1.Investigation of vaginal myomectomy and recovery after operation

ObjectiveTo explore the effect of vaginal myomectomy and compared with abdominal myomectomy.Methods80 patients with myomectomy were randomly divided into two groups with 40 cases in each group. Patients of the group A were operated by vaginal myomectomy, that of the group B were operated by abdominal myomectomy. Effects of operation and recovery of two groups were compared.ResultsIn the group A, 38 cases (95%) successfully undertook vaginal myomectomy, whereas all 40cases completed the planned surgery in the group B. There were no significant differences on the mean operating time, blood lose during operation, mean maximal postoperative temperature and average postoperative hospital stay between two groups (P>0.05). There was no uterine hematomas developed postoperatively in all patients. The symptoms were mostly relieved after 6 moths'follow-up and patients of the group A showed a rapid recovery time and lower rate of complaints.ConclusionVaginal myomectomy can be performed in selected patients with low complications, good short-term effect, minimum operative injury compared with abdominal myomectomy.
2.Effect of tension-free vaginal tape procedure on stress urinary incontinence

Objective To compare the effect of tension-free vaginal tape (TVT) and vaginal reconstructive surgery (VR) on women with severe genital prolapse and stress urinary incontinence.Methods 43 patients with stage I or II vaginal anterior defect and stress urinary incontinence were assigned to TVT group(21 patients) and VR group (22 patients) . Results Maximum quantity of urine, pressure of detrusor at maximum quantity of urine, and abdominal leakage point pressure occured significantly in TVT group after operation(P<0.05). Postoperative satisfaction rate were higher in TVT group(P<0.05). Conclusion TVT is more effective to improve the quality of life in women with stress incontinence.
3.Effect of raloxifen on postmenopausal osteoporosis

ObjectiveTo observe the effect of raloxifen, a selective eastrogen receptor modulator, on postmenopausal osteoporosis.MethodsSixty six patients with postmenopausal osteoporosis were randomly divided into test group and control group with 33 cases in each group. Patients in the test group were treated with raloxifen (60 mg/day) for six months, at same time; cases in the control group were treated with placebo. Before and after treatment,routine laboratory examinations, transvaginal B type ultrasonic examination, and diagnostic curettage were performed. ResultsThe bone mineral densities of lumbar vertebrae and hip were significantly higher in the test group(60.6%) than in the control group(21.2%) (P<0.01). ConclusionRaloxifen is an effective medicine to treat postmenopausal osteoporosis, and it is also safe, without side effect of stimulating endometrim, etc.

5.Investigation of the prevention and cure of human brucellosis in Zhengzhou from 2002 to 2007

Objective To understand the epidemicological condition of human brucellosis in Zhengzhou city to provide scientific evidence for future preventive and control measures. Methods Surveillance on high risk populations were done in the designated monitoring Gongyi City and other randomized monitoring cities or counties (4 or 5 towns or farms were selected in each city or county) during the year from 2002 to 2007. The intradermic allergy test with brucellin were carried out among high risk populations who had been in close touch of domestic animals or their products in those cities, and standard tube agglutination test(SAT) were used for those who had suspected clinic symptom or positive result of intradermie allergy test during 2002 to 2005. The SAT investigations for enterprises of livestock breeding were performed in non-designated monitoring cities. All manipulation methods and judging standards were according to the manual of prevention and treatment for human brucellosis. Results In the designated monitoring city Gongyi from 2002 to 2007, a total of 13 831 people were investigated, among whom 3744 peoples were tested by intradermic allergy test and the positive rate was 1.01%(38/3744). Fifty-four cases were positive in SAT among 2693, and the positive rate of SAT was 2.01%(54/2693). Forty-four new cases were diagnosed. In the 11 non-designated monitoring city from 2002 to 2005, 13 136 persons were treated by intradermic allergy test, the positive rate was 0.62% (82/13 136), among these 1316 persons were treated by SAT and 66 patients were positive. The positive rate was 5.02% (66/1316) from 2002 to 2007. Fifty one new cases were diagnosed. In the enterprises of domestic animals breeding, among 242 and 688 employees were treated by SAT, 17 and 16 patients were positive, the positive rate were 7.02%(17/242) and 5.02%(16/688) in 2006 and 2007, respectively. Conclusions The infection of human brucellosis in Zhengzhou is rising up. The governments at all levels must put large amounts of funds for the monitoring, investigating and disinfecting to prevent the disease from increasing and outspreading.
6.Vacuum sealing drainage combined with discontinuous windowing technique for repairing large area exposed wound of Achilles tendon.

OBJECTIVETo explore clinical effect of vacuum sealing drainage (VSD) combined with discontinuous windowing technique for repairing large area exposed wounds of Achilles tendon.
METHODSFrom July 2009 to May 2014, 11 patients with large exposed wounds of Achilles tendon were treated, including 5 males and 6 females with an average age of 43 years old (aged from 7 to 65 years old). Among them, 4 cases were skin necrosis caused by heavy objects abrasion and contusion; 3 cases were caused by distal tibiofibula fractures; 3 cases were caused by bicycle-spoke injuries; 1 case was caused by diabetes. Areas of exposed Achilles tendon were from 6 cmx3 cm to 14 cmx5 cm without tendon rupture or bone exposed. After debridement, discontinuous fenestration on Achilles tendon was made by knife blade parallel with longitudinal axis of Achilles tendon, combined with Vacuum Sealing Drainage (VSD) treatment.
RESULTSAfter drainage treatment with one VSD cycle (5 to 7 days), abundant fresh granulation tissues were growing on all wounds and survived well after the second phase dermatoplasty. All patients were followed up for 12 to 24 months, the color of skin flap was good, the texture was soft without burst. At 3 to 4 months after operation, subcutaneous fat was appeared under the flap, the skin was sliding, movement of ankle joints was good. No delayed Achilles tendon rupture were occurred.
CONCLUSIONVacuum sealing drainage (VSD) combined with discontinuous fenestration is a simple, safe and effective method for repairing large area exposed wounds of Achilles tendon,which could minimize the secondary damage caused by wounds of skin flap grafting.

