2.Clinicopathological Analysis on Hepatitis B Virus-Associated Glomerulonephritis in Children

Objective To investigate the clinicopathological features of hepatitis B virus-associated glomerulonephritis(HBV-GN) in children.Methods Clinical and pathological features were analyzed and correlated in 73 cases with renal biopsy-confirmed(HBV-GN).Sixty-six boys and 7 girls aged from 1 to 14 years old were included in this study,accounting for 14.4% in children under-(going) renal biopsy in the same period.Results Nephrotic syndrome(NS) was the prodominant clinical manifestation(53/73,72.6%),followed by glomerulonephritis(15/73,20.5%) and hematuria with proteinuria(5/73,6.9%).Serologic markers of HBV were found in all cases.Among them,55 cases(75%) were positive for hepatitis B surface antigen(HBsAg),hepatitis B e antigen(HBeAg) and anti-hepatitis B core(anti-HBc),14 cases(19.2%)positive for HBsAg,anti-HBe and anti-HBc,2 cases positive for HBsAg and anti-HBc,1 child positive for HBsAg and HBeAg,and another only positive for anti-HBc.The most common pathological type of HBV-GN was membranous nephropathy(MN),which was found in 69 cases(94.5%),followed by membranoproliferative glomerulonephritis(MPGN) and mesangial proliferative glomerulonephritis(MsPGN),2 cases(2.7%),respectively.All cases with NS presented themselves with MN.The pathological grades were significantly correlated with clinical manifestations through double trend analysis(?~2=5.22 P=0.022).Conclusions MN is predominant in childhood HBV-GN,which mainly presented with NS.Their clinical patterns are somewhat correlated with pathological grades.
4.CT Diagnosis of Mediastinal Cystic Mature Teratoma

Purpose To explore the CT features of mediastinal cystic mature teratoma. Materials and Methods CT images of 11 patients with pathologically confirmed mediastinal cystic mature teratoma were retrospectively analyzed, distribution within mediastinum, shape of cysts, capsule contents density, walls of cysts and mass cardiovascular interface (MCI) were analyzed. Results All the 11 cases were located in the anterior mediastinum, including three cases confined to the anterior mediastinum, seven cases located mainly within or involving the anterior and middle mediastinum, and one case confined to the anterior inferior mediastinum. Nine cases were round, one case was oval, one case was half round. Seven cases with fat ingredients displayed within the lesions, four cases with fat nodules within the cystic walls;ten cases showed as cysts with significant walls;MCI protrude type in six cases and smooth type in five cases. Fat, hair, keratosis, mucus and other ingredients were found in each cavity in surgery and pathology examination. Conclusion CT features of mediastinal cystic mature teratoma are complex and diverse, but with certain characteristics, thus CT is of great significance in the diagnosis and differential diagnosis of mediastinal cystic mature teratoma.
5.Etiological analysis of infection after eye surgery and the investigation of drug sensitivity

Background Clinical researches showed that antimicrobial-resistance of bacterium is gradually serious.It is very important to master pathogen distribution and antimicrobial susceptibility after eye surgery.Objective This study was to investigate the distribution and drug sensitivity of pathogenic bacteria from ocular operative infection in 3-year duration to provide a reference for reasonable selection of antibiotics.Methods Sixtyfive positive specimens were obtained from 65 patients due to postoperative infection in Department of Ophthalmology of Shenzhou Hospital and Shengjing Hospital from 2009 January through 2011 December.The pathogenic bacteria were isolated and then the drug sensitivity test of conventional antibiotics was performed.Results Among the 65 ophthalmic specimens,35 strains of Gram-positive bacteria were isolated with the constituent ratio 53.8%,and 15 strains of Gram-negative bacteria accounted for 23.1%.Fungus was found in 15 cases with the constituent ratio 23.1%.Staphylococcus epidermidis accounted for 42.9% in Gram-positive bacteria,and Pseudomonas aeruginosa accounted for 33.3% in Gram-negative bacteria.Aspergillus was the main component of fungus,which accounted for 60.0% of the proportion 3 strains of methicillin-resistant Staphylococcus aureus (MRSA) in 8 strains of Staphylococcus aureus,but no vancomycin-resistant Enterococcus (VRE) and vancomycin-resistant Staphylococcus (VRS) were seen.The in vitro drug sensitivity test showed that the main Gram-positive bacteria,Staphylococcus epidermidis and Staphylococcus aureus,were sensitive to linezolid,vancomycin,levofloxacin and ciprofloxacin with the sensitive rates >50.0%,and these bacteria were lowly sensitive to penicillin and erythromycin with the sensitive rates ≤30.0%.Pseudomonas aeruginosa appeared to be the main Gram-negative bacteria,showing a sensitive rate ≥ 80.0% to tobramycin,amikacin,ceftazidime,cefepime,cefoperazone/sulbactam,piperacillin/tazobactam,imipenem and meropenem.Conclusions Staphylococcus epidermidis,Staphylococcus aureus,Pseudomonas aeruginosa and Aspergillus are primary pathogens in ocular infection after operation.Antibiotics such as penicillin,erythromycin,levofloxacin,cefotaxime seem to be relatively high resistant to eye infection.To choose sensitive antibiotics timely and reasonably is a key to the prevention and control of ocular infection after operation.
6.Epidemiological investigation of a brucellosis outbreak transmitted through a lamb slaughtering site

