1.Comparative early results on off-pump versus on-pump coronary artery bypass grafting(CABG) in elderly patients

Objective To compare the outcomes of off-pump versus on-pump CABG.Methods From 2002 to 2008,CABG was performed in 105 patients aged 80 years and over,including 45 without cardiopulmonary bypass (CPB) or off-pump (OP) CABG (OPCAB) and 60 with CPB (onpump CABG).The outcomes were compared between two groups.Results The mean ICU stay was (37.1±30.3) h in OPCAB group and (60.6±58.2) h in on-pump CABG group (P<0.01).Average ventilator-assisted time was (10.8±9.7) h for OPCAB group versus (22.3±35.7) h for onpump CABG group (P<0.01).Postoperative atrial fibrillation occurred in 31.1% of OPCABG patients and 41.7 % of on-pump CABG patients (P<0.01).The mortality rate was 5.0% in OPCABG group versus 8.3% in on-pump CABG group (P<0.05).Conclusions OPCABG is a safe and efficient method of myocardial revascularization in the elderly patients with lower morbidity and complications.
2.Diagnostic value of serologic intestinal-fatty acid binding protein in necrotizing enterocolitis: a Meta analysis

Objective To study the diagnostic value of serologic intestinal-fatty acid binding protein (i-FABP) in neonatal necrotizing enterocolitis (NEC).Method The standard search strategy included electronic search and manual search.Electronic search was carried out in databases including PubMed,Ovid,Cochrane Library,Chinese Biomedicine Database (CBM),CNKI,WANFANG DATA and VIP database.As a supplement,relevant references from previous reviews and studies were examined by manual search.Two independent reviewers assessed the quality of included studies and extracted data,and performed further meta-analysis.RevManS.3.5 and Meta-Disc1.4 and Stata12.0 were used in the statistical analysis.Result A total of 11 studies with 700 cases were identified,including 308 NEC cases and 392 control cases.The blood samples collected at the time of suspected NEC stage showed pooled sensitivity 0.56 (95% CI 0.44 ~ 0.67),pooled specificity 0.93 (95 % CI0.87 ~ 0.97),area under curve (AUC) =0.8282,and Q* =0.7610,respectively.The blood samples collected after clinical diagnosis of NEC showed pooled sensitivity 0.78 (95% CI 0.72 ~ 0.83),pooled specificity 0.81 (95% CI 0.76 ~ 0.86),AUC =0.8680,and Q* =0.7985,respectively.Conclusion i-FABP is a valid serologic biomarker for early diagnosis of NEC in neonatal infants.
3.Experimental research of CT-guided thermochemotherapy for pig's healthy pancreas

Objective To investigate the influence of CT-guided thermochemotherapy for pig's healthy pancreas and in turn to discuss the feasibility and efficacy on pancreatic tumors.Methods 9 healthy pigs were undergone CT-guided 80℃themochemotherapy with 18 G fine meedle injecting individually of 5-Fu 750mg(10 ml)into the pancreatic tail within 2min.CT,MRI and serum analyses test were carried out as the follow up study at 1h,3rd,7th and 14th day after the procedure;since the 3rd day,3 pigs were sacrificed right after every follow up and together with comparative study between diagnostic imagings and pathologic changes. Results The pig pancreatic tail necrotic lesions appeared to be most conspicuous at the 3rd day after thermochemotherapy and shrank gradually from 7th day to 14th day.Serum amylase level reached the highest at 3rd day after the treatment and lowered down at 7th day;and near approximately normal at 14th day.The outcomings of diagnostic imagings and macroscopic pathology were in accordance.Obvious pancreatitis occurred only in one pig and all others were nearly uneventful without serious complications of pancreatitis,intestinal adhesion and mortaligy.Conclusion CT-guided themochemotherapy can induce pancreatic,focal necrosis in healthy pig but with no serious complications;and these can further more establish the foundation for clinical practiee.(J Intervent Radiol,2007,16:844-846)
4.Effect of Shengjiang San on NF-κB Activity in LPS-activated Rat Alveolar Marcrophage Cell Line

ABSTRACT:OBJECTIVE To investigate the effect of Shengjiang San on expression of TLR4 and NF-κB in LPS-induced rat pulmonary alveolar macrophage cell.METHODS NF-κB signal pathway in NR8383 cells was stimulated with LPS.The ex-pression level of TLR4 was measured by QRT-PCR and Western Blotting separately.NF-κB activity was detected by Dual Lu-ciferase Reporter Gene Assay kit.The protein expression level of p-p65 was detected by Western Blotting.The mRNA level of TNF-α,A20,IL-6,IL-1β was measured by QRT-PCR,and the production of TNF-α,IL-6,IL-1β in the supernatant of NR8383 was determined with ELISA.Shengjiang San treated NR8383 cells,which were induced NF-κB signal pathway by LPS.NF-κB activity was detected by Dual Luciferase Reporter Gene Assay kit,qRT-PCR,Western Blotting and ELISA.RE-SULTS LPS induced TLR overpression in both mRNA and protein levels.Moreover,NF-κB signaling pathway and down-stream target cytokines were increased with dose-dependent.10 μg/mL Shengjiang San treatment abrogated the induction of LPS to TLR4 and NF-κB signaling pathway.CONCLUSION Shengjiang San significantly reduced LPS-activated NF-κB signa-ling pathway in NR8383.

