1.The expression and significance of unfolded protein response-related gene in synovial fluid macrophages in patients with spondyloarthritis and other arthritis

Objective To investigate whether unfolded protein response (UPR) plays a role in the pathogenesis of spondyloarthritis (SpA),and to assess UPR-related gene expression in SpA and other arthritis patients.Methods Eighteen patients with SpA,12 with rheumatoid arthritis (RA) and 6 with osteoarthritis (OA) were recruited.Macrophages were isolated from synovial fluid samples by immunomagnetic separation.The expression of UPR-regulated genes,including binding immunoglobulin protein (BiP),glucose-regulated protein 94 (GRP94),C/EBP homologous protein (CHOP),growth arrested and DNA damage-inducible 34 (GADD34),X-box binding protein 1 (XBP-1) and endoplasmic reticulum DnaJ homolog 4 (ERdj4),was tested by real time polymerase chain reaction (PCR).Results Compared with macrophages in OA patients,the expression of BiP and GRP94 mRNA [(6.06 ± 2.08) × 10-2 vs (1.11 ±0.72) × 10-2 for BiP mRNA,11.80(7.30-38.40) × 10-3 vs 1.27(1.02-4.18) × 10-3 for GRP94 mRNA,both P values <0.01] was significantly increased in macrophages in SpA patients.XBP1 mRNA was up-regulated [(12.70 ± 5.20) × 10-3 vs (4.14 ± 2.56) × 10-3,p < 0.01] in SpA group as well.UPR-regulated gene expression in SpA patients with HLA-B27 positive or HLA-B27 negative was similar.However,none of UPR-regulated genes showed different expression between the SpA group and RA group except for GADD34 mRNA [7.30 (5.56-15.40) × 10-3 vs 21.30 (12.20-27.60) × 10-3,P =0.009].Conclusions Our data suggest that UPR possibly participates in the pathogenesis of SpA,although the relationship between HLA-B27 and UPR still needs further investigation.
2.Percutaneous cervical diskectomy in the management of radicular type cervical spondylosis

Objective To discuss the clinical significance of percutaneous cervical diskectomy (PCD) in the management of radicular type cervical spondylosis. Methods A retrospective analysis was carried out on clinical records of 106 radicular type cervical spondylosis cases treated by PCD from July 1993 to June 2002. Results Follow-up observations for 6 months~8 years(mean 3.6 years) in 91 cases showed the outcomes were excellent in 36 cases, good in 40, and unchanged in 15, the rate of excellent-or-good results being 83.5%. Conclusions PCD has an excellent therapeutic effect for nerve-root symptoms resulted from cervical disc herniation, such as numbness and pain of the upper extremities or shoulders.
3.The Study of Total Flavonoids in Leaves of Lonicera Hypoglauca Miq

Objective To study the qualitative analysis and quantitative analysis of flavonoids and other ingredients in leaves of Lonicera hypoglauca miq,and to optimize the reflux extraction condition for total flavonoids from leaves of lonicera hypoglauca miq. Methods Preliminary test was used for qualitative analysis. UV-spectrophotometry was used to determinate content of total flavonoids. By orthogonal design,the extraction rate of flavonoids was determined in different factors of ethanol reflux extracting. Results Leaves of lonicera hypoglauca miq contained alkaloids and flavonoids.The optimum conditions of ethanol reflux extracting total flavonoids are 12 times 50% ethanol,and refluence for 1.0h at water bath for two times.According to validate experiment,the average total flavonoids content was 16.6% in leaves of lonicera hypoglauca miq,with RSD=1.52%(n=3). Conclusion Flavonoids are in higher level in leaves of Lonicera hypoglauca miq. There are some chemical similarity in leaves of lonicera hypoglauca miq and leaves of flos lonicerae.
4.Change of Angiopoietin-1 in the Peripheral Blood of Patients with Acute Respiratory Distress Syndrome and Its Relationship with Prognosis

