1.Inhibitory effects of LY267108 on nuclear factor kappa B during osteoclast activation

BACKGROUND:No ideal drugs can be used in the prevention and treatment of aseptic loosening of artificial joints. Some researchs showed that erythromycin has strong inhibitory effects on periprosthetic osteolysis. Its antibacterial activity, however, limits its application in artificial joint loosening prevention. LY267108 is a new type of erythromycin derivatives, eliminates the antibacterial activities, and retains the anti-inflammatory activity.
OBJECTIVE:To evaluate inhibitory effect of LY267108 on nuclear factor kappa B during osteoclast activation.
METHODS:RANKL and macrophage colony-stimulating factor were added to RAW264.7 cellline of a mouse model induced by osteoclasts. Simultaneously, different concentrations of alendronate sodium, erythromycin and LY267108 were cocultured for 48 hours. The activity of nuclear factor kappa B and content of intracytoplasmic inhibitory subunit of nuclear factor kappa B alpha were measured by electrophoretic mobility shift assay and western blot assay.
RESULTS AND CONCLUSION:LY267108 has a strong inhibitory effect on nuclear factor kappa B. 10 mg/L LY267108, 25 mg/L erythromycin and 10 mg/L alendronate sodium had similar inhibitory effects on nuclear factor kappa B, which was obviously stronger than 10 mg/L erythromycin. However, 25 mg/L LY267108 had strongest inhibitory effects. No significant difference in intracytoplasmic inhibitory subunit of nuclear factor kappa B alpha levels was detected among 10 mg/L LY267108, 25 mg/L erythromycin and 10 mg/L alendronate sodium groups, but was stil apparently higher than 10 mg/L erythromycin group. Levels of intracytoplasmic inhibitory subunit of nuclear factor kappa B alpha were highest in the 25 mg/L LY267108 group. Results indicated that LY267108 in the process of osteoclast activation had stronger inhibitory effects on nuclear factor kappa B compared with erythromycin, and its safety was higher than alendronate sodium. Simultaneously, LY267108 did not have antimicrobial activity, and became a potential ideal drug for prevention and treatment of aseptic loosening of artificial joints. However, the inhibitory effects of LY267108 on the degradation of inhibitory subunit of nuclear factor kappa B alpha would be a mechanism of inhibiting the activation of nuclear factor kappa B.

5.Clinical Experience of Diagnosis and Medicine Therapy of Aortic Dissection (A Report of 20 Cases)

Objective To investigate the clinical manifestations and diagnosis and treatment of aortic dissection(AD).Methods The clinical data of 20 patients with aortic dissection were analyzed retrospectively.Results 20 patients were diagnosed by CT, MRI and colour ultrasonography (5,5,10 cases, respectively). According to DeBakey typing, this group of patients was composed of type I(11 cases, 55%), type II(2 cases, 10%)and type III (7 cases, 35%). All patients were treated with internal medicine ,average time in hospital stay was(29 8?25 5)days. The patients' conditions were improved in 19 person-times (76%). 4 patients died (20%) and 2 patients abandoned treatment (8%).Conclusions The accurate diagnosis as early as possible and active therapy was a key of improvment of prognosis in patients with aortic dissection.
6.Treatment of diabetic peripheral neuropathy

Diabetic peripheral neuropathy (DPN) is the most common complication of diabetes mellitus in nervous system. However, the pathogenesis is still unkown. Therapeutic options are mainly pathogenetically oriented therapy and symptomatic therapy. With an increasing understanding of the pathogenesis of DPN, many new therapies have emerged. Comprehensive therapy targeting different pathogenesis will be frequently applied in the future. In this article, the common therapy and advance in treatment of DPN are reviewed.
7.The value of transcutaneous oxygen saturation in early diagnosis of newborns with critical congenital heart disease

Objective To study the value and significance of transcutaneous oxygen saturation monitoring from different parts in early diagnosis of neonatal critical congenital heart disease. Methods The transcutaneous oxygen saturation in right upper extremities and right lower extremities of neonates born in our hospital from July 2010 to July 2015 were detected at 2 hours to 24 hours after birth. If the transcutaneous oxygen saturation ≤ 0. 95 or difference between upper and lower extremities≥0. 03, color doppler ultrasound echocardiography were done. Results There were 24 385 cases of newborns in our study. 1 680 newborns with abnormal transcutaneous oxygen saturation ( ≤0. 95 or difference between upper and lower extremities ≥0. 03) were examined with color doppler ultrasound echocardiography. 204 cases of congenital heart disease (12. 1% ) contain 9 cases of critical congenital heart disease ( 0. 5% ) were detected. The critical congenital heart disease included complete transposition of great artery ( n = 3), tetralogy of Fallot ( n = 4), artery malformation ( n = 1), and pulmonary atresia with right ventricular dysplasia ( n = 1 ) . Conclusions Transcutaneous oxygen saturation monitoring from different parts has a high value for early detection of critical congenital heart disease in the newborn. It is recommended for clinical use in grass-roots hospitals.
8.Relationship between Plasma Brain Natriuretic Peptide,Endothelin-1 and Left Ventricular End Diastolic Vo-lume Index,Left Ventricular Ejection Fraction in Children with Heart Failure