Abstract
On August 11, 2022, Tongxiang Center for Disease Control detected a case of brucellosis, and the case was not engaged in related work. Case finding and risk factor investigation were immediately conducted to trace the source of infection. It was revealed that the case sold aquatic products at a farmer's market and frequently picked up goods at a seafood warehouse adjacent to a lamb slaughtering site. There was a potential risk of infection due to indirect contact with slaughtered lambs or contaminants. Serological tests were conducted on 9 employees of the slaughtering site and 48 residents nearby, and the brucellosis cases diagnosed in hospitals in the same area were searched. A total of 11 brucellosis cases were identified, including 9 confirmed cases and 2 asymptomatic infections. There were 2 cases of slaughtering workers and 9 cases of non-occupational individuals from the surrounding area of the slaughtering site. Brucella melitensis biovar 3 were isolated from a slaughtering worker and a non-occupational individual. The slaughtered lambs primarily came from northern regions such as Inner Mongolia and Heilongjiang. It was concluded that it was a cluster caused by Brucella melitensis biovar 3 and spread through direct or indirect contact with imported infected lambs or contaminated environments from a lamb slaughtering site. It is suggested to strengthen the quarantine of imported sheep, legally shut down non-compliant lamb slaughtering sites, implement designated slaughtering and enhance occupational protection.
7.Effect of buyang huanwu decoction on expressions of angiopoietin-1 and its receptor mRNA in brain of rat after intracerebral hemorrhage.

OBJECTIVETo investigate the mechanisms of Buyang Huanwu Decoction (BYHWD) by observing its effects on expressions of angiopoietin-1 (Ang-1) and the endothelial-specific receptor tyrosine kinase (Tie-2) mRNA in damaged region of rats' brain after intracerebral hemorrhage (ICH).
METHODSOne hundred and sixty Sprague-Dawley rats were randomly divided into four groups, 10 in the normal control group, 60 in the sham-operative group, 60 in the ICH model group, and 30 in the BYHWD-treated group. The ICH model was established by injecting collagenase type VII 0.5 U stereotaxically into right globus pallidus. Animals in the BYHWD-treated group were administered orally with BYHWD, while animals in the sham-operative group and the ICH model group were administered orally with equal volume distilled water, and those in the normal control group drank water freely. The positional variations of the expression of Ang-1 and Tie-2 in the sham-operative group and the model group were assayed by immunohistochemistry on dayl, 4, 7, 14, 21 and 28 after modeling, in the meantime, the dynamic changes of Ang-1 and Tie-2 mRNA expressions in all groups were assayed by reverse transcription-polymerase chain reaction (RT-PCR).
RESULTSNo significant expression of Ang-1 and Tie-2 in brain of rats in the normal or the sham-operative group was found during the experiment. In the model group, the Ang-1 and Tie-2 positive micrangio-segments appeared at the edge of clot on day 1 to day 4, they gradually penetrated to hematoma area from day 7; with Ang-1 and Tie-2 mRNA expressed from day 1, but very weak until day 4, showing no significant difference to that on day 1; thereafter, they increased gradually, and reached the peak on day 28 (P <0.05). While the two expressions in the BYHWD treated group reached the peak on day 21, and from day 7 to day 28, they were all significantly higher than those in ICH model group at the corresponding time points (P <0.01).
CONCLUSIONBYHWD can promote the up-regulation of Ang-1 and Tie-2mRNA expressions in brain of intracerebral hemorrhagic rats, which might accelerate the angiogenesis in the reconstruction of microvascular network in the damaged zone, and thus facilitating the repairing of damaged tissue.

