optimal status in the first postoperative year.However, in the second year, the results of fecal incontinence reduced slightly in Groups A, and Group B became stable.Two years after surgery, four cases in Groups A relapsed while there was no recurrence in Group B.Difference of Grade I to Grade Ⅲ complications among the two groups was statistically insignificant (x2 =0.05, P ＞ 0.05).Conclusions The clinical effect of laparoscopic rectopexy with Douglas pouch elevation associated with the procedure for prolapse and hemorrhoids (PPH) is better than that without PPH for female and male CRP patients with severe symptoms.

Objective To evaluate a modified liver hanging maneuver(retrohepatic tunnel of the IVC) in patients undergoing hemihepatectomy.Methods Twenty-four patients undergoing hemihepatectomy were divided into two groups:modified liver hanging maneuver group(n=12)and Pringle's maneuver group(n=12).The amount of intraoperative bleeding,operation time,postoperative liver function,liver function recovery and complications were compared between the two groups.Reset All operation were performed successfully and there were no difference in the time of operation etween the two groups.There was a difierence in the amount of mean intraoperative blood loss between the two groups.It was(160±40)ml in liver hanging group and(560±120)ml in Pringle's group(P＜0.01).Liver function recovery measured on postoperative day 3 and day 7 was better in liver hanging groupthan that in Pringle's group(P＜0.01).The volume of postoperative peritoneal serous fluid dranage was significantly less in liver hanging group(P＜0.01).Conclusion The modified liver hanging maneuver is useful for hemihepatectomy.
