1.Evaluation on the effect of competency based education practice teaching model of vocational nursing students with ability training

Objective To discuss the effect of competency based education practice teaching model on vocational nursing students with ability training.Methods Totally 96 vocational nursing students were divided into control group from July 2014 to May 2015,receiving routine mentoring teaching mode and totally 99 vocational nursing students were divided into learning group from July 2015 to May 2016,receiving competency based education practice teaching model.Theoretical examination and clinical operation skill test as well as self evaluation of caring ability and transitional nursing ability were conducted to both groups after the course to compare the test scores and the change of their caring ability and transitional nursing ability.SPSS 19.0 was used for statistical analysis,and independent samples t test was used between the two groups.Results The theoretical scores of the nursing students in the experimental group ([93.70 ± 4.27) vs.(88.00 ± 4.83)] and skill test [[95.70 ± 2.49] vs.(91.40 ± 2.87)] were higher than those of the control group;caring ability score [(189.3 ± 13.5) vs.(179.4 ± 10.4)],with the average score of cognitive dimension (51.6 ± 9.8) vs.(50.2 ± 10.3)],the dimension of courage (70.2 ± 6.5) [vs.(65.7 ± 8.4)],average patient dimension (63.4 ± 4.8) vs.5.4)];transitional nursing ability score [(124.95 ± 24.06) vs.(118.18 ± 20.41)],the dimension of knowledge sharing [(38.43 ± 8.22) vs.(36.08 ± 7.23)],average score of attitude dimension [(54.08 ± 10.66)vs.(51.02 ± 9.16)],average score of behavior dimension [(32.43 ± 6.25) vs.(32.43 ± 6.25)],which were all increased compared with the control group,and the difference was statistically significant.Conclusion Competency based education practice teaching mode achieves good teaching effect and it is worth recommending in clinical teaching.
2.Expressions and significance of Survivin and VEGF in hepatocellular carcinoma

Objective To explore the expressions of Survivin and VEGF and relationship between them in hepatocellular carcinoma(HCC).Methods The expressions of Survivin protein and VEGF protein in 50 HCC.30 cirrhosis and 10 normal tissues were assessed by immunohistochemical method.The expressions of Survivin mRNA and VEGF mRNA in 50 HCC,30 cirrhosis and 10 normal tissues were assessed by in situ hybridization.Results The expressions of Survivin and VEGF in cancer tissues,cirrhosis tissues,normal tissues weresignificantly different. The expression of Survivin in HCC tissues was stronger than that in cirrhosis,but the expreesion of VEGF in cirrho- sis was stronger than that in HCC tissues.Conclusion The expression of survivin.is closely associated with the ex- pression of VEGF in HCC and they take positive correlation.The abnormal expressions of Survivin and VEGF are closely associated with the development of HCC.They may play important roles in the development of HCC.
3.Transurethral ureteroscopy in the diagnosis and treatment of hemospermia.

OBJECTIVETo explore the clinical application value of transurethral ureteroscopy in the diagnosis and treatment of hemospermia.
METHODSWe summed up and analyzed the experience in the diagnosis and treatment of 43 hemospermia patients by transurethral ureteroscopy and douching therapy.
RESULTSThe disease causes were clearly determined and the therapeutic procedures successfully accomplished in all the cases. The mean surgical time was 20 (18 -45) minutes. No significant complications developed either intraoperatively or postoperatively. The patients were followed up for 1 -24 months, during which hemospermia symptoms completely disappeared in 35 and were relieved in 6 of the cases, but the other 2 remained unimproved. All the patients had normal sexual life and none experienced retroinfection postoperatively.
CONCLUSIONTransurethral ureteroscopy, advantageous for its safety, high rate of detection, good effect of treatment and fewer complications, deserves to be popularized in the clinical diagnosis and treatment of hemospermia.

4.Influencing factors for measles vaccine immunization among health care workers in Hangzhou

Objective To determine factors influencing the immunization of measles vaccine(MV) among health care workers in Hangzhou, and to provide recommendations to promote their MV immunization. Methods In 2016, we used typical sampling method to select 2 general hospitals of 3 different levels, 1 infectious diseases hospital and 1 children's hospital, and interviewed health care workers in high and low measles risk departments to investigate their MV immunization by using a structured questionnaire. Factors influencing their immunization were analyzed by logistic regression model. Results A total of 141 of 349 health care workers investigated had MV immunization history, and the MV immunization coverage rate was 40.40%.The logistic regression analysis showed that working in low measles risk department(OR=1.91, 95%CI: 1.20-3.04) was risk factors for MV immunization, and having confidence with the effectiveness of MV(OR=0.40, 95%CI: 0.21-0.78) . Knowing the "measles vaccination suggestion" (OR=0.50, 95%CI: 0.28-0.91) and the hospital had organized measles vaccination for health care workers in recent years(OR=0.35, 95%CI: 0.22-0.57) were protective factors for MV immunization. Conclusions Health care workers in Hangzhou had low MV coverage but high willingness. We should enhance education activity of MV immunization and organize measles vaccination for health care worker at regular intervals by hospitals to increase the MV coverage.
5.The surgical treatment and prognosis for recurrent colon cancer.

OBJECTIVETo investigate the surgical treatment and prognosis for recurrent colon cancer after curative resection.
METHODSThe clinical data of 102 recurrent colon cancer cases from January 1997 to December 2005 were analyzed retrospectively. Obtained data were analyzed by Statistical Package for the Social Sciences (Release 11.5, SPSS, Inc). The related factors were underwent chi2 analysis,survival analysis were estimated using the Kaplan-Meier method and compared using the Log-rank test. COX regression was used in multivariate analysis.
RESULTSUnivariate analysis revealed that obstruction of primary tumors, CEA level before reoperation, number of recurrence, time of recurrence, and reoperation type were significant statistically. COX regression analysis revealed that number of recurrence, reoperation type was the most important prognostic factor.
CONCLUSIONThe recurrent colon cancer still need active surgical treatment in order to prolong the survival time.