Objective To investigate the change of angiopoietin‐1(Ang‐1)in the peripheral blood of patients with acute re‐spiratory distress syndrome and its relationship with prognosis.Methods A total of 85 hospitalized patients with ARDS were recruited and divided into mild group(n=25) ,moderate group(n=36)and severe group(n=24)according to the oxidation states of patients.Another 40 hospitalized patients without ARDS were included as the control group.The basic clinical data of all pa‐tients were obtained.The expressions of Ang‐1 and Tie2 in the peripheral blood were detected by RT‐PCR.The correlation be‐tween the different variables was analyzed by using Pearson correlation analysis.All patients were divided into survival group(n=47)and death group(n=38)according to the deaths of patients after 28 days.The relevant factors for prognostic of patients with ARDS were analyzed by using multivariate Logistic regression analysis.The effectiveness of Ang‐1 in predicting ARDS was analyzed by using ROC curves.Results In ARDS patients relative to the control patients ,PaO2/FiO2 was decreased ,A‐PACHEⅡ score ,LIS score ,SOFA score ,C‐reactive protein(CRP) ,IL‐6 ,and mortality rates were increased ,and the relative ex‐pressions of Ang‐1 mRNA and Tie2 mRNA in the peripheral blood were decreased ,with the differences being statistically sig‐nificant(P<0.05).The relative expressions of Ang‐1 mRNA and Tie2 mRNA in the peripheral blood in severe group were much lower than in the mild group and moderate group ,those in the moderate group were lower than in the mild group ,and there were significant differences(P<0.05).Pearson correlation analysis showed that the expressions of Ang‐1 and Tie2 in the peripheral blood of patients with ARDS were positively correlated with PaO2/FiO2 (P<0.05) ,but were negatively correlated with APACHE Ⅱ score ,LIS score ,SOFA score ,CRP and IL‐6 levels(P< 0.05).Multivariate Logistic regression analysis showed that PaO2/FiO2 and APACHEⅡ score were independent risk factors for the mortality in patients with ARDS(P<0.05) ,while the relative expressions of Ang‐1 mRNA and Tie2 mRNA were protective factors for the mortality in patients with ARDS(P>0.05).ROC curve analysis showed that when the relative expression of Ang‐1 mRNA was used for the diagnosis of ARDS ,the area under the curve was 0.874(95% CI:0.809-0.938) ,the sensitivity was 72.5% ,and the specificity was 85.9% . Conclusion Down‐expression of Ang‐1 in the peripheral blood of patients with ARDS is closely related with the severity of the patient’s condition.It may accelerate the endothelial cell damage and increase the vascular permeability by reducing the binding to Tie2.It may be used as a prognostic indicator for patients with ARDS.
5.Treatment of Lisfranc joint injury with the operation of the Kirsehnerwlres and screw

Objective To summarize the clinical experience of operation treatment of Lisfranc fracture dislocation,and evaluate its clinical effect.Methods Thirty-nine patients with Lisfranc joint injuries (46 sides) were managed with open reduetlon as well as internal fixation with serews and Kirsehnerwlres.Results All the patients were followed up from12.0 to 42.0 months,the average was (21.1 ± 1.8) months.Evaluated the clinical effect according to the American Orthopaedic Foot and Ankle Society (AOFAS) midfoot score standard,there were 29 cases of excellent,8 cases of good,and 3 cases of aceeptable.Conclusion By Kirschner wire combining with screw fixation,open reduction and internal fixation may achieve satisfactory clinical results for the treatment of Lisfranc joint injurie.The operation is simple and fast,small trauma,less complications and can obtain good effects.

