OBJECTIVE:To observe the effect of Xuezhikang on serum MCP-1 and endothelial function of brachial artery in patients with chronic heart failure but with normal cholesterol level. Objective To learn the control status of iodine deficiency disorders in Baoji City of Shaanxi Province.Methods From 2009 to 2011,according to the National Iodized Salt Monitoring Program,the iodized salt was monitored,and thyroid was examined in 12 counties(areas) of Baoji City.According to rural and urban area stratification,60 children aged 8 to 10 were selected in each county(district),and their urine samples were collected for determination of iodine content.In 2011,per capita daily salt intake was surveyed by the three weighing method at children's home whose urine was collected.Chencang Area,Fufeng County and Taibai County,representatives of Baoji City different geomorphic features were selected for investigation of water iodine,and urinary iodine of pregnant and lactating women,and 15 people were selected in each county (district).Results From 2009-2011,in the 12 counties(areas) in Baoji City,the coverage rate of iodized salt was all 100.00％(3468/3468); the qualified rate of iodized salt and the consumption rate of qualified iodized salt were all ＞ 99.00％.Children's goiter rate was 3.41％(87/2548),3.06％(77/2520) and 3.33％(84/2520),and they were all less than that of the national standard (＜ 5％).Medians of urinary iodine of 8-10 years old children were 368.20,293.80 and 332.50 μg/L,respectively,and the ratios of urine iodine ≥300 μg/L were accounted for 66.42％ (797/1200),48.05％(692/1440) and 56.67％(816/1440),respectively.Median urinary iodine of pregnant women was 301.81 μg/L and lactating women was 329.79 μg/L.A total of 1116 households were investigated,the median of per capita daily salt intake was 8.9 g.Eighteen water samples were collected,range of water iodine value were 0.60-10.25 μg/L.Conclusions Iodine nutrition in general population of Baoji City is exceeded the optimum level,and the current iodized salt concentration has some down space,but fully consideration should be taken on iodine needs in different iodine deficiency areas and among different groups of people.

Objective To explore the effects of beta-amyloid precursor protein (APP17) peptide on the changes in the expressions of phosphoinositide 3-kinase(PI3K), protein kinase B(PKB) and phosphorylation of cAMP response element binding protein (p-CREB) in the neurons of hippocampal gyms in rats after cardiopulmonary resuscitation. Method Twenty-one Wistar rats were randomly divided into three groups, namely the sham-operated control group, the resuscitation group and resuscitation with APP17 peptide-treated group. The rat model of asphyxial cardiac arrest was made by clamping the endotracheal tube and the standard external cardiopulmonary resuscitation ( CPR) was performed until the restoration of spontaneous circulation ( ROSC) observed.ROSC was defined by the appearance of normal QRS waves of electrocardiogram and mean artery pressure ( MAP)≥60 mmHg for more than 10 minutes. Rats of resuscitation group and control group received intravenous 0.9%NaCl, and the rats of the APP17 peptide group were treated with APP17 peptide(10μg·300 g~(-1), i. v.) after ROSC. Rats were sacrificed by decapitation after reperfusion 2 hours and then the cerebral hippocampal gyrus was immediately separated to detect PI3K, PKB and p-CREB by immunohistochemistry ( IHC) and Western-blot analysis. Statistical comparisons were made by one-way analysis of variance (ANOVA) . Results IHC showed that there was no significant difference in PDK positive cells between resuscitation group and control group (2.75 ±1.80 vs. 2.53 ± 1.53, P > 0.05) . The PDK obviously more increased in the APP17 peptide group than in resuscitation group(5.85 ± 2.83 vs. 2.75 ± 1.80, P < 0.01) .The counts of PKB and p-CREB positive cells were obviously lower in resuscitation group than those in control group (2.45 ± 1.36 vs. 5.22 ± 2.50, P < 0.05);(2.41 ± 1.11 vs. 8.31 ±3.02, P < 0.01 ). The PKB and p-CREB positive cells were significantly higher in the APP17 peptide group than in resuscitation group (9.66±4.32 vs. 2.45 ±1.36, P < 0.01);(14.18 ± 3.96 vs.2.41 ± 1.11, P <0.01). The PKB and p-CREB protein levels in resuscitation group were lower than those in the control group. Conclusions The expressions of PKB and p-CREB in neurons of hippocampus gyrus 2 hours ROSC, and APP17 peptide could restorer the expression of PDK, PKB and p- CREB and thereby protect the neurons of hippocampus gyrus from the injury of CPR.

