Hypoxia-inducible factor-1α (HIF-1α)is a transcription factor that acts as a master regulator of gene expression induced by low oxygen conditions.HIF-1α is the most sensitive indicator of hypoxia and can be activated by systemic hypoxia,cerebral hemisphere ischemia and cerebral focal ischemia.HIF-1 expression can be induced by hypoxic-ischemic damage after cerebral infarction and exert its effects on hypoxia-inducible downstream genes,thus protecting hypoxic tissues and cells under hypoxic conditions via regulating the expression of its downstream target genes.This article reviews the expression of HIF-1α and its downstream target genes in acute cerebral ischemia,in order to provide a theoretical basis for gene therapy.

Objective To utilize the expert resource in central hospital for helping doctors in community hospital to make suitable diagnosis and treatment,improve the technical level of community medical care. Methods The Telemedicine Diagnosis System is developed based on B/S mode and Microsoft Visual Studio.net C#2005 in Windows platform. Results The Telemedicine Diagnosis System has been developed, and realized the function of interface display, information management, medical image transmission and telemedicine diagnosis. Conclusion The system has friendly interface, simple operation, more reliability and quick data transmission, and it can accurately realize the experts' telemedicine diagnosis in time.

Objective To explore the psychological availability of the full-time candidates for master degree of nursing during the clinical work. Methods A phenomenological approach was adopted in this study. About 11 full-time candidates for master's degree of nursing took part in the semi-structured and in-depth interviews and the acquired data were analysed. Results About 4 themes promoting psychological availability were identified: strong research knowledge and skills, career development confidence, perception of attention from leaders and more satisfactory salary. The four themes hindering psychological availability were:clinical work tasks, lower level of expertise, lack of social support and lower perception of self benefit. Conclusions The full-time candidates for master's degree hold active perception and much expectation about their clinical work but meanwhile they feel passive and disappointed. Nursing administrators should develop distinguishing and definite clinical personnel training and continuing education programs for them, offer them an access to learning chances and emotional support in order to improve the psychological availability and stabilize the nursing team.

Objective To investigate the expression changes of the protein tyrosine phosphatase receptor T (PTPRT) in temporal lobe epileptic foci in the experimental animals and epileptic patients and the relationship between PTPRT and epileptogenesis.Methods After getting the epilepsy lobe tissue from the experimental and control groups,immunohistochemistry,immunofluorescence and Western blot analysis were used to assess the expression of PTPRT and its changes.Results In the temporal lobe tissue of intractable patients and control group,PTPRT was mainly expressed in the neurons.PTPRT was significantly increased in patients with intractable epilepsy (0.277 ± 0.048) than that in the control group(0.171 ±0.025 ; t =9.586,P ＜ 0.05).PTPRT was mainly expressed in the neurons in the temporal lobe brain tissue of the rats in the control group and experimental group.Compared with control group,the expression of PTPRT in the temporal lobe tissue was increased within 24 h post-seizure,and decreased 1 and 2 weeks post-seizure,then it was increased 1 and 2 months post-seizure (A ratio:control 0.443 ± 0.039,6 h 0.840±0.032,24 h 1.113 ±0.064,7 d 0.564 ±0.039,14 d 0.570 ±0.029,30 d 0.899 ±0.034,60 d 1.011 ± 0.074,F =256.427,P ＜ 0.05).Conclusions Through researches into the expression and function of PTPRT in the temporal lobe brain tissue of patients with intractable epilepsy and animal models,we presume that the PTPRT plays a role in the synapses reorganization and mossy fiber sprouting,and participates in the reconstruction of the neural network which leads to the intractable epilepsy.

Objective To explore the clinical application of adaptive support ventilation (ASV) in elderly patients with acute respiratory failure.Methods A total of 46 mechanically ventilated patients aged over 65 years with acute respiratory failure admitted from January 2013 to June 2015 were enrolled.Comparison between the ASV mode and synchronized intermittent mandatory ventilation (SIMV) mode was carried out in respects of the impacts of both modes on respiratory mechanics, hemodynamics, oxygen availability and comfort rate.Results Difference between ASV and SIMV in respiratory rate was [(20.84 ±4.04) vs.(24.50 ±4.60) cycles/min, t =4.04, P ＜0.05], in inspiratory resistance was [(13.24 ±4.76) vs.(16.54±5.25) cmH2O/ (L·s), t=3.16, P＜0.05], in mean airway pressure was [(13.58±2.58) vs.(16.63 ±1.57) mmHg, t =6.84, P＜0.05], in peak airway pressure was [(25.96 ± 3.69) vs.(27.87 ± 2.45) mmHg, t =2.92, P ＜ 0.05], and tidal volume was [(378.41 ± 85.61) vs.(341.52 ± 86.84) mL, t =2.05, P ＜ 0.05], and comfort rate of patients was increased in ASV mode.There were no statistically significant differences in arterial oxygen partial pressure, carbon dioxide partial pressure, lactate, heart rate, mean arterial pressure and central venous pressure between the two modes (PP ＞ 0.05).Conclusions Compared with the synchronized intermittent mandatory ventilation mode, the adaptive support ventilation mode can improve the respiratory mechanics and can increase the comfort rate in the elderly patients with mechanical ventilation.
