3.Clinical pathologic analysis of urologic primary primitive neuroectodermal tumor

Objective To explore the clinico-pathological features, immunophenotype, treatment and prognosis of urologic primary primitive neuroectodermal tumor (PNET). Methods The clinical data of 3 patients with urologic PNET were analyzed retrospectively. All patients were male, aged 29, 32 and 75 years respectively. Two of the lesions were located in the kidney, and the third was located in the bladder. The sizes of renal tumors were 7.7 cm×6.2 cm and 12.6 cm×9.4 cm respectively. Imaging examinations revealed a well-defined mass with inhomogeneous echo inside. The size of bladder tumor was 10.0 cm×10.0 cm. CT scan demonstrated irregular thickening of the bladder wall, and the density of the wall was inhomogeneous. In the 2 cases of renal PNET radical surgery was performed, while an emergency palliative surgery to remove a blood clot and biopsy were performed in the bladder PNET case. Results In light microscope, the tumors were characterized by uniform small round or oval cells and nest-like or dense sheet structures surrounded by sparse fibrovascular stroma. Homer-Wright rosettes or pseudorosettes were observed, as well as mitoses. Immunohistochemical study revealed that all cases showed positive staining for CD99, synaptophysin and vimentin. One of the renal tumor cells showed positive for CD56, and the other renal tumor and urocystic tumor cells were focally positive for chromogranin A. Additionally, in 1 of the cases of renal tumor there was a high positive rate of 80% for Ki67 staining while the other case showed less than 5%. All 3 cases were eventually diagnosed as PNET. The first renal tumor case was not treated with radiotherapy and chemotherapy postoperatively, and the patient died of recurrence 14 months after surgery. Both the second renal tumor case and the bladder tumor case underwent chemotherapy postoperatively, and they died 4 and 6 months after surgery respectively. Conclusions The urologic primary PNET is a very rare, highly malignant soft tissue tumor, and the diagnosis must be based on pathologic findings and immunohistochemical phenotypes. The multimodal treatment for urologic primary PNET consists of surgery, chemotherapy and radiotherapy.
5.A Sampling Survey on Prevalence and Risk Factors of Overweight and Obesity among Inhabitants in Shenchi County, Shanxi

Objective To comprehend the status and risk factors of overweight and obesity among urban and rural inhabitants of Shenchi county.Methods With stratified cluster sampling,two residents" committees were selected as investigation spots from each town of Longquan,Yijing,Bajiao and Hezhi of Shenchi county.Permanent inhabitants aged 20 to 74 yrs were investigated on their status of overweight and obesity and its realted risk factors.Results In this study,2231 inhabitants were investigated,among which male 871 and female 1 360.The prevalence rate of overweight and obesity were 28.6%(standardized rate was 26.7%) and 5.1% (standardized rate was 4,5%).There was a statistic significance in prevalence rate of overweight between male and female(X~2=4.595, P0.05).It was discovered that the age,the degree of education, the annual income of family,drinking,exercise status,alimentary control status,family history and so on had the relation with BMI, the exercise and diet control status assumed negative association by multiple linear regression.Conclusion There is a high prevalence rate of overweight and obesity among residents in Shenchi county in Shanxi province.We should make the comprehensive intervention measures according to its status and risk factors,and make the prevalence rate decreased.

9.Expression of Recombinant Nematode Anticoagulant Peptide in Pichia Pastoris

Objective: To acquire recombinant nematode anticoagulant peptide ( NAP) with high anticoagulant activity. Methods: Pichia pastoris GS115 strain was transformed with recombinant yeast expression vector pPICS. 5K-rNAP. Expression of rNAP was induced with methanol after the identification of positive strains. NAP expressed in the collected yeast culture supernatant was confirmed with SDS-PAGE and Western blot. The biological activity of the products was validated with PT (prothrombin time) , INR (international normalized ratio) and APTT (activated partial thromboplastin time) , respectively. Results: The yeast strains expressing NAP were identified. The rNAP was secreted into culture supernatant with a molecular weight of about 10 kDa due to glycosylation, which is a little bigger than that predicted (8.7kDa). The anticoagulant efficiency of rNAP was confirmed with the in vitro assays. Conclusion: The recombinant nematode anticoagulant peptide with high biological activity was successfully expressed in Pichia pastoris and can be used in the future development of novel anticoagulant agent.
10.Effect of combined therapy with bailing capsule and benazepril on urinary albumin excretion rate and C-reactive protein in patients with early diabetic nephropathy.

OBJECTIVETo observe the effect of combined therapy with Bailing Capsule (BC) and benazepril on the levels of urinary albumin excretion rate (UAER) and C-reactive protein (CRP) for exploring its protective effect on early diabetic nephropathy.
METHODSSixty patients with early diabetic nephropathy were randomly assigned to the control group treated by benazepril alone, and the treated group treated by BC and benazepril, and the treatment lasted for 16 weeks. The changes of UAER and CRP levels were measured to estimate the protective effect of the combined therapy.
RESULTSLevels of 24h urinary protein, UAER, CRP were (0.85 +/- 0.32) g/24 h, (83.34 +/- 38.27) microg/min, (2.67 +/- 1.72) mg/L before treatment in the control group, and (0.43 +/- 0.17) g/24 h, (71.22 +/- 31.12) microg/min, (1.05 +/- 0.78) mg/L after treatment and they were (0.87 +/- 0.31) g/24 h, (81.59 +/- 35.69) microg/min, (2.55 +/- 1.66) mg/L before treatment in treated group, and (0.25 +/- 0.29) g/24 h, (57.32 +/- 31.11) microg/min, (0.49 +/- 0.38) mg/L after treatment respectively, all of them decreased after treatment in both groups, showing significant differences as compared with those before treatment (P<0.05, P< 0.01), and the reduction in the treated group was more significant (P<0.01); meanwhile, levels of serum creatinine, fasting blood glucose and HbA1c had somewhat decrease, showing no statistical difference with those before treatment (P >0.05).
CONCLUSIONCombined use of BC and benazepril could significantly lower the UAER and CRP levels in patients with early diabetic nephropathy to alleviate the renal impairment, showing an effect better than that of using benazepril alone.