Objective To compare the perioperative outcomes and long-term therapeutic response of laparoscopic splenectomy versus open splenectomy for idiopathic thrombocytopenic purpura.Methods A retrospective analysis of 124 patients who under-went splenectomy(68 LS and 56 OS)for ITP between January 2011 and January 2015 was conducted.Results Patients undergoing LS were found to require a longer operative time(P <0.05 )but had reduced hospital stay,lower intra-operative blood loss(P <0.05),less postoperative pain,earlier drain removal,and decreased incidence of complications(P <0.05).Conversion to OS was re-quired in 4 patients for excessive loss of blood(5.8%).Deep venous thrombosis(DVT)was observed in 1 patients after OS.One pa-tient died from pneumonia after LS.Mean follow-up of (33±11.8)months was performed in LS group and of (32±12.9)months in OS group.50 patients(73.5%)in LS group and 43(76.7%)in OS group reached sustained complete haematological response(P >0.05).Kaplan-Meier analysis showed that there was no significant difference in the relapse-free survival rate between the groups (P =0.679).Conclusion Compared with open splenectomy,laparoscopic splenectomy for patients with ITP has similar long-term therapeutic response,but it has advantages of minimally invasiveness.

Objective To investigate the contribution of single nucleotide polymorphisms (SNP)variation in folate metabolism pathway genes and its interaction with environmental risk factors to the etiology of NTD. Methods In 275 families from central China, a total of 278 aborted fetal tissues or blood samples were collected from NTD individuals, 478 maternal and/or paternal blood samples were also obtained as controls. Folate supplementation, maternal diabetes mellitus and medication before pregnancy and during the first trimester of pregnancy were investigated. SNP analyses of all samples were performed by CEQ 8800. Case-parent control study and transmission/disequilibrium tests (TDT) were performed according to environmental cofactors stratification to evaluated 28 SNP in 12 folate pathway genes associated with human NTD. Results Only gene MTHFR rs1801133 was significantly associated with NTD, and synergistic effects of environmental risk factors (no folate supplementation and maternal diabetes) were shown on the occurrence of NTD. Linkage disequilibrium between BHMT rs3733890 and NTD existed in case of no folate supplementation,whereas the genotype alone did not contribute to the etiology of NTD. Other SNP were not significantly associated with NTD. Conclusions MTHFR rs1801133 is a risk factor of NTD, but BHMT rs3733890 is not an independent risk factor. Further investigations in folate and methionine cycle genes are requird in larger scale to enclose the interactions between gene and gene, or gene and environmental factors.

Objective To provide anatomical basis for elevating iliac tissue flap pedicled on the iliolumbar artery.Methods The course,number,outer diameter and distribution of iliolumbar artery were observed on 13 human cadavers.Results All the iliolumbar artery originated from the internal iliac artery.The iliolumbar artery gave off 2 branches (iliacus branch and lumbar branch) when passing between the obturator nerve and the lumbosacral trunk,posteriorly to the psoas major.The mean distance between origin of the iliolumbar artery and bifurcation point to iliacus and lumbar branches was 7.1 (7.1 ± 0.5) cm.The iliacus branch divided into two branches:one artery curved forward and anastomosed with the iliacus branch of deep circumflex iliac artery; the other artery supplied the tissue around the posterior superior iliac spine.The lumbar branch supplied the psoas major and the quadratus lumborum.Conclusion The iliolumbar artery and the iliacus branch is one of the most constant and reliable vessels supplying the iliac bone,and can be used as the pedicle of free or pedicled iliac tissue flaps.

AIM: To observe the clinical curative effect of sparse wind clearing heat method combined with western medicine treatment on epithelial herpes simplex keratitis ( HSK) .
METHODS: A total of 75 cases ( 75 eyes ) of patients with epithelial HSK were divided into the treatment group 38 cases ( 38 eyes ) and the control group 37 cases ( 37 eyes) . The patients of the control group were treated with ganciclovir drop and symptomatic treatment. On the basis of the above treatment plan, the patients of the treatment group were treated with sparse wind clearing heat decoction. Clinical curative effect was observed and analyzed.
RESULTS:In the treatment group, curative rate was 79% and effective rate was 95%. In the control group, curative rate was 54% and effective rate was 78%. The differences had significant difference between the two groups (P<0. 05). The corneal lesion healing index in the treatment group was better than that in the control group (P<0. 05). At 6mo follow-up the recurrence rate was 10%in the treatment group, and the control group with 40%, and there was significant difference between the two groups ( P < 0. 05 ). The average healing time of the treatment group was 14. 74±8. 58d, and the control group was 19. 68 ± 8. 71d. There was significant difference between the two groups (P<0. 05).
CONCLUSION: Sparse wind clearing heat method combined with western medicine treatment on epithelial HSK has preferably curative effect.