7.Effect of treatment and intervention on COPD in stable phase with chronic obstructive pulmonary disease

Objective To investigate the analysis of Symbicort in stable stage of chronic obstructive pulmonary disease treatment and intervention. Methods 80 patients with chronic obstructive pulmonary disease in stable period from June 2014-2016 year in May in our hospital were randomly divided into control group and experimental group, 40 cases in each group. The control group of patients with the use of simple Symbicort treatment and routine nursing care to patients, patients in the experimental group using Symbicort combined with Bailing Capsule for treatment, and the implementation of targeted nursing intervention measures, compare the effect of two groups of patients and nursing satisfaction. Results the clinical treatment of patients in the experimental group the total efficiency 95.0%, nursing satisfaction was 97.5%, the control of clinical patients in the treatment group the total effective rate was 80.0%, nursing satisfaction was 87.5%, the experimental group was significantly higher than the control group, the difference was statistically significant (P<0.05). Conclusion Chronic In the stable period of obstructive pulmonary disease, the combination of Xin Yi capsule combined with Bailing capsule can effectively improve the pulmonary function of patients and improve the clinical therapeutic effect, and is worthy of popularization and application in clinic.
8.A report of the solitary fibrous tumor in nasal cavity.

A 56-year-old woman, with a 1-day history of bilateral nose haemorrhagia, predominantly on the right side, sometimes manifested rhinocnesmus, sneeze and water rhinorrhea, no nose obstruction, and no fever. Endoscopy revealed a smooth and pinkish mass that completely obstructed the right nasal cavity and extended to the base of nose. Computed tomography showed a large mass that occupied the entire right nasal cavity and part of the right maxillary sinus, ethmoidal sinus and frontal sinusitis.

9.A Novel Human Rab26 cDNA Coloning,Expression and Its Endocytosis Enhancing Function

Rab GTPases serve as master regulators of vesicular membrane transport on both the exo-and endocytic pathways. Though there are many reports on Rab proteins, the function of these small proteins still remain in speculation. And no report has ever clarified the character of human Rab26. Here it was reported that a novel Rab protein Rab26 is membranous organelle related and in volved inendocytosis of HeLa cells. By using RT-PCR method a novel Rab26 cDNA full-length cDNA of Rab26 that is 1656 bp was identified.The cDNA sequence that at 1197 is 'A' other than 'G', while 'C' at 956 substitutes for 'T', and has 'GCC' insertion at 48 to 50 compared with published sequences. The complete open reading frame (ORF) is 771 bp in length encoding 256-residue protein with a calculated molecular mass of 27.9 ku (GenBank accession No.AY646153), rather than a shorter one with 190-amino acid residue as reported previously. GFP labeled full-length Rab26 expression showed that Rab26 was mainly sublocated in membranous organelles and could enhance endocytosis which means could took PE labeled protein as an endocytic tracer. RT-PCR analysis showed Rab26 was detected to express in several kinds of adenocarcinoma cell lines such as Acc2, AccM, SPC-A1 and HeLa cell lines, which indicated that Rab26 expression might be associated with some carcinomas.
10.The Clinical Characteristics and Incidence Tendency Analysis on 1 628 Cases of Colorectal Carcinoma

Objective To provide the basis for clinical diagnosis and treatments of colorectal cancer by studying the clinical characteristics and incidence tendency of colorectal cancer in recent 12 years in Kunming. Methods The data of 1628 colorectal cancer patients from 2001 to 2012 in the First Affiliated Hospital of Kunming Medical University were collected. The clinic pathological data of all patients was divided into 2001-2006 (Agroup) and 2007-2012 (B group) two consecutive time periods for statistical analysis according to its time of onset. Results The incidence of colorectal cancer in the city rose from 49.2%to 56.0%, and the incidence of colorectal cancer in rural areas decreased from 50.8%to 44%( <0.05) .The location ratio of colorectal cancer in the rectum decreased from 74.8%to 73.9%, in the left half colon ratio increased from 14.8%to 19%, while the right colon ratio decreased from 10.4% to 7.1% ( < 0.05) . The change in the three parts had statistically significant difference between two consecutive time periods. The percentage of colorectal cancer patients in Dukes A stage rose from 13.4% to 18.8%, while the percentage of patients in Dukes D stage decreased from 22.3% to 14.5% ( <0.01) . Conclusions The incidence of colorectal cancer in Kunming gradually increases.The morbidity in the city is significantly higher than that in the country. The ratio of left half colon cancer is rising, but the ratio of rectal cancer and right half colon cancer is falling. The early diagnosis rate of colorectal cancer is on the rise.