7.TME or TSD with pelvic autonomic nerve preserving operation for 247 patients with advanced rectal cancer

Objective To investigate effects of rectal cancer to undergo total mesorectal excision (TME)or three space dissection(TSD)with pelvic autonomic nerve preservation(PANP).Methods TME or/ and TSD was applied in 247 Patients with advanced rectal cancer in which 185 cases (74.9 %) underwent PANP(Group P)including TME-PANP(Group Pro)139 cases and TSD-PANP(Group Ps)46 cases.The other 62 cases underwent none-PANP(Group P-)due to tumor invasion.Results There were no death cases for operation inall patients.Group Pm was better than Group Ps in the operation time and the difficulty of proce- dure(P0.05).Conclusion The procedure with TME to preserve pelvic autonomic nerves adapts to the majority of rectal cancer patients.TSD procedure is more complex than TME.Statistically,the survival differ- ence between Patients with TSD and with TME is no defective.The survival time is determined to the tumor's earlier diagnosis and therapy.
8.Observation on therapeutic effect of treatment for children with cerebral palsy by acupuncture.

OBJECTIVETo evaluate the clinical therapeutic effect of acupuncture treatment of children cerebral palsy with gaining in wisdom and activating channels method.
METHODSSixty cases were randomly divided into a gaining in wisdom and activating channels group, a JIN's 3-needling group and a routine acupuncture group, 20 cases in each group. The gaining in wisdom and activating channels group was treated with point group of the head selected as main points combined with Dazhui (GV 14), Jinsuo (GV 8), Mingmen (GV 4), Yaoyangguan (GV 3), Hegu (LI 4), Neiguan (PC 6), Sanyinjiao (SP 6), etc., the JIN's 3-needling group was treated with Four-shen needling and Dazhui (GV 14), Shenzhu (GV 12), Fengfu (GV 16), etc. were selected as main acupoints in the routine acupuncture group. Comprehensive Function Evaluation for Disabled Children was used for assessment of comprehensive function of disabled children.
RESULTSThe total effective rate of 90.0% in both the gaining in wisdom and activating channels group and the JIN's 3-needling group, with no significant difference between the two groups (P > 0.05), were better than that of 60.0% in the routine acupuncture group (P < 0.05).
CONCLUSIONAcupuncture treatment with gaining in wisdom and activating channels method can significantly improve the comprehensive function of disabled children with more obvious therapeutic effects.

9.Epitope Tagging of the rpoS gene of Y. pestis by Recombineering Technique

Objective: To facilitate the functional analysis of chromosomal genes and their products, the recombineering technique to epitope tagging of chromosomal genes of Y. pestis was adapted. Methods: The epitope tag was generated by primer annealing and then fused with resistance gene by fusion PCR. The epitope-resistance cassette was inserted into pBluecript, resulted in the template plasmid, pBS-MH. The tagging cassette for rpoS was obtained by PCR amplification from pBS-MH with primers containing homology specific to the target gene. PCR products were transformed into recombination competent cells and recombinants were selected. PCR and DNA sequencing were used to confirm the correct tagging event. The expression of the tagged protein was detected with Western blot by using monoclonal antibody to the epitope. Results: The template plasmid containing fusion of epitope and resistance gene was successfully constructed. The sigma factor gene, rpoS, was tagged with a myc-his tag at the COOH terminus. Expression of the tagged rpoS was successfully detected indirectly by the antibody against His tag. Conclusion: The chromosomal gene tagging by recombineering technique represents a powerful tool in the functional study of bacterial genes and their products.
10.Diagnostic Value of 64-Slice Dual-Source CT Coronary Angiography in Patients with Atrial Fibrillation: Comparison with Invasive Coronary Angiography.

OBJECTIVE: We wanted to evaluate the image quality and diagnostic value of 64-slice dual-source computed tomography (DSCT) coronary angiography in patients with atrial fibrillation (Afib). MATERIALS AND METHODS: The coronary arteries of 22 Afib patients seen on DSCT were classified into 15 segments and the imaging quality (excellent, good, moderate and poor) and significant stenoses (> or = 50%) were evaluated by two radiologists who were blinded to the conventional coronary angiography (CAG) results. The sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) for detecting important coronary artery stenosis were calculated. McNemar test was used to determine any significant difference between DSCT and CAG, and Cohen's Kappa statistics were calculated for the intermodality and interobserver agreement. RESULTS: The mean heart rate was 89 +/- 8.3 bpm (range: 80-118 bpm). A range from 250 msec to 300 msec within the RR interval was the optimal reconstruction interval for the patients with Afib. The respective overall sensitivity, specificity, PPV and NPV values were 74%, 97%, 81% and 96% for reader 1 and 72%, 98%, 85% and 96% for reader 2. No significant difference between DSCT and CAG was found for detecting a significant stenosis (reader 1, p = 1.0; reader 2, p = 0.727). Cohen's Kappa statistics demonstrated good intermodality and interobserver agreement. CONCLUSION: 64-slice DSCT coronary angiography provides good image quality in patients with atrial fibrillation without the need for controlling the heart rate. DSCT can be used for ruling out significant stenosis in patients with atrial fibrillation with its high NPV for detecting in important stenosis.
