1.Effect of Xuezhikang on Serum MCP-1 Level and Endothelial Function in Patients with Chronic Heart Failure

OBJECTIVE:To observe the effect of Xuezhikang on serum MCP-1 and endothelial function of brachial artery in patients with chronic heart failure but with normal cholesterol level. METHODS:Patients with chronic heart failure (NYHAⅡ-Ⅳ) were randomly allocated into two groups(n=24 each). Both groups received routine treatment in accordance with the guide of CHF treatment,additionally,the treatment group received Xuezhikang 0.6 g bid for 8 weeks. Objective To summarize perioperative care and effect of surgical treatment of total anomalous pulmonary venous connection (TAPVC) in newborns and babies within 6 months.Methods From September 2001 to May 2013,ninetytwo consecutive patients with TAPVC received surgical correction under cardiopulmonary bypass (CPB) with moderate or deep hypothermia.The anatomic subtype included supracardiac type 48,intracardiac type 36,infracardiac type 6 and mixed type 2.There were 36 newboms.Pathological diagnosis was made by echocardiogram,computerized tomography and operative findings during operation.The option of procedure was determined by findings.Respirator assisted breathing after operation and cardiac function was maintained by a variety of positive inotropic drugs,to prevent the happening of the pulmonary hypertension crisis.Results Severely low cardiac output syndrome occurred in 8 patients and they died within 24 hours after operation.The remaining cases postoperative recover smoothly and receive postoperative follow-up about 1 month to 12 years,3 cases of postoperative pulmonary vein stenosis (PPVS) occurred.Conclusion Total anomalous pulmonary venous connection should be surgical treatment as soon as possible with satisfied effect.The key of perioperative care is maintenance of left heart function,negative balance of liquid,prevention and treatment of pulmonary hypertension.
4.Antitumor effect of chlorophyllin in vitro

AIM: To study the effect and mechanism of chlorophyllin (CHL) inhibiting HT29 cells. METHODS: IC 50 value and growth curve of HT29 cells were detected with MTT method. Apoptosis was detected with Wright-Giemsa staining, FCM and DNA electrophoresis. Telomerase was detected by PCR-ELISA, and protein and mRNA expression of COX-2 gene were detected through RT-PCR and Western blot. RESULTS: CHL inhibited the growth of HT29 in a dose-dependent manner. CHL blocked HT29 cells in G 1 phase but did not induce apoptosis. Different concentration of CHL inhibits the expression of telomerase and COX-2 in HT29 cells. CONCLUSION: CHL inhibits the growth of HT29 cells by inhibiting the expression of telomerase and COX-2 and blocking cells in G 1 phase. [
5.Experimental study of the protective effect of combined medication on acute cerebral ischemic reperfusion injury

ObjectiveTo study neural protective effect of combined medication with nimodipine and mannitol on injury of cerebral ischemia-reperfusion for screening the better medication method in acute cerebral ischemia-reperfusion. MethodsA model of cerebral ischemia-reperfusion was performed by clipping bilateral common carotid artery of rats with vago- and releasing them 3 hours later. 40 Wistar female rats within 1 month were divided into 5 groups randomly with 8 rats each: model group (no use of medicine), nimodipine group(0.2mg/kg), mannitol group(0.5g/kg), nimodipine and mannitol group, sham-operated group (no use of medicine and no clipping process). The changes of SOD and MDA in brain tissue were measured 24 hours after cerebral ischmic reperfusion in all groups. At the same time pathologic study was performed to compare the different groups.ResultsThere were significant differences between nimodipine and mannitol group and other groups in changes of SOD, MDA and pathological changes(P<0.01). Conclusions Combined medication with nimodipine and mannitol is the better way to protect brain tissue from acute cerebral ischemia-reperfusion than other way in present experiment, by synergistic action.
6.Effect of rhubarb on inflammatory cytokines and complements in patients with systemic inflammation reaction syndrome and its significance.

OBJECTIVETo study the effect of rhubarb in treating patients with systemic inflammation reaction syndrome (SIRS) and its mechanism.
METHODSThe 40 patients with SIRS in the treated group were treated with conventional treatment plus rhubarb powder orally or by nasal feeding, the 38 patients in the control group were treated with conventional treatment alone. Serum tumor necrosis factor-alpha (TNF-alpha) was determined by ELISA during the admission and the 3rd day after admission, C-reactive protein (C-RP), complement 3 and 4 (C3, C4) were also determined by auto-scattering turbidimetric quantitative analysis. The parameters were compared between groups and with normal control group.
RESULTSCure rate in the treated group was significantly higher than that in the control group accompanied with lesser occurrence of multiple organ dysfunction syndrome (MODS) and lower mortality. Serum TNF-alpha, C-RP, C3 and C4 in the SIRS patients were increased during admission, which were significantly higher than normal control, but these parameters would be reduced together with the alleviating of symptoms after treatment, particularly after rhubarb treatment.
CONCLUSIONRhubarb could improve the prognosis of patients with SIRS, its major mechanism is that rhubarb has the antagonizing effect against inflammatory cytokines and complements.

