2.Application of SIEMENS Ysio DR amplification in radiation dose reduction

Objective To investigate the application of SIEMENS Ysio DR amplification in the radiation dose reduction. Methods Totally 160 patients with similar heights and body shapes were divided equally into A,B,C and D groups,and then underwent radiological examinations with SIEMENS Ysio DR from September 2014 to May 2016.A and B groups went through right hand AP oblique examination with the same kV while different mAs, and C and D groups had chest posteroanterior examination with different kV and the same mAs. Image quality was regulated with the amplification coefficient, and then evaluated with double-blind method by two senior radiologists.Statistical analysis was executed over the values of mAs in A and B groups and those of kV in C and D groups. Results A group had the mAs value being(2.26 ±0.16)mAs, which was significantly different from that of B group [(4.05±0.19)mAs] (P<0.05);while C group had kV value being (104.79±6.39)kV, which was statistically different from that of D group [(83.48±3.67)kV] (P<0.05). Satisfactory image were obtained by regulation with amplication coefficient.There was no obvious difference between the image quality in either A and B groups or C and D groups(P>0.05).Based on the principle that low mAs resulted in low radiation dose and high kV lead to low effective dose,A group had the radiation dose lower than those of B group,and C group had the effective dose lower than those of D group. Conclusion SIEMENS Ysio DR has the photographing parameters regulated according to different sites and the amplification coefficient for enhancing image quality, which decreases X-ray radiation dose and effective dosage with the diagnosis results unaffected.
3.Clinical study on left ventricular remodeling patterns and its relationships with cardiovascular risk factors in patients with obstructive sleep apnea syndrome

Objective To investigate the left ventricular remodeling (LVR) patterns and its relationships with cardiovascular risk factors in patients with obstructive sleep apnea syndrome (OSAS).Methods 148 patients with habitual snoring were diagnosed OSAS by polysomnography with AHI≥5 /h and blood pressure was monitored at same time.The clinical characteristics were collected and echocardiography was done next morning.LVR patterns was categorized as normal geometry (NG),concentric remodeling (CR),eccentric hypertrophy (EH) and concentric hypertrophy (CH) on the basis of RWT >0.42 and LVMI >46.7 g/m2.7 (female) or 49.2 g/m2.7 (male).Multinomial logistic regression was used to analysis the correlation of cardiovascular risk factors and LVR patterns.Results ①Of 148 OSAS patients,there were 115 (77.7%) with abnormal LVR,NG 33 (22.3%),CR 35 (23.6%),EH 37 (25%)and CH 43 (29.1 %).②General clinical characteristics:Compared with NG group,BMI and waistline were increased in EH group (P <0.05 or P <0.01) ; Age,BMI,waistline,hypertension,dSBP and nSBP were increased in CH group (P <0.05 or P <0.01).Compared with CR group,male,age,BMI and waistline were increased in EH group (P <0.05 or P <0.01).Male,age,BMI,waistline,hypertension,nSBP and TG were increased in CH group (P < 0.05 or P < 0.01); Compared with EH group,the prevalence of hypertension was increased in CH group (P <0.05); Compared with EH group,the prevalence of hypertension was increased in CH group (P<0.05).③Polysomnography parameters:Compared with NG group,AHI and ODI were increased in CR group (P<0.05 or P<0.01).AHI,ODI and T90 were increased,while Mean SaO2 and Lowest SaO2 were decreased in CH group (P <0.05 or P < 0.01);Compared with CR group,AHI was decreased in EH group (P <0.05).Mean SaO2 was decreased,but T90 was increased in CH group (P<0.05 or P <0.01); Compared with EH group,Lowest SaO2 was decreased in CH group (P<0.05).④Blood pressure:Compared with NG group,4:00 SBP and 6:00 SBP were increased in CH group (P<0.05) ;Compared with CR group,22:00 SBP was increased in CH group (P <0.05).⑤ In multinomial logistic regression analysis,after adjusting the confounding factors,CR was associated with AHI [odds ratio (OR) 1.035,P =0.024.EH was associated with Age (OR 1.094,P =0.016),BMI (OR 1.397,P =0.011) and 4:00 SBP (OR 1.124,P =0.026).CH was associated with Age (OR 1.084,P =0.028).Conclusions OSAS could cause result in LVR and the prevalence of four patterns were similar..Age and AHI were significant determinants of CH and CR respectively.Age,BMI and 4:00 SBP were significant determinants of EH.
4.Adult bilateral Crowe type-IV developmental dysplasia of the hip:is the prosthesis an optimal therapeutic strategy?

