1.Controlled study on different hepatic blood flow occlusion in hepatectomy

Objective To evaluate the indications of different hepatic blood flow occlusion in hepatectomy of cancer patients. Methods One hundred and fifty-six patients admitted between 1991-2001 underwent hepatectomy with hepatic blood flow occlusion in different ways, among them 48 cases underwent hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter, 71 cases underwent hepatectomy with porta hepatis occlusion by Pringle method, 37 cases treated by hemihepatectomy or partial hepatectomy with hemihepatic occlusion. Results Intraoperative blood loss in patients using balloon catheter was smaller and postoperative liver function recovered faster compared with other ways of blood flow occlusion. Conclusion The preliminary result shows that hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter is safe and useful technique for hepatectomy.
2.Study on the value of T-SPOT.TB test in tuberculous pleurisy

Objective To explore the value of T-SPOT.TB test in diagnosis of tuberculous pleurisy by comparing the T-SPOT.TB test, adenosine deaminase (ADA) of hydrothorax and tuberculosis antibody (TB-Ab). Methods 62 pleural effusion patients are included in the research , of which 32 cases have tuberculosis and 30 cases have no tuberculosis. All patients underwent T-SPOT.TB, pleural effusion ADA, and TB-Ab test. The results were compared with final clinical diagnosis for sensitivity and specificity evaluation. Results The sensitivity of T-SPOT.TB, ADA, TB-Ab were 90.6%, 71.9% and 62.5% respectively. The specificity of T-SPOT. TB, ADA, TB-Ab were 90.0%, 83.3% and 86.7% respectively. The sensitivity of T-SPOT.TB was the highest one among the three methods. The sensitivity of T-SPOT.TB has statistically significant difference compared with TB-Ab (P < 0.05), but no statistically significant difference compared with ADA (P > 0.05). Conclusions The T-SPOT.TB test had higher sensitivity and specificity for the diagnosis of tuberculous pleurisy , and had important reference value in early diagnosis of patients with tuberculous pleurisy. The T-SPOT.TB and TB-Ab combination examination had a complementary effect.
3.Endoscopic sphincterotomy for choledocholithtasis

Objective To evaluate endoscopic sphincterotomy(EST)for the management of choledocholithiasis. Methods Between July 1987 and March 2007,991 cases of choledocholithiasis treated with EST were reviewed,and 710 cases were followed-up. Results All cases were treated with EST.The common bile duct stones were removed in 909 cases(91.7%,909/991).The rate of complications after EST was 6.0%(59/991)including pancreatitls in 29 cases(2.9%),hemorrhage in 18 cases(1.8%),cholangitis in 11 cases(1.1%),and duodenal perforation in 1 case(0.1%).There was no mortality in this group.Follow-up found reflux cholangitis in 51 cases(7.2%)and recurrent choledocholithiasis in 42 cases(5.9%). Conclusion Therapeutic endoscopy for choledocholithiasis is safe and effective.

Objective Our retrospective study was aimed to analyze the clinical value of serum CEA and CA19-9 in patients with chole-lithiasis.Methods The clinical data of 98 patients with cholelithiasis and 44 patients with inguinal hernia received treatment in our hospital from February 2011 to February 2015 were retrospectively analyzed.The expressive levels of CEA and CA19-9 of the patients were detected and compared.The important roles of CEA and CA19-9 in the patients with cholelithiasis were analyzed.Results The levels of CEA,CA19-9 and inflammatory factors in normal group and control group had no statistical differences (P>0.05).The levels of CEA,CA19-9 and inflam-matory factors in rising group were higher than those in control group (P<0.05).The levels of CEA,CA19-9,inflammatory factors before and after the treatment had no statistical differences (P>0.05),but the levels of CEA,CA19-9,inflammatory factors in rising group were obvi-ously decreased( P<0.05) .Conclusion The levels of CEA and CA19-9 in patients with cholelithiasis had correlation with inflammation of biliary tract,which will increased by severe choleithiasis.
6.Differential diagnosis of the etiologies of fetal megacystis by prenatal ultrasonography

Objective To explore the clinical value of prenatal ultrasonography in the differentiation among the etiologies of fetal megacystis.Methods Twenty seven fetuses,diagnosed as fetal megacystis by prenatal ultrasonography,were retrospectively analyzed.The etiologies of fetal megacystis were presumed by such characteristics as keyhole sign,thickness of the bladder wall,amniotic fluid index,fetal sex and other combined signs.All fetuses were followed up until to the induction of labor or birth.Results Twenty seven singleton fetuses (19 males and 8 females) were diagnosed as megacystis.According to the characteristics and other combined signs,8 cases of posterior urethral valves (PUV),1 of prune belly syndrome(PBS),1 of megacystis-microcolon intestinal hypoperistalsis syndrome(MMIHS),1 of urethral atresia and 5 of chromosomal abnormality were presumed by prenatal ultrasound.Multiple malformations were found in 5 fetuses and there were also 6 fetuses with unknown reason originally.Among the 27 fetuses,21 were induced labor and 6 continued pregnancy to birth.Except for the 6 cases of unknown reason,etiologies of 17 fetuses with megacystis were confirmed by autopsy,genetic tests,surgery or further examination after birth.The accuracy rate of prenatal ultrasonography in the differentiation among the etiologies of fetal megacystis was 80.95% (17/21).Conclusions On the basis of detailed prenatal ultrasonography and typical characteristics,it is reliable to differentiate the etiologies of fetal megacystis.Sometimes fetal megacystis may be one part of multiple malformations or complex syndrome,such as VACTERL syndrome.However,it is difficult for ultrasonography to diagnose vesicoureteral reflux(VUR)prenatally.

Objective To study the prevalence of the mtDNA A1555G gene mutation in Chinese population with nonsyndromic hearing impairment.Methods PCR-RFLP,directional sequencing of PCR products were applied in 325 patients with nonsyndromic hearing impairment and 50 normal controls.Results The mutation rate of the mtDNA A1555G was 14.5% (47/325),28 of 47 cases were homozygosis,19 of 47 cases were heterozygosis.The same mutation was not detected in the control subjects.Conclusion The mutation rate of the mtDNA A1555G is relatively high in the Chinese NSHI patients,the mutation type includes both heterozygosis and homozygosis.

10.Expression of miR-216a in pancreatic cancer and its clinical significance.

OBJECTIVETo explore the clinical significance of miRNA-216a expression in pancreatic cancer.
METHODSFourteen patients with pancreatic cancer undergoing pancreaticoduodenectomy and 6 patients with benign pancreas lesions were examined for miR-216a expressions in the tumor or lesion tissues using Agilent Human miRNA Microarray (V12.0). The relationship between miR-216a expressions and the clinicopathological features of the patients was analyzed.
RESULTSThe expression of miRNA-216a was significantly lower in pancreatic cancer than in benign pancreas lesions (P=0.000). The expression of miRNA-216a was significantly correlated with the T stage of the tumor (P=0.002), but not with the patients' age, gender, smoking status, tumor stage, lymph node metastases, distant metastasis, tumor differentiation, nerve invasion, vessel invasion or serum CA19-9 level (P>0.05).
CONCLUSIONSThe down-regulated expression of miR-216a in pancreatic cancer suggests the involvement of miR-216a in the tumorigenesis and development of pancreatic cancer. miR-216a may potentially serve as a novel tumor marker and also a prognostic factor for pancreatic cancer.
