1.The study of perioperation parenteral nutrition in patients with malignant gastrointestinal tumor and diabetes

Objectives: To study safety method of control of blood glucose in perioperation patients with malignant gastrointestinal tumor and diabetes. Methods: The twenty eight cases with malignant gastrointestinal tumor and diabetes were analyzed retrospectively. Results: The patients with diabetes were safely controled in blood and urine glucose during perioperetion.There were no significant changes in serum lipids.The excretions of urea nitrogen in urine increased 24 hours after operation.The body weights declined and plasma proteins decreased after operation. Conclusions: The nutritional support is necessary and of feasibility in patients with malignant tumor and diabetes during perioperation.It is a key step to control blood glucose by using insuline.The nutritional support can protect function and structure of oragans.
2.Qualitative and quantitative analysis of cocaine by GC/MS and GC

Objective To establish GC and GC/MS qualitative and quantitative analysis methods for identification of cocaine cases. MethodsChoosed and optimized the best analysis parameters of cocaine by GC/MS and GC. We prepared 0.10, 0.20, 0.40, 0.60, 0.80, 1.00, 1.20mg/ml ethanol solution of cocaine standard which all contained 0.6mg/ml diazepam (internal standard) to study the analyse standard curve and the detection limit of cocaine drugs. ResultsThe linear equation of cocaine by GC/FID was Y=1.055X-0.0021, R2=0.9999; That of GC/NPD was Y=0.556X-0.0016, R2=0.9996; The detection limits of cocaine were 10ng by GC/FID method and 2ng by GC/NPD method. On the other hand, we used GC/FID and GC/NPD methods and internal standard to quantitate the cocaine drugs sized by police respectively, the result of concentration was 72%?2.3%. The quantitative data of both methods had good repeatability. ConclusionThe methods established are useful to examine and identify cases of cocaine.
3.Clinicopathological analysis of primary small intestinal tumors in 121 cases

Objective By analyzing the clinical and pathological characteristics of small intestinal neoplasms of patients presenting at our hospital,this study was to improve our cognition of this disease and the prognosis.Methods We collected and reviewed the medical records of 121 patients suffering from small intestinal neoplasms,who underwent surgery at Ruijin hospital from January 2003 to June 2009.Diagnosis was confirmed by pathological examination,and patients were followed-up.Results Intestinal hemorrhage,anemia and abdominal pain were the three main symptoms for all patients.CT,and gastrointestinal endoscopy were valuable for the diagnosis of small intestine neoplasms.Compared with open surgery,laparoscopic procedures can shorten the operation time and the postoperative length of hospital stay.Conclusions Surgical procedure is the key treatment for patients with small intestinal neoplasms.Long term follow-up plays important role in the detection of other synchronous or metachronous gastrointestinal tumors and improves the prognosis.
4.The latest research progress of torpor occurrence mechanism

Torpor refers to a state in which the metabolic activity in the body of the living animal is greatly reduced during the period of reduced food supply, which is manifested as a substantial decrease in body temperature, metabolic level, and exercise level. Mammals have a strict body temperature regulation system to maintain a constant body temperature. When the energy supply is insufficient for a long time, some mammals will enter a hibernation state. Torpor is very similar to the hibernation state. The research on the mechanism of torpor state is of great significance in aerospace, military medicine and other fields. This review summarizes the specific mechanisms regulating the occurrence of torpor from four aspects: adenylate cyclase activating polypeptide (adcyap) neurons, leptin, pyroglutamylated RFamide peptide (QRFP) neurons, and sympathetic nervous system, aiming to provide ideas for further research on the mechanism of torpor.
5.Comparative analysis of percutaneous balloon pulmonary valvuloplasty for pulmonary valve stenosis by three methods

