Objective To evaluate the safety of dynamic anterior plate fixation for cervical distractive flexion injuries and compare its rigidity between different types of plate design. Methods Twelve sets of cadaveric calf spine were used in this test. All the specimens were made into distractive flexion injury models (C4-C5) ac- cording to Allen's method. After discectomy and grafting, they were randomized into three groups in which Orion, Codman, and Window instrumentations were used respectively. The stiffness of each construct was tested in flexion, extension, lateral bending and axial torsion conditions sequentially. Results Compared with an intact cervical spine, the range of motion (ROM) of an injured cervical spine increased whatever plate was applied. Orion in- strumentation presented stiffness the closest to that of the normal control, except for less torsional stiffness. Codman instrumentation provided stiffness close to that for normal and Orion groups only in lateral bending. Window's was the weakest mad not enough in all kinds of movement. Conclusions Static anterior fixation is the first choice for cervical injuries. Dynamic plate fixation may sacrifice stiffness to some extent, especially when a shifting kind of design is to be chosen.

The aim of this experiment is to investigate the effect of teaseed oil which contains about 76% oleic acid on blood lipids and platelet function. Adult Newzealand female rabbits of 2-3 kg body weight were divided into 3 groups of 6-8 animals each. Stock diet was given with 5 ml of coconut, teaseed or safflower oil to each group plus 0.5g of cholesterol by using stomach tube every day for 8 weeks. The results showed serum total cholesterol (TC), LDL-C, platelet aggregation and plasma TXB2 level were significantly reduced in teaseed oil group as compared with that of coconut oil group. Serum TC, LDL-C were lowered too in safflower group, but it was not significantly different from that of coconut oil group. Ratio of serum HDL-C/TC in teaseed oil group was significantly higher than other two groups, and TXB2/6-keto-PGF1a ratio was decreased about 30% when compared with the two other groups, but with no statistical significance.Male Wistar rats were divided into 4 groups of 30 animals each and were fed for 9 weeks with semisynthetic diets containing 15% coconut, teaseed or safflower oil and 1% cholesterol respectively. Teaseed oil was found to cause a lower level in platelet aggregation and a significant reduction in plasma TXB2/PGF1a ratio. Analyses of fatty acid composition of platelet membrane showed the MUFA content was much higher and that of the ?-6 PUFA was obviously less in teaseed oil group. The results together with that obtained in rabbit experiment clearly indicated MUFA (mainly oleic acid) exerting beneficial effect on the prevention of cardiovascular diseases. Its possible pathway was discussed in the text.

As a method of choice in the treatment of ampulary carcinoma, pancreaticoduodenectomy often demands open procedure. With the development of minimally invasive techniques, laparoscopic pancreaticoduodenectomy was realized in recent years.Because laparoscopic pancreaticoduodenectomy is high technique-demanding,its popularization is restricted. With flexible robotic arnls and three-dimensional imaging,Da Vinci surgical system has overcome the shortcomings of traditional laparoscope to some extent. In March 2010.a 60-year-old female patient with ampulary carcinoma underwent Da Vinci surgical system-assisted pancreaticoduodenectomy at the Southwest Hospital. The mean operation time and blood loss were 490 minutes and 450 ml respectively, and no blood transfusion was required.Out-of-bed activity began shortly after the operation. The gastric tube was removed and fluid diet was given on postoperative day 3.The patient was discharged on postoperative day 10 without incidence of complications. The Success of this case preliminarily demonstrats that Da Vinci snrgical system-assisted pancreaticoduodenectomy is safe and feasible.

Objective Overexpressions of epidermal growth factor(EGF)and EGF receptor have been associ- ated with progression and invasive phenotype of pancreatic cancer. However, the underlying molecular mechanism by which EGF worked in pancreatic cancer cells has not been completely understood. In this study, effect of EGF on the invasion and metastasis of pancreatic cancer cells and its regulatory mechanism were investigated. Methods The effects of EGF on the proliferation, adhesion and invasion of pancreatic cancer cells were detected by WST-1 prolif- eration assay, adhesion assay and invasive assay, respectively. The activity and expression of MMP-2 and MMP-9 were examined by zymography,Western blot and RT-PCR,respectively.The activity of NF-?B was examined by EMSA.Results EGF could significantly promote the invasiveness of pancreatic cancer cells but did not affect cell proliferation or adhesion. The expressions of NF-?B and MMP-9 were significantly increased by EGF,but EGF did not affect the activity and expression of MMP-2.Furthermore,EGF stimulated the NF-?B binding activity.Pre- treatment with NF-?B inhibitors, pyrrolidine dithiocarbamate(PDTC),could significantly inhibit the activity of NF-?B induced by EGF.Meanwhile, the EGF-induced expression and activity of MMP-9,as well as cell invasiveness were also inhibited by NF-?B inhibitor. Conclusion EGF could increase the expression and promote the invasiveness of MMP-9 via the activation of NF-?B in pancreatic cancer cells, which implies that NF-?B inhibitant, such as PDTC,may diminish the invasiveness of pancreatic cancer cells.

Academic Journal of Second Military Medical University 2013;34(2):214-218
Objective: To explore the clinical efficacies of transpedicular lumbar wedge resection osteotomy for correction of kyphosis in ankylosing spondylitis. Methods: From January 2005 to March 2010,32 patients with ankylosing spondylitis kyphotic deformity received one stage posterior transpedicular wedge osteotomy and internal fixation, with 27 receiving single-level ones and 5 receiving two-level ones. All patients underwent X-ray examination of the total spine to obtain radiographic parameters and were asked to accomplish simplified Chinese scoliosis research society-22 (SRS-22) questionnaire to assess quality of health before and after operation. Results: The mean operation time was (260±42) mi n and the mean blood loss was (1 360±282) mL in the patients. The patients were followed up for a mean of (31±8) months (range: 24-76 months) and there were no neurological complications and pseudarthrosis. The chin-brow vertical angle (CBVA), global thoraco-lumber kyphosis angle (TLKA), thoracolumbar kyphosis angle (TKA) and lumber lordosis angle (LLA) were (65. 9±11. 6)° (78. 2±15. 9)° (38. 9±10. 3)° and (-14. 6±17. 3)° before operation and (11. 7±4. 7)°, (38. 9±10. 3)°, (1. 3±7. 8)° and (26.2±5. 6)° after operation, respectively; and significant differences were found for each parameter (P<0. 01). The body height and the sagittal imbalance distance were improved from (135. 4±15. 2) cm and (37. 2±11. 3) cm before operation to (166.2±9. 6) cm and (12. 7±7. 7) cm after operation, respectively. The average SRS-22 score increased from (1. 8±0. 4) before operation to (4. 0±0. 6) after operation, showing a satisfactory outcome. Conclusion: Transpedicular lumbar wedge resection osteotomy is a safe and effective method for treatment of the ankylosing spondylitis kyphosis.
