1.The clinical application of whole chromosome painting probes in preimplantation genetic diagnosis for translocation carriers.

OBJECTIVETo make preimplantation genetic diagnosis (PGD) for female translocation carriers by analyzing first polar bodies (1PBs) with whole chromosome painting probe (WCP).
METHODSWCP was used in fluorescence in situ hybridization (FISH) analysis of 1PBs for four female Robertsonian carriers presented for PGD with 45 XX, der(13;14)(q10;q10) karyotype. All the patients underwent ovarian stimulation and during 6 h after oocyte retrieval 1PBs were biopsied and WCP were used in FISH. On day 3 after fertilization embryos diagnosed as normal or balanced were transferred.
RESULTSA total of 61 oocytes were collected in 4 PGD cycles. Of the 54 matured oocytes, 50 were biopsied and 45 were fixed successfully. Results were obtained in 40 1PBs. Overall, 74.1% (40/54) oocytes were diagnosed. The fertilization rate and good embryo rate were 64.8% (35/54) and 65.7% (23/35) respectively. Two clinical pregnancies were obtained. One patient delivered a normal female baby with karyotype 46, XX in June 2006. For another patient, the fetus spontaneously aborted at 9th week of pregnancy with karyotype of 45, X confirmed by amniotic villus diagnosis.
CONCLUSIONWCP can differentiate normal, balanced and unbalanced oocytes accurately and can be used as an efficient PGD method for female carriers of translocation.

2.Association of calcitonin receptor gene polymorphism with bone mineral density in Shanghai women.

OBJECTIVETo investigate the association of calcitonin receptor (CTR) gene polymorphism with bone mineral density (BMD) in premenopausal and postmenopausal women.
METHODSCTR genotypes were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) in 184 premenopausal women and 199 postmenopausal women in Shanghai area. BMD at lumbar spine (L2-4) and femoral neck (FN) were measured by dual-energy X-ray absorptiometry (DEXA).
RESULTSThe distribution of CTR genotypes in 383 Shanghai women were CC genotype 83.8%, TC genotype 14.6%, TT genotype 1.6%, respectively. BMD at FN of CC genotype was significantly higher than TC and TT genotypes (P < 0.01) in postmenopausal women. But there was no difference in BMD of different CTR genotypes in premenopausal women. Multiple regression analysis showed that CTR genotypes were associated with FN BMD in postmenopausal women (P < 0.05).
CONCLUSIONSThe polymorphism of CTR gene was associated with BMD in postmenopausal women.

3.Relationship between body composition and bone mineral density in healthy premenopausal women.

OBJECTIVETo investigate the relative contribution of fat mass and lean mass on bone mineral density (BMD) in premenopausal healthy women.
METHODSThe BMD at lumbar spine, proximal femur and total body, together with fat mass and lean mass was determined by dual-energy X-ray absorptiometry (DEXA), the body height, weight, waist, and hip circumference were also measured, and body mass index (BMI) and waist-hip ratio were calculated in 282 premenopausal women.
RESULTSFat mass was a major determinant for BMI, BMI and lean mass were positively related with L2-4, proximal femur and total body BMD (P = 0.000 for all), and lean mass were the only independent factor contributing to L2-4 (standardized coefficient beta = 0.282, P = 0.000), proximal femur (beta = 0.336, P = 0.000) and total body BMD (beta = 0.361, P = 0.000) in stepwise regression analysis. The relationship between BMI and BMD was further improved after controlling fat mass, while decreased or even lost when controlling lean mass.
CONCLUSIONSLean mass was an important factor determining BMD in premenopausal women.

Objective:To discuss the clinical efficacy of Xuesusheng granule for cancer related fatigue (CRF) deficiency of both Qi and blood in clinical practice, and to investigate its effect on immune function and quality of life. Method:According to random number table method, 118 patients were divided into observation group (59 cases) and control group (59 cases). Patients in control group got Ejiao Huangqi oral liquid, 10-20 mL/time, 2 times/day. Patients in observation group got Xuesusheng granule, 10 g/time, 3 times/day. The treatment was continued for 8 weeks in both groups. Before and after treatment, scores of cancer fatigue scale (CFS), functional assessment of cancer therapy (FACT) and Qi and blood deficiency syndrome were graded, and degree of fatigue was discussed with brief fatigue inventory scale-Chinese (BFI-C). Kangfu Xiaoyan Suppository is widely used in the treatment of gynecological inflammatory diseases. Long-term clinical application and a certain amount of research evidences show that Kangfu Xiaoyan Suppository can alleviate the clinical symptoms of pelvic inflammatory diseases,reduce the recurrence rate,and relieve sequelae,with a better safety and economic characteristics. As a type of nationally protected traditional Chinese medicine and type B medicine included in medical insurance,it has been selected as a Chinese patent medicine for rectal administration. It was included in the Guidelines for diagnosis and treatment of common gynecological diseases of traditional Chinese medicine published by the Chinese Academy of Traditional Chinese Medicine in 2012,the Pelvic inflammatory diseases diagnosis and treatment guidelines issued by the Infectious Diseases Collaborative Group of the Obstetrics and Gynecology Branch of the Chinese Medical Association in 2014,and the group standard of Single use of traditional Chinese medicine/combined antibiot guidelines for clinical practice-pelvic inflammatory diseases of the Chinese Academy of Traditional Chinese Medicine in 2017. To further enhance clinicians' understanding of the drug and better guide its rational clinical use,experts from the field of gynecology of traditional Chinese and Western medicine were invited to develop and compile this expert consensus. This consensus takes full account of clinical evidences and expert clinical experience,and form recommendations for clinical problems based on evidences and consensus recommendations for clinical problems without evidence by nominal grouping method. The expert consensus is mainly formed in the consideration of six factors: quality of evidence,economy,efficacy,adverse reactions,patient acceptability and others. Based on clinical research evidences and expert experience,this consensus provides a preliminary reference for the clinical use of the drug in a concise and clear format. However,evidence-based support is still required in a large number of high-quality studies,and this consensus will be revised in the future according to new clinical problems and the update of evidence-based evidence in practical application.
