2.Preliminary clinical report of bilateral decompression under microscope via unilateral approach for the treatment of lumbar stenosis

Objective To report the surgical technique and preliminary clinical results of bilateral decompression under microscope via unilateral approach for the treatment of lumbar stenosis. Methods Sixteen eases of lumbar stenosis were treated in our institute. For the surgical treatment, only one side paraspinous muscle was dissected. The upper and lower margins of the adjacent laminae were drilled, and the underlying ligament flavum was exposed. By changing the direction of the microscopy, the base of the spinous process and the internal side of the contralateral laminae were also drilled off till the lateral recess. Finally, decompression of the spinal canal was achieved by removing the ligament llavum. Results Follow-up ranged from 6 to 47 months. Intermittent claudication was totally relieved in 14 out of 16 cases, and markedly relieved in 2 cases. Back pain was relieved totally in 6 cases, marked improved in 8 cases, not change in 2 cases. Pain was significantly relieved in all 5 cases with sciatica, among them, diseetomy had to be performed in 3 cases. During follow-up, there was no symptom recurrence, and dynamic X ray did not show lumbar in-stability. Conclusion Bilateral decompression under microscope via unilateral approach for the treatment of lumbar stenosis is minimally invasive surgery, the effect of the operation is good. The technique has little in-fluence on spinal stability.
3.The correlation of serum creatine kinaseisoenzyme-MB and the prognosis of patients with acute coronary syndrome

Objective To investigate the correlation of serum ereatine kinase iseenzyme-MB (CK-MB) and recent cardiac events(30 days, 180 days) and late cardiac events (360 days) in patients with acute coronary syndrome (ACS). Methods Serum creatine kinase (CK) and CK-MB in 168 patients with ACS were measured, and these patients were divided into four groups according to the levels of CK and CK-MB. The occurrence of cardiac events analysis of CK and CK-MB was followed up for 30, 180 and 360 days and the relationship between the occurrence of cardiac events and the levels of CK and CK-MB were studied. Results There was statistical correlation between increased CK-MB and abnormal ECG Q wave, ST segment change (P< 0.05), 168 patients had followed up 30, 180 and 360 days, in which cardiac events happened in 57 common patients (64 examples), 27 examples (16.1%) happened in the recent cardiac event, and 37 examples(22.0%) happened in the late cardiac event. The cardiac event and CK-MB elevation had statistical correlation (P<0.05). Conclusion ACS patients with increased CK-MB levels prompt a poor prognosis, and could be an independent indicator of recent and late cardiac events in patients with ACS.
5.An experiment on tooth movement in new bone after distraction osteogenesis

Objective: To compare the similarities and differences of teeth movement at different time points after distraction osteogenesis, and to explore the best time point of teeth movement after distraction osteogenesis. Methods: The experiment was carried out on 8 beagle dogs. Group I: started teeth movement at the second week after distraction osteogenesis. Group Ⅱ: started teeth movement at the sixth week after distraction osteogenesis. Analyzed the root, periodontal ligament and alveolar bone by physical measurement, X-rays and HE staining methods. Results: In the second and sixth week after distraction osteogenesis, the distance between the third and fourth premolar teeth reduced significantly(P<0.05), there was a remarkable difference between group I and group Ⅱ(P<0.05). Conclusion: Under the same force, the speed of tooth movement in the second week is faster than that in the sixth week, but periodontal absorption in the second week is more serious than that in the sixth week.
6.Construction and immunoscreening of cDNA library of Armillifer agkistrodontis nymphs

Objective To construct cDNA entry library and cDNA expression library of Armillifer agkistrodontis (A.) nymphs and make a preliminary immunoscreening for the cDNA expression library.Methods The nymphs were collected from the Kunming mice infected experimentally with A.agkistrodontis eggs and the total RNA were extracted from the nymphs using TRIzol Reagent.After purifying the mRNA,the synthesized cDNAs were cloned into the donor vector pDONR222 by BP reaction of Gateway technology and the recombinants were transformed into the DH10B cells by electroporation,the cDNA entry library was obtained.Next,the expression vector pDEST17 was ligated with entry clones by LR reaction,and the recombinants were transformed into the BL21 (DE3) cells.Hence,the cDNA expression library was constructed.Then,the expression library was immunoscreened with the mixed sera of mice infected with A.agkistrodontis,and the insertions of positive clones were sequenced.After that,the open reading frame(ORF) of positive slone sequence,the homology of the screened genes and their encoded proteins were analyzed by Finder and BLAST (basic local alignment search tool) program of National Center of Biotechnology Information(NCBI),and the discovered new genes were submitted into the GenBank.Besides,the physico-chemical properties,secondary structure and B cell epitopes of encoded proteins were also analyzed by bioinformatics software.Results The average titer and total clones of the cDNA entry library were 1.45 × 105 CFU/ml(colony-forming unit,CFU) and 1.74 × 106 CFU,respectively,and the range of fragment length of the inserted cDNA was between 0.2-4.0 kb,with an average of 1.4 kb.The total clones of cDNA expression library were 1.00 × 105 CFU,and the fragment length of the inserted cDNA was between 0.3-2.2 kb,with an average of 1.0 kb.Five positive clones,coded S1,S5,A1,D1 and F1,respectively,were obtained through preliminary immunoscreening.The sequence and homology of the five positive clones were sequenced and analyzed by BLAST program.No significant similarities were found in pentastomida species,which meant that they were all novel genes of A.agkistrodontis.The gene sequences were submitted to GenBank,with the accession number from JQ180451 to JQ180455.Also,results obtained by bioinformatics software showed that the predictive encoding proteins were all potential to be valuable recombinant diagnostic antigens.Conclusions The cDNA library of A.agkistrodontis nymphs is successfully constructed,and five new genes of A.agkistrodontis are discovered.The establishment of cDNA library and the discovery of the new genes will lay a foundation for further studying the gene functions and screening the immunodiagnostic antigens.

