2.Clinical Observation of Bifid Triple Viable Capsule in the Treatment of Ulcerative Colitis

OBJECTIVE:To observe the efficacy and safety of Bifid triple viable capsule in the treatment of ulcerative colitis (UC). METHODS:94 patients with UC were randomly divided into control group and research group. Control groups was given nutrition support,light diet and 5-amino salicylic acid and other conventional treatment;research group was additionally given 420 mg Bifid triple viable capsule,3 times aday. Treatment course for both group was 8 weeks. The clinical efficacy,and serum D-lac-tic acid,diamine oxidase(DAO)levels before and after treatment,and incidence of adverse reactions in 2 groups were observed. RESULTS:The total effective rate in research group was significantly higher than control group,the difference was statistically sig-nificant(P<0.05);after treatment,serum D-lactic acid,DAO levels in 2 groups were significantly lower than before,and re-search group was lower than control group,the differences were statistically significant(P<0.05 or P<0.01);and there was no sig-nificant difference in the incidence of adverse reactions(P>0.05). CONCLUSIONS:Based on the conventional treatment,both the efficacy and safety of Bifid triple viable capsule are good in the treatment of UC.
3.Evaluation of Boned Metabolism Quota in the Prediction of Osteoporosis of Climacteric Female in Xi' an Area

Objective To investigate the relationship among age,menopausal status,serum 25 (OH)VD and skeletal metabolism index as PTH (Parathyroid Hormone),CA(Calcium),P (Phosphorus) and boned mineral density (BMD) of climacteric female in Xi'an.Methods 352 cases of healthy women at the age of 40~60 were collected who did routine examination in outpatient building of the First Affiliated Hospital of Xi'an Jiaotong University from January 2016 to April 2016.According to the results of skeletal density test,all subjects were divided into 3 groups as the normal group (T>-1),the reduction group (-2.5<T<-1),the osteoporosis group (T<-2.5).Electrochemiluminescent immunoassay (ECLIA) was used to detect levels of serum 25 (OH)VD,PTH,CA and P.Kruskal-Wallis test and chi-square test were used to analyze differences of age,serum 25(OH)VD,PTH,CA and P in different levels of bone mineral density groups.Results Compared with bone density normal group 2.24(2.18~2.29)mmol/L,serum CA levels of bone mineral density reduction group 2.27 (2.20 ~ 2.32)mmol/L and osteoporosis group 2.27(2.23~2.33)mmol/L were significantly higher (Z=9.669,P<0.01).Serum P levels of bone mineral density reduction group 1.15 (1.09~ 1.23)mmol/L and osteoporosis group 1.18 (1.09~ 1.25)mmol/L were obviously higher than bone density normal group,1.11 (0.99 ~ 1.23) mmol/L (Z =13.64,P< 0.01).Ages of osteoporosis group 54(50~57) years and bone mineral density reduction group 51 (47~ 54) years were significantly older than Bone density normal group 47(43~50) years (Z=73.08,P<0.01).The rate of menopause in osteoporosis group and bone mineral density reduction group were significantly higher than in bone density normal group (x2 =13.81,P< 0.01).There was no statistical difference in serum 25(OH)VD and PTH between groups.Conclusion This study found boned density of climacteric women is more likely correlated with age,menopause status and levels of serum CA and P.There was not enough evidence to prove that 25 (OH)VD and PTH have effect on skeletal density.Therefore,to monitor and detect the menopause status and levels of serum CA and P of climacteric female is critical for the prevention and treatment on osteoporosis of women.
4.Adenovirus-mediated transfer of TIMP-4 gene inhibits neointimal formation after balloon injury

Objective: To investigate the effects of human tissue inhibitor of matelloproteinase 4 (TIMP 4) on vascular smooth muscle cells (VSMCs) migration and the neointimal formation after balloon injury in rats. Methods: The cultured VSMCs were transfected with an adenoviral vector containing human TIMP 4 gene, AdTIMP 4. Effect of TIMP 4 on VSMC migration was investigated by monolayer cell scrape. AdTIMP 4, control adenoviral vector or PBS was transduced into the rat carotid artery from the adventitial after carotid artery injury. Cell number within the internal elastic lamina 4 days after gene transfer was counted and neointima/media area ratio 28 days after gene transfer was calculated. Results: The migration distance of VSMCs infected with AdTIMP 4 was inhibited markedly. Morphometric analysis demonstrated a statistically significant reduction in the number of cells migrated into neointima compared with controls [(32.5?4.8) cells per section, (33.8?7.0) cells per section and (8.2?2.4) cells per section for uninfected, AdGFP treated and AdTIMP 4 treated arteries, respectively]. 5.Rapid identification and drug susceptibility assay of mycobacterin in HIV patients

