1.Probe into establishment of voluntary reporting system of nursing errors

Errors and flaws of clinical nursing practice can be fully exposed to voluntary reporting system of nursing errors, and then dramatically improving patient safety. This paper aims at exploring the theoretical basis, the characteristic of the system and essentiality for establishing voluntary reporting system of nursing errors, introducing the methods practicing both in abroad and in domestic, explaining its precondition and put forward a suggestion that the voluntary reporting system would be established in our own country.
2.Research on the relation between upper gastrointestinal bleeding and non-steroidal anti-inflammatory drugs

Objective To investigate the clinical features and the correlated reason of upper gastrointestinal bleeding (UGIB) induced by non-steroidal anti-inflammatory drugs (NSAIDs). Methods Five hundred and twenty-eight patients were divided into two groups according to consumption of NSAIDs in the week previous to the onset of bleeding, 116 cases were in NSAIDs group and 412 cases were in non-NSAIDs group. The clinical data was analysed and compared between two groups. Results There was no significant difference in sex, smoking history, and ulcer history between two groups (P> 0.05). The average age was (57.4 ± 12.1) years old and the infection of Hp was 65.5% (76/116) in NSAIDs group, and they were higher than those in non-NSA1Ds group [(44.6 ± 11.9) years old and 25.0% (103/412)](P < 0.01). More gastric ulcer and complex ulcer was seen in NSAIDs group (P <0.01 or <0.05). Conclusion Recognition about the clinical characteristics of NSAIDs relevance UGIB should be strengthened, and side effect of NSAIDs should be reduced as far as possible.

4.Soft tissue repair of large defects of lower limbs with deep inferior epigastric perforator flap

Objective To study the method and effect of deep inferior epigastric perforator flap(DIEP)in repairing the large defects of lower limbs.Methods Eight cases,from July 2009 to November 2011,including 3 cases of plantar skin defects with bone exposure after foot injuries,three cases of plate exposure after tibia fracture surgery and 2 cases of heel repeated ulceration after skin graft,were repaired by deep inferior epigastric perforator flap.Results All deep inferior epigastric perforator flaps survived with good functions,except 1 case whose distal with poor blood supply and the flap survived after treatmenting,three cases of flap bloated with good appearances after second operation.Conclusion DIEP is a proper option for repair of large defects of lower limbs.It has the advantages of abundant blood supply,large flap area,abdomen can suturing without abdominal complications.
5.Analysis of clinical manifestations and pathogenic septicemia of newborn babies late onsetsepsis in community acquired

Objective To summarize the clinical manifestation ,the main pathogenic bacteria distribution and drug resistance of neonatal community acquired sepsis late onset in our hospital .Methods Retrospectively analyse the clinical material of 122 cases (41 premature cases and 81 cases of full term) with neonatal community acquired sepsis late onset ,which were clinically diagnosed , from January 2009 to December 2012 in our hospital .Results The main clinical manifestation of neonatal community acquired sep-sis late onset was poor response(64 .7% ) ,repellent milk(57 .4% ) ,temperature changes(61 .5% ) ,and the respiratory tract and um-bilical region were the main infection ways .42 cases were checked out with pathogen in the 122 cases ,blood culture positive rate was 34 .4% ,and there was no statistically differences between the premature and the full term infant group ,In the 42 cases ,there were 29 cases with staphylococcus ,including 10 cases of staphylococcus aureus ,14 cases of coagulase negative staphylococcus and 5 cases of enterococcus ;and there were 10 cases are checked out with e .coli .All of the coccus detected were resistant to penicillin and erythromycin ,but sensitive to vancomycin ,teicoplanin ,linezolid .The e .coli was sensitive to amikacin ,imipenem ,meropenem ,and al-so had a higher sensitivity to cefazolin ,ceftriaxone ,cefepime ,cefoperazone and nitrofurantoin .Conclusion Blood culture positive rate is not high in neonatal community acquired sepsis late onset ,and its′clinical manifestations are nonspecific .The main pathogenic bacteria is coagulase negative staphylococcus ,staphylococcus aureus ,followed by escherichia coli .
6.Inhibitory Efficiency on Growth in vivo of B16 Melanoma Cell Expressing Angiostatin

Objective: Study on the growth character of B16 melanoma cell which can express angiostatin. Methods: Angiostatin gene was constructed from human plasminogen cDNA by deletion mutation. A B16 melanoma cell clone named BAG28 which stably expresses angiostatin was established by introducing gene into it. 8.The clinical observation of oxaliplatin combined with capecitabin for patients with advanced colorectal cancer

Objective To observe the therapeutic effect and toxicity reaction of Oxaliplatin com- bined with XeLoda in the treatment of 42 patients with advanced colorectal cancer.Methods All the pa- tients were treated with Oxaliplatin(130 mg/m~2,ivgtt for 2 h,d1)combined with XeLoda(2000 mg?m~(-2)?d~(-1), po,bid,d1 to d14).The regime was repeated every 21 days for at least 3 consecutive cycles.Results The to- tal response rate was 40.5%(17/42)in which 2 got CR and 15 PR.24 patients got Kamofsky score increased. The major toxic effects were alopecia,peripheral neuritis,gastrointestinal tract reactions and myelosuppression. Conclusion Oxaliplatin combined with XeLoda regimen is effective in the treatment of advanced colorectal cancer,and its toxicity is tolerable.It is worth studying in the future.

10.Endovascular therapy for complex subclavian artery occlusive diseases

Objective To discuss the key points of endovascular therapy for complex subclavian artery occlusive diseases. Methods During the period from January 2012 to December 2013, a total of 92 patients with complex subclavian artery occlusive disease were admitted to Xuanwu Hospital of Capital Medical University, Beijing, China. The clinical data were retrospectively analyzed. The features of the lesions, the success rate of endovascular therapy, the use of combined approaches, the relief of symptoms after treatment, etc. were evaluated. Results The complex subclavian artery occlusive diseases could be divided into three types. Type Ⅰ: long segment of the left subclavian artery was occluded; type Ⅱ: ostial stenosis or occlusion of the right subclavian artery; and type Ⅲ: subclavian artery stenosis or occlusion was associated with the ostial disorder of the vertebral artery, or the opening of vertebral artery was affected by the subclavian artery stenosis or occlusion. The technical success rate was 82.6%. Combination use of femoral artery and brachial artery approach was employed in 27.2% of patients, which had improved the technical success rate. After the treatment the symptom improvement rate was 81.6%. Conclusion Upper limb artery approach can improve the re-canalization rate of left subclavian artery with long segment occlusion, and can ensure the accurate positioning of stent at the site of right subclavian artery opening. During the procedure of endovascular intervention for subclavian artery occlusion disease, attention should be paid to the protection of the vertebral artery.