Objective To investigate the effect of expression of pituitary tumor transforming gene(PTTG) on the expression of inhibitory of vascular endothelial growth factor(VEGF).Methods The constructed(recomibinant) vector,pcDNA3.1-PTTGas that contained full-length antisense PTTG,was transfected into the cholangiocarcinoma cell line QBC939 in different quantity.After 48h incubation,the variation of expressions of PTTG and VEGF mRNAs and proteins were observed by RT-PCP and Western-blot method.Results(After) successful transfecting the recomibinant vector into QBC939,the mRNAs and proteins of PTTG and VEGF were all inhibited.Linear correlation analysis showed positive correlation between descendent extents of PTTG and VEGF(P
8.Medium and long-term follow-up outcome after arthroscopic repair of type II SLAP lesions with two Loops

Objective To determine the medium-long term functional outcome of arthroscopic repair of type II SLAP lesions with two loops.Methods According to retrospective study,29 cases patients with arthroscopic repairs of isolated type II SLAP lesions were collected from March 2011 to June 2013,the mean patient age at the time of surgery was 31.8 years(range17 to 47 years).All patients with ground anchor suture,and through double loop knot technology to fix SLAP injury.Visual analogue scale(VAS),Kerlan-Jobe Orthopaedic Clinic(KJOC),Roche Shouder function Score(ROWE) were used to determine the function of shoulder joint.The shoulderjoint range of motion before treatment and post treatment were recorded,shoulder joint mobility was recorded,satisfaction survey was done,compared the preoperative and postoperative data to evaluate its clinical effect.Results All patients were available for follow-up of 2.7-5.2 years.Overall,functional outcome was improved from baseline compared with final follow-up for VAS scores((4.3±1.2) points vs.(1.5±0.8) points,t=5.435,P<0.05),KJOC scores((72.1±4.9) points vs.(92.8±5.4) points,t=5.921,P<0.01),and ROWE scores((54.1±12.4) points vs.(84.3±10.2) points,t=7.326,P<0.01).Only a minority of patients feel pain when they did srenuous exercise.According to satisfaction investigation of patients,patient satisfaction was reported in 89.7%,3 cases had not good results,they could not return to previous level of play,including 2 cases were athletes,1 case was not athlete.Conclusion There is no more persistent pain and dysfunction after surgery,a good or excellent medium-long term functional outcome can be anticipated after arthroscopic repair of type II SLAP lesions with two loops.

10.The study of sustain attention of CNV in patients with subcortical ischemic vascular cognitive impairment

Objective To investigate the characteristics of contingent negative variation(CNV) in patients with subcortical ischemic vascular cognitive impairment (SIVCI) and their relationship with continuous performance task (CPT) to establish a neuroelectrophysiological index of attention impairment in earlier period of Vascular Cognitive Impair-ment. Methods CNV was assessed in 45 SIVCI patients [30 patients with subcortical ischemic vascular cognitive impair-ment no dementia (SIVCIND ) and 15 patients with subcortical ischemic vascular dementia (SIVD )] and 15 normal con-trol (NC) by using a EB-Neuro ERP Instrument. Results ①CNV in SIVCI group were less regular compared with nor-mal control;CNV in SIVCIND group had decreased Expectancy Wave (EW) amplitudes (9.98± 4.10μV vs. 16.13±2.75μV) and EW areas (14848.10 ± 3199.16 μV · ms vs 20058.87 ± 1025.95 μV · ms) compared with normal control (P<0.05);EW amplitudes (6.25±1.52μV vs. 9.98± 4.10μV) and EW areas (8474.00±3511.94 vs. 14848.10±3199.16μV · ms) of CNV in SIVD group were significantly lower compared with SIVCIND group (P<0.05);However, there was no sig-nificant difference in CNV EW latency among three groups(F(2,57)=1.90,P>0.05);②Pearson correlation\Spearman rank correlation analysis revealed that there were significant, positive correlations between EW latency and CPT Reacting time(R=0.748, P<0.01);there were no significant relationships between CPT missing rate and EW latency(R=0.22,P>0.05)、EW amplitude(R=-0.191,P>0.05)and there were significant, negative correlations between EW area and CPT Reacting time(R=-0.718,P<0.01)、CPT missing rate(R=-0.829 ,P<0.01), EW amplitude and CPT Reacting time(R=-0.616, P<0.01). Conclusion SIVCI patients in early stage have attention deficits in sustained attention and CNV EW ar-ea may be a good neuroelectrophysiological index for sustained attention impairment .
