3.Manifestation and diagnosis of 14 patients with systemic lupus erythematosus and centric nervous system infection

Objective The aim is to inquiring into the diagnosis and discrimination of systemic lupus erythematosus (SLE) complicated with centric nervous system (CNS) infection.Method The retrospective analysis of 14 patients with SLE and CNS infection was made.Results These patients were treated by corticosteroid for a long time before CNS infection.There were 4 patients with cryptococcal meningitis,3 with suppurative meningitis,5 with tuberculosis meningitis,1 with encephalopyosis and 1 with unclear diagnosis.Conclusion Pathogenic microbiological assay of cerebrospinal fluid is a reliable basis for diagnosis of SLE with CNS infection.The diagnosis of patients without pathogenic organism depends on original infection,manifestations,difference of cerebrospinal fluid and head CT.The differentiation of SLE complicated with CNS infection from SLE encepalophathy is important.
4.One stage treatment of acetabular fractures combined with ipsilateral femoral head or neck fractures with total hip arthroplasty

Objective To discuss method,operative characters and curative effect of total hip arthroplasty(THA)in treatment of acetabular fractures combined with ipsilateral femoral head or neck fractures.Methods One stage THA was done in seven patients with acetabular fractures combined with ipsilateral femoral head fracture(one patient)or femoral neck fractures(six patients)7-21 days after fractures.There were five males and two females,at age range of 41-65 years(average 50 years).According to AO classification,there were one patient with type A fracture,three with type B and three with type C.Of all,five patients received biological acetabular replacement after reduction and internal fixation of acetabular fracture and two received PMMA acetabular replacement with metal enforcement ring.All patients received biological femoral stem.Results Of all,six patients were followed up for from 2 years and 3 months to 6 years and 5 months(mean 3 years and 9 months),which showed heterotopic ossitlcation in one patient and dislocation in one who recovered after manipulative reduction.The Harris score was increased from mean prooperative 48 points to mean postoperative 91 points.X-ray found no loosening of prosthesis during follow-up.Conclusions THA can improve function of hip joint significantly,avoid further operation and lessen the burden of patients mentally and economically and hence is effective in treatment of acetabular fracture combined with ipsilateral femoral head or neck fractures.
5.Clinical Research on the Relationship between Type 2 Diabetes Mellitus and Colorectal Cancer

Objective: To investigate the relationship between type 2 diabetes mellitus (T2DM) and the pathogenesis and metastasis of colorectal cancer. Methods: A case-control study was performed to compare 852 colorectal cancer patients with 940 controls (patients without cancer) recruited from 2001 to 2006, with respect to their sex, cancer subsite, the course of T2DM, hepatic metastasis, smoking and drinking. Correlated risk factors were analyzed. Results: The risk of colorectal cancer was increased in patients with T2DM and the relative risk (OR) was 2.466. The OR of male patients was higher than that of female patients, but with no significant difference (2.775 vs 2.070, P=0.394). The incidence of T2DM in patients with left hemicolon cancer was higher than that in those with right hemicolon cancer and rectal cancer, but with no significant difference between them. The colorectal cancer risk in T2DM patients with a DM course of 10 ~ 20 years was the highest, and the OR was 4.696. The rate of hepatic metastasis was higher in T2DM patients with colorectal cancer than that in celorectal cancer patients without T2DM and the OR was 2.888. Conclusion: T2DM may be one of the important pathogenic risk factors for colorectal cancer. The OR is increased with the extension of DM course within 20 years. Colorectal cancer patients with T2DM may be more prone to hepatic metastasis.
6.Effects of dexamethasone sodium phosphate on the proliferation of in vitro cultured rabbit articular chondrocytes

BACKGROUND: Studies have demonstrated that dexamethasone can reduce type Ⅱ collagen and increase type I collagen in articular cartilage surface matrix, but the action mechanism of glucocorticoid to articular chondrocyte proliferation remains unclear. OBJECTIVE: To investigate the effects of dexamethasone sodium phosphate on rabbit articular chondrocytes cultured in vitro. DESIGN: Comparative observation.MATERIALS: New Zealand rabbits, 1 month old, were used for chondrocyte isolation and culture.METHODS: Rabbit articular chondrocytes cultured in vitro were randomly divided into control and experimental groups. Cells of the control group were cultured in DMEM media without dexamethasone sodium phosphate. Cells of experimental groups were cultured in DMEM media with different concentrations of dexamethasone sodium phosphate (0.02, 0.1, 0.5 g/L), respectively. MAIN OUTCOME MEASURES: Primary and passage chondrocytes were observed. Flow cytometry and immunocytochemistry were used to observe the effect of dexamethasone sodium phosphate on cell proliferation and type Ⅱ collagen synthesis. The ultrastructural changes of cultured chondrocytes were observed by transmission electron microscopy. RESULTS: The attachment and proliferation of experimental group chondrocytes was slower than control group. There average gray scale values of the experimental groups were significantly greater than the control group (P < 0.05). The cellular proportions of S phase and G_2+M phase of the experimental groups decreased but the cellular proportions of G0/G1 period increased. Under transmission electron microscope the amount of rough endoplasmic reticulum and mitochondria of the experimental groups decreased. CONCLUSION: Dexamethasone sodium phosphate inhibited articular chondrocyte proliferation, possibly due to the decrease of type Ⅱcollagen and protein synthesis ability of chondrocyte.
7.Late course accelerated hypofractionated three-dimensional conformal radiotherapy for stageⅢnon-small cell lung cancer

