1.Epidemiology investigation on rheumatoid arthritis and chronic kidney disease among population older than 45 years in Luohe city

ObjectiveTo study the prevalence of chronic kidney disease(CKD) among population with rheumatoid arthritis(RA) in Luohe city.Methods 3072 residents (older than 45 years) with eligible data from Luohe city were randomly selected using a stratified,multistage sampling.All residents were interviewed and given physical examination,tested for sample of uria and blood,and also given special examination about RA.Results The prevalence of RA was 6.90% in 3072 subjects.The albuminuria was detected in 10.42% of subjects,hematuria in 8.59%,reduced renal function in 1.82%,Howerer,which was higher in patients with RA,was 14.62%,10.85% and 4.72% respectively.Especially the prevalence of albuminuria and reduced renal function in RA patients was significantly higher compared to that in population without RA (14.62% vs 10.10%,P<0.05 and 4.72% vs 1.61%,P<0.01),but no significant differences in hematuria (10.85% vs 8.43%,P>0.05).The prevalence of CKD was 16.93% in tougher,and RA patients had higher prevalence of CKD than those population without RA (22.17% vs 16.54%,P<0.05).ConclusionThe prevalence of CKD among population older than 45 years in Luohe city is high,and the patients with RA have special characteristic in epidemiology of CKD.
3.The feasibility of autologous mononuclear bone marrow cells transplantation in patients with anterior myocardial infarction

Objective To evaluate the feasibility of autologous mononuclear bone marrow cells (BMCs) transplantation in patients with anterior myocardial infarction. Methods Twenty-two patients were enrolled in this prospective, nonrandomized, control study (14 patients in cell transplantation group; 8 in control group). All patients underwent standard percutaneous coronary intervention (PCI) and drug therapy. 14 patients in the cell transplantation group were transplanted with autologous mononuclear (BMCs) via a over-the-wire (OTW) balloon catheter slowly placed into the left anterior descending (LAD) coronary artery during inflation. Results One patient in the cell transplantation group died of acute drug eluting stent thrombosis. The other 13 patients and the 8 patients in the control group had no procedure related complications and ventricular arrhythmia. All patients in both groups underwent 3-month and 6-month follow-up, 6 minutes test, 2D Doppler echocardiogram, single photon emission computed tomography and cardiac magnetic resonance imaging (MRI). After 3 months of follow-up, a significant increase of left ventricular ejection fraction (LVEF) determined by ultrasonic cardiography was found [(50.37?7.31)% vs (40.12?5.52)% before transplantation, P =0.000?1] and was also significantly greater compared with the control group [(50.37?7.31)% vs (44.09?3.50)%, P =0.035]. Conclusion The result indicates that selective intracoronary transplantation of autologous mononuclear BMCs seems to be feasible and may improve the left ventricular systolic function.
4.Studies on the Pharmacology of Cajanin Preparation

Cajanin preparation could significantly reduce the mouse pinna inflammation iuduced by croton oil. The higher the dose, the stronger the effect .It also inhibited the infiltration increase of intraperitoneal blood capillary of mice led by acetic acid.In addition, the preparation had an analgesic effect. These experimental results indicated that cajanin may be hopefully developed to a new drug for the treatment of scald trauma, and infection.
5.Effect and mechanism of intravenous bone marrow stromal cells transplantation against chronic ethanol-induced cognitive dysfunction in rats

Objective To investigate the effect and mechanism of intravenous bone marrow stromal cells (BMSCs)transplantation against chronic ethanol-induced cognitive dysfunction in rats.Methods BMSCs from rats were isolated,cultured and identified in vitro.Male Sprague-Dawley rats(n=28)were randomly divided into control group(n=7),chronic ethanol group(n=7),BMSCs group(n=7)and PBS group(n=7).BMSCs(1×10~7 cells/animal)or PBS were intravenously injected to the rats of BMSCs or PBS group at the 7th week after ethanol gavage,respectively.All rats were sacrificed at the end of the 8th week.The learning and memory behavior was assessed using Moms water maze task.Immunofluorescence was used to examine cell proliferation maker(BrdU)in hippocampal dentate gyrus.Immunohistochemistry was used to evaluate NGF and BDNF expression in hippocampal region.Results In Morris water maze,escape latency at each time point in BMSCs group((33.83±3.26)s,(13.14±2.64)s,(6.53±0.98)s,(4.45±0.85)s)were significantly shorter than those in chronic ethanol group((53.78±4.29)s,(36.53±2.39)s,(18.28±2.13)s,(8.67±0.46)s,all P<0.01))and time percentage of platform quadrant in BMSCs group((73.99±1.49)%) was dramatically longer than that in chronic ethanol group((51.25±3.69)%),but no significant difference with contro1.The number of BrdU-positive cells in the dentate gyrus was significantly higher in BMSCs group than that in chronic ethanol group(P<0.01).Comparing to chronic ethanol gavage,BMSCs transplantation significantly increased the expression of NGF alld BDNF (P<0.01)in hippocampus.Conclusion Intravenous BMSCs transplantation is able to result in enormous attennation of the learning and memory abnormalities in chonic alcoholism rats,and which might be involved to promote hippocampal endogenous cellular proliferation by secretary neurotrophic factors.

