AIM:To study the effect of different surgeries for vision and postoperative complications in leprosy patients with lagophthalmos.METHODS:A retrospective analysis.Totally 68 leprosy patients (97 eyes) with lagophthalmos were selected who treated with different surgeries during May 2007 to September 2015 in our sanatorium.The surgeries included nylon thread correction surgery, tarsorrhaphy, temporalis transposition surgery.We observed and compared the best corrected visual acuity(BCVA) and the rate of postoperative complications, such as epiphora, lacrimal duct injury, hard to open eyes, infection at the inner and lateral canthus and scar formation of lacrimal apparatus.RESULTS:The effect on BCVA of patients treated with nylon thread correction surgery and tarsorrhaphy:the preoperative and postoperative BCVA was significantly different of the two groups (Z=-4.193,-4.213;P<0.05);the preoperative and postoperative BCVA was not significantly different of the patients underwent temporalis transposition surgery (Z=-1.532, P >0.05).The postoperative complications:(1) epiphora:there was 91% in patients underwent nylon thread correction surgery, 89% in patients underwent tarsorrhaphy, 59% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=12.198, P<0.05);(2) lacrimal duct injury:there was 66% in patients underwent nylon thread correction surgery, 95% in patients underwent tarsorrhaphy, 41% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=22.415, P<0.05);(3) hard to open eyes:there was 56% in patients underwent nylon thread correction surgery, 55% in patients underwent tarsorrhaphy, 22% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=8.795, P<0.05);(4) infections:there was 84% in patients underwent nylon thread correction surgery, 3% in patients underwent tarsorrhaphy, 11% in patients underwent temporalis transposition surgery, the difference was statistically significant (χ2=60.858, P<0.05).CONCLUSION:There are certain clinical efficacy for lagophthalmos caused by leprosy after surgery, especially patients after temporalis transposition surgery can close eyes by oneself.And their vision does not change significantly, which suggests that this surgery may prevent exposure injury for cornea.But the iatrogenic injury, the vision decrease in patients underwent other surgeries and the ocular tissue damage, such as epiphora, lacrimal duct injury, hard to open eyes, infection at the inner and lateral canthus and scar formation of lacrimal apparatus should be pay more attention to.

OBJECTIVETo study the blood enriching effects of Paeoniae Radix Rubra and Paeoniae Radix Alba, paeoniflorin and albiflorin on mouse model of blood deficiency caused by γ-ray radiation.

METHODBuild mouse model of blood deficiency induced by γ-ray radiation. Paeoniae Radix Rubra and Paeoniae Radix Alba were given during modeling. The amount of WBC was detected af- ter the treatment. Based on the result of WBC and paeoniflorin content, albiflorin content in Paeoniae Radix Rubra and Paeoniae Radix Alba, the same model and the same method were used to comparatively study the effect of blood enriching of paeoniflorin and albiflorin.

RESULTOn the 7th day, the amount of WBC in model mice treated with 2 g x kg(-1) Paeoniae Radix Alba and 2 g x kg(-1) Paeoniae Radix Rubra significantly increased compared with that of model group (P < 0.05). In another experiment with the same model, the amount of WBC in model mice treated with 120 mg x kg(-1) paeoflorin and 120 mg x kg(-1) albiflorin significantly increased (P < 0.05) compared with that of model group on the 7th day. On the 10th day, the amount of WBC in rats treated with 120 mg x kg(-1) paeoflorin increased significantly (P < 0.05) compared with that of model group. Compared with the same dose of paeoniflorin, the amount of WBC in mice treated with albiflorin had no significant difference.

CONCLUSIONAll Paeoniae Radix Alba, Paeoniae Radix Rubra, paeoniflorin and al- biflorin can raise the amount of WBC and have the effect of enriching blood induced by radiation, while paeoniflorin and albiflorin have a similar result in this model. The result indicated that both paeoniflorin and albiflorin are effective constituents in Paeoniae Radix Alba, and paeoniflorin work as the common effective constituent in both Paeoniae Radix Rubra and Paeoniae Radix Alba.

Objective To strengthen special knowledge training in newly engaging-system nurses in laminar air flow cleaning ward so as to improve nursing quality.Methods Those measures such as establishing prospective target,standardizing training content,enhancing clinic technician and theoretic knowledge tutoring by experienced nurses,paying attention to looking ward and proposing nursing questions,strengthening rehearsing in clinic and periodical examination were put forward so as to ensure all-sided training in newly engaging-system nurses in laminar air flow ward.Results The newly engaging-system nurses have poor speciality knowledge on laminar air flow word.Mter three months' training,their speciality knowledge had improved a lot,and the score on speciality knowledge was significantly higher than that of pre-training.Conclusions Speciality knowledge training should be strengthened in newly engaging-system nurses so as to ensure improvement of nursing quality.

OBJECTIVETo study the protective effect of Acai berries(Euterpe oleraceae) on chronic alcoholic hepatic injury in rats and their mechanism.

METHODWistar rats were fed for 1 week and randomly divided into blank group, model group, Dongbao Gantai group, Acai 1.6, 0.8, 0.4 g . kg-1 groups. The blank group was given distilled water, and the other groups were orally given 56% white spirit (Erguotou) for eight weeks at the dosages of 8 mL . kg-1 in the 1st week, which increased by 0.1 mL week by week till to 15 mL . kg-1, in order to establish the chronic hepatic injury model, and observe the effect of Acai berry freeze-dried powder on hepatocyte membrane permeability, liver lipid peroxidation, changes in inflammatory cytokines and pathological changes in hepatocytes.