Objective Objective To evaluate the efficacy and comphcations of late course acceler- ated hypofractionated three dimensional conformal radiation therapy (3DCRT) for patients with stageⅢnon small cell lung cancer (NSCLC). Methods Sixty patients with stageⅢNSCLC were randomized into 2 groups: Late course accelerated hypofractionated 3DCRT group(group A—30 patients) and conventional fractionated radiation therapy group (group B—30 patients). In group A, 30 patients, at first, received a dose of 40 Gy at 2 Gy per fraction, 5 times a week, which followed by late course accelerated hypofractionat- ed 3DCRT with a dose of 16-20 Gy at 4 Gy per fraction, 3 times a week. In group B, 30 patients received a dose of 60-66 Gy at 2 Gy per fraction, 5 times a week. Chemotherapy, including vinorelbine and cisplatin, was given one cycle during radiotherapy and 3 cycles after radiotherapy for all patients. 8.Infectious Cloning Approach for Herpesvirus Based on Bacterial Artificial Chromosomes

The genetic analysis of herpesviruses has been a constant challenge, due to the large, complex genomes of herpesviruses and mutagenesis of viral genes by conventional recombination methods in cell culture. Recently, a completely new approach for full-length infectious clones of herpesviruses based on bacterial artificial chromosomes (BACs) has been developed. This technique allows the maintenance, propagation and genetic modification of the viral genome as a BAC plasmid in E.coli, thus making the procedures fast, safe and effective in prokaryotic cells. This technique also makes it possible for the reconstitution of viral progeny or mutants by transfection of the BAC plasmid into eukaryotic cells, thereby facilitating the analysis of viral gene functions in the context of genome. In this presentation, Epstein-Barr virus was used as an example to describe the principle, establishment of the technique and mutation introduction into the BAC plasmid, and to discuss the perspective in the use of BAC-cloned herpesviruses.
9.Analysis of coronary arteriographic and fluorescence fundus angiographic characteristics in type 2 diabetic patients with coronary heart disease

Objective To study the relationship between coronary arteriographic and fluorescence fundus angiographic characteristics in type 2 diabetic patients with coronary heart disease.Methods The study was carried out by the analysis of the data from coronary arteriography and fluorescence fundus angiography in 203 type 2 diabetic patients with coronary heart disease in different groups divided according to age or total cholesterol level. Logisitic regression analysis was applied to explore various risk factors to angiographic characteristics.Results With advancing age,there were more involvement of 3 coronary vessels or the left main branch along with stageⅢretinopathy,but less single vessel diseases in the coronary arteries and less stageⅠretinopathy.The difference in coronary angiographic and fluorescence fundus angiographic characteristics between groups with different total cholesterol levels was not significant.Logistic regression analysis suggested that coronary artery diaease was related to age,sex and blood glucose and triglyceride levels while diabetic retinopathy was related to blood glucose level and age.Conclusion There is great difference in coronary arteriography and fluorescence fundus angiography among different age groups.Aging may aggravate the lesions both in the coronary arteries and fundal vessels in type 2 diabetic patients with coronary heart diseease.
10.Bone grafting and internal fixation for treating fibrous dysplasia of the adult proximal femur A 13.case report

The present study retrospectively analyzed 13 patients with fibrous dysplasia of the adult proximal femur who received treatment at the Department of Orthopedics,Shenzhen Hospital,Peking University between March 2000 and June 2006.These patients comprised 6 males and 7 females,with an age of 21-35 years.Among 11 patients suffering from mild hip pain,6 presented with mild limping due to worsened pains,and 2 exhibited obvious manifestations of bone fracture.All patients underwent X-ray,CT examinations,and 3 were subjected to MRI examinations,prior to bone grafting and internal fixation.Monostotic lesion was observed in 10 patients and polystotic lesion in 3 patients,but endocrine disturbance was not observed in any patient.Nine patients suffered from lesions in the femoral neck and 4 from lesions in the femoral rotator.Curettage,autogenous and allogeneic bone grafting,and internal fixation with dynamic hip screws and proximal femoral nails were successfully performed in each patient.Internal fixation time averaged 165 minutes (range 120-210 minutes).During the early stage after surgery,weight-bearing activities were not permitted.Bone fracture healed after an average of 3 months.Eighteen to forty-eight months of follow-up demonstrated that clinical symptoms were relieved,with good hip joint function and basically normal gait.X-ray plains showed thickened cortical bone,as well as compacted ossification shadow in bone grafting region.In addition,partial bone absorption,but not recurrent clinically,was observed in 4 patients.These findings imply that thorough curettage,autogenous and allogeneic bone grafting,and internal fixation with dynamic hip screws and proximal femoral nails is a reliable method to treat fibrous dysplasia of the adult proximal femur and it can acquire satisfactory curative effects.
