Objective: To investigate the clinical efficacy and safety of compound bismuth potassium citrate capsules(CBPCC) combined with omeprazole capsules in eradication of Helicobacter pylori (H. pylori) infection. Methods: A total of 240 patients with gastric H. pylori infection were enrolled in this double-blinded, randomized controlled study. Patients in group A(control group) were given a combination of bismuth potassium citrate/tinidazole/clarithromycin tablets and ormprazole capsule(n= 120); those in group B(treatment group) were given compound bismuth potassium citrate capsules and ormprazole capsules (n = 120). H. pylori eradication rates and clinical safety of both treatment regimens were analyzed using SAS 6.12 software system. Results: A total of 236 patients from 6 resreach centers completed in this study, with 117 in group A and 119 in group B. Two patients in the contol group and 8 in the treatment group did not complete the experiment; six patients in the control group were excluded due to other reasons. There were 236 patients entered full analysis sets(FAS), 220 entered per protocol set (PPS), and 236 entered safety analysis sets (SS). FAS showed that the eradication rates of H. pylori were 84.87% in group B and 74.36% in group A(P<0.05); PPS showed the eradication rates were 76.15% in group A and 90.09% in group B (P<0.05). Intragroup comparison showed that the symptoms of patients in both groups were effectively improved; intergroup comparison showed that the symptom improvement rates were similar between the 2 groups. The rates of adverse events were 14.53% in group A and 14.29% in group B(P > 0.05), with no severe adverse event found in both groups. The treatment regimen in group B was more cost-effective than that in group A(P<0.05). Conclusion: This study demonstrates that compound bismuth potassium citrate capsules combined with omeprazole capsules have a high efficacy in eradication of H. pylori infection, and it is safe, cost-effective and worth popularizing in clinic.

Objective To evaluate the inhibitory effect of small interfering RNA (siRNA) targeting peroxisome-proliferator-activated receptor-γ coactivator-1α (PGC-1α) on retinal neovascularization in the mouse.Methods Eighty seven-day-old C57BL/6J mice were divided into normal group,model blank group,model control group and PGC-1α siRNA group,twenty mice in each group.Mice in the normal group were kept in normal room air.Mice in the model blank group,model control group and PGC-1α siRNA group were induced for retinal neovascularization by hypoxia.Liposome with PGC-1α siRNA (1 μl) and liposome with negative control siRNA (1 μl) were injected into the vitreous in the PGC-1α siRNA group and model control group respectively when mice were moved out to room air from the cabin (Postnatal 12).No injection were performed in the model blank group.At postnatal 17,fluorescein angiography was used to assess the vascular pattern.The proliferative neovascular response was quantified by counting the nuclei of new vessels extending from the retina into the vitreous in cross-sections.PGC-1α and vascular endothelial growth factor (VEGF) level in retina were measured by real-time polymerase chain reaction (real-time PCR) and Western blot.Inhibition efficiency of PGC-1α siRNA on PGC-1α and VEGF was calculated.Results Mice in the normal group showed reticular distribution of retinal blood vessels.Central nonperfused retina,neovascular tufts and fluorescein leakage were seen in the model blank group and model control group.Neovascular tuft and fluorescein leakage were decreased in the PGC-1α siRNA group compared to the model blank group and model control group.The neovascular nuclei were increased in the model blank group and model control group compared to the normal group (P＜0.05).The neovascular nuclei were decreased in the PGC-1α siRNA group compared to the model blank group and model control group (P＜0.05).The expression of PGC-1α mRNA and protein in retina was increased significantly in the model blank group and model control group as compared with normal group,while decreased 54％ and 53％ respectively in the PGC-1α siRNA group as compared with model blank group and model control group (P＜0.05).The expression of VEGF mRNA and protein in retina was increased significantly in the model blank group and model control group as compared with normal group,while decreased significantly in the PGC-1α siRNA group (decreased 48 ％ and 40 ％ respectively) as compared with model blank group and model control group (P＜0.05).Conclusions Intravitreal injection of PGC-1α siRNA mediated by liposome can inhibit retinal neovascularization in the mouse effectively.

AlM: To investigate the effect of exogenous peroxisome-proliferator-activated receptor-γcoactivator-1α ( PGC-1α) on vascular endothelial growth factor ( VEGF) expression in human retinal vascular endothelial cells ( HRVEC) .METHODS:Recombinant PGC-1α protein was added to HRVEC, and no recombinant PGC-1α protein was added to HRVEC as control group. After 24h of incubation, two groups of cells were then placed into a normoxic ( 20%O2 ) or hypoxic ( 1% O2 ) environment for another 16h. The expression of VEGF mRNA and protein were detected by real - time PCR, ELlSA and immunofluorescence cytochemistry.RESULTS: VEGF mRNA and protein levels in the cells were significantly increased by recombinant PGC - 1αprotein both under normoxia and hypoxia conditions as compared with control groups (P<0. 01).CONCLUSlON: PGC-1α can upregulate the expression of VEGF in HRVEC under normoxia and hypoxia conditions.

