Objective To explore the significance of ultrasound in choosing the delivery modality in 200 cases scarred uterus gravida′s second deliveries .Methods The study selected 200 cases late pregnancy gravidas who had once uterine incision delivery history and analyzed clinical data retrospectively ,all of them were chosen from September 2010 to September 2013 in our hospital . All cases were taken by transabdominal ultrasound or transvaginal ultrasound firstly ,then taken by transabdomina transvaginal ul‐trasound ,finally contrast obsenved the transabdomina transvaginal ultrasound detections with their caesarean section observations , meanwhile observe how the uterus scar ranking affect the delivery consequence .Results (1)A total of 200 cases had higher uterus scars visualization ratio(100 .0% ) than ransabdominal ultrasound (92 .0% )and transvaginal ultrasound(96 .5% )(P<0 .05);con‐trasting the transabdomina transvaginal ultrasound with the caesarean section observations ,there was no significant difference(P>0 .05) .The grading diagnosis coincidence of transabdomina‐transvaginal ultrasound was 89 .74% .(2)The UD rate ,the bleeding vol‐ume during operation and the 24 hours postoperative bleeding volume of scarred uterus gravida grade Ⅱ and grade Ⅲ were signifi‐cantly higher than grade Ⅰ (P<0 .05) .Conclusion Transabdomina transvaginal ultrasound can effectively improve the diagnosis coincidence rate of uterine scar detection and can help to chose the better delivery mode according to the scars healing .

BACKGROUND: Abnormal curvature of the cervical spine reflects in large measure the degenerative changes of the cervical vertebrae. OBJECTIVE: The effect of manipulative therapy has been recognized in restoring normal cervical curvature, and aimed to further probe into the effect of this therapy on blood flow velocity in the vertebrobasilar arteries of patients with cervical spondylosis. DESIGN: A non-randomized and controlled concomitant study. SETTING: Departments of Spinal Orthopaedics and General Sugery of a University hospital, and the Outpatient Department of Orthopaedic Institute of a college of Traditional Chinese Medicine. PARTICI PANTS: From January 1999 to December 2001, 60 patients with cervical spondylosis were treated in the Department of Spinal Orthopaedics in Ruikang Hospital, Third Department of Surgery, First Affiliated Hospital of Guangxi College of Traditional Chinese Medicine, and Outpatient Clinic of Institute of Orthopaedics, Guangxi College of Traditional Chinese Medicine. The patients were divided into spondylosis group with abnormal curvature(30 cases) and spondylosis group without normal curvature(30 cases) . Thirty healthy subjects were also included to serve as the control group.METHODS: Manipulation was applied in these patients and the cervical curvature and blood flow velocity in the left vertebral artery (LVA) and right vertebral artery(RVA) as well as in the basilar artery(BA) were measured before and after the treatment for comparison. The correlation coefficients between them were also calculated.MAIN OUTCOME MEASURES: ① The cervical curvature before and after manipulation in spondylosis group and control group; ② The blood flow velocity in maximal systolic phase and end-diastolic phase in the specified arteries.RESULTS: The blood flow velocity in maximal systolic phase and end-diastolic phase decreased in LVA, RVA and BA in patients with abnormal cervical curvature, whose blood velocities in maximal systolic phase in these arteries was less than those in control subjects and patients with normal curvature ( P < 0.05). After manipulation, the velocity in these arteries in creased apparently in patients with abnormal curvature. The changes in systolic velocity in LVA and systolic in BA were correlated to the curvature before manipulation, and after that, the maximal systolic velocity in LVA,maximal systolic velocity in RVA and BA were all related to the changes of the cervical curvature. CONCLUSION: Abnormal cervical curvature is the anatomical basis for abnormalities of the vertebrobasilar arteries. Manipulative therapy can restore the cervical curve and blood flow velocity in these arteries.

The plastic splint PCL,which is introduced in this article,is a new type of medical exter- nal immobilization material.During the course of its development, the burden has been optimality seeked with regressive orthogonality after screening of the material. Its function has attained the international levels during the eighties. This splint may be used not only in the treatment of burns and various orthopedic conditions, but also in the immobilization of fracture and rehabilation treatment of orthopedic patients etc.

Model-informed drug development (MIDD) in the development of pediatric drugs is drawing more and more attention due to the insufficiency of subjects, lack of research on ontogeny, and the limitation of ethic. The core of MIDD used for dose selection includes the population pharmacokinetic (PopPK) model and physiologically based pharmacokinetic (PBPK) model, as well as model-based simulation and prediction. PBPK model has the advantage of predicting the optimal pediatric dose before the clinical trials and has the ability of extrapolation from adult model to pediatric model. PopPK model characterizes the pediatric PK feature based on the analysis of clinical data and can be used to explore the significant covariates, which is a power tool for individualized medicine in children. With their own advantages and disadvantages, PBPK and PopPK model should be jointly used in the pediatric drug development to refine the dose regimen for children at different ages. In this study, the pediatric drug development of rivaroxaban was taken as an example to introduce the combined application of PBPK model and PopPK model in the design and validation of pediatric dose regimen in Phase Ⅰ, Ⅱ and Ⅲ trials, which may provide reference to MIDD in other pediatric drug development.

Objective To investigate the correlation between obstructive sleep apnea syndrome(OSAS)with myocardial ischemia and arrhythmia. Methods To observe the occuring rate of premature beats and change of ST- segment,90 eases of OSAS patients were detected by the polysomnogram(PSG)and dynamic electrocardiogram at the same time. Results Total morbidity of myocardial ischemia was 32.2 % in OSAS patients, and it was 59.4 %, 15.8 %,20 % in serious, moderate and mild groups respectively. There was a statistically significant difference be- tween the three groups and the control group(P0.05).Conclusion As one of the risky factors of cardiovascular diseases,OSAS can induce myocardial ischemia and arrhythmia.
