1.Analysis of the relevant factors for ischemic necrosis of the talus fracture.

OBJECTIVETo explore the relationship of the classification of talus fracture and surgery methods with avascular necrosis.
METHODSFrom March 2009 to November 2013, 78 patients with talus fracture were treated, of them, 43 cases were followed up from 2 to 5 years. There were 27 males and 16 females, aged from 17 to 65 years old with the mean of 38.6 years. Thirty-nine cases had talar neck injury and 4 cases had talar body injury. Different treatments were performed according to different injury conditions. The time from injury to treatment was from 6 to 48 hours. The effect of classification of talus fracture and surgery methods on avascular necrosis was analyzed.
RESULTSIn 43 cases,19 cases occurred avascular necrosis, including talar neck fracture of type I in 2 cases, type II in 5 cases, type III in 5 cases, type IV in 5 cases and talar body fracture in 2 cases (combined with talar neck fracture). And 29 patients were treated with operation, there was no statistically significant differences in avascular necrosis with different operations.
CONCLUSIONCompared with talar body fracture, talar neck fracture is more easily to develop into avascular necrosis. In the 4 types of talar neck fracture, the possibilities of type III and IV were the most ones.

2.Diagnosis and treatment of prostate cancer with prostate specific antigen 4-10?g/L

Objective To investigate the diagnostic and therapeutic characteristics of prostate cancer with prostate specific antigen(PSA)4-10?g/L. Methods The data of prostate biopsies for the patients with PSA 4-10?g/L from May 1998 to February 2004 and the treatment and prognosis of these patients were retrospectively analyzed. Results In the 141 cases, 34 were diagnosed as prostate cancer, including 3 cT1, 21 cT2, 6 cT3 and 4 cT4. Mean Gleason score was 5.8, and the mean Gleason scores of cT1 and cT2 were significantly lower than those of cT3 and cT4. Twenty-four cases of cT1 and cT2 underwent radical prostatectomy. Four cases of cT3 recieved radical prostateetomy after 4 months of neoadjuvant endocrine therapy. Two cases of cT3 and 4 cT4 received castration. Pathological section from radical surgeries showed that 21 cases were organ confined and 7 were locally invasive. The difference of mean Gleason score between these two groups was significant in statistics. After 18 to 69 months (mean 42 months)follow-up, 21 organ confined cases were free survived, 4 locally invasive cases had biochemical recurrence and 1 case had multifocal bone metastasis. No cancer-specific death happened. Two of 4 cT4 cases were still alive and 2 died of the tumor. Conclusions Routine needle biopsy is necessary for the cases with“grave area”PSA of 4~10 ?g/L. Prostate cancers with PSA 4~10/?g/L are not always early diseases. Gleason score is a very important index for determine the stage of pathology after surgery. Radical prostatectomy is an effective treatment for organ confined disease.
3.The influence of acupuncture on Nestin expression in the cortex of hypoxia-ischemic neonatal rats

Objective To investigate the influence of acupuncture on Nestin expression in the cerebral cor- tex of hypoxia-ischemic neonatal rats.Methods A total of 72 Sprague-Dawley(SD)rats aged 7 days were random- ly divided into three groups:A normal group(n=24),the hypoxic-ischemic encepbalopathy(HIE)group(n=24), and an acupuncture+HIE group(n=24).The rats were sacrificed at time points including 3d,7d,14d and 28d af- ter insult,their brains were removed and sections were made.Any nerve stem cells(NSCs)in the cerebral cortex were detected using immunohistochemical staining of Nestin.Results Nestin-positive cells were found in the cere- bral cortex in the normal neonatal SD rats early after birth.After HI insult,the Nestin-positive cells had increased in the injured areas of the cortex and in contralateral mirror areas,especially around the injured areas.Peak Nestin ex- pression was 3 days after insult.In the acupuncture+HIE group,Nestin positive cells were significantly increased when compared with that in the HIE group,the peak was postponed,and the peak was longer than in the HIE group. The Nestin positive cells decreased,were seldom seen,and were not different among three groups 28 days after the HI insult.Conclusions Endogenous neural stem cells in the cerebral cortex were increased by acupuncture in hy- poxia-ischemic in neonatal rats,and acupuncture made the peak of Nestin expression longer.
5.Comparison of baicalin,paeoniflorin and glycyrrhetic acid in different decoctions of Huangqin Decoction

