3.Establishment and Application of Drug Cold Chain Monitoring Platform in Our Hospital

OBJECTIVE:To guarantee the reliability of cold chain drug storage device and the quality of the drugs in the hospi-tal. METHODS:The establishment and effects of drug cold chain monitoring platform in our hospital were introduced as well as re-lated improvement and measures. RESULTS:The cold chain monitoring platform was established in our hospital through equipping with related equipment, constructing platform network, determining monitoring point, setting early warning condition, etc. All-weather and real-time monitoring of temperature and humidity were realized for different districts of the hospital and different cold chain equipments. The platform could provide the information about operating condition of equipment,and complete drug re-frigeration temperature record;equipment failure could be dissolved in time based on early warning message. Through analyzing rea-sons of early warning message,the unqualified equipment was updated in time,which further improve the system and procedure of cold chain monitoring platform. Within 2 years after the implementation of the platform,53 sets of special refrigerator for drug were updated. Monthly early warning message and the rate of refrigeration equipment early warning decreased from 6869 and 100% in Sept. 2014 at the early stage of short message warning function to 1083 and 40% in Feb. 2016,respectively. CONCLU-SIONS:The establishment of drug cold chain monitoring platform guarantees the reliability of cold drug storage condition so as to ensure the quality of drug in the hospital.
4.Effects of early rehabilitation on upper extremity function of patients with shoulder-hand syndrome after stroke

ObjectiveTo study the effects of early rehabilitation on symptom, range of motion (ROM) and motor function of upper extremity of patients with shoulder-hand syndrome after stroke.Methods47 patients were divided into two groups,early rehabilitation group and non-early rehabilitation group.Two groups were treated with comprehensive rehabilitation. The passive ROM, the pain and motor function of affected limb was assessed by Fugl-Meyer Assessment.The evalution was done in pre-treatment and post-treatment respectively. Results1.The passive ROM decreased and the pain of affected limb of the early rehabilitation group were less than the non-early rehabilitation gronp when the shoulder-hand syndrome occour.2.After comprehensive rehabilitation treatment,their clinical symptom ROM and motor function of upper extremity impoved significantly in patients with shoulder-hand syndrome after stroke( P<0.05).The early rehabilitation treatment has better effects. Conclusions The early rehabilitation treatment could decrease the degree of their passive ROM decrease and the pain of affected limb in patients with shoulder-hand syndrome after stroke,and the patients who accepted earlyrehabilitation seemed easly to improve by comprehensive treatment.
5.Physicochemical properties and biocompatibility of novel sutureless silicone corneal ring

Background Corneal contact lens have been widely used in vitrectomy.The corneal ring accompanying with the contact lens therefore is also much concerned.We designed a sutureless silicone corneal ring to renew a suturing?ring,but its detection of physicochemical properties and hiocompatihility is necessary for clinical application. Objective This study was to investigate the physicochemical properties and biocompatibility of the newly designed sutureless silicone corneal ring. Methods The Shore hardness,tensile stress-strain tests,tear strength,transmittance and the haze of the novel silicone ring were tested.The bioeompatibility of sutureless silicone corneal ring to corneal epithelial cells was evaluated by determining cytotoxieity.The ring was implanted into the counjunctival soc of 6 Japanese white rabbits for 4 hours to determine its stimulation response by measuring the area of corneal fluorescence staining,and then the ring was placed on the skin surface of 6 SD rats for 4 hours to assess the presence of allergic reaction by calculating the swelling area of skin. Results The new sutureless silicone corneal ring showed good physiological features with a Shore hardness of 63.4 degree,tensile strength more than 5.86 MPa,elongation>100%and tear strength 34 kN/m.The transmittance of 3 legs in the ring was more than 93%;however,the haze was less than 0.1%.Cytotoxicity of the ring was 3.1 1×10-6%.No obvious corneal fluorescence staining was seen at any time point in all the rabbit eyes in the stimulating test.Furthermore,there was not any skin swelling in various time points in all the SD rats in the allergy test. Conclusion The physicochemical properties of the silastic used in our newly designed sutureless silicone cornea ring can meet the demands of the ring.The biological evaluation complies with the ISO i0993-1 international standards and meets the demands of the organism.Thus,it is all ideal material to use in constructing the corneal ring.
6.Effect of electro-acupuncture at zusanli (ST36) on the expression of ghrelin and HMGB1 in the small intestine of sepsis rats.

