1.Clinical manifestations of pyogenic liver abscesses of cryptogenic origin

Total 118 patients with pyogenic liver abscess admitted to Zhongshan Hospital were classified into pypgenic liver abscesses of cryptogenic origin (PLAC,n =54),pyogenic liver abscesses of biliary origin (PLAB,n =48) and pyogenic liver abscesses of hemagenous origin (PLAH,n =16)according to the causes.The clinical features of three groups were compared and analyzed.The results showed that no statistically significant difference existed among three groups in clinical manifestation and signs.Compared to PLAB,the abscesses in right lobe of liver were more common in PLAC,diabetes mellitus was more prevalent in PLAC,but the co-existing diseases including chronic liver disease,non biliary malignant tumor and ALP elevation were less in PLAC.Klebsiella pneumoniae was frequently detected in PLAC,but there was no significant difference comparing to other two groups.No significant difference was found among three groups in the choice of therapeutic methods,the course of antibiotics,length of hospital stay and the days from admission to diagnosis.
2.Pyogenic liver abscess: a clinical analysis of 118 cases

The clinical data of 118 patients with bacterial liver abscess admitted to a tertiary hospital from January 2007 to December 2009 were analyzed retrospectively.Etiology study showed that 45.8%(54/118) cases of bacterial liver abscess were cryptogenic,40.7% (48/118) were biliary source and 13.5% (16/118) were hematogenous.The positive rate of pus culture was 57.1% (36/63),among which klebsiella pneumoniae accounted for 72.2% (26/36).The positive rate of blood culture was 31.2%(20/64) and klebsiella pneumoniae accounted for 95% (19/20).One hundred and sixteen patients (98.3%) underwent color ultrasound examination with an accuracy rate of 93.2%.84 patients (71.2%)underwent the CT scan with an accuracy rate of 95.3% and 20 patients underwent MRI with an accuracy rate of 85%.Seventy five patients (63.6%)accepted ultrasound-guided percutaneous aspiration and catheter drainage combined with antibiotic therapy; 40 patients(33.9%)were treated with antibiotics alone.On the discharge 27 patients(22.9%) achieved clinical cure,87 patients (73.7%) were classified as effective,4 patients (3.4%) had no effect and no death in this group of patients.The average length of hospital stay was 16 days.Cryptogenic was the most common cause of bacterial liver abscess,biliary source took the second place.Kleibsiella pneumoniae was the predominant organism.Detailed medical history inquiry,physical examination and assistant examination may increase the accuracy rate of diagnosis.Needle biopsy should be performed if necessary.Ultrasound-guided percutaneous aspiration and catheter drainage combined with antibiotics should be the first-line treatment.

5.The Study of Body Position in Stomach CT Scanning

Objective To evaluate the role of different kinds of body positon in the stomach CT scanning.Methods It included 30 normal adults who underwent CT scanning in four differnent kinds of body positions.The relationship between the body position and the image appearence in the each part of stomach has been analysed.Results The results showed that the adjusting of body position was useul in revealing gastric wall and perigastric organs.The best imaging result of fundus ventriculi part of stomach can be obtained with the head down and the feet high position while patients lie on his back.The best body position to reveal corpus ventriculi part is in prone position and the best body position to reveal pars pylorica part is in right lateral position.There is a very significant difference between supine position and the best revealing position of each part of stomach prespectively(?
6.The Roleof Using the DBP-CT in the Staging of Advanced Gastric Cancer

Objective:To evaluate the role of using the double body position CT(DBP-CT)in the staging of advanced gastric cancer.Methods:We performed CT examination in 85 patients with advanced gastric cancer which were divided into the double postion group(55 cases)and the single position group(30 cases),after oral intake water agent.The preoperative CT findings of each group was compared with surgical findings respectively and the accuracy in the CT staging of advanced gastric cancer of each group was estimated.Results:The accuracy in the preoperative CT staging of double position group was 89.09%,and in single postition group was 60%.There was a significant difference(P<0.05).Conclusion:Our findings show that the DBP-CT is surperior to the single position group in finding lesion,confineing the range of the tumour,and determing the degree of adjacent invasion.It can improve the diagnostic accuracy in the preoperative staging of advanced gastric cancers significantly.
7.Effectiveness of doctor-patient communication course on improvement of doctor-patient communication ability

