1.Role of Nrf2/Keap1/ARE signaling pathway in respiratory diseases
Chinese Pharmacological Bulletin 2015;(11):1488-1492
Nrf2/Keap1/ARE is an important signaling pathway in maintaining the balance between peroxide and antioxidants. Once oxidative stress happened, Nrf2/Keap1/ARE would be ac-tivated to regulate the expression of downstream antioxidant pro-teins that could reduce the injury from oxidative stress and the level of oxidative stress. Recent researches show that Nrf2/Keap1/ARE signaling pathway is closely related to refractory re-spiratory diseases such as pulmonary fibrosis (PF), lung canc-er, chronic obstructive pulmonary disease (COPD), suggesting that Nrf2/Keap1/ARE signaling pathway may be a potential ther-apeutic target. This review focuses on the role of Nrf2/Keap1/ARE signaling pathway in refractory respiratory diseases in order to further understand the related mechanism and provide evidence for the diagnosis and treatment of refractory respiratory diseases.

Objective To investigate the expression of the hypoxia-inducible factor (HIF)-1α in rat brain neurons and the intervention of β-sodium aescinate after restoration of spontaneous circulation (ROSC).Methods Sixty SD adult rats were randomly (random number) divided into 3 groups (n =20),namely experiment group, control group and sham operation group.(1) The rats of experiment group were injected intraperitoneally with β-sodium aescinate (5 mg/kg) immediately after ROSC.(2) The rats of control group received normal saline injected intraperitoneally instead of β-sodium aescinate solution.(3)The rats of sham operation group did not have cardiac arrest and β-sodium aescinate intervention.Cardiac arrest rat model was established by using asphyxiation and intra-venous potassium chloride solution.Blood samples were taken 1 h,6 h,12 h and 24 h after ROSC,and subsequently rats were sacrificed and their brain tissues were harvested. The expressions of HIF-1 α mRNA,vascular endothelial growth factor (VEGF)mRNA and erythropoitin (EPO) mRNA and their protein levels in rat brain neurons were detected by using RT-PCR and immunohistochemistry, and the levels of serum neuron-specific enolase (NSE) and S100β proteins were determined by using enzyme-linked immunosorbent assay. The t test or one-way ANOVA was used to assess overall differences among groups for each of the variables, followed by Bonferroni test for multiple comparisons.Pearson method was used for correlation analysis. Results Compared with the sham operation group at intervals of 1 h,6 h,12 h and 24 h after ROSC,levels of serum S100β and NSE proteins were significantly increased in rats of the control group (P < 0.05).Meanwhile,the expressions of HIF-1 α mRNA,VEGF mRNA and EPO mRNA and their protein levels in rat brain neurons were significantly increased in the control rats (P <0.05).Compared with the control group at intervals of 1 h,6 h,12 h and 24 h after ROSC,levels of serum NSE and S100β proteins were significantly decreased in rats of the experiment group (P < 0.05).Whereas,the expressions of HIF-1 α mRNA,VEGF mRNA and EPO mRNA and their protein levels in rat brain neurons were significantly increased in rats of the experiment group (P <0.05).HIF-1 α mRNA was positively correlated with EPO mRNA and VEGF mRNAs (r =O.866,P <0.05 ; r =0.952,P < O.01).Conclusions The expression of hypoxia-inducible factor-1 α is increased in rat brain cells after ROSC,and β-sodium aescinate up-regulates the expression of hypoxia-inducible factor1 α mRNA and protein levels. The up-regulated expression of hypoxia-inducible factor-1α improves the resistance of brain cells to ischemia and hypoxia contributing neuronal protection, which might be due to upregulated EPO and VEGF expressions induced by hypoxia-inducible factor-1α.