Objective To establish the rapid pathogen identification method for HIV and Mycobac-terium tuberculosis (Mtb)co-infection and the assay for the drug susceptibility. Methods Geneprobe and 16S rDNA sequencing were used to differentiate mycobacterium species and modified microscopic observation drug susceptibility (MODS) was used for the drug susceptibility test. The above assays were compared with acid-fast smear, L-J culture and proportional drug susceptibility tests. Results (1) Thirty-four mycobacte-rial isolates were obtained from 112 samples collected from 68 HIV patients. Among these isolates, the strain species were determined by Geneprobe and 16S rDNA sequencing as the followings: 21 Mtb complex, 10 NTM including 5 M.avium complex, 2 M.gordonae, 2 M.kansasii, 1 M.colombiense, and 3 co-infection. (2) The sensitivity of Mtb to rifampicin, ethambutol, isoniazid and streptomycin were 100%, 100%, 76.2%, 90.5% respectively, while the sensitivity of NTM to rifampicin, ethambutol, isoniazid and strepto-mycin were 40%, 60%, 0%, 30% respectively. There is no significant statistic difference between the two methods, MODS and the reference standard, for the drug susceptibility test. (3) Six to eight weeks are nee-ded for the identification of the species of mycobacteria and the drug susceptibility test by using traditional method. In this study, 5-14 d, 6-15 d and 10-14 d are needed for Geneprobe, 16S rDNA sequencing, and MODS respectively. The time for the testing has been dramatically shortened. Conclusion The identifica-tion of mycobacterial species and the drug susceptibility test using clinical samples could be completed within 15 days by using combined Geneprobe, 16S rDNA sequencing and modified MODS. This combined method can be used for the pathogen identification and drug resistant test in HIV patients who are co-infected by my-cobacteria.

Oxaliplatin, the third generation of the platinum based chemotherapy agent, is effective in the treatment of multiple solid tumors and is also quite safe. However, there is a high incidence of peripheral neurotoxiciy, which is dose-limiting. Oxaliplatin induces two distinct forms of neurotoxicity: an acute syndrome that is triggered or aggravated by exposure to cold, and chronic cumulative sensory neurotoxicity which in nature resembles characteristics of cisplatin associated neurotoxicity. In this article, the clinical manifestations, electrophysiologic abnormalities, mechanism of neurotoxicity and therapy are reviewed.
7.Lung recruitment maneuver effects on respiratory mechanics and extravascular lung water index in patients with acute respiratory distress syndrome

BACKGROUND: Animal experiments showed that recruitment maneuver (RM) and protective ventilation strategy of the lung could improve oxygenation and reduce extravascular lung water. This study was to investigate the effects of RM on respiratory mechanics and extravascular lung water index ( EVLWI) in patients with acute respiratory distress syndrome (ARDS). METHODS: Thirty patients with ARDS were randomized into a RM group and a non-RM group. In the RM group, after basic mechanical ventilation stabilized for 30 minutes, RM was performed and repeated once every 12 hours for 3 days. In the non-RM group, lung protective strategy was conducted without RM. Oxygenation index (PaO2/FiO2), peak inspiratory pressure (PIP), Plateau pressure (Pplat), static pulmonary compliance (Cst) and EVLWI of patients before treatment and at 12, 24, 48, 72 hours after the treatment were measured and compared between the groups. Hemodynamic changes were observed before and after RM. One-way ANOVA, Student's t test and Fisher's exact test were used to process the data. RESULTS: The levels of PaO2/FiO2 and Cst increased after treatment in the two groups, but they were higher in the RM group than in the non-RM group (P<0.05). The PIP and Pplat decreased after treatment in the two groups, but they were lower in the RM group than in the non-RM group (P<0.05). The EVLWI in the two groups showed downward trend after treatment (P<0.05), and the differences were signifcant at all time points (P<0.01); the EVLWI in the RM group was lower than that in the non-RM group at 12, 24, 48 and 72 hours (P<0.05 or P<0.01). Compared with pre-RM, hemodynamics changes during RM were significantly different (P<0.01); compared with pre-RM, the changes were not significantly different at 120 seconds after the end of RM (P>0.05). CONCLUSIONS: RM could reduce EVLWI, increase oxygenation and lung compliance. The effect of RM on hemodynamics was transient.
8.Effect of static pressure on the expression of glial fibrillary acidic protein and heat shock protein 70 by the cultured retinal M?ller cells

