Chinese Journal of Orthopaedic Trauma 2004;0(06):-
Monteggia fracture dislocation refers to the fracture of the ulna with associated dislocation of the radial head. This rare injury only accounts for less than 5%of all the forearm fractures. By literature review of the history of Monteggia fracture dislocation, the authors further clarify the definition, classification, injury mechanism and treatment protocol of this special type of fracture. The key to a good result of the treatment for a Monteggia fracture dislocation is timely diagnosis and management so that anatomic reduction and stable fixation of the ulna can be achieved. In addition, enough attention must be paid to the associated injuries at the level of the elbow, including fractures of the coronoid process and the radial head as well as posterolateral rotatory instability of the ulnohumeral articulation. The factors strongly affecting prognosis include comminuted fractures of the ulna combined with fractures of the radial head and coronoid process.

Aim To investigate the activation of prote-ase-activated receptor 2 ( PAR2 ) in regulation of the expression of epidermal growth factor receptor ( EGFR) and apoptosis of lung cancer ( LC) cells. Methods LC cells A549 and its EGFR-silenced counterpart were incubated in the medium added with tryptase. Quanti-tative RT-PCR and Western blotting were used to de-tect the expression of EGFR in A 5 4 9 cells . The apop-tosis and Bax/Bcl-xL of cells were also recorded. Re-sults Treating A549 cells with tryptase in the culture for 48 hrs resulted in a marked increase in the expres-sion of EGFR in A549 cells. Marked decreases in a tryptase dose-dependent manner in apoptotic A549 cells were detected in the presence of tryptase. Expo-sure to tryptase markedly decreased the levels of Bax and increased the levels of Bcl-xL in A549 cells, which were not shown in EGFR-deficient A549 cells. Conclusion Tryptase can increase the expression of EGFR in LC cell line, A549 cells, which protects A549 cells from apoptosis, increases Bcl-xL, and sup-presses Bax in A549 cells.

Objective To compare the clinical effect of drug-eluting stents implantation or pure balloon expansion for the treatment of patients with knee artery lesions. Methods Sixty-eight patients with knee artery lesions were randomly and voluntarily divided into the control group( n=34 )and observation group( n=34 ) . The patients in the control group were given balloon expansion treatment, while in observation group were implanted drug-eluting stents besides balloon expansion. Pathological changes of skin temperature, percutaneous oxygen partial pressure( TcPO2 ),ankle brachial index( ABI),and the recurrence rate of patients before and after treatment for 6 months and 12 months were recorded and compared. Results After 6 months treatment,TcPO2 and ABI in observation group were(35. 4 ± 4. 5)mmHg and 0. 85 ± 0. 04,significantly higher than that in control group(( 28. 2 ± 3. 5 ) mmHg and 0. 62 ± 0. 03 ),and the differences were statistically significant( t=2. 535,2. 185;P﹤0. 05). At 12 months after treatment, skin temperature,TcPO2,ABI in observation group were(32. 4 ± 4. 3)℃,(34. 3 ± 4. 2)mmHg and 0. 80 ± 0. 04,significantly higher than that of the control group ((28. 6 ± 3. 7)℃,(26. 4 ± 3. 6)mmHg,0. 53 ± 0. 02;t =2. 354,2. 648,2. 064;P ﹤0. 05). Meanwhile, the recurrence rate was 5. 9% in observation group, significantly lower than that of the control group( 32. 4%;χ2=8. 463,P﹤0. 05). Conclusion The clinical effect of drug-eluting stents implantation in treatment of patients with knee artery lesions is superior to that of balloon expansion.