AIM: To compare the contents of baicalin,paeoniflorin and glycyrrhetic acid in the seperated and mixed decoction of Huangqin Decoction(Radix Scutellariae,Radix Paeoniae alba,Radix et Rhizoma Glycyrrhizae and Fructus Jujubae). METHODS: The contents of baicalin,paeoniflorin and glycyrrhetic acid were analyzed by HPLC.Chromatographic conditions included: Hypersil BDS C_(18) column and the mobile phase consisted of a mixture of methanol-water-phosphoric acid(47(∶)53(∶)0.2),acetonitrile-water-phosphoric acid(18(∶)82(∶)0.1) and methanol-water-phosphate buffer solution(70(∶)29(∶)1).Baicalin,paeoniflorin and glycyrrhetic acid were detected at 280 nm,230 nm and 250 nm respectively. RESULTS: The linear range of baicalin,paeoniflorin and glycyrrhetic acid were 2.88—72.00 ?g/mL,2.85—22.80 ?g/mL and 4.16—52.00 ?g/mL respectively.The contents of baicalin in the mixed decoction was higher than that in the seperated decoction.The contents of paeoniflorin and glycyrrhetic acid in the mixed decoction were almost as same as that in seperated decoction. CONCLUSION: The contents of active ingredients in the mixed and seperated decoction are different.
6.Digital geometry processing and its applications in maxilla and mandible reconstruction surgery.

A new method to fabricate exact-fit medical implants is proposed. Medical image processing and digital geometry processing are adopted for bone triangular mesh modeling from CT image series. After geometric operations such as decimation, defragmentation, cutting and mirroring are applied to the original triangular mesh, an RP model of same size as the bone is fabricated with LOM method. RP models are used for manufacturing exact-fit implants and they also serve surgery-guiding purpose. This novel technique has been applied in over 30 clinical cases, and it is proven to be practicable.

9.Robotic-assisted thoracic surgery using Da Vinci robotic system: a report of 25 cases

Objective To summarize the basic experience of robotic-assisted Thoracic Surgery using da Vinci Robotic system and to evaIuate its value in clinical application.Methods From Jan 2009 to Sep 2012,the clinical data of 25 patients who underwent robotic-assisted Thoracic Surgery using da Vinci Robotic system were analyzed.Results All 25 patients were successfully operated and no conversion to thoracotomy occurred,including 10 cases of pulmonary lobectomy,14 cases of rumor mainly in anterior mediastinum and a cases of esophageal carcinoma.The operative time of pulmonary lobectomy was 180-390min,mean(241 ± 90.98)min,the estimated blood loss was 150-300 ml,mean (195 ± 43.78)ml,and the post-operative 24 h drainage was 250-300 ml,mean(305 ± 28.38)ml.The operative time of rumor from thymus mainly in anterior mediastinum was 70-210 min,mean (116.36 ± 45.23)min,the estimated blood loss was 50-100 ml,mean (63.64 ± 23.36)ml,and the post-operative 24 h drainage was 20-270 ml,mean (123.64 + 69.93) ml.No other major complications were experienced,no peri-opermive mortality occurred.Conclusion Da Vinci robotic-assisted thoracic surgery is a feasible and safe surgical procedure with clear operation field,precise dissection,minimal trauma and fast recovery.
10.An Aldose Reductase Inhibitor Screening Model Constructed by Transfection of pSNAV-AR into HEK293 Cells

Objective:Aldose reductase,involved in the pathogenesis of diabetic complications,was recombinated with an adeno associated virus vector pSNAV2.0,and it was transfected into human embryonic kidney 293(HEK 293)cells.The gene engineering produced AR would be used as a target protein to screen aldose reductase inhibitors.Restriction endonuclease digestion and ligation procedures were performed to construct the AR expression plasmid vector pSNAV-hAR.Methods:After confirmation the recombinant plasmid by PCR,restriction endonuclease digestion,and DNA sequencing,pSNAV-hAR was transfected into HEK293 cells.Western blot and immunofluorescence analysis were performed to detect the expression of AR and its enzyme activity.Results:The results of a series of analysis including AR activity assay,Western blot and immunofluorescence analysis shown the expressed protein mediated by the adeno associated virus vector transfecting HEK 293 cells,was functional AR.The traditional aldose reductase inhibitors,Sobinil and Zopolrestat,were used to test and verify the constructed cell model.Conclusion:The established AR expression model can be used in mechanismresearch of activation of polyol pathway on diabetic complications and screening potential aldose reductase inhibitors.