Objective To observe the effects of static pressure on the number of cultured retinal M?ller glial cells(RMGC)and expression of glial fibrillary acidic protein(GFAP)and heat shock protein(HSP)70 by these cells.Design Experimental study. Participants Cultured rat RMGC.Methods Rat RMGCs were cultured and identified according to previous method described by Reichenbach.These cells were treated with different static pressures and divided into 4 groups:A(1.33kPa),B(2.67kPa),C(5.33kPa)and D(10.67 kPa)while the cells without treatment was as control group(NC).The morphologies of RMGC in these groups were observed under inverted phased contrast microscope,the number of RMGC counted with conservative method and the viability were studied with trypan blue staining.The expressions of GFAP and HSP70 in RMGCs were detected with the method of western blot.Main Outcome Measures The morphologies of RMGC,cell number,cell viability.Results There were pressure-dependent changes of RMGC number. The cell number of group C and D was less than that of group NC,A and B(P<0.01).High static pressure resulted directly in the decreased ratio of unstained RMGCs(P<0.01).The ratio of unstained RMGCs in group C and D was less than that in group NC,A and B(P<0.01).Many cells in group C and D were injured and the higher the pressure elevated,the more the degree of injury became.The expressions of GFAP and HSP70 in group NC were less than other pressure treated groups and the expression of GFAP in group C and D was higher than that in group A and B.There was no obvious difference between these pressure treated groups.Conclusions High static pressure could cause the injuries of RMGCs.The increased expression of GFAP and HSP70 in RMGC might be regarded as a sign of retinal injury response to high intraocular pressure.
9.Association between single-nucleotide polymorphisms of vascular endothelial growth factor gene and Alzheimer’s disease in Shaanxi Han population

Objective To evaluate the association between 8 single nucleotide polymorphisms (SNPs)of vascular endothelial growth factor (VEGF ) gene and the risk of Alzheimer’s disease (AD ) in Shaanxi Han population.Methods We examined the potential association between AD and 8 SNPs of VEGF gene using the MassARRAY system.The participants enrolled in this study included 214 patients with AD and 249 healthy controls from Shaanxi Han population.SPSS16.0 and Haploview 4.2 were employed to analyze differences in genotypes, alleles and haplotypes between the two groups.Results The results showed that rs3025039 (3’UTR)were significantly associated with AD (P<0.05).Greater frequency of rs3025039 T allele (P=0.008,OR=1.527,95%CI=1.116-2.088)was found in AD subjects.Furthermore,strong linkage disequilibrium (LD)was observed in 2 locks (block1:rs699947-rs1570360-rs2010963;block 2:rs3024997-rs3024998-rs3025006)(D’>0.9).There were no significant haplotypes in block 1 and block 2 (P=0.034)found between the patients and controls.Conclusion These findings point to the role for VEGF gene polymorphisms (rs3025039)in AD of a Shaanxi Han population. Individuals with T allele of rs3025039 may be at a higher risk for AD.
10.Molecular characteristics of pyrazinamide resistance in Mycobacterium tuberculosis strains

Objective To investigate the correlation between the mutation of pncA gene and the susceptibility to pyrazinamide ( PZA) in Mycobacterium tuberculosis complex ( MTBC) strains and to analyze the mutation of panD and rpsA genes in wild type isolates without pncA gene mutation.Methods The sus-ceptibilities of 108 MTBC strains to first-line drugs including PZA were detected by using the MGIT 960 TB system.PCR was performed to amplify the 16S rDNA and pncA, panD and rpsA genes.The PCR products were analyzed by DNA sequencing analysis .Results Among the 78 multidrug-resistant MTBC strains , 47 isolates (60%) were resistant to PZA.Four out of 30 (13%) strains that were sensitive to ethambutol , iso-niazid, rifampicin and streptomycin (EIRS) were resistant to PZA.The drug-resistant MTBC strains showed higher resistance rate to PZA than that of the EIRS sensitive strains .There were 49 ( 96%) PZA-resistant isolates and 4 (7%) PZA-sensitive isolates occurred pncA gene mutation.Most of the pncA gene mutations in the genomes of PZA-resistant strains were base substitution mutation , especially the His57Asp substitu-tion.The pncA gene mutations centralized in the regions of 160-169, 203-289, 309-396 and 413-467.Seven novel mutation sites of pncA gene were observed including T175C, C188A, G insertion at 68, AGC insertion at 235, C insertion at 339, CC insertion at 392 and GT deletion at 395.The mutation sites founded in the genomes of PZA-sensitive strains were different from those of the PZA-resistant strains .No mutation of the pncA gene and the upstream regulatory sequence was found in two PZA-resistant strains , NJ44 and NJ108 . The sequence analysis of panD and rpsA gene showed that the NJ 108 strain had panD gene mutation at G419A, but no mutation was detected in the NJ 44 strain.Conclusion The multidrug-resistant MTBC strains showed higher resistance rate to PZA .The pncA gene mutation was common in PZA-resistant MTB strains and the panD gene mutation was also worthy of attention .
