Chinese Journal of Hospital Administration 2011;27(11):863-865
ObjectiveTo imitate the process of identifying episode separations, which helps to improve the methodology of control episode separations. Methods Abstracts of inpatient record on all patients discharged from the hospitals in Beijing in 2010 were drawn from Beijing Public Health Information Center.Risk-adjusted by Diagnosis Related Groups-Beijing version (BJ-DRGs),two-weeks readmission rate was calculated at the whole city and hospital levels respectively.Comparing the readmission rate between these two levels,“suspicious objects”of episode separations can be found. In turn, the episode separations can be identified by detail analysis of these suspicious objects. Results In 2010,there were 13 DRGs with readmission rate exceeding 10%.Take the cases of dementia and other chronic brain disorders for example, the readmission rate for this type of inpatients in hospital G was much higher than the average level of the whole city. After the detail analysis of hospital G,the episode separations were found. Conclusion It is an effective method to indentify episode separations that calculating readmission rate based on the DRG risk-adjustment and using city level readmission rate as bench-mark.

Objective To evaluate MRI in diagnosis and follow-up of Achilles tendon rupture. Methods From April 2003 to May 2005, 24 cases of closed Achilles tendon rupture were examined in our department with MRI. It was performed through sagittal T1WI, T2WI, GE-STIR and axial TME imaging. Four cases were followed up with MRI after 4 to 8 weeks. Results MRI could determinate the severity and exact location of Achilles tendon rupture by demonstrating signal changes of Achilles tendon. In this series, complete rupture of Achilles tendon mostly occurred at the median part, with augmentation and high signal on T2WI. MRI demonstrated high signal on T2WI in partial rupture of Achilles tendon. The signal intensity in the four cases followed up by MRI was shown to decrease. Conclusion MRI is a valuable tool for clinical diagnosis and follow-up of Achilles tendon, because it provides enough information through signal changes at the tendon.

BACKGROUND: Dynamic glucose monitoring can reflect the fluctuation of blood glucose by successive examination at the interval of 5 minutes, which contributes to the overall and objective evaluation of glucose metabolism.OBJECTIVE: To investigate the tendency and characteritics of blood glucose fluctuation in type 2 diabetic and normal subjects. DESIGN: A case-controlled retrospective experiment based on patients and healthy controls.SETTING: Endocrine Metabolism Ward and Clinic of Sixth People's Hospital Affiliated to Shanghai Jiaotong University.PARTICIPANTS: Totally 47 patients were newly diagnosed as having type 2 diabetes mellitus at Endocrine Metabolism Ward and Clinic, Sixth People's Hospital Affiliated to Shanghai Jiaotong University, between December 2002 and July 2004. They had not received intervention (diet, exercise or medicine) before. Those with acute complications such as ketoacidosis and hyperosmolar coma were excluded. Meanwhile, 43 healthy controls were selected from those coming for routine health examinations, and they had no family history of diabetes, and impaired fasting glucose and impaired glucose tolerance were excluded. Their lipid and liver and kidney indexes as well as blood pressure were within normal range. They all signed informed consent.METHODS: Totally 43 normals and 47 patients with type 2 diabetes mellitus were subjected to dynamic glucose monitoring for consecutive (71±10) hours, and (846±122) results of glucose level were obtained.MAIN OUTCOME MEASURES: The average glucose level, diurnal maximum and minimum, fluctuation amplitude, the average preprandial and postprandial glucose values and the time percentage of diurnal glycemic fluctuations.RESULTS: According to the objective of this study, all subjects went through the experiment without loss. ① The fluctuation tendency of blood glucose in two groups: The average glucose, diurnal maximum and maximum fluctuation amplitude in type 2 diabetes mellitus group were [(12.2±2.6) mmol/L], [(18.5±2.7) mmol/L] and [(11.2±2.8) mmol/L,respectively], which were significantly higher than those in control group [(5.3±0.5), (7.8±1.4) and (3.9±1.6) mmol/L](t=16.68, 23.04, 15.14;P < 0.001). ② The percentage of diurnal glycemic fluctuations time: The time percentage of glycemic fluctuations within 2.8-7.8 mmol/L was 99% (87%-100%) in control group, 95% (28%-100%) and 62% (6%-100%) when blood glucose.was increased to > 7.8 mmol/L or > 11.1 mmol/L in type 2 diabetes mellitus group.CONCLUSION: Blood glucose in type 2 diabetes mellitus has been proved to fluctuate at a relatively higher level than that of normals, with greater fluctuation amplitude.Dynamic glucose monitoring is benefical to evaluating the tendency and characteritics of blood glucose fluctuations in type 2 diabetic and normal subjects.

Objective:To investigate the therapeutic efficacy of immunotherapy with dendritic cells-based vaccine against intracranial gliomas in rats.Methods:C6 glioma cells were injected into the brain of Wistar rat under stereotactic monitor to establish an animal model of glioma. The precursors of dendritic cells were isolated from bone marrow of rats, stimulated in vitro with recombinant rat granulocyte-macrophage colony-stimulating factor(rrGM-CSF)and interleukin-4(rrIL-4).Cultured cell populations were confirmed to be functional DCs.These DCs were then pulsed ex vivo with C6 tumor-lysates prepared by three cycles of freezing to -80℃ and thawing to 0℃ and subsequently injected subcutaneously into rats harboring intracranial C6 tumor.Rats from different group were treated with five weekly subcutaneous injections of either control media, unpulsed DCs,or DCs pulsed with tumor-lysates.The animals were followed for survival, the percentage of CD8 +T cells in peripheral blood and cytotoxicity assay in vitro were determined by FACS.The level of cytokine IFN-? and IL-10 were detected by ELISA.Results:The results indicated that C6 glioma model rats treated with tumor-lysate-pulsed DCs led to prolonged survival time, increased the percentage of CD8 + T lymphocyte in peripheral blood in comparing with control group.Cytotoxicity assays suggest that vaccination with these tumor-lysate-pulsed DCs can induce specific cytotoxic T lymphocytes against C6 tumor cells compared with control group.Furthermore, significantly enhanced IFN-? and reduced IL-10 (even undetectable)were observed in rats treated with pulsed-DCs.Conclusion:Data supported the therapeutic efficacy of systemic vaccination with DCs pulsed with tumor-lysates against intracranial glioma.
