1.Effect and prognosis of nimodipine on the patients with severe traumatic brain injury

ObjectiveTo observe the therapeutic effect and prognosis of nimodipine on severe traumatic brain injury. Methods64 patients with severe traumatic brain injury were divided into the nimodipine group(32 cases) and the routine treatment group(32 cases). The Glasgow Coma Scale(GCS) was assessed before and after the treatment. The Activities of Daily Living(ADL) and cognitive ability were evaluated in clear-headed patients. After 6 months follow-up, the Glasgow Outcome Scale (GOS), Bathel index and Mini Mental State Examination (MMSE) score were carried on.ResultsIn both of the two groups, GCS score were increased distinctively after treatment. The MMSE score in nimodipine group was higher than that of routine treatment group. There was no statistic difference in GOS and ADL between two groups after 6 months, but MMSE in nimodipine group was higher than that of routine treatment group. Conclusions Nimodipine could be helpful in cognitive function. The prognosis of severe traumatic brain injury lied on the degree of cerebral damage.
2.Cloning and Sequencing of Envelope Gene of Subgroup J Avian Leukosis Virus

The envelope gene of ADOL-4817 strain of avian leukosis viru s subgroup J (ALV-J) was amplified by polymerase chain reaction (PCR) and clo ned into TA vector. The sequence analysis results showed that the envelope gene is composed of 1?746 bp, 1?554 bp of which could be translated into 517 amino acids for gp85 and gp37. The molecular weight of envelope protein is 57.7kD. T here are 15 potential glycosylation sites in the envelope protein, 13 of which i s located in gp85. Analysis of sequences of envelope gene indicate that ADOL -4817 showed high degree of sequence identity to other ALV-J strains, and m ost close ly related to the like-envelope gene of endogenous virus EAV-HP but divergent from these of other ALV subgroup A-E . These data support the hypothesis that envelope gene of avian leukosis virus subgroup J maybe acquired by recombination with expressed sequences.
3.Fluoride analysis of drinking water in endemic fluorosis areas in Shandong province from 2005 to 2007

Objective To investigate the distribution of water-borne fluoride and the current status of water defluoridation project by improving drinking water quality in endemic fluorosis areas in Shandong province,and to provide scientific basis for making strategies in prevention and control of the disease. Methods According to "the National Technical Scheme for Endemic Disease Control in 2004, 2005 and 2006", 113 endemic fluorosis diseased counties(cities, and districts) of the province's 17 cities were screened in order to investigate the fluoride level in drinking water in fluorosis villages, recheck the fluoride level after implementing the water defluoridation project, and investigate the current status of the water defluoridation project. The fluoride level in drinking water was determined by F-ion selective electrode. Results There were a total of 5816 water defluoridation projects in the province. Most of them were carried out by drilling a deep well to get under-ground water. The wells still in good condition were accounted for 72.80% (4234/5816). Intermittent operated wells were accounted for 3.11% (181/5816). Abandoned wells were accounted for 24.09%(1401/5816). Level of water fluoride was determined in 6940samples from fluorosis villages(villages that not carry out the water defluoridation project as well as villages carried out the water defluoridation project with abandoned wells were included) and the value that lower or equal to 1.00mg/L was determined in 2987 villages which accounted for 43.04% (2987/6940). Level of water fluoride that over 1.00 mg/L was found in 3953 villages which accounted for 56.96% (3593/6940), and the highest level of water fluoride was 11.33 mg/L. Level of water fluoride were determined in 4415 samples from water defluoridation project and the value lower or equal to 1.00 mg/L was in 2983 wells which accounted for 65.53%(2983/4415). The value over 1.00 mg/L was in 1522 wells which accounted for 34.47%(1522/4415), the highest value of water fluoride was 9.71 mg/L. Conclusions Level of water fluoride in up to 1/2 of the villages and 1/3 of the projects, is still higher than the standard in Shandong province. Nearly 1/4 of the project has been abandoned. The current situation for endemic fluorosis control is still not good in the province, countermeasures for endemic fluorosis must be carried out as soon as possible and surveillance of water defluoridation project must be strengthened.
4.Association of 8-hydroxyguanine glycosidase OGG1 Ser326Cys polymorphism with male infertility.