RESULTAcai berries could significantly reduce serum ALT and AST(P<0.05), MDA(P<0.05), TG(P<0.05) and serum TNF-α and IL-6(P <0.05) and increase GSH and SOD(P <0.05). According to liver histopathological observation, livers in the model group were dominated by steatosis, some livers suffered spotty necrosis and inflammatory cell infiltration; The positive drug and Acai groups showed different changes in pathologic changes in rat livers.

CONCLUSIONAcai berries show s specific protective effect on alcoholic hepatic injury. Its mechanism may be correlated with the inhibition of such inflammatory factors as TNF-α and IL-6.

Objective To investigate the effects of alcohol extract of bark and male flower of Eucommia ulmoides Oliv. on airway allergic inflammation induced by chicken ovalbumin (OVA) in mice; To explore its mechanism of action. Methods On day 0, day 7, mice were intraperitoneally injected OVA for sensitization, followed by nasal stimulation for 21 days to establish airway allergic inflammation mice models. The mice were divided into normal group, model group, alcohol extract of bark of Eucommia ulmoides Oliv. group, alcohol extract of male flower of Eucommia ulmoides Oliv.group,and Dexamethasone group.Each medication group was given relevant medicine for gavage. The lung tissue was embedded in HE and PAS dyeing, to observe the pathological changes of bronchus and surrounding lung. The levels of serum OVA-IgE, IL-4, IFN-γ and IL-13 were measured by ELISA. The expression of ICAM-1, VEGF, MMP9 and TIMP1 were detected by immunohistochemistry. Flow cytometry was used to detect the expression of Th17 cells in peripheral blood. The expressions of TNF-α and IL-6 mRNA in lung tissue were detected by RT-PCR. Results The model group showed changes of airway allergic inflammatory such as eosinophils and other inflammatory cell infiltration, bronchial spasm, and mucus secretion. Lung histopathology in alcohol extract of bark and male flower of Eucommia ulmoides Oliv.groups was improved significantly(P<0.05).Compared with the normal group, the levels of serum OVA-IgE, IL-4 and IL-13 increased in model group, while the level of IFN-γ decreased (P<0.05, P<0.01). The expressions of ICAM-1, VEGF and MMP9 increased, while the expression of TIMP1 decreased (P<0.01); peripheral blood IL-17+cells increased (P<0.01); the expressions of TNF-α and IL-6 mRNA increased. Compared with the model group, the levels of serum OVA-IgE, IL-4 and IL-13 decreased in alcohol extract of bark and male flower of Eucommia ulmoides Oliv. groups (P<0.05, P<0.01); the expressions of ICAM-1 and VEGF decreased (P<0.05, P<0.01); the expression of TIMP1 increased. Alcohol extract of bark and male flower of Eucommia ulmoides Oliv.could down-regulate IL-17+cells,reduce the expression of IL-6 mRNA(P<0.05,P<0.01). Alcohol extract of bark of Eucommia ulmoides Oliv. group could induce the secretion of IFN-γ (P<0.01), and down-regulate the expression of TNF-α mRNA(P<0.05).Alcohol extract of male flower of Eucommia ulmoides Oliv. group could significantly down-regulate the expression of MMP9 (P<0.05). Conclusion Alcohol extract of bark and male flower of Eucommia ulmoides Oliv.can induce the production of OVA-IgE,inhibit secretion of Th2 cytokines, inhibit the expression of adhesion molecules, depress Th17 cells, so as to inhibit the airway allergic inflammation.

Objective To retrospectively analyze medical data of patients with colorectal cancer (CRC) so as to provide evidence for clinical use of opportunistic screening.Methods A total of 2450 CRC patients (male 1377,female 1073) who were treated at five hospitals in North China during October 2001 and September 2011 and had complete medical records and pathological results were recruited.The correlations of incidenceofCRCwithage,gender,tumorlocationandhistologicaltypeswere analyzed.Results Of all the CRC patients,those less than 50 years old accounted for 18.14％ ; and the incidence of CRC was substantially increased in those over 50 years old.Seventy-three percent of tumor occurred at the rectum and sigmoid colon,6％ at descending colon,7％ at transverse colon and 14％ at ascending colon.Moderately,well or poorly differentiated adenocarcinoma accounted for 50.33％,40.35％and 9.32％,respectively.Histological differentiation was not correlated with age and gender ( P ＞ 0.05 ).Conclusions Age and gender should not be considered a determination of opportunistic screening for CRC.Colonoscopy is recommended as an alternative CRC screening procedure.

ObjectiveTo observe the clinical efficacy of hyperbaric oxygen (HBO) in treatment of diabetes complicated with acute cerebral infarction (ACI) and its effect on insulin resistance (IR).Methods One hundred and twenty-eight patients with diabetes complicated with ACI were divided into control group and treatment group with 64 cases each by table of random digit.The control group was treated with routine medication,while the treatment group was treated with HBO besides routine medication.The two groups were treated for 14 days.The neural function defect degree score,fasting blood glucose(FPG),fasting insulin (FINS) and insulin action index(IAI) were assessed before and after treatment in each group.Correlation between the difference value of IAI and neural function defect degree score was analyzed with Pearson correlation analysis.ResultsThe total effective rates in treatment group and control group were 87.50％ (56/64) and 70.31％(45/64) respectively,with a significant difference between two groups (P＜ 0.05).IAI after treatment in control group and treatment group showed significant difference compared with that before treatment(P＜ 0.05 or ＜ 0.01 ),but IAI after treatment in treatment group and control group had significant difference (-4.03 ± 0.51 vs.-4.22 ± 0.55,P ＜ 0.05).There was negative correlation between the difference value ofIAI and neural function defect degree score(r =-0.696,P ＜ 0.01 ).ConclusionHBO may increase the clinical efficacy in treatment of diabetes complicated with ACI,and also it improves IR significantly.