7.Rish factors and drug resistance of nosocomial infections caused by multidrug resistant Pseudomonas aeruginosa

Objective To analysis the risk factors and drug resistance of multidrug-resistant pseudomonas aeruginosa(MDRP) infection and to provide the basis for clinical anti-infective therapy .Methods Predisposing factors and drug resistance to clinical commonly used antibacterial drugs of MDRP which were separated from September 2010 to December 2011 were adopted for retro-spective analysis .Results A total of 235 hospital MDRP infection were collected ,97 multidrug resistant strains were concluded ,the separation rate was 41 .2% .The separation rate of ICU and neurosurgery were higher ,account for 35 .54% and 22 .31% respective-ly .The main resource of specimen is respiratory tract ,about 75 .21% .The resistance of MDRP was very serious .The resistant rate of Amikacin was the lowest ,account for 37 .11% ,followed by piperacillin/tazobactam ,ceftazidime ,account for 47 .4% and 48 .45%respectively .The resistant rate of other antimicrobial agents were greater than 50% .Advanced age ,serious underlying diseases ,long hospital stay ,long-term repeated application of broad-spectrum antimicrobial drugs ,admission to ICU ,invasive treatment operations were the risk factors for MDRP resistance .Conclusion The resistance is very serious ,in order to reduce the generation and spread of drug-resistant strains ,the hospital infection control ,drug resistance monitoring ,rational use of antimicrobial drugs and prevention of cross-infections should be strengthened .
8.Analysis and assessment of real-time contrast-enhanced ultrasonography in the diagnosis of breast masses

Objective To investigate the perfusion characteristics of intraductal breast lesion by real-time gray-scale contrast ultrasound and to determine the value of real contrast ultrasound in the diagnosis of breast masses.Methods A total of 30 breast lumps by ultrasound contrast enhancement were observed from the enhanced level.An enhanced mode and enhanced border were observed when the lesion was clear.The perfusion characteristics were compared between the benign and malignant lesions.Results Thirty breast lumps include 17 benign lumps and 13 benign lumps by pathological operation.After injected with the microbubble contrast medium,all breast lumps enhanced to varied extent.And malignant lesions showed significant enhancement for more than 3 grade(69.2%,9/13).The radial enhancement around lesion were mainly observed in the malignant lesions (P<0.05).Conclusion The microvascular perfusion of breast intraductal lesions can be clearly displayed by real-time gray-scale contrast-enhanced ultrasound.The feasibility of differentiation between benign and malignant lesions according to their perfusion characteristics appears to be promising.
9.Retrospective analysis of epidemiological and clinical features of 86 patients with acute brucellosis

Objective To investigate epidemiological and clinical features of patients with acute brucellosis.Methods The epidemiological,clinical,laboratory and treatment data of patients diagnosed as acute brucellosis during 2002 to 2004 in our hospital were retrospectively analyzed. Results Fifty-one patients had a history of close contact with sheep or cows with brucellsis.Twenty- seven patients had drunk milk or eaten instant boiled mutton.The transmission routes were unknown in 8 patients.All the patients had fever and most had low-grade fever.Fifty-five patients had irregu- lar fever and 20 patients had intermittent fever.The most common manifestations were fever(86/86), fatigue(63/86),sweating(43/86),arthralgia(68/86),orchiditis(7/86),hepatomegaly(8/86),sple- nomegaly(7/86)and headache(18/86).Forty-seven patients had liver dysfunction and 17 patients had thrombocytopenia.Eighty patients recovered and 6 patients relapsed after combination therapy with rifampicin,sulfamethoxazole and quinolone.Conclusion The changes in epidemiological and clinical features of patients with acute brucellosis should be noticed.
10.Studies on chemical constituents of Paederia scandense.

OBJECTIVETo study the chemical constituents of Paederia scandense.
METHODThe constituents were isolated and purified by silica gel and Sephadex LH - 20 column chromatography. Their structures were elucidated by physicochemical properties and spectral analysis.
RESULT20 compounds were obtained and identified as rubiadin-1-methylether (1), diadzein (2), cleomiscosin B (3), cleomiscosin D (4), isolariciresinol (5), linarin (6), isoscopoletin (7), caffic acid (8), coumarinic acid (9), p-hydroxyl-benzoic acid (10), oleanolic acid (11), ursolic acid (12), beta-sitosterol (13), daucosterol (14), paederoside (15), paederosidic acid (16), paederosidic acid methyl ester (17), saprosmoside E (18), paederoscandoside (19), caffeic acid 4-O-beta-D-glucopyranoside (20).
CONCLUSIONCompounds 1-10, and 20 were isolated from this plant for the first time.