OBJECTIVETo explore the expression of Ghrelin and high mobility group protein B1 (HMGB1) in the serum and the intestinal tissue of sepsis model rats, and to evaluate the effect of electro-acupuncture (EA) at Zusanli (ST36) on the expression of HMGB1 and Ghrelin.
METHODSForty-eight male Wistar rats were randomly divided into four groups, i.e., the sham-operation (sham), the cecal ligation and puncture group (CLP), the CLP + EA at Zusanli (ST36) group (EA), and the CLP + Ghrelin receptor blocking agent + EA group (GHSRA), 12 in each group. A sepsis rat model was prepared by CLP. The incision of the abdominal wall was immediately sutured along the ventral midline for rats in the Sham group. In the EA group EA at Zusanli (ST36) was performed 20 min after CLP surgery with the constant voltage (2 - 100 Hz, 2 mA) for 30 min. In the GHSRA group, Ghrelin receptor blocking agent, [D-Arg1, D-Phe5, D-Trp79, Leu11]-substance P (700 nmol/kg), was administered through intravenous injection immediately after CLP, and 20 min later, EA at Zusanli (ST36) was performed in the same way as for rats in the EA group. Blood samples were withdrawn 12 h after CLP. The serum levels of Ghrelin and HMGB1 were detected using ELISA. Ghrelin expressions and the number of Ghrelin immunopositive cell in the jejunum were determined by immunohistochemistry. HMGB1 contents of the jejunum tissue were detected by Western blotting.
RESULTSCompared with the Sham group, the number of serum immunopositive cells and the expression of HMGB1 in the jejunum tissue significantly increased and levels of Ghrelin and the expression rate of immunopositive cells significantly decreased in the CLP group (P < 0.05). Compared with the CLP group, the number of serum immunopositive cells and the expression of HMGB1 in the jejunum tissue significantly decreased, but levels of Ghrelin and the expression rate of immunopositive cells significantly increased in the EA group (P < 0.05). Compared with the EA group, the number of serum immunopositive cells and the expression of HMGB1 in the jejunum tissue significantly increased in the GHSRA group (P < 0.05), but there was no statistical difference in levels of Ghrelin between the two groups (P > 0.05). The serum level of HMGB1 was negatively correlated with Ghrelin in the Sham group, the CLP group, and the EA group (r = -0. 528, P < 0.01).
CONCLUSIONSEA at Zusanli (ST36) could inhibit the expression of HMGB1 in the jejunum of septic rats, and promote the expression of Ghrelin. The expression of HMGB1 was inhibited by Ghrelin receptor blocking agent, which suggested that the anti-inflammation of EA at Zusanli (ST36) might be associated with Ghrelin.

7.Effect of TCM five-tone therapy on chronic fatigue syndrome

Objective To study the effect of TCM five-tone therapy on chronic fatigue syndrome(CFS).Methods Fifty nine CFS patients were divided into the treatment group(n=30)and the control group(n=29),which received TCM five-tone therapy and common music therapy,respectively for 3 months.Both groups were assessed with fatigue Scale-14,depression status inventory and visual analogue scale.Result After treatment,the treatment group was scored lower than the control group in FS-14,DSI and VAS(all P<0.05).Conclusion TCM five-tone therapy may be more effective in decreasing the CFS patients with fatigue and depression and alleviating their pain symptoms.
8.The B lymphocytes D8/17 antigen expression and characteristics of patients with rheumatic heart disease in Guangdong

ObjectiveTo investigate the D8/17 antigen expression of patients with rheumatic heart disease (RHD) in Guangdong province and study the antigen's characteristics.Methods The level of D8/17 antigen expression on B lymphocytes was determined with flow cytometry assay in 96 RHD patients and 83 unaffected controls.The percentage of B-cells expressing the D8/17 antigen having more than 10% was considered to be positive. D8/17 antigen was extracted by immunopreeipitation,and the antigen characteristics was analyzed by tandem mass spectrometry. Results The mean percentage of B-cells expressing the D8/17 antigen was (85.36 ± 15.15)% in the RHD patients and (82.89 ±4.55)% in the controls,with no significant difference between the two groups ( P =0.436).Moreover,the positive rate of the D8/17 cxprcssion was 100% in either the RHD patients or the controls.The molecular weight of D8/17 antigen was found to be 40 000-67 000,and the purified protein was most likely to match moesin or β-actin.ConclusionsB-cell antigen D8/17 is not associated with RHD in Guangdong province of China.Moesin or β-actin is the most likely protein to match D8/17 antigen.
9.Prevalence and Predictor Analysis of Left Ventricular Reverse Remodeling in Patients With Primary Hypertension Combining Left Ventricular Systolic Dysfunction

