Objective To illustrate the details and effects of a new technique of penile augmentation-a dual plane approach to enhance the penile girth with human acellular dermal matrix (ADM)through the incision on the dorsal penile root. Methods Firstly,a reversed V incision was made at the dorsal root of the penis and the superficial suspensory ligament of the penis was released.A Dartos fascia incision was then made and the plan between Dartos fascia and Buck's fascia was dorsally dissected toward the coronary sulcus.A Buck's fascia incision was made 1.5-2 cm from the coronary sulcus and the fascia was undermined distally. One or two sheets of ADM was dorsally placed by a dual plane method which combined partial sub-Buck's fascia plane and partial sub-Dartos fascia plane to enhance the penile circumference. Finally, the Dartos fascia incision was closed and followed by the closure of the wound with V-Y advancement. Results A total of 35 patients underwent dual plane penile augmentation No dorsal penile skin necrosis and exposure of ADM sheet happened in our group postoperatively.25 patients were followed from 6 to 24 months after operation. All patients were satisfied with their penile appearance and 21 married patients reported satisfied sexual activity. The increase of penile circumference was (2.65±0.41) cm at flaccid state and (1.85±0.35) cm at erectile state 6 months postoperatively. No pleat or absorb of the ADM sheet, losses of sensitivity of the penis occurred during the follow-up period. Conclusions By adjusting the plane of ADM placement, the dual plane penile augmentation ensures adequate soft-tissue coverage and sufficient blood supply of penile skin and offers increased benefits and fewer tradeoffs compared with a single plane. This technique could be an effective way to lengthen and enlarge the penis in one stage without major complication.

Chinese Journal of Nuclear Medicine 2010;30(6):395-399
Objective To investigate the iodide uptake by U251 glioma cell lines which were transfered with both human sodium/iodide symporter (hNIS) and human thyroperoxidase (hTPO) genes. Methods Recombinant adenosine virus AdTPO was constructed through cloning, recombination, packaging and amplifying. The viral titers were calculated after purification. The protein expression of AdTPO was tested by Western-Blotting and the recombinant plasmids PcDNA3. 1/hNIS were constructed. After hNIS gene was transfected into human glioma cell lines U251 through liposome, the cell lines with stable hNIS expression (hNIS-U251) selected by G418 antibiotics were defined as hNIS-U251 group. Then, hTPO was transducted into hNIS-U251 with adenosine virus (AdTPO-hNIS-U251 group). U251 cells with no plasmid were used as the control group (U251). Cultured cells from each group were studied for 125I uptake as well as 125I efflux rate. Student-Newman-Keuls in multiple range test was used. Results AdTPO-hNIS-U2.51 with stable expression was successfully established by transfecting hNIS and hTPO genes into human glioma cell lines. The 125I uptake by AdTPO-hNIS-U251, hNIS-U251 and U251 cell lines was (74 647.53 ±3605.88), (55 769.96 ±4353.26) and ( 507.67 ± 57.69 ) counts/min, respectively ( F = 836. 17, P < 0.05 ). The uptake compacity by AdTPO-hNIS-U251 was 147 fold higher than that by U251 (q =55.64, P<0.01 ) and 1.3 fold higher that by hNIS-U251 (q = 14. 17, P <0.01 ). 125I efflux rate was prolonged in AdTPO-hNIS-U251 group and its effective half time was 13 min. Conclusion Enhanced 125I uptake by the human glioma cell lines can be achieved with combined transfection of hNIS and hTPO genes.
9. Assessment of left ventricular systolic and diastolic function in type 2 diabetic patients and in type 2 diabetes mellitus with coronary heart disease by two-dimensional speckle-tracking
Medical Journal of Chinese People's Liberation Army 2019;44(3):248-253
[Abstract] Objective To investigate the early changes of myocardial systolic and diastolic function in patients with type 2 diabetes mellitus (T2DM) and in patients with both T2DM and coronary heart disease (CAD). Methods A retrospective analysis was taken of 128 patients who were hospitalized for suspected CAD, underwent coronary angiography and received echocardiography between August 2015 and November 2017. In these patients, 32 patients with T2DM with normal coronary angiography (DM group), 66 patients with T2DM and CAD (DM+CAD group) and 30 patients with normal coronary angiography without DM (control group) were enrolled in the study. The two-dimensional speckle tracking strain technique was used to measure the global longitudinal peak strain (GLS) of the left ventricular myocardium, and peak systolic longitudinal strains (PSLS) of the basal segment, middle segment and apical segment of the three groups. The further results were compared and analyzed. Generalized linear regression analysis were taken to analyze the factors affecting left ventricular myocardial GLS in the DM patients. Results The interventricular septal (IVS), left ventricular posterior wall (LVPW) and left artrial dimension (LAD) were increased in the DM group and (DM+CAD group), with the E peak and E/A were decreased, compared with the control group. The peak value and E/e' value were higher in the DM+CAD group, compared with the DM group. Importantly, the left ventricular GLS of the DM group, and the DM+CAD group were significantly lower than that of the control group. Moreover, the left ventricular GLS, the basical, the middle and the apical segment PSLS were all significantly lower in the DM+CAD group, than the DM group. In addition, by generalized linear regression analysis, whether combination of coronary heart disease significantly affected GLS value in DM patients, including glycated hemoglobin levels, hypertension, and E/e' values. Conclusion The DM Patients or the patients of DM+CAD were both have left ventricular systolic and diastolic dysfunction in the early stage, even if the ejection fraction is normal. The left ventricular systolic and diastolic dysfunction is more obviously in the DM+CAD patients. The two-dimensional speckle-tracking strain can be used to evaluate left ventricular diastolic and systolic dysfunction in diabetic patients in the early stage.

Objective To assess the harvest method and application value of free-style anterolateral thigh perforator flap (ALTPF) in oral-maxillofacial reconstruction. Methods Fifty-three patients who suffered from oral and maxillofacial tumor underwent simultaneous reconstruction using free-style ALTPFs after radical resection from August,2013 to May,2014.Perforators of the ALTPF region were marked through hand-held Doppler probe preoperatively.Flaps were designed centered on perforators according to the defect size of the accepting site.Incisions were first made at the inner border of the designed flap.Perforators were exposed above the fascia lata femoris and then dissected retrogradely through the vastus lateralis muscle to harvest a vascular pedicle with desired caliber and length.Flap thinning was applied under microscope in some cases to compromise the need of the accepting site. Results All 53 flaps survived after transplantation while skin exfoliation occurred in 5 cases due to flap thinning.Four cases sustained partial necrosis and was cured by trimming and dressing changes.Five to 14 months' post-operative followup showed satisfactory accepting-site morphology with good speech function and swallowing recovery. All donor sites were closed primarily without skin-grafting,leaving no donor-site complications including incision disruption, scar hyperplasia and muscle strength degeneration of the lower limb. Conclusion Multiple perforators have been accu rately located preoperatively in free-style harvest approach of ALTPFs,thus optimal effects can be reached with decreased donor-site morbidity and improved aesthetic outcome to the uttermost, which accords with the refined, personalized and minimal invasive development concept of modem reconstructive surgery.