7.Analysis of the Effect of Double-wavelength Diode Laser for Relase of Pain

Objective: Adopting quantified index to observe and analyze the effect of acute and chronic pain through double-wavelength diode laser. Methods: Using diode laser with the wavelength of 650nm / 810nm to irradiate the special tissues or aching points of patient, selecting different frequency, intensity and time to make treatment. Results: VAS scoring: P<0.05, the total efficient rate is 96%. Conclusions: Treatment with double-wavelength diode laser to various acute and chronic pain is just a definitely good curative effect, and it can release the effects of patient sleeping, daily life, communication with people and life interests caused by pain, and finally enhancing the quality of life.

9.Comparative study of image quality and radiation dose between prospective and retrospective ECG gating technique in coronary artery imaging with 64-slice spiral CT

Objective To compare the image quality and radiation dose between prospective ECG-trigering and retrospective ECG gating technique in coronary artery imaging. Methods 33 patients suspected coronary artery disease were included in this study and divided into experimental group (prospective ECG-triggering coronary artery imaging, heart rate < 65 bpm, 16 patients)and control group (retrospective ECG gating coronary artery imaging, heart rate < 75 bpm, 17 patients). Radiation dose was recorded for statistical analysis. At the same time, image quality was evaluated. Results There were no significant differences in image quality 3.2 msy. Effective dose reduced 76.47 %. Conclusions Prospective ECG gating coronary artery imaging can obtain the similar image quality compared with prospective ECG gating coronary artery imaging but the effective dose reduced 76.47 %. Prospective ECG gating coronary artery imaging has clinical value to peoples who are able to not accept high radiation dose and with low heart rate.
10.Development of gastro-esophageal reflux during laparoscopic surgery in lateral jack-knife position: a comparison with reverse Trendelenburg/Trendelenburg position in patients lying supine

Objective To evaluate development of gastro-esophageal reflux (GER) during laparoscopic surgery in lateral jack-knife position under general anesthesia through comparing with reverse Trendelenburg/ Trendelenburg position in the patients lying supine.Methods Ninety patients of both sexes,aged 18-64 yr,of ASA physical status Ⅰ or Ⅱ,with body mass index of 18-30 kg/m2,scheduled for elective laparoscopic surgery under general anesthesia,were randomly divided into 3 groups (n =30 each):lateral jack-knife position group (group L),Trendelenburg position group (group T) and reverse Trendelenburg position group (group Tre).Anesthesia was induced with midazolam,sufentanil,propofol and cisatracurium besylate and maintained with propofol and remifentanil given by target-controlled infusion.A pH-sensitive probe was inserted through nose into the lower esophagus and pH value was continuously recorded until 1 min after extubation.GER was defined as pH value ≤ 4 lasting for ≥ 1 min in the lower esophagus during surgery.The development of GER during surgery and the lowest pH value in the lower esophagus when GER developed were recorded.Results Compared with group Tre,the incidence of GER (27%) and total number of times GER had occurred were significantly increased in group L,and no significant changes were found in the indices mentioned above in L and T groups.When GER developed,the lowest pH value in the lower esophagus was 2.1 ± 1.3,2.6 ± 1.2 and 3.5 in L,T and Tre groups,respectively.Conclusion The incidence of GER is 27 % during laparoscopic surgery when the patients are in lateral jack-knife position and it is higher than that obtained with reverse Trendelenburg position in the patients lying supine.
