Research on osteoporosis diagnosis has always been a critical isuue in the field of international academia and medicine. Recent progress in quantitative ultrasound (QUS) has suggested that ultrasound, due to its obvious advantages, be considered as an effective and noninvasive tool for assessment of bone status and diagnosis of osteoporosis. This paper presents the principle and recent development in the ultrasonic assessment of bone status and osteoporosis including assessment with through-transmission and backscattering measurement, and also introduces the latest progress in ultrasonic axial transmission technique.Limitations of current research are discussed and suggestions are proposed for future research.

Objective To discuss the clinical efficacy of internal fixation assisted by thoracoscope in treatment of rib fractures. Methods The study enrolled 180 patients with rib fractures associated with thoracic deformity hospitalized from July 2010 to June 2013.Ninety out of the patients were operated on by thoracoscope-assisted internal fixation (operation group),and the remaining 90 fractures were treated non-operatively (non-operation group).Clinical markers recorded were duration of pain, time of ventilator use, hospital length of stay and complications.Patient mental health was measured with self esteem scale (SES).Patient mobility was evaluated at follow-up.Results Between operation and non-operation groups differences were observed in duration of pain [(3.1 ± 1.0)d vs (8.9 ± 1.2) d],time of ventilator use [(3.0 ± 1.0) d vs (4.8 ± 1.0) d] and hospital length of stay [(10.0 ± 1.1) d vs (15.8 ± 1.0) d] (P < 0.01).SES in operation group was (28.3 ± 2.1) versus (24.4 ± 3.3) points in non-operation group (P < 0.01).No major complications occurred in operation group, but there were 20 pleural effusion,15 severe thoracic collapse or deformity,14 lung infection,10 refractory chest pain and 2 upper limb dysfunction in non-operation group (P <0.01).Two patients presented mobility limitation in operation group, but 12 had evident loss of mobility in non-operation group (P <0.01).Conclusions Thoracoscope-assisted internal fixation is effective to accelerate the pace of recovery, relieve pains, reduce complications and thus can be a priority method for treatment of rib fractures.

0.05).However,50.0% of T1 patients with TNBCs had lymph node metastasis, which was much higher than the other 2 groups. The recurrence rate in TNBC,non-TNBC or Her-2 positive patients were 57.1%,40.5% and 44.2%,respectively.The triple negative group had higher tumor recurrence rate than the other two groups(P=0.03).The overall 3 year survival rate of the TNBC group was 58.9%,which was much lower than the other two groups(76.4% for the luminal a and b group and 74.8% for the Her-2 positive group(P=0.04).Conclusion:The TNBCs was higher in premenopausal patients well as a higher lymph nodes metastasis rate with small tumor size. Moreover, three years recurrence and mortality rates in this group is higher than the non-TNBC groups.

Journal of Chinese Physician 2021;23(4):563-567
Objective:To discuss the computed tomography (CT) and magnetic resonance (MR) imaging features of primary adrenal cortical carcinoma (PACC) and improve the imaging diagnostic level of primary adrenocortical carcinoma. Methods:The imaging features of 15 patients with biopsy or pathology confirmed PACC were retrospectively analiezed. There were 12 patients underwent CT examination, 7 patients underwent MR examination, and 4 patients underwent CT and MR exminations. Results:All of 15 PACCs were unilateral (10 on the right, 5 on the left). The maximum diameters of the lesions ranged from 4.4 to 15.1 (8.9±3.3)cm. The shape of the tumor was oval in 6 cases and irregular in 9 cases. CT findings: 1 cases showed homogeneous density and 11 cases were heterogeneous attenuation (including 2 cases with hemorrhage and 1 case with fat) on plain scan. The CT values of triple-phase of PACC ranged from 28.8 to 62.0(39.2±8.5)HU, 32.7 to 83.4(52.5±14.4)HU and 43.4 to 86.4(61.1±15.0)HU respectively. All cases showed mild (7 cases) and moderate (4 cases) gradual enhancement, only one case excluded. MRI findings: inhomogeneous mild and moderate enhancement were detected in 6 cases and homogeneneous enhancement was found in 1 case. The solid parts of PACC showed iso-signal intensity in T1WI, hyper-signal intensity in T2WI and diffusion limited in the diffusion weighted imaging (DWI). There were 14 cases of vascular shadow in 15 patients, and 3 cases of involvement and metastasis of surrounding tissues and organs. Conclusions:PACC often present as a big solitary heterogenous mass with necrotic area, hemorrhage and intratumoral vascular. The mild to moderate heterogenous gradual enhancement can be seen after the contrast.

World Science and Technology-Modernization of Traditional Chinese Medicine 2013;(4):743-745
The research on clinical efficacy evaluation of traditional Chinese medicine (TCM) is a hot and difficult issue in the field of TCM. However, the overall level of current related research also needs to be improved. There are still a lot of academic controversies in the field of TCM clinical efficacy evaluation. The author believed that before conducting research, it is necessary to sort out core issues of TCM clinical efficacy evaluation, which in-clude the concept of TCM clinical efficacy and its evaluation. In recent years, Chinese scholars have conducted a series of studies on these issues and have some achievements. To think and answer these questions will play an important role for the ultimate resolution of problem of TCM clinical efficacy evaluation.

Through analyzing the current situation and coverage controversy of Chinese clinical trial insurance, this paper stated that the attending insurance rate in domestic clinical trials was entirely low.The sponsors, clinical trial institutions, investigators and insurance companies paid attention of different levels to clinical trial insurance. Therefore, the risk awareness of drug/medical device clinical trials should be enhanced. It is necessary to give impetus to clinical trial insurance system, during which all parties need to make a joint effort including government departments, ethics committees, sponsors, clinical trial institutions, investigators and insurance companies.

Objective To analyze and summarize the risk factors of failed internal fixation for intertrochanteric fracture. Methods From April 2008 to April 2011,267 patients with intertrochanteric fractures in 4 hospitals were treated with internal fixation. The relationship between the failure of internal failure and possible factors as age, gender, hypertension, diabetes, the abuse of alcohol and tobacco, use of glucocorticoid, the degree of osteoporosis and fractures type were studied.According to the surgical risk assessment table, the patients were divided into low-risk,mid-risk,and high-risk group. The rate of internal fixation failure was compared in the 3 groups. Results We found 42 cases which showed radiographic failures. The internal fixation failure directly related with advanced age, diabetes, severe osteoporosis, unstable type fracture, but not gender, hypertension, the abuse of alcohol and tobacco, use of glucocorticoid.Risk factors of internal fixation failure included diabetes, osteoporosis degree, and fracture stability.Failed intertrochanteric fracture fixation mainly occurred in the mid-risk and high-risk groups. Conclusion Severe osteoporosis, unstable fracture, diabetes are risk factors of failure of intertrochanteric fracture fixation. These factors will affect the quality of surgery. For the patient with intertrochanteric fractures in the low-risk groups, internal fixation should be the first choice for treatment. For the patients in the mid-risk and high-risk group, internal fixation should be applied cautiously. For the aged patients in high-risk groups, hip arthroplasty is a wise option.
