2.Vertical reduction mammaplasty.

OBJECTIVEThe inverted "T" operation is a typical and classic method for breast hypertrophy. Although having good results, it leaves significant scar and tends to have bottom-out deformity with time. The purpose of this study is to introduce our experience in vertical mammaplasty.
METHODSThe typical Lejour's design was used. The lower part of the breast was excised. The nipple-alveolar complex was elevated to a proper position with the superior dermal-glandular pedicle. Only vertical scar was left after the skin was closed.
RESULTS24 patients were treated using this method. The results were satisfactory except unilateral nipple-alveolar complex necrosis occurred in one patient. Another patient had a minor revision for the dog-ear 6 months after breast reduction.
CONCLUSIONSVertical reduction mammaplasty is effective with reduced scar and highly satisfactory results. It deserves recommendation.

3.Production and Characterization of Monoclonal Antibodies of Shrimp White Spot Syndrome Virus Envelope Protein VP28

BALB/c mice were immunized with purified White spot syndrome virus (WSSV).Six monoclonal antibody cell lines were selected by ELISA with VP28 protein expressed in E.coll in vitro neutralization experiments showed that 4 of them could inhibit the virus infection in crayfish.Westernblot suggested that all these monoclonal antibodies were against the conformational structure of VP28.The monoclonal antibody 7B4 was labeled with colloidal gold particles and used to locate the VP28 on virus envelope by immunogold labeling.These monoclonal antibodies could be used to develop immunological diagnosis methods for WSSV infection.
4.Vertical reduction mammaplasty with wide superior pedicle.

OBJECTIVETo report the reduction mammaplasty with vertical incision and superior wide pedicle.
METHODSTypical Lejour mosque-dome design was performed. The inferior part of glandular tissue and skin were excised. The nipple-and-areola complex (NAC) was elevated to normal position with superior wide pedicle. The breast morphology was modified with vertical scar left.
RESULTS46 patients were treated. 4 patients had unilateral breast reduction. 14 breasts had wound dehiscence. 3 breasts received debridement, others were treated conservatively with dressings. No complete NAC necrosis occurred.
CONCLUSIONSThe reduction mammaplasty with vertical incision and superior wide pedicle is a safe and effective method with a low risk of NAC necrosis.

5.Flap transplantation combined with liposuction to treat upper limb lymphedema after mastectomy.

OBJECTIVETo investigate a new surgical method to treat unilateral limb lymphedem after radical mastectomy.
METHODS10 cases of upper limb lymphedema after radical mastectomy were treated using flap transfer (the lateral thoracic skin flap or latissimus dorsi musculocutaneous flap combined with liposuction).
RESULTSAfter the treatment, the upper limb perimeter reduced in varied degrees. Nuclear lymphatic radiography showed notable changes in lymphatic circulation. The effective results were steady during the follow-up of 3-18 months.
CONCLUSIONFlap transplantation combined with liposuction is a useful treatment for limb lymphedema from radical mastectomy.

6.Plastic surgery after solid organ transplantations.

BACKGROUNDMore patients receive organ transplantation surgeries due to the advancement in immunosuppressive agents and surgical techniques. Some of those patients may need to undergo plastic or reconstructive surgery. Long-term use of immunosuppressive agents raises some serious problems. Therefore, this study aimed to introduce our experience about the safety and effectiveness of plastic surgeries after solid organ allograft transplantation.
METHODSA retrospective review of 17 transplant recipients who underwent different reconstructive or cosmetic operations was carried out. The subjects included 1 heart transplant, 1 liver transplant and 15 kidney transplant recipients.
RESULTSAll patients tolerated the plastic surgery procedures well. Flaps and skin grafts were the main constructive methods. There were no postoperative infections and wound dehiscence. Transferred flaps survived completely. Skin grafts took well. Three of the cosmetic surgery patients were satisfied with the results.
CONCLUSIONSImmunosuppressed organ transplant recipients can successfully undergo major reconstructive and cosmetic surgery when given special attention.

7.The treatment of sternal wound infection by internal fixation of sternum with titanium plate after cardiac surgery.