ObjectiveTo explore the prevalence state of children in coal-burning borne endemic fluorosis in Fuyuan county Qujing city,to know the implementation of preventing measures, and to provide a scientific basis for prevention of endemic fluorosis.MethodsIn 2009,105 families were sampled to check the service condition of defluoridation stove and the effect of health education (the awareness of use defluoridation stove reasons) in Xiaoyuwang,Tuoga and Shaliang villages,Fucun town of Fuyuan county.Food fluorine of indoor stored peppers and corns was monitored of 20 families in Xiaoyuwang village,5 household were randomly selected from east, west, south, north and middle places of the village.Air fluorine was monitored from indoor air samples of 5families,dental and urinary fluorine was monitored of the native-born children aged 8 to 12,and dental fluorosis of these children was checked.Clinical endemic fluorosis and X-ray endemic fluorosis was monitored of local adult residents over the age of 30 that had been lived in the area for more than 5 years.ResultsThe rate of correct use of the defluoridation stove was 100%,and adult awareness rate of use defluoridation stove reasons was 82.86%(87/105).Of the 19 pepper samples, fluorine content were 9.98 - 538.40 mg/kg and median was 32.38 mg/kg,all exceeded the standard(1.5 mg/kg).Of the 25 corn samples, fluorine content were 0.42 - 101.54 mg/kg and median was 6.32 mg/kg,with 18 samples exceeded the standard(1.0 mg/kg).Of the 6 air samples, fluorine content were 0.000 98 - 0.003 33 mg/kg and median was 0.001 45 mg/kg.Detection rate of dental fluorosis of children aged 8 - 12 was 42.3%,urinary fluorine content were 0.13 - 1.80 mg/L,geometric mean was 0.58 mg/L,4 adults were diagnosed clinical skeletal fluorosis, detection rate was 2.46%(4/162),5 adults were found by X-ray with endemic fluorosis, detection rate was 9.61%(5/52).ConclusionPeppers and cons fluorine pollution is serious, general public should be guided to change their lifestyles to minimize the fluorine pollution.
7.Thrombolysis during extended cardiopulmonary resuscitation for autoimmune-related pulmonary embolism
World Journal of Emergency Medicine 2015;6(2):153-156
BACKGROUND: Massive pulmonary embolism (MPE) and acute myocardial infarction are the two most common causes of cardiac arrest (CA). At present, lethal hemorrhage makes thrombolytic therapy underused during cardiopulmonary resuscitation, despite the potential benefits for these underlying conditions. Hypercoagulability of the blood in autoimmune disorders (such as autoimmune hemolytic anemia) carries a risk of MPE. It is critical to find out the etiology of CA for timely thrombolytic intervention. METHODS: A 23-year-old woman with a 10-year medical history of autoimmune hemolytic anemia suffered from CA in our emergency intensive care unit. ECG and echocardiogram indicated the possibility of MPE, so fibrinolytic therapy (alteplase) was successful during prolonged resuscitation. RESULTS: Neurological recovery of the patient was generally good, and no fatal bleeding developed. MPE was documented by CT pulmonary angiography. CONCLUSIONS: A medical history of autoimmune disease poses a risk of PE, and the causes of CA (such as this) should be investigated etiologically. A therapy with alteplase may be used early during cardiopulmonary resuscitation once there is presumptive evidence of PE. Clinical trials are needed in this setting to study patients with hypercoagulable states.

To observe the effect of calycosin on the proliferation and activation of primary hepatic stellate cells (HSCs) in rats, and prove calycosin shows the effects through peroxisome proliferator-activated receptor γ(PPARγ) and farnesoid X receptor (FXR). The results indicated that calycosin could inhibit HSC proliferation and expressions of activation marker smooth muscle actin-α and type I collagen. With the increase in HSC activation time, FXR expression reduced, but with no notable impact from calycosin. Calycosin could up-regulate PPARγ expression and its nuclear transition in a concentration-dependent manner. Its prohibitory effect on HSC activation could be blocked by PPARγ antagonist. In conclusion, calycosin could inhibit HSC activation and proliferation, which may be related with the up-regulation of PPARγ signal pathway.
