1.Effect of exogenous PGC -1α on VEGF expression in human retinal vascular endothelial cells

AlM: To investigate the effect of exogenous peroxisome-proliferator-activated receptor-γcoactivator-1α ( PGC-1α) on vascular endothelial growth factor ( VEGF) expression in human retinal vascular endothelial cells ( HRVEC) .METHODS:Recombinant PGC-1α protein was added to HRVEC, and no recombinant PGC-1α protein was added to HRVEC as control group. After 24h of incubation, two groups of cells were then placed into a normoxic ( 20%O2 ) or hypoxic ( 1% O2 ) environment for another 16h. The expression of VEGF mRNA and protein were detected by real - time PCR, ELlSA and immunofluorescence cytochemistry.RESULTS: VEGF mRNA and protein levels in the cells were significantly increased by recombinant PGC - 1αprotein both under normoxia and hypoxia conditions as compared with control groups (P<0. 01).CONCLUSlON: PGC-1α can upregulate the expression of VEGF in HRVEC under normoxia and hypoxia conditions.
5.A Comparative Study on Sensitivity of HE,IHC and RT-PCR in Detection of Breast Cancer Metastases in Axillary Lymph Nodes

Objective To compare the sensitivity of HE,immunohistochemistry (IHC) and RT-PCR in detection of breast cancer metastases in axillary lymph nodes.Methods Twenty female patients with newly diagnosed and clinically node-negative breast cancers underwent modified radical mastectomy, including a complete axillary lymph node dissection. The ages of the patients ranged from 31 years to 65 years, and the diagnosis of breast cancer was approved by pathological finding. Two hundred and thirty-nine axillary lymph nodes were found in these 20 patients. Metastases in axillary lymph nodes were explored by HE, cytokeratin 19 IHC and RT-PCR for cytokeratin 19 respectively. Results Seven(2.9%) lymph nodes were found to have metastatic cancers by HE in 3 patients,all nodes were found in level Ⅰ. Metastatic cancers were found in 13 (5.4%) nodes by IHC in 7 patients,11 nodes in level Ⅰ and 2 nodes in level Ⅱ; and 52(21.8%) nodes were found to contain tumor cells by RT-PCR in 14 patients,30 nodes in level Ⅰ and 22 nodes in level Ⅱ. All of 7 histologically(HE) positive nodes were found to contain tumor cells by IHC and RT-PCR. Among 232 histologically(HE) negative nodes,6(2.6%) nodes were found to contain tumor cells by IHC,and 45(19.4%) nodes were found to contain tumor cells by RT-PCR, all 6 IHC positive nodes showed the expected 460-base pair products on gel electrophoresis ( P
6.Effect of Diabetic Status on the Preventive Efficiency of Methylcobal on Peripheral Neuropathy in Rats

ObjectiveTo investigate the effect of varied hyperglycemia and durations on preventing peripheral neuropathy (PN) with methylcobal in diabetic rats.Methods80 Sprague Dawley rats were selected and randomly chosen 16 as normal control (NC) group, others 64 animals as diabetic model induced by alloxan. The model rats were evenly divided into relatively good control (GC) group and relatively poor control (PC) group based on levels of hyperglycemia regulated with exogenous insulin. 16 rats of each GC/PC group were intramuscularly injected with methylcobal (500 μg/kg). The sensory nerve conduction velocity (SNCV), motor nerve conduction velocity (MNCV) and evoked potential amplitute (EPA) of sciatic nerve of rats were detected by evoked electromyogram respectively at 2nd, 8th and 12th week after onset of diabetes. Fructosamine in serum was also detected.ResultsThe rats injected with methylcobal had a significantly delayed reduction of SNCV, MNCV and EP in GC group (fructosamine,1.0 mmol/L) than that in PC group (fructosamine, 1.2 mmol/L) ( P<0.05~0.01). Up to 8th week after onset of diabetes, there was no obvious difference in physioelectroparameters between the GC group and NC group. At 12th week after onset of diabetes, physioelectroparameters in the PC group fell to the level of non-methylcobal injected group. No effect of methylcobal on blood glucose revealed in any groups ( P>0.05).ConclusionMethylcobal has a beneficial effect on delaying the onset of diabetic PN, and is of high efficacy with a good control of diabetic state, especially at early stage.