Objective: To evaluate the effects of percutaneous balloon pulmonary valvuloplasty ( PBPV) for pulmonary valve stenosis (PS) by single-balloon, double-balloon and Inoue-balloon methods. Methods: PBPV was performed in 42 patients with PS. The systolic right ventricular pressure (SRVP) , systolic right ventricular outflow pressure and systolic pulmonary arterial pressure ( SPAP) were contrasted before and after the procedure. Systolic transpulmonary valve pressure gradient (TVPG) and total pressure gradient from right ventricular to pulmonary arterial after the procedure were compared among the three gropes. Results:SRVP, SRVOP were decreased from (14. 69?6. 01 )kPa, (12. 16?3. 48)kPa to (8.14?5.26)kPa, (5.39?2.21)kPa and PAP increased from (1.57?0.56)cm to (17. 16? 2.16)cm after PBPV, respectively. TVPGs were (2. 40?2. 05) kPa, (3. 29?2. 25 ) kPa, (2.52? 1.95)kPa and TPGs were (2. 40?2. 05)kPa, (4.98?4.26)kPa, (6.16?6.93)kPa, (5.65?4.75) kPa after PBPV by single balloon, double balloon and Inoue balloon methods, respectively. During the follow-up period (2. 5?1. 6) years, bloodflow velocity at pulmonary valve measured by supersonic Doppler decreased from (2. 38?1. 89) m/s immediately after PBPV to (2.22?1. 96) m/s, further TVPG decreasing was indicated. Conclusion: The short and long term results of PBPV for PS by each of the three methods were excellent. Inoue balloon may be the first choice for elder children and adults, while for younger children, single balloon method may be preferred. Double balloon method can be remained as the second choice when Inoue method failed.
6.The Clinical Characteristics and Incidence Tendency Analysis on 1 628 Cases of Colorectal Carcinoma

Objective To provide the basis for clinical diagnosis and treatments of colorectal cancer by studying the clinical characteristics and incidence tendency of colorectal cancer in recent 12 years in Kunming. Methods The data of 1628 colorectal cancer patients from 2001 to 2012 in the First Affiliated Hospital of Kunming Medical University were collected. The clinic pathological data of all patients was divided into 2001-2006 (Agroup) and 2007-2012 (B group) two consecutive time periods for statistical analysis according to its time of onset. Results The incidence of colorectal cancer in the city rose from 49.2%to 56.0%, and the incidence of colorectal cancer in rural areas decreased from 50.8%to 44%( <0.05) .The location ratio of colorectal cancer in the rectum decreased from 74.8%to 73.9%, in the left half colon ratio increased from 14.8%to 19%, while the right colon ratio decreased from 10.4% to 7.1% ( < 0.05) . The change in the three parts had statistically significant difference between two consecutive time periods. The percentage of colorectal cancer patients in Dukes A stage rose from 13.4% to 18.8%, while the percentage of patients in Dukes D stage decreased from 22.3% to 14.5% ( <0.01) . Conclusions The incidence of colorectal cancer in Kunming gradually increases.The morbidity in the city is significantly higher than that in the country. The ratio of left half colon cancer is rising, but the ratio of rectal cancer and right half colon cancer is falling. The early diagnosis rate of colorectal cancer is on the rise.
7.Efficacy of combination therapy with 5 alpha-reductase inhibitor and alpha-receptor blocker for the treatment of intravesical prostatic profusion in patients with benign prostatic hyperplasia

Objective To evaluate the efficacy of combination therapy with finasteride and doxazosin in benign prostatic hyperplasia/ lower urinary tract symptoms (BPH/LUTS) patients with intravesical prostatic profusion (IPP).Methods A total of 322 BPH/LUTS patients who accepted combination therapy with finasteride and doxazosin were enrolled in this study.Patients were divided into 4 groups according to the degree of IPP:group Ⅰ(IPP>10 mm),group 2 (IPP between 5 mm and 10 mm),group 3 (IPP<5 mm),control group (without IPP).All patients were received inasteride 5 mg once per day and doxazosin 4 mg once per day for 6 months.International prostate symptom score (IPSS),prostatic specific antigen(PSA),ultrasonographic and urcdynamic evaluation were compared before and after treatment.The correlations between the above factors and IPP were estimated by Logistic regression analysis.Results After 6 month of treament,the changes of IPP degree and the maximal urinary flow rate (Qmax) had no significant differences in group 1,group 2 and group 3 as compared with before treatment (all P>0.05).The IPSS in group 1 was not significantly different before and after treatment (P>0.05).There were significant differences in the PSA level,IPSS,total prostate volume (TPV),transition zone volume (TZV),residual urine volume (PVR) in the 4 groups before and after treatment (all P<0.05).Logistic regression analysis showed that PVR and Qmax had positive and negative correlations with IPP (P<0.001 and P=0.024),respectively.Conclusions Combination therapy with finasteride and doxazosin can significantly improve the symptoms of LUST and reduce the total prostate volume in patients with BPH/LUTS,but for BPH patients with IPP,the combination therapy can not effectively alleviate the degree of IPP.The increase of residual urine volume and decrease of Qmax may enhance the risk of bladder outlet obstruction in BPH patients with IPP.
8.A study on calculating the ankylosing spondylitis disease activity score according to the C-reactive protein level