Objective To investigate the difference of IOL Master and A-ultrasound combined with corneal topography meas-urement in intraocular lens ( IOL) power calculation .Methods A total of 84 eyes of 60 cataract patients received phacoemulsification and intraocular lens implantation surgery were selected in Xiangya Hospital from March 2010 to September 2011.There were 57 eyes of 42 age-related cataract patients and 27 eyes of 18 complicated cataract patients .The patients were divided into IOL Master group and A ultrasound combined with corneal topography measurement group .Before surgery , in IOL Master group , axial length ( AL) and corneal curvature were measured with IOL Master , IOL power was calculated according to the Haigis , SRK/T formula and so on.At the same time the axial length ( AL) and corneal curvature were measured with A ultrasound combined with corneal topography respectively and IOL power was calculated by SRK/Ⅱ formula.In A ultrasound combined with corneal topography measurement group , axial length ( AL) and corneal curvature were measured with A ultrasound combined with corneal topography respectively , IOL power was calculat-ed according to the SRK/Ⅱformula.3 months postoperatively , all the patients were conducted refractive outcome and calculating mean absolute refractive error(MAE).Finally the data were analysed.Results ⑴In IOL Master group, before operation there was no sig-nificant difference in mean axial length between IOL Master and A-ultrasound measurement ( P >0.05 ) , while in the patients with AL>26 mm the axial length was (28.53 ±0.57)mm and (29.42 ±0.64)mm using IOL Master and A-ultrasound measurement respec-tively ( P <0.05).The mean corneal curvature was (42.12 ±0.31)D and (43.09 ±0.27)D using IOL Master and corneal topogra-phy measurement respectively ( P <0.01).The mean IOL power were (17.06 ±0.48)D and (16.37 ±0.56)D in IOL Master group and A ultrasound combined with corneal topography measurement group respectively ( P <0.05 ) .⑵3 months postoperatively , the MAE was (0.07 ±1.05)D and (0.16 ±0.81)D in IOL Master group and A-ultrasound combined with corneal topography measure-ment group( P <0.05) respectively.Conclusions In cataract patients with normal axial length , IOL Master and A-ultrasound com-bined with corneal topography have high consistency for axial length measurement and IOL power calculation .But in cataract patients with high myopia , IOL Master is more accurate for axial length measurement and IOL power calculation compared to A -ultrasound com-bined with corneal topography .

Objective To measure the daily nursing workload by simplified therapeutic intervention scoring system and provide information for reasonable dynamic management of nursing manpower and improving nursing service satisfaction of patients in elderly ICU.Methods 113 patients in elderly ICU were selected from July 2012 to June 2013.The Therapeutic Intervention Scoring System 28(TISS-28) was used to measure the nursing workload,reasonably arranged the nursing staff according to the daily total nursing workload.The average daily working time and service satisfaction were compared before and after the application of dynamic management of nursing manpower.Results After the application of dynamic management of nursing manpower,the average daily working time of nurses decreased,satisfactory degree of patients with nursing service increased,and satisfactory degree of medical workers with nursing manpower management also increased.Conclusions Measurement of nursing workload by TISS-28 can provide the basis for the dynamic management of nursing manpower in elderly ICU and improve satisfaction degree of medical workers.

BACKGROUND:Previous study has observed that the calf acellular dermal membrane exhibits slow repair efficiency, fast degradability speed and other shortcomings in the repair of cartilage defects. OBJECTIVE:To investigate the repair effect of the col agen type Ⅱ-modified acel ular dermal membrane on cartilage defects in rabbits. METHODS:The fetal rabbit chondrocytes were seeded onto the col agen type Ⅱ-modified acel ular dermal membrane, and the composite was then observed under scanning electron microscope at 3, 7 and 14 days. Cartilage defect models were established on the bilateral femoral condyles of 24 New Zealand white rabbits, and these model rabbits were randomly allocated to three groups. The cartilage-acellular dermal membrane and cartilage-collagen type Ⅱ-modified acellular dermal membrane were implanted into the defect regions of control and experimental groups, respectively. Those received no intervention were as blank control group. Collagen type Ⅱ immunohistochemical staining and Wakitani scoring system were performed at 6 and 12 weeks postoperatively. RESULTS AND CONCLUSION:Chondrocytes grew and adhered well in the scaffold. The Wakitani scores in the experimental group were significantly lower than those in the control and blank control groups at postoperative 6 and 12 weeks (P<0.05). At 6 and 12 weeks postoperatively, collagen type Ⅱ immunohistochemical staining was the strongest in the experimental group, with yellow and brown particles in the cytoplasm;the control group was positive for collagen type Ⅱ immunohistochemical staining, while the blank control group was negative for the staining. Our findings suggest that the collagen type Ⅱ-modified acellular dermal membrane is beneficial for the repair of cartilage defects.