8.Expression of Recombinant Nematode Anticoagulant Peptide in Pichia Pastoris

Objective: To acquire recombinant nematode anticoagulant peptide ( NAP) with high anticoagulant activity. Methods: Pichia pastoris GS115 strain was transformed with recombinant yeast expression vector pPICS. 5K-rNAP. Expression of rNAP was induced with methanol after the identification of positive strains. NAP expressed in the collected yeast culture supernatant was confirmed with SDS-PAGE and Western blot. The biological activity of the products was validated with PT (prothrombin time) , INR (international normalized ratio) and APTT (activated partial thromboplastin time) , respectively. Results: The yeast strains expressing NAP were identified. The rNAP was secreted into culture supernatant with a molecular weight of about 10 kDa due to glycosylation, which is a little bigger than that predicted (8.7kDa). The anticoagulant efficiency of rNAP was confirmed with the in vitro assays. Conclusion: The recombinant nematode anticoagulant peptide with high biological activity was successfully expressed in Pichia pastoris and can be used in the future development of novel anticoagulant agent.
9.Effect of neoadjuvant hormonal therapy in different time combined with intensity modulated radiotherapy for locally advanced prostate cancer

ObjectiveTo investigate the ideal method of neoadjuvant hormonal therapy (NHT) for locally advanced prostate cancer.MethodsSixty cases of patients diagnosed with locally advanced ( T3 -T4 N0M0) prostate cancer were treated with NHT combined with intensity modulated radiotherapy (IMRT),They were randomly divided into 3 groups with 20 cases in each group.Group A with NHT 2 weeks,Group B with NHT 3 months,Group C with NHT 6 months.Endocrine duration began with NHT until 12 months after the end of IMRT.The PSA and prostate volumes were detected by transrectal ultrasound and Qmax was tested after NHT and every 3 months after IMRT.Results After NHT,the median PSA of different groups were decreased to 24.88,0.20 and 0.07 μg/L,respectively.There was significant difference ( P < 0.05 ).The prostate volume in groups B and C reduced significantly ( P < 0.05 ).The group B reduced 20.8% and the group C reduced 39.5%.The Qmax of group B and C were ( 11.70 ± 2.81 ) and ( 14.45 ±2.61 ) ml/s respectively.After 12 months of endocrine combined with IMRT:(①)PSA.There was significant difference (P <0.01 ) with group C < group B < group A.②The prostate volume.The reducing of groups B and C were more obvious than group A ( P < 0.01 ).There was no significant difference between group B and group C ( P > 0.05).③Qmax.There was significant difference (P < 0.01 ) among the 3 groups with group C > group B > group A.ConclusionsNHT combined with IMRT is an ideal method for locally advanced prostate cancer.The NHT time before IMRT treatment should last at least 3 months.

Objective To investigate ideal solution of neoadjuvant hormomal therapy (NHT) for locally advanced prostate cancer.Methods 60 patients diagnosed with locally advanced (T3-4N0M0) prostate cancer were treated with NHT.They were randomly divided into 3 groups of 20 cases.A group:NHT 2 weeks,B group:NHT 3 months,C group:NHT 6 months.Results The median PSA of A,B and C group after NHT were 24.88 (6.62-55.86),0.20 (0.05-12.07) and 0.07 (0.01-2.01) ng/ml,respectively.There was statistically significance compared with those in untreatment ( all P =0.00).There was statistically significant (P =0.00)among groups.The prostate volume of A,B and C group were (49.50+14.19),(47.35±17.99) and (36.15±7.17)ml,respectively.There was statistically significance in the B and C group compared with that in untreatment (P =0.04,0.00).There was statistically significant between A and C group and between B and C group (P =0.00,0.01).The Qmax of A,B and C group were (8.75±2.15),(11.7±2.81) and (14.45±2.61) ml/s,respectively.There was statistically significance in the B and C group compared with untreatment (both P =0.00).There was statistically significance among groups (all P =0.00).Conclusion The NHT time should last at least 3 months in order to reduce PSA and prostate volume and to increase the Qmax.