OBJECTIVETo investigate the internal fixation of sternum with titanium plate for the treatment of sternal wound infection after cardiac surgery.
METHODSA retrospective study of 26 patients from Jan 2009 to Dec 2011 was carried out. All the 26 cases with sternal wounds infection after cardiac surgery, were treated by internal fixation with titanium plates.
RESULTSAll the patients had chronic infection, lasting for more than 3 months. 3 patients received vacuum-assisted closure treatment after complete debridement. All the wounds were covered with pectoral major muscle flap. All the wounds healed completely with no recurrence of infection.
CONCLUSIONSInternal fixation of sternum with titanium plate can increase the stability. It helps to improve the treatment efficacy of sternal wound infection after cardiac surgery.

8.Prognostic value of soluble MICA levels in the serum of patients with advanced hepatocellular carcinoma.

Serum levels of soluble MHC class I-related chain A (sMICA) are related with the prognosis of various types of cancer; however, few studies on the prognostic value of sMICA in hepatocellular carcinoma (HCC) have been reported. In this study, we retrospectively investigated the relationship between sMICA levels and clinical features of advanced HCC, and we assessed the prognostic value of sMICA in advanced HCC. Furthermore, the relationship of serum sMICA levels and natural killer group 2, member D (NKG2D) expression on natural killer (NK) cells was also evaluated. We detected sMICA levels in the serum of 60 advanced HCC patients using enzyme-linked immunosorbent assay (ELISA) and measured expression levels of NKG2D on NK cells using flow cytometry. We found that serum sMICA levels in HCC patients were in the range of 0.10-6.21 ng/mL. Chi-square analyses showed that sMICA level was significantly related with only tumor size. Survival analysis showed that a high sMICA level was significantly related with poor prognosis among HCC patients. Multivariate analyses indicated that sMICA was an independent prognostic factor. In addition, the levels of CD56+NKG2D+ NK cells were within the range of 11.2%-55.4%, and correlation analyses indicated that sMICA level was negatively correlated with the level of NKG2D+ NK cells. Our results suggest that serum sMICA levels may be an independent prognostic factor for advanced HCC.

9.The study on donor site sequelae after autologous breast reconstruction with an extended latissimus dorsi flap.

OBJECTIVETo study the prevention and treatment on the donor site sequelae after autologous breast reconstruction with extended latissimus dorsi flap.
METHODS88 patients received breast reconstruction with extended latissimus dorsi flap between May 1999 and Nov 2004 were concerned. We analyzed the donor site sequelae by objective and subjective evaluation and we assessed the functional condition. Results Dorsal hematoma developed in 1 patient. There are 7 patients with dorsal seromas and 2 of them developed capsules; 5 patients with mild skin exfoliate and 1 patient with dry skin necrosis, 2 winged shoulders. No infection and no hypertrophic scars appeared in donor site. There is no significant functional limitation and no influence in daily life.
CONCLUSIONSThe main complication after autologous breast reconstruction with extended latissimus dorsi flap was seromas and it can be controlled to decrease. There is no severe complication. It's a worthy method in autologous breast reconstruction in oriental people.

10.Epidemiological study on an outbreak caused by E. coli O157:H7 in Jiangsu province.

OBJECTIVETo carry out epidemiological study on an outbreak caused by E. coli O157:H7 infection in Jiangsu province in 1999.
METHODSEpidemiological, microbiological and moleculebiological methods were used to find out the source, route of transmission and risk factors.
RESULTS95 severe O157:H7 infected patients with acute renal failure in 9 counties and districts of 2 municipalities were reported in Jiangsu province, 1999 while 83 of the patients died with a death rate of 87.37%. Most patients were seen in mid or late June. The ratio of male to female was 1 to 1.44 and 88.42% of the patients were over 50 years old. 38 patients occurred in 2000 with 34 deaths. Major factors contributing to the outbreak would include without drinking tap water, eating leftover food, poor sanitary status in kitchen, not washing hands before meal and after bowl movement. 2 strain of O157:H7 was isolated from severe patients and 3 from diarrhea cases. Carrier rate among animals was up to 9.62% and 99.41% of the strains carried toxic gene. Strains isolated from feces of patients and animals belonged to the same colonies.
CONCLUSIONThis outbreak was severe which caused by O157:H7 and was first seen in China, which was closely related to the high carrier rate of O157:H7 in animals and to the positive rate of high toxic gene of the strains. There were various routes of transmission and the main factors of infection would include poor personal health habits and poor sanitation of the household.