Obstructive sleep apnea-hypopnea syndrome (OSAHS) is a common clinical disease that seriously threatens human health and life. Accurate location of the upper airway obstruction is the key to the diagnosis and treatment of OSAHS. Acoustic pharyngometry uses sound reflection to quickly assess the cross-sectional area and volume of the upper airway. Acoustic pharyngometry represents a simple, quick, non-invasive method for measuring upper airway dimensions which could predict sleep apnea risk. In this article we sought to introduce the application of acoustic pharyngometry in the diagnosis and treatment of OSAHS.

Objective To investigate cognitive status of medical students of a medical university on laboratory biosafety,and provide basic data for laboratory biosafety management in Chinese universities.Methods 900 full-time undergraduate medical students were chosen by cluster random sampling,questionnaires were filled out in by them.Results 900 questionnaires were distributed,877 (98.21％) valid questionnaires were obtained,49.03％ (n =430) were from sophomores,50.97 ％ (n =447) from juniors,148 (16.88 ％) students have ever participated in students'scientific research.The overall awareness rate of laboratory biosafety was 58.72％,only 32.16％ of students understood the detailed contents of laboratory biosafety regulations,only 8.21％ of students have received training in laboratory biosafety;the awareness rate of laboratory biosafety cabinet was only 14.14％,only 7.75％ of students knew which operation should be performed in biosafety cabinet;28.28％ of students could deal with waste according to the rules,68.19％ of students were able to identify warning signs of biological hazard;92.82％ of the students thought that laboratory biosafety-related courses should be set up.The overall awareness rate of laboratory biosafety knowledge and safety behavior was low,which were 42.65％ and 41.96％ respectively,juniors was higher than that of sophomores(P＜0.05);in the aspect of chemical hazards and biological hazards,students with scientific research experience scored higher than those who did not participate in scientific research(all P＜0.05).Conclusion Medical students' cognition on knowledge of laboratory biosafety is not optimistic,it is imperative to strengthen the management of education and publicity of laboratory biosafety.

Objective To probe the value of ultrasound in detection of malignant foci among multiple thyroid nodules. Methods Ultrasonograms of 83 patients with pathologically confirmed thyroid cancer combined with benign nodules were analyzed.Results Among 83 patients of thyroid carcinoma, 89 malignant and 87 benign nodules were detected with ultrasound. Significant differences of two-dimensional ultrasounic signs including low echoes, border fuzzy, micro-calcification and rear echo attenuation were found bteween malignant and benign nodules (P<0.01). There were significant differences in the grade Ⅱ, Ⅲ blood flow between malignant and benign ones (P<0.01). Of 89 malignant nodules, small thyroid carcinoma with diameter ≤1 cm accounted for 52.81% (47/89), and ultrasound diagnostic rate of all cancer nodules was 67.42% (60/89).Conclusion Ultrasounic characteristics including low echoes, border fuzzy, micro-calcification, rear echo attenuation and internal rich blood flow contribute to differentiate the thyroid microcarcinoma from multinodules.In particular, the microcarcinoma should not be ignored.

Objective To explore the diagnostic value of DTI for prostate cancer.Methods From October 2009 to December 2010,44 patients suspected of prostate cancer received MRI and DTI.The data of MRI and DTI were analyzed retrospectively.By histopathology,prostate cancer was proved in 16 patients,and benign prostatic hyperplasia ( BPH ) was proved in 28 patients.Differences in ADC and FA values between prostate cancer and BPH were compared by independent samples t test.Diagnostic accuracy of FA value and ADC value for prostate cancer was analyzed by using ROC curve,and the diagnostic threshold of FA value and ADC value for prostate cancer was determined.Results The mean FA value of the tumor regions and BPH were 0.308 +0.084 and 0.203 ±0.029,respectively.The mean ADC value of the tumor regions and BPH were (0.883 +0.192) × 10 -3 mm2/s and ( 1.408 ±0.130) × 10-3 mm2/s,respectively.There were statistically significant differences in ADC and FA values between tumor regions and BPH (t values were 4.833 and 10.779 respectively,P＜0.01).The ADC value area under curve of ROC was 0.996 (95％ CI was 0.984 to 1.007) ; the FA value area under curve of ROC was 0.904(95％ CI was 0.812 to 0.996) ; Combined the FA and ADC value area under curve of ROC is 0.996(95％ CI was 0.984to 1.007) ; Using the ADC value of 0.725 × 10 3 mm2/s as the ROC cut off point,the diagnostic sensitivity and specificity were 100.0％ and 96.0％,respectively; Using the FA value of 0.311as the ROC cut off point,the diagnostic sensitivity and specificity was 100.0％ and 68.7％,respectively.Conclusion DTI imaging can provide valuable information for prostate cancer diagnosis and differential diagnosis,and improve the diagnosis ability of prostate cancer.