OBJECTIVETo explore the association of 8-hydroxyguanine glycosidase OGG1 Ser326Cys polymorphism with semen quality and the risk of male infertility.
METHODSThis case-control study included 620 idiopathic infertile patients and 385 normal fertile controls. We determined their genotypes by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and analyzed their semen quality by computer-aided semen analysis (CASA).
RESULTSThe individuals with OGG1 326 Cys/Cys showed significantly lower sperm motility and concentration ([52.1 +/- 26.7]% and (3.75 +/- 0.91) x 10(6)/ml, ln transformed value) than the Ser/Ser carriers ([59.0 +/- 21.8] % and (4.12 +/- 0.88) x 10(6)/ml, ln transformed value) (P < 0.05). The risk of male infertility increased 69% in the OGG1 326Cys allele carriers as compared with the Ser carriers (OR = 1.69, 95% CI: 1.24 -2.31).
CONCLUSIONOGG1 326 Ser/Cys polymorphism might contribute to the risk of male infertility in the southern Chinese population.

5.Action mechanisms of prolactin and its receptors on penile erection and ejaculation.

Prolactin is a polypeptide hormone which mainly acts on the reproductive system and plays an important role in penile erection and ejaculation. Prolactin receptors have a variety of short forms apart from the classic long form, which are widely expressed in male reproductive glands. High levels of prolactin can induce erectile dysfunction and results in secondary male infertility, which are mainly associated with the inhibition of dopaminergic activity, reduction of the testosterone level, and contraction of the cavernous smooth muscle. Moreover, low levels of prolactin can result in ejaculatory dysfunction. This article updates the views on the expressions of prolactin receptors in the male reproductive system, the effects of prolactin on penile erection and ejaculation, and its action mechanisms.

6.Clinical curative effect of laparoscopic left hepatectomy for primary hepatic carcinoma in 47 cases

Objective To evaluate the safety and feasibility of laparoscopic left hepatectomy in patients with primary hepatic carcinoma. Methods Retrospective analysis was conducted in clinical data of left hepatectomy from 2007 to 2014,including 47 cases of laparoscopic hepatectomy and 46 cases of open hepatectomy. The intraoperative and postoperative data of the two surgical methods were compared to evalu-ate whether laparoscopic left hepatectomy is safe and feasible. Results The tumor size were bigger in the open hepatectomy [(56. 57 ± 24. 56) mm vs. (64. 11 ± 33. 39) mm,P=0. 218]. The laparoscopic left hepatectomy resulted in shorter operation time [(217. 53 ± 60. 22) min vs.(306.80±119.91)min],andtherewasasignificantlydifference(P<0.05).Theintraoperativebloodlosswerelesserinthelaparo-scopic left hepatectomy [(350. 21 ± 197. 98) mL vs. (556. 74 ± 471. 41) mL],and there was a significantly difference (P<0. 05). The lap-aroscopic left hepatectomy had a smaller intraoperative blood transfusion rate (12. 8% vs. 32. 6%,P<0. 05). The length of ICU stay,time for gastroentestinal function recovery, postopetative hospital stay were shorter than those of open left hepatectomy (P<0. 05). There was no significant difference of postoperative complication rate between them (14. 9% vs. 23. 9%,P=0. 271). The survival rates of 1-and 3-year after operation in patients with laparoscopic left hepatectomy were 91. 5% and 83. 0% respectively,while 84. 8% and 76. 1% in patients un-derwent open hepatectomy. The tumor-free survival rates 1-and 3-year after operation were 74. 5% and 59. 6% in patients with laparoscopic left hepatectomy respectively,while 65. 2% and 54. 3% in patients underwent open hepatectomy. Conclusion Laparoscopic left hepatectomy for primary hepatic carcinoma is safe and feasible. Laparoscopic left hepatectomy could be a consideration as the standard surgical methods for hepatic carcinoma.
7.Clinical Analysis of 10 Children with Takayasu′s Arteritis