Objective To analyze the effectiveness of doctor-patient communication course on improvement of doctor-patient communication ability.Methods Totally 360 eight-year program medical students of clinical medicine from Shanghai Jiaotong University School of Medicine were chosen as the research object and were divided into educational group(n=l80) and control group(n=180).Students in educational group study doctor-patient communication course while those in control group do not.SEGUE Framework was used to conduct exploratory research between educational group and control group.Accuracy of questionnaire database was examined by Excel software and logical test.KoImogorov-Smimov(K-S test) was applied to analyze the scores and (x ± s) was used to express the data through SPSS 16.0 statistical software.Comparison between group differences are statistically significant(P<0.05).Results Overall score was higher in educational group than in control group.Concerning of communication initiation,infomation collection and understanding of patients,scores of educational group were significantly higher than those of control group(P=0.001,P=0.002 and P=0.007,respec-tively).Furthermnore,KoInogorov-Smimov(K-S test) and (x ± s) statistical results showed that scores of detailed items were higher in educational group than in control group as well.Conclusions Doctor-patient communication course is of great importance to help medical students practice physician-patient communication based on related knowledge learning,but course content setting remains to be further improved.
9.Liver protective effects of Siphonostegia chinensis decoction in rats with liver fibrosis

Objective To investigate the liver protective effects of Siphonostegia chinensis decoction in rats with liver fibrosis induced by carbon tetrachloride(CCl4).Methods A total of 120 male SD rats were randomly divided into six groups: a blank control group, a model group, a Siphonostegia chinensis decoction group, a Salvia miltiorrhiza decoction group, a Rhizoma polygoni cuspidati decoction group and a polyene phosphatidylcholine capsules group, with 20 rats in each group. The model of liver fibrosis in rats was induced by subcutaneous injection with CCl4, except the blank group. Except rats in the blank control group and the model group, the rats in the other four groups were treated with Siphonostegia chinensis decoction, Salvia miltiorrhiza decoction, Rhizoma polygoni cuspidati decoction and polyene phosphatidylcholine capsules(10 ml/kg). The serum level of tumor necrosis factor-α(TNF-α)was determined with enzyme-linked immunosorbent assay, the fibrosis stage assessment were determined by histopathological examination.Results Compared with the model group, Siphonostegia chinensis decoction group significantly reduced the stage of liver fibrosis(P<0.01)and there were no obvious difference in liver fibrosis stage among the Salvia miltiorrhiza decoction group, Rhizoma polygoni cuspidati decoction group and polyene phosphatidylcholine capsules group, and the Siphonostegia chinensis decoction group(allP>0.05).Compared with liver fibrosis model group(368.06±24.90)U/L, the serum level of TNF-α in the Siphonostegia chinensis decoction group(336.61± 20.20)U/L significantly reduced(P<0.01), and there were no obvious difference among the Siphonostegia chinensis decoction group, the Salvia miltiorrhiza decoction group(337.81±21.04)U/L, the Rhizoma polygoni cuspidati decoction group(338.95±24.43)U/L and the polyene phosphatidylcholine capsules group(337.11± 23.64) U/L(allP>0.05).Conclusion Siphonostegia chinensis decoction has certain liver protective effect in rats with liver fibrosis induced by CCl4.
10.Hypoglycemic effects of Trigonella foenum-graecum on mice

ObjectiveTo study the effects of aqueous extract of Trigonella foenum-graecum(AET) on the blood glucose in normal and alloxan(ALX)-diabetic mice.MethodsFasting blood glucose and glucose tolerance in normal and ALX-diabetic mice were measured respectively 7 days after AET had been given.ResultsAET had not significantly effected the fasting blood glucose of normal mice, but improved their glucose toleranc. Otherwise, AET reduced fasting blood glouse of diabetic mice induced by ALX significantly.ConclusionAET can be used on treatment of diabetes mellitus.