Objective To build a set of inexpensive wireless wide area network PACS based on open source code.Methods To improve the open source code MyFreePACS on Internet.Results The basic function of PACS can be achieved in an economical way,using internet explorer to browse and deal with the DICOM Images,with the capability of such functions as displaying and managing of the pictures.It is possible to make rounds at patient bedside with wireless device and remote medical consultation with specialists through Internet.Conclusion It is possible to utilize the free open source code MyFreePACS on the internet to construct an economical PACS on Web in the medium and small-scale hospital.
9.Prediction of radiation-induced lung toxicity in locally advanced non-small cell lung cancer treated with chemoradiotherapy by functional lung dose-volume histogram

Objective To investigate the correlation between functional lung dose-volume histogram (f-DVH) parameters and radiation-induced lung toxicity (RILT) in patients of locally advanced non-small cell lung cancer (NSCLC) treated with late-course accelerated hyperfractionated radiotherapy and chemotherapy,and to identify the excellent predictors of f-DVH and their reference thresholds.Methods A total of 51 patients of NSCLC at stage Ⅲ underwent PET/CT/SPECT coregistered image guided radiotherapy.Philips Pinnacle3 planning system was used for delineation of the target volume and organs at risk so as to establish the three dimensional conformal radiotherapy or intensity-modulated radiotherapy treatment plans.The version 3.0 of the NCI Common Terminology Criteria for Adverse Events was used to evaluate the grade of RILT,and analyze the correlation of the DVH parameters of the total lung (TL),ipsilateral lung (IL),and functional lung (FL) and RILT,and to identify the excellent predictors.The median follow-up was 15 months.Results During the follow-up,10 cases of RILT (19.6%) ≥grade 2 were observed.Single factor analysis showed that the V5-V40 of TL,V5-V/20 of IL,and V5-V50 of FL were all related to the occurrence of RILT,and multiple factor analysis showed that TL-V15 and FL-V20remained associated with RILT (P = 0.005 and P = 0.016).According to ROC analysis,the V10 (45.38%) of FL was the most sensitive predictor with a sensitivity rate of 90.0% and 1/25 (27.78%) of FL was the most specific predictor with a specificity rate of 90.24%.The sensitivity,specificity and accuracy of V20 of FL were 70.00%,73.17%,and 74.90%,respectively.Conclusions The occurrence of RILT is closely associated with multiple f-DVH parameters of FL,and f-DVH has good sensitivity and specificity for prediction.
10.Application of percutaneous nephroscopy in the treatment of upper urinary tract carcinoma

Objective To evaluate the application of percutaneous nephroscopy in the treatment of upper urinary tract transitional cell carcinoma, particularly renal pelvic carcinoma. Methods From June 2006 to June 2010, eight cases (with 10 sides) of renal pelvic carcinoma received percutaneous nephroscopy tumor resection. There were six males (with 7 sides) and two females (with 3 sides) in the study group. There were six cases with solitary kidney and two cases with bilateral renal pelvic tumors. There were four cases with high-grade tumors and six cases with low-grade tumors. The age of patients ranged from 52 to 72 yrs (average 61.2 yrs). Tumor sizes ranged from 0.5 to 3.5 cm (average 2.6 cm). Patients were treated with laser or electrocautery through percutaneous nephroscopy. A ureteral stent was placed in the patients after the procedure. Chemotherapy was administered postoperatively through the nephrostomy tube. Results All the operations were successfully completed uneventfully. The operative time was 45-95 min (average 73 min), estimated blood loss was 20-300 ml (average 50 ml). No remarkable differences were found in serum creatinine levels before and after operation. After 10 to 36 mon. follow-up by CT, MRI, and ureteroscopy, one patient died of tumor metastasis and two patients had local tumor recurrence. The remaining patients had no local recurrence. Conclusions Percutaneous nephroscopy in treating renal pelvic tumor is safe and feasible. This is a better choice for the renal pelvic carcinoma patients who are unsuitable for ureteronephrectomy.