BACKGROUND:Crowe type-IV developmental dysplasia of the hip presented complete dislocation of the femoral head, significantly shortening lower limb, serious adverse development of upper segment of acetabulum and femur, and even deformity. At present, there are no reports concerning its therapeutic strategy.
OBJECTIVE:To explore the therapeutic strategy of adult bilateral Crowe type-IV developmental dysplasia of the hip.
METHODA total of 12 cases of adult bilateral Crowe type-IV developmental dysplasia of the hip, who were treated in the Department of Orthopedics, The First People’s Hospital of Changzhou from January 2000 to January 2013, were col ected. There were 5 males and 7 females, at the age of 19-47 years old, averagely 33.5 years old. After two or three weeks of skeletal traction, they received bilateral total hip arthroplasty. Hip rotation center was reconstituted at the level of the“true”acetabulum. The subtrochanteric osteotomy was performed for patients with reduction difficulty. For patients with femoral neck anteversion>40°, subtrochanteric rotary osteotomy was performed.
RESULTS AND CONCLUSION:Disappearance of hip pain, satisfied move function and normal gait were gained after operation. Lower limbs recovered to be isometric. The average leg lengthening was 3.1 cm (range, 2.5 to 4.8 cm). One patient affected sciatic nerve irritation. The average fol owed-up time was 3.5 years. No fracture, dislocation or loosening of the prosthesis appeared. The post-operative Harris Hip Score averaged 86.3. By reconstruction of the hip rotation center in the“true”acetabular level, correction of femoral neck anteversion, reconstruction of function of the abduction muscle and proper selection of prosthesis, total hip arthroplasty for adult bilateral Crowe type-IV developmental dysplasia of the hip could obtain good curative effects.
5.Effect of Shenfu injection in postoperative recovery after abdominal surgery.

OBJECTIVETo investigate the effect of Shenfu injection (SF) in postoperative recovery after abdominal surgery.
METHOD150 patients were randomly divided into control group (75) and SF group (75). The efficacy were evaluated with the time of passage of gas by anus, information of wound healing, blood routine, blood coagulaton function, liver function. Changes recovery factor, immune index and hemorheological index were detected too.
RESULTThe time of passage of gas by anus in SF group was significantly shorter than that in control group. Blood routine, blood coagulaton function, liver function showed no significant difference in the two group of preoperative and postoperative. Whole blood viscosity, blood reduced viscosity and plasma fibrinogen viscosity in SF group were significantly lower than that in control group after operation. IgG and IgA in SF group was significantly higher than that in control group after operation. IgG and CD4+/CD8+ in control group was significantly higher after operation than that in before operation. Among tissue damage plerosis correlation factor, the blood levels of superoxide dismutase (SOD) in SF group and control group is significantly lower than that in preoperation (P < 0.05), SOD in SF group was significantly higher than that in control group after operation. The blood levels of malondialdehyde (MDA) in SF group and control group is significantly higher than that in preoperation (P < 0.05). MDA in SF group was significantly lower than that in control group after operation. Whole blood viscosity, blood reduced viscosity and plasma fibrinogen viscosity in control group after operation were significantly higher than that before operation.
CONCLUSIONSF can stimulate the immune system and reduce the blood viscosity. It is contribute to the patients' rehabilitation.

Triphala consists of Terminalia chebula Retz, Terminalia bellirica (Gaertn.) Roxb. and Phyllanthus emblica L, which are made into Dasanguo Powder. Triphala is rich in a variety of chemical components, including tannins, phenolic acids, triterpenoids and flavonoids. The content of tannin is abundant in Triphala, which are often used as the main indicators of analysis. Modern research found that Triphala has a variety of pharmacological activities such as prevention of gastrointestinal diseases, antibacterial, anti-inflammatory, anti-oxidation, anti-tumor and so on. This paper briefly summarized the chemical constituents and pharmacological effects of Triphala, combining the relevant national and international literature in recent years to provide reference for the development and further study of Triphala.
7.Study on Water-Soluble Constituents from Tibetan Medicine Terminalia billirica (Gaert.) Roxb

This paper aimed at investigating the water-soluble constituents of Terminalia billirica (Gaert.) Roxb.Compounds were isolated and purified by Diaion HP-20, Sephadex LH-20, Toyopearl HW-40, MCI gel CHP 20 P and ODS column chromatographies. The chemical structures were elucidated according to spectral data and physicochemical properties. The results showed that ten compounds were isolated and identified as corilagin, salicoside, methyl-α-Darabinofuranoside, 1, 4-dimethyl-β-d-fructopyranose, β-fructopyranose, β-fructofuranose gallic acid, ellagic acid, 1, 2, 3, 4, 6-penta-O-galloyl-β-D-glucopyranonside, Chebulagic acid. Except compound 7 and 8, others compounds are separated from Terminalia billirica (Gaert.) Roxb for the first time, and except corilgin, the other compounds are separated from Terminalia for the first time.