8.Expression of ITF Gene in Lacococcus lactis

To develop an oral drug,ITF gene encoding ITF proein,was expressed in a live delivery vehicle lactococcus lactis.First,the ITF gene was cloned into the prokaryotic expressive vector pNICE:sec.Second,the recombinant vector pNICE:sec-ITF was transformated into Lactococcus lactis strain NZ9000 to express ITF protein.Then the recombinant ITF was induced to express and was identified by SDS-PAGE and Western blot.Rabbits are divided into blank control group,preparation group and therapeutic group which are respectively administrated wih PBS and pNICE:sec-ITF Lactococcus lactis.By grades of ulcer test whether administrated pNICE:sec-ITF Lactococcus lactis protects against HCl-induced gastric injury in rabbits.The results were described as follows.The ITF was amplified and cloned in the vector pNICE:sec successfully.The fusion protein(5.9kDa) was expressed in L.lactis by the induction of the nisin.The quantity of expression accounted for 5% of the total bacterial protein.Western bolt analysis confirmed that fusion protein could be recognized specially by Monoclonal Anti-human TFF3 Antibody.Preparation groups and therapeutic groups do good than control group.Prove that administrated pNICE:sec-ITF Lactococcus lactis is biologically active in an HCl-induced rabbit gastric mucosal injury model.
9.Effect of Heroin on DLG4 Expression in Hippocampus, Amygdala and Frontal Cortex of Rats.

OBJECTIVE:
To observe the expression of discs large homolog 4 (DLG4) protein in hippocampus, amygdala and frontal cortex of rats and evaluate postsynaptic density in heroin dependence.
METHODS:
The rat heroin dependent model was established by increasing intraperitoneal injection of heroin. DLG4 proteins in hippocampus, amygdala and frontal cortex of heroin dependent 9, 18, 36 days rats were detected with immunohistochemical staining and compared with that in the control group.
RESULTS:
DLG4 proteins in hippocampus, amygdala and frontal cortex were gradually reduced with extension of heroin dependent time.
CONCLUSION
Heroin dependence can affect postsynaptic density of hippocampus, amygdala and frontal cortex. The changes become more apparent with extension of heroin dependence time.

10.Correlation analysis between liver and spleen stiffness measured by acoustic radiation force impulse and hepatic venous pressure gradient

Objective To investigate the correlation between liver and spleen stiffness measured by acoustic radiation force impulse (ARFI) and hepatic venous pressure gradient (HVPG),and to evaluate its efficiency in the diagnosis of portal hypertension.Methods From April 2014 to March 2016,20 cases underwent HVPG measurement because of liver cirrhosis were enrolled.Before HVPG measurement,liver and spleen stiffness were assessed with ARFI.The correlation between HVPG and age,alanine aminotransferase (ALT),aspartate aminotransferase (AST),total hilirubin,serum albumin,platelet count,prothrombin time,aspartate aminotransferase to platelet ratio index (APRI) score,Child-Pugh score,model for end-stage liver disease (MELD) score,liver stiffness and spleen stiffness were analyzed.Pearson correlation and Spearman rank correlation were performed for statistical analysis.Results HVPG,liver and spleen stiffness were successfully measured in all 20 patients.The mean liver stiffness was (1.78±0.29) m/s,the mean spleen stiffness was (3.37±0.44) m/s and HVPG was (16.10±5.14) mmHg (1 mmHg=0.133 kPa).Age,ALT,AST,total bilirubin,serum albumin,platelet count,prothrombin time,APRI score,Child-Pugh score and MELD score were all not correlated with HVPG (all P>0.05).But HVPG was positively correlated with liver and spleen stiffness (r=0.449,P=0.047;r=0.487,P=0.030).In the diagnosis of HVPG≥12 mmHg,the area under curve (AUC) of liver stiffness was 0.875,the optimal cut-off value was 1.77 m/s,the sensitivity was 68.6 % and the specificity was 100.0%.In the diagnosis of HPVG≥20 mmHg,the AUC of liver stiffness was 0.798,the optimal cut off value was 1.85 m/s,the sensitivity was 100.0% and the specificity was 68.8%.The AUC of spleen stiffness was 0.820,the optimal cut-off value was 3.23 m/s,the sensitivity was 100.0 % and the specificity was 56.3%.Conclusion In patients with liver cirrhosis,liver stiffness and spleen stiffness assessed by ARFI are positively correlated with HVPG and therefore ARFI has certain application value in the noninvasive diagnosis of portal hypertension.