Objective: To analyze the prevalence and predictor for left ventricular reverse remodeling (LVRR) in patients of primary hypertension combining left ventricular systolic dysfunction (LVSD) with tailored medication.
Methods: A total of 118 consecutive patients admitted in our unit from 2010-08 to 2012-10 with the base line left ventricular ejection fraction (LVEF)≤40%were enrolled. The demographic and clinical information with the findings of echocardiography at admission were collected. The patients were followed-up until 2013-12 or until the all cause death/cardiac transplantation. According to echocardiography, LVRR was deifned by 2 criteria at the same time:①the absolute elevation of
LVEF≥10%than base line and the follow-up LVEF≥50%,②the relative reduction of left ventricular end-diastolic diameter (LVEDD) index≥10%than base line and the follow-up LVEDD index≤27 mm/m2. LVRR prevalence with its base line predictor was investigated.
Results: The overall mean follow-up time was (23 ± 15) months, and 39/118 (33.1%) patients acquired LVRR as LVEF from the base line level (30.6 ± 6.8)%increased to the follow-up level (57.0 ± 4.9)%;LVEDD index from the base line level (31.6 ± 3.9) mm/m2 decreased to the follow-up level (24.4 ± 1.9) mm/m2, all P<0.01. The average time length for reaching LVRR was (11 ± 9) months, and 27/39 (69.2%) patients reached LVRR within 12 months. There were 79 patients not reached to LVRR, while their LVEF also from the base line level (28.6 ± 6.1)%increased to the follow-up level (39.0 ± 13.2)%;LVEDD index from the base line level (38.1 ± 5.6) mm/m2 decreased to the follow-up level (36.1 ± 6.9) mm/m2. Multivariable logistic regression analysis indicated that the patients with the shorter duration of heart failure (HF) as>6 months vs≤6 months (OR=0.244, P<0.01), shorter QRS interval as≥120ms vs<120ms (OR=0.276, P<0.05) and the higher quartile of systolic blood pressure (SBP)/LVEDD index (OR=2.724, P<0.01) at admission were the independent predictors for LVRR.
Conclusion:With tailored medication, about 1/3 of patients with hypertension combining LVSD could acquire LVRR, the patients with shorter duration of HF, shorter QRS interval and higher ratio of SBP/LVEDD index had more possibilities.
10.Expression of hypoxia-inducible factor 1 and neuroglobin in piglet cortex during deep hypothermic circulatory arrest

AIM:To observe the expression of hypoxia-inducible factor 1 (HIF-1) and neuroglobin (NGB) in piglet cortex during deep hypothermic circulatory arrest.METHODS:Wuzhishan piglets were randomly assigned to car-diopulmonary bypass group ( CPB group) , 40 min of circulatory arrest ( CA) at 18 ℃ without cerebral perfusion ( DHCA group) or with selective antegrade cerebral perfusion ( SACP group) .After 180 min of reperfusion, cortical tissue was har-vested for determining HIF-1αand NGB expression by HE staining, Western blot and real-time PCR.RESULTS:Severer cerebral injury was observed in DHCA group than that in SACP group.After 180 min of reperfusion, HIF-1αprotein and mRNA levels were significantly higher in DHCA group than those in CPB group (P<0.05).Accordingly, SACP animal had higher levels of HIF-1αprotein and mRNA than those in DHCA group (P<0.05).Simultaneously, higher NGB pro-tein and mRNA levels were found in DHCA group than those in CPB group after 180 min of reperfusion ( P<0.05) .The SACP animal had higher levels of NGB protein and mRNA than those in DHCA group (P<0.05).CONCLUSION:Up-regulation of HIF-1 and NGB are involved in the mechanism against cerebral injury resulting from DHCA in the cortex and possibly a part of cerebral protective effect of SACP.