The aim of this work was to identify and analyze the chemical constituents of Terminalia chebular Retz by establishing high performance liquid chromatography-linear ion trap/electrostatic field orbitrap combined high resolution mass spectrometry (HPLC-LTQ-Orbitrap MS) technique. With the application of C18 reverse phase column (4.6 mm×250 mm, 5 μm), gradient elution was performed with methanol and water (0.2% acetic acid) as mobile phase.In negative ionization mode, the data of LTQ-Orbitrap was collected. Accurate molecular mass of molecular ion peaks and fragment ions provided by the high-resolution mass spectrometry were compared with the literature and the reference substance to determine the possible structure of the compound. The results indicated that 62 compounds from the extract of Terminalia chebular Retz. were identified, including acids and hydrolysable tannins (60), triterpenoids (2) . There were14 compounds were detected from this specie for the first time. It is proved as an effective method to provide reference for Chemical composition mass spectrometry and the quality control in further phytochemical studies of Terminalia chebular Retz.
9.Study on Quality Evaluation of Terminalia Chebular Based on Simultaneous Determination of Total Tannin and 7 Components

The spectrophotometric method was established for the determination of total tannin content in gardenia medicinal materials and tannins, and HPLC method was applied to simultaneously determine gallic acid, punical glucoside A, methyl gallate, punny glucoside B, corilagin, pentagalloyl glucose and the content of ellagic acid. Taking gallic acid as a reference substance, a phosphomolybdate tungstic acid colorimetric method (Pharmacopoeia method) was used to determine the total tannin content. With the chromatographic column Agilent ZORBAX SB-Aq-C18 (250 × 4.6 mm, 5 microns), mobile phase for phosphoric acid (0.1%), methanol, water flow rate of 1 ml/min, column temperature 30℃, detection wavelength of 270 nm, gradient elution, 7 kinds of component contents in myrobalan medicinal materials were measured at the same time. The result showed that the average content of total tannins of Radix Scutellariae was 30.31%, RSD was 0.55%; the average recovery rate was 99.70%, and the RSD was 21.97% (n = 6) . In the scorpion medicinal materials, gallic acid, punical glucoside A, punny glucoside B, methyl gallate, punny glucoside B, creatin, pentagalloglucose and ellagic acid have a good linear relationship in their respective ranges. The recovery rate of the sample is between100.10% and 102.77%. The establishment of the determination method of total tannin, gallic acid, punical glucoside, methyl gallate, corridain, pentagalloglucose and ellagic acid in the scorpion medicinal materials is simple and accurate, which has strong specificity and can be used for quality control of Terminalia chebular.
10.Study on HPLC Fingerprint of Terminalia chebular Retz Based on Chemometrics Analysis

This paper was aimed to establish the HPLC fingerprint of Terminalia chebular Retz The analysis was performed on Agilent ZORBAX SB-Aq C18 column (4.6×250 mm, 5 μm) . The mobile phase was 0.1% phosphate watermethanol solution with gradient elution. The flow rate was 1 mL·min-1. The temperature was maintained at 35℃. The detection wavelength was 270 nm. The injection volume was 20 μL. The chromatographic data of 10 samples was analyzed by similarity of chromatographic data, SPSS software and SIMCA software. The results showed that there were17 common peaks in the diagram. The similarity of 10 samples was between 0.984 and 0.999, which showed that there was no significant difference among these chemical components. The samples were classified into four types according to cluster analysis, which shared the similar results as the principal components analysis (PCA) and partial least sq uares analysis (PLS-DA) . VIP analysis result showed that peak 5 (methyl gallate), 11 and 1 (gallic acid), 10, 12 may be the main chromatographic peaks to identify the samples. It was concluded that chemometrics analysis could be used to analyze, find and distinguish the marker ingredients of Terminalia chebular Retz. This method was accurate and stable, which can provide a reference for the quality control and evaluation of Terminalia chebular Retz.
