1.Biodegradation of a novel bionic scaffold with nanostructure in vivo

BACKGROUND: Bone implant materials have been previously reported to be not coincident between inducing velocity of new bone formation and degradation velocity itself; therefore, the materials could not be completely degraded but formed into foreign substances. A novel artificial bone implant material, characterizing by well biocompatibUity, biodegradation, and biomechanics, is focused in biomaterials field recently.OBJECTIVE: To study the biodegredation of a novel bionic scaffold with nanostructure, i.e., poly (3-hydroxybutyrate-co-3-hydroxyvalerata)/sol gel bioactive glass (PHBV/SGBG), in vivo. DESIGN, TIME AND SETTING: A controlled animal experiment was performed at Animal Experimental Center of the Third Hospital affiliated to Sun Yat-sen University from May 2005 to October 2006. MATERIALS: PHBV/SGBG was provided by Materials Institute of South China University of Technology, and ethylene oxide was sterilized for preparation.METHODS: Eight hybrid dogs were used to make models of Ubia diaphyseal defect, having two defects on both left and right sides. The tibia diaphyseal defects at proximal part were considered as the control group, and those were not performed with any treatment; while, the tibia diaphyseal defects at distal part were considered as the experimental group, and PHBV/SGBG was fully implanted into the defect regions. Every two dogs were sacrificed at different time points of 2, 4, 8, and 12 weeks, respectively. MAIN OUTCOME MEASURES: In vivo biodogradation and osteogenesis were monitored under optic microscopy and electron microscope.RESULTS: The PHBV/SGBG scaffold had well biodegradation and rapid degradation velocity, and it began to degrade at two weeks after operation. The PHBV/SGBG scaffold was almost replaced by new bone tissues at 8 weeks after operation and completely degraded at 12 weeks after operation. In addition, the PHBV/SGBG scaffold had a good ability to induce new bone formation from edge to center. Whereas, surface depression in the defect region was still visible in the control group, cortical bone was not formed in embedded region of soft tissue; furthermore, electron microscopy demonstrated that calcium salt deposition was increased in the bone defect region, and the structure was tight; however, the defect was not completely repaired, and some voids were still visualized.CONCLUSION: The novel bionic scaffold, PHBV/SGBG, degrades fast in vivo to generate new bone tissues. The new bone regenerate accompanied by a fitting degradation of the novel bionic scaffold that achieve complete repair.
2.Biopolymer poly hydroxybutyrate-hydroxyvalerate membrane

BACKGROUND: Poly hydroxybutyrate-hydroxyvalerate (PHBV) has been used to construct bioprosthetic heart valve. It remains unclear whether it can be used as membrane for guided bone regeneration. OBJECTIVE: To investigate the biocompatibility of PHBV membrane and evaluate its efficiency of promoting bone regeneration in vivo. METHODS: Effects of 100%, 75%, 50%, 25% PHBV extract solution on relative growth rate of dog bone marrow mesenchymal stem cells were measured by MTT method and cytotoxicity of the biomaterials was evaluated. Bone defects were made on distal bilateral tibias and treated with PHBV membrane; the proximal bilateral tibias undergoing reduction of periosteal flap and were used as control. RESULTS AND CONCLUSION: The toxicity gradation of PHBV membranes was grade 0-1. That is, they were not toxic to growth and proliferation of bone marrow mesenchymal stem cells. New bone regeneration was observed in the defects covered with PHBV membranes at week 2 post-surgery. The defects covered with PHBV membranes were filled with mature bone at week 12 post-surgery. The bone repair in experimental groups was earlier and better than that in control groups. Results demonstrated that PHBV membrane, which has no cytotoxicity to mesenchymal stem cells in a wide range of extract concentration, could be a promising biopolymer membrane for guided bone regeneration.
3.Clinical Research on the Relationship between Type 2 Diabetes Mellitus and Colorectal Cancer

Objective: To investigate the relationship between type 2 diabetes mellitus (T2DM) and the pathogenesis and metastasis of colorectal cancer. Methods: A case-control study was performed to compare 852 colorectal cancer patients with 940 controls (patients without cancer) recruited from 2001 to 2006, with respect to their sex, cancer subsite, the course of T2DM, hepatic metastasis, smoking and drinking. Correlated risk factors were analyzed. Results: The risk of colorectal cancer was increased in patients with T2DM and the relative risk (OR) was 2.466. The OR of male patients was higher than that of female patients, but with no significant difference (2.775 vs 2.070, P=0.394). The incidence of T2DM in patients with left hemicolon cancer was higher than that in those with right hemicolon cancer and rectal cancer, but with no significant difference between them. The colorectal cancer risk in T2DM patients with a DM course of 10 ~ 20 years was the highest, and the OR was 4.696. The rate of hepatic metastasis was higher in T2DM patients with colorectal cancer than that in celorectal cancer patients without T2DM and the OR was 2.888. Conclusion: T2DM may be one of the important pathogenic risk factors for colorectal cancer. The OR is increased with the extension of DM course within 20 years. Colorectal cancer patients with T2DM may be more prone to hepatic metastasis.
5.Relationship between levels of plasma coagulation factors and acute myocardial infarction in low age period