Objective To investigate the clinical features,treatment response and prognosis in children with Takayasu′s arteritis(TA) in order to improve the understanding of TA.Methods A retrospective study of 10 children with TA was performed.All of them were admitted and diagnosed in Peking University First Hospital from Jan.1998 to Oct.2008.The clinical features,laboratory tests,imaging modalities,treatment response and prognosis were all collected and evaluated.Results There were 3 boys and 7 girls in the 10 patients with TA,and the ratio of male to female was 12.3.The onset was from 4 months to 9 years old,with average age at 5.5 years old.The average duration of diagnosis was 7.6 months.The incidences of hypertension,vascular bruits,albuminuria,convulsion were present in 100%,100%,70% and 40%,respectively.The clinical types included typeⅡ(60%),type Ⅲ(10%) and type Ⅳ(30%).The acute phase inflammatory indices of activity such as erythrocyte sedimentation rate(ESR),C-reactive protein(CRP) and white blood cell(WBC) were not evidently increased.Tuberculosis infection was found in 6 out of 10 patients and anti-tuberculosis treatment was performed.Six patients were treated with steroids and 3 cases of them were also given immunosuppressives cyclophosphamide or methotrexate.Three of the 10 patients received anti-hypertensive and vasodilator.Two patients received percutaneous translurminal angioplasty and 1 patient received nephrectomy.One patient died of renal failure,heart failure and shock.Conclusions The patients with TA had high prevalence of tuberculosis infection,diagnosis as often late because of lack of specific clinical features at the acute inflammatory period.When organic ischaemia occurred,treatment response was usually unsatisfactory.Patients with multi-systemic and multi-viscera lesions should have comprehensive examination,especially for those with hypertension,pulseless and vascular bruits,in order to rule out TA.Early ultrasonography,computed tomography and magnetic resonnance image methods are valued in eariler diagnosis and they are the key factors to improve prognosis.

9.Clinical study on a randomized, double-blind control of Shenwu gelatin capsule in treatment of mild cognitive impairment.

OBJECTIVETo evaluate the effectiveness of Shenwu gelatin capsule (compound Chinese ginseng and fleeceflower root etc) in the treatment on senile mild cognitive impairment.
METHOD166 patients meeting criteria for mild cognitive impairment were selected from patients in Dongzhimen Hospital and were randomly assigned into a treatment group (n = 83 cases), given 5 capsules of Shenwu gelatin capsule, and a positive control group (n = 83 cases) , given 2 capsules of aniracetam with 3 placebo capsules. All subjects took this medication 3 times a day for 3 months, double-blind and double-moulding control were used in this study. At a baseline and end (three months later), all subjects were assessed using a battery consisting of MMSE and clinical memory scale (CMS).
RESULTBoth Shenwu gelatin capsule and aniracetam can remarkably increase the score of memory quotient (MQ), P < 0. 01. They have the similarly effectiveness, there are no statistical difference between the two groups in effectiveness of increasing memory scores.
CONCLUSIONShenwu gelatin capsule has certain effects on mild cognitive impairment.

10.A follow-up study on a randomized, single-blind control of King's Brain pills in treatment of memory disorder in elderly people with MCI in a Beijing community.

OBJECTIVETo evaluate the effect of King's Brain pills (Compound Chinese ginseng extract from herbs) on the treatment and the delaying of memory decline in the elderly with mild cognitive impairment (MCI) in a community by a year follow-up of neuropsychology.
METHOD75 patients with MCI were selected from Beixinqiao community of Beijing by a cutoff score of 0.0/0.5 on CDR and were randomly assigned into a treatment group (n = 30 cases), given 4 pills of a compound Chinese ginseng extract (King's Brain) with 2 placebo tablets, and a positive control group (n = 30 cases), given 2 tablets of Piracetam with 4 placebo tablets, as well as a placebo group (n = 15 cases), given a placebo of 4 tablets and 2 pills. All subjects took this medication 3 times a day for 3 months. Single-blind and double-moulding control were used in this study. At a baseline and a middlepoint (after 3 months), and a follow-up end (one year later) following a three months of medication therapeutics, all subjects were assessed using a battery consisting of MMSE and 5 memory items on BNPT battery.
RESULTIn the treatment group, MMSE score 27.50 +/- 1.68 was increased to 28.27 +/- 1.70 after 3 months but decreased to 26.90 +/- 1.90 after one year of the treatment. However, the latter score was higher than that in a placebo group 26.33 +/- 1.03 (P < 0.05). Verbal Learning Test score was significantly increased from 68.73 +/- 28.74 at baseline to 87.33 +/- 29.78 at follow-up point in the treatment group, which was significantly higher than that in the placebo group (P < 0.01). The total score of memory items on BNPT battery was significantly increased from 78.23 +/- 28.98 at baseline to 93.53 +/- 35.56 at follow-up point in the treatment group, higher than that in Piracetam group (P < 0.05) and the placebo group (P < 0.01).
CONCLUSIONBoth King's Brain spills and piracetam tablets have protective effect on cognitive and memory decline in elderly with MCI.