Objective To study the prediction value of C-reactive protein (CRP) level to ankylosing spondylitis disease activity score (ASDAS),the most widely used activity score in ankylosing spondylitis (AS).Methods A total of 386 patients with AS were enrolled and divided into 2 groups by CRP≥3.5 mg/L (n =266) and CRP < 3.5 mg/L(n =120).ASDAS-CRP was evaluated in patients with CRP below the normal range (3.5 mg/L) in different subgroups.Three methods were used to test the consistency between ASDAS-CRP and ASDAS-erythrocyte sedimentation rate (ESR).Results The ASDAS-CRP showed a good correlation with the ASDAS-ESR when CRP level was ≥ 3.5 mg/L (r =0.899,P =0.000).In the group of CRP < 3.5 mg/L,ASDAS-ESR showed better consistency with ASDAS-CRP when CRP level was below 1.5 mg/L(intra-class correlation coefficient 0.902;kappa coefficient 0.70).By the evaluation of variant gradient matrix,CRP 1.5 mg/L coincided with disease activity states.Conclusion There is good consistency between ASDAS-CRP and ASDAS-ESR.When CRP level is below the normal range,1.5 mg/L could be a cut-off value to calculate the optimal ASDAS-CRP score.
9.Secretory Expression of the Fusion Protein PTH-HSA in Pichia pastoris

The fused gene (PTH-HSA) of parathyroid hormone (PTH) gene and Human Serum Albumin(HSA) gene was amplified without linker by Overlapping PCR technology. The spliced gene was clone into Pichia pastoris secretory vector pPIC9K. With the help of promoter AOX1 and mat ? signal peptide, the PTH-HSA gene was designed to secretory expression.Linearized by restriction enzyme SalI, The recombinant plasmid pPIC9K/PTH-HSA was transformed into Pichia pastoris KM71 by electroporation. The recombinant strains which were identified by G418 and PCR analysis were induced by methanol to express protein PTH-HSA. The target protein was expressed in fermentation supernatant. Western blot analysis of the fusion protein showed that the expressed fusion protein PTH-HSA had the antigenicity of HSA.adenylate cyclase assay proved that the fused protein exhibited the bioactivity to stimulate cAMP synthesis The specific activity of broth was about 318IU/ml.
10.The research of surface measurement in calculating chest volume of pectus excavatum rats

Objective To calculate the chest volume for an evaluation of lung development ,the degree of thorax narrowing and pectus excavatum deformity by the surface measurements .Methods Ninety normal Sprague Dawley (4 weeks) rats were ran‐domly divided into experimental and control groups .Experimental group had 70 rats and control group had 20 rats .Cutting off the lower three cartilage from parasternal of rats to produce Pectus excavatum model in experimental groups .We measured many chest radial lines before surgery ,2 ,4 ,8 and 12 weeks after surgery respectively ,then made comparison between parallel group .Thoracic volume size was calculated by a mathematical formula ,Archimedes measurement and the CT three dimensional reconstruction ,and the t test and linear regression were analyzed by SPSS17 .0 .Results this experiment used multiple radial lines to get measurement results ,and through mathematical calculations ,Archimedes measurement and CT reconstruction ,we found there were significant differences between the experimental group and the control group (P<0 .05) .Conclusion It is practicable of surface measurement to calculate chest volume .This method could replace the CT examination to evaluate the change of the chest volume along the process of pectus excavatum formation .