Objective To explore the relationship between the levels of plasma coagulation factors (F) and acute myocardial infarction (AMI) in low age period (<60 years old) and their diagnostic value in diagnosing AMI in low age period.Methods One hundred and sixty inpatients with low age AMI in the cardiology department of the Heze Municipal Hospital were selected as the case group,and contemporaneous 160 cases of low age non-AMI served as the control group.F Ⅱ,FⅦ,FⅧ,fibrinogen (Fg) and von willebrand (vWF) were measured with enzyme-linked immunoabsorbent anti-sandwich assay.The relationship between coagulation factors and low age AMI was analyzed with univariate and multivariate analysis,and their value for diagnosing low age AMI was evaluated with diagnostic test and receiver operating characteristics (ROC) curve.Results The univariate analysis showed that FⅡ,FⅦ,FⅧ and Fg levels had significantly statistical difference between the case group and control group(P<0.05),and the vWF level had no statistically difference(P>0.05).The multivariate analysis indicated that the FⅡ level≥ 14.27 μg/L and FⅦ level ≥22.99 μg/L were the independent risk factors for low age AMI.The value of FⅡ for diagnosing low age AMI was lower,and the optimal cut off value of Fg for diagnosing low age AMI was 22.99 μg/L,its area under ROC curve was 0.709 with a moderate diagnostic value,and the sensitivity (91.88%) and negative predictive value (86.02%) were higher,the false negative rate (13.98%) was lower,and the accuracy (70.94%) was moderate.Conclusion The FⅡ level ≥14.27 μg/L and Fg level ≥22.99 μg/L are the independent risk factors for low age AMI,and detecting the Fg level could have hint significance in diagnosing low age AMI.

OBJECTIVE: To observe the effects of Ginkgolides inhalation on eosinophils infiltration in airway of asthmatic guinea pig model. METHODS: Guinea pigs were divided into seven groups(n=8 in each group): model group, ginkgolides inhalation low, medium and high dosage groups(5, 10, 20mg?kg-1, respectively), ginkgolides ip group (at a dosage of 20 mg?kg-1), Dexamethasone ip group (at a dosage of 10 mg?kg-1), normal control group. OBJECTIVE:To establish a HPLC method for the determination of ferulic acid in pedo spleen energy activating Misture.METHODS:The determination was performed on Hypersil ODS column,the mobile phase consisted of1%acetic acid-methanol(70∶30)with a flow rate of0.8ml/min,the column temperature was26℃,the detection wavelength was320nm and the sample size was20?l.RESULTS:Good linear relationship with peak area score was achieved when the detection con?centration range of ferulaic acid was within a range of0.02~1.2mg/ml(r=0.9997),the average recovery was97.88%(RSD=1.19%).CONCLUSION:The method can be served as a quality control for pedo spleen energy activating Misture.
9.End-to-end anastomosis of esophagus after partial resection for early cervical esophageal carcinoma

Objective To investigate the feasibility of treatment mode of end-to-end anastomosis of esophagus(EAS) af ter partial resection for early-stage cervical esophageal carcinoma(ECEA).Methods 7 patients were substantially confirmed as squsmous cell carcinoma of cervical esophagus by endoscopy,the nearest distance of the lesion from the incisors was 17cm,and the furthest was 20 cm,the maximum extent was 2.5 cm,and the minimum was 1 cm.None of them with longitudinal muscularis invasion.Confirmed by PET/CT or chest enhancement CT examination preoperatively,intrathoracic and cervical lymphatic metastasis was excluded,cT1 -2 N0 M0.Incisal margin length was not less than 1 cm,the maximum was 5 cm and the minimum was 3 cm.Meanwhile,the cervical lymph node should be dissected,and the average number was 6.43 per case.After surgery,all the patients were fixed by plaster slab to release the tension of anastomosis.Postoperative adjuvant radiotherapy or chemotherapy was received.Results None of the patients had severe postoperative complications,and the average hospital stay was 14.5 days.All the patients are alive,the longest follow-up lasts for 3 years and 4 months,all of them can take normal food,without anastomotic stenosis.Conclusion Treatment mode of EAS after partial resection for ECEA significantly decrease the operative damage,apparently improve the patient's quality of life(QOL),so that the patients can better receive adjuvant treatment subsequently; it is a feasible and effective method for cervical esophageal carcinoma at the early stage.