6.Changes of endothelial cell function and platelet activation in rabbit spinal cord with ischemia-reperfusion injury.

OBJECTIVETo study the changes of vascular endothelial cell function and platelet activation in rabbit spinal cord following ischemia-reperfusion (I/R) injury and their roles in the spinal cord injury.
METHODSRabbit spinal cord I/R injury models were established using Zivin method, and the changes in plasma NO and GMP140 levels were dynamically monitored after the injury.
RESULTSPlasma NO level increased significantly in the I/R group at the end of the ischemia, and reached the peak level at 2 h of reperfusion as compared to that in sham-operated group (P<0.01). Plasma NO level decreased at 6 h of reperfusion, but still significantly higher than the level in the sham-operated group (P<0.05). Plasma GMP140 underwent no significant changes in the sham-operated group, but significantly increased in the I/R group at the end of the ischemia, followed by gradual declination to the normal level at 2 h of reperfusion.
CONCLUSIONSpinal cord I/R injury causes overexpressions of NO and GMP140, suggesting the involvement of endothelial cell injury and platelet overactivation in the pathological process and repair of spinal cord I/R injury.

7.Single plus one port laparoscopic radical prostatectomy: a report of 8 cases in one center.

Laparoscopic radical prostatectomy is considered the first treatment of choice for local prostate cancer due to its minimal invasion advantage. To further achieve the goal of minimal invasion, single port laparoscopic radical prostatectomy has been developed to minimize the complications associated with puncture tracks. The aim of this study was to illustrate the technique for single port laparoscopic radical prostatectomy and evaluate its efficacy and safety. We reported 8 cases of radical prostatectomy with excellent early outcome carried out in Shanghai Changzheng Hospital from June 2009 to August 2009 using a home-made multiple instrument access port and adding an additional small incision at McBurney point.

8.Surgial treatment and prognosis of ovarian metastasis from colorectal cancer.

OBJECTIVETo summarize the surgical treatment experiece and to investigate the prognosis of the patients with ovarian metastasis from colorectal cancer.
METHODSThe data of 67 patients with synchronous or asynchronous ovarian metastasis from colorectal cancer surgically treated between January 1989 and December 2005 were collected and analyzed retrospectively using Statistical Package for the Social Sciences (Release 11.5, SPSS, Inc). Prognostic factors were analyzed using chi2 test. Survival analysis was estimated by the Kaplan-Meier method and compared using the log-rank test. Multivariate analysis was carried out by Cox regression.
RESULTSThe overall 1-, 3- and 5-year survival of these 67 patients was 71.0%, 18.7% and 9.2%, respectively. Univariate analysis revealed that the metastasis was confined in the ovary or pelvis only, unilateral/double ovarian metastasis, and operation mode were all statistically significant prognostic factors (P <0.05). Cox regression analysis showed that the operation mode was the most important prognostic factor (OR = 3.531, P <0.001).
CONCLUSIONSurgical treatment is still the most effective mode in the treatment for the ovary metastasis from colorectal cancer.

9.Clinical significance of solitary lymph node metastasis in carcinoma of the ampulla of Vater.

OBJECTIVETo investigate the rule of distribution of solitary lymph node metastasis and its relation with clinico-pathologic factors in carcinoma of ampulla of Vater.
METHODSThe data of 26 patients who were discovered to have solitary lymph node metastasis, from 152 patients with carcinoma of the ampulla of Vater who had received pancreatoduodenectomy were retrospectively reviewed. The related clinico-pathologic factors affecting it's metastasis were analyzed and compared with 105 such patients without any lymph node metastasis.
RESULTSOf these 152 patients with carcinoma of ampulla of Vater, 47(30. 9%) had lymph node metastasis and 26 had only solitary lymph node metastasis with a rate of solitary lymph node metastasis of 55. 3% (26/47). The majority of the solitary lymph node metastasis (84. 6% , 22/26) were located at the pancreaticoduodenal region, only 4 patients had skip metastasis. It was revealed by Chi-square test (chi(2) ) that solitary lymph node metastasis was correlated with the tumor size (P = 0. 007) , histological differentiation(P = 0. 003) , T stage(P = 0. 000) and pancreatic infiltration (P =0. 009).
CONCLUSIONThe majority of solitary lymph node metastasis are located at the pancreaticoduodenal region. Sentinel lymph node assessment may be helpful to determine the extent of lymph node dissection for carcinoma of the ampulla of Vater.

10.Changes in serum and spinal cord nitric oxide levels in rabbits with spinal cord ischemia-reperfusion injury

Objective To observe the changes in serum and spinal cord nitric oxide (NO) levels after spinal cord ischemia-reperfusion (IR) injury in rabbits. Methods The Zivin method was used to induce spinal cord IR injury in rabbits, in which the NO levels in the serum and spinal cord tissues were observed dynamically. Results In rabbits with spinal cord IR injury, the serum NO levels increased gradually after spinal cord IR injury and reached a peak level at 2 h, showing significant difference from the levels of before ischemia (P<0.05), and followed by obvious reduction at 6 and 12 h after the injury (P<0.05). In the spinal cord tissue homogenate, the NO levels increased significantly to reach the peak level at the end of the ischemia (P<0.05), and then decreased obviously at 2 and 6 h to the level comparable to that in the sham-operated group (P>0.05). Conclusions NO is over-expressed in the serum and spinal cord tissue in rabbits following spinal cord IR injury, and may play a role in neuronal damage and repair in the event of spinal cord IR injury.
