3.Forensic Analysis of 33 Cases of Fatal Pulmonary Thromboembolism.

OBJECTIVE:
To explore the related risk facts of pulmonary thromboembolism (PTE) and analyze the relation between PTE and the trauma or medical behavior by investigating the cases of PTE.
METHODS:
Thirty-three cases were selected from Institute of Forensic Science (IFS) from 2000 to 2014.
RESULTS:
In 33 cases, 16 decedents were male, 17 decedents were female; different degrees of dyspnea, chest tight- ness and syncope symptoms were the clinical manifestation of the deceased; the thrombus was mainly distributed in the left and right pulmonary arteries. The main source of embolism was the deep vein of lower limb and the left probability was higher. Trauma, limited position, operation and cardiovascular disease showed high-risk factors of PTE; D-Dimer test, hemolytic test and computer tomography pul- monary angiography were the diagnostic tools for PTE. In some cases, trauma and medical malpractice could be involved in the cause of death.
CONCLUSION
Non-typical clinical symptoms present in the most cases caused by PTE, and these cases always show many high-risk factors. The relation between PTE and injury or medical behavior should be considered carefully in the forensic pathological practice.

4.Immunophenotypic Features of NK/T Cell Lymphoma in Children and Its Association with Epstein-Barr Virus

Objective To study the immunophenotypic features of NK/T cell lymphoma in children and its association with Epstein-Barr virus (EBV) infection. Methods Five cases of children′s NK/T cell lymphoma were studied. CD45RO, CD3?, CD56, CD20, TIA-1 and granzyme B were detected by immunohistochemistry staining for investigating immunophenotype. The expression of EBV-latent membrane protein (LMP-1) were detected by immunohistochemistry. EBV-encoded RNA (EBER1/2) were detected by in situ hybridization (ISH). Results CD45RO, CD3?, TIA-1 and granzyme B were positive in 5 cases, CD56 was positive in 2 cases, while CD20 negative in 5 cases.EBER1/2 positive in 4 cases and LMP-1 positive in 3 cases.Conclusions NK/T cell lymphoma in children is strong associated with EBV infection,and EBV infection may play an important role in the pathogenesis of children NK/T cell lymphoma.

Objective To detect the levels of transforming growth factor-?_1(TGF-?_1) in plasma,serum and urinary of children with Immunoglobulin A glomerulonephritis(IgAGN) and mesangial proliferate glomerulonephritis(MsPGN) and explore the different effects of TGF-?_1 in the two diseases.Methods The plasma,serum and urinary TGF-?_1 levels were measured in 24 children with IgAGN,and 30 children with MsPGN and 30 healthy controls by enzyme-linked immunosorbent assay(ELISA).Results The TGF-?_1 levels in plasma,serum and urinary samples of IgAGN group were increased.The TGF-?_1 levels of IgAGN were significantly higher than those of MsPGN and heathy controls(P(0.05)).Conclusion It is showed that TGF-?_1 plays a diffenent role in IgAGN and MsPGN.
6.Regulation of syringin, chlorogenic acid and 1,5-dicaffeoylquinic acid biosynthesis in cell suspension cultures of Saussurea involucrata.

Syringin, chlorogenic acid and 1,5-dicaffeoylquinic acid are three main bioactive ingredients in herbs of Saussurea involucrata with various pharmacological properties, while their contents are very low. In this study, the biosynthesis of syringin, chlorogenic acid and 1,5-dicaffeoylquinic acid in the cell suspension cultures of S. involucrata were regulated by feeding carbon sources and precursors, which resulted in a great increase of the contents and yields of the above three bioactive ingredients. After 16 days of fermentation, the yields of syringin, chlorogenic acid and 1,5-dicaffeoylquinic acid reached 339.0, 225.3, 512.7 mg x L(-1), respectively. Meanwhile, their contents increased up to 67.9, 1.9, 10.6 times of wild medicinal material, respectively. The results provided a solid basis for further studies on application of cell suspension cultures of S. involucrata for large-scale production of bioactive compounds syringin, chlorogenic acid and 1,5-dicaffeoylquinic acid.

7.Clinical observation on Qidi Yiqi Yangyin Huoxue Recipe in treating diabetic nephropathy at stage III and IV.

OBJECTIVETo observe the clinical effect of Qidi Yiqi Yangyin Huoxue Recipe (QYYHR) on diabetic nephropathy (DN) at stage III and IV.
METHODSOne hundred type 2 DN patients of qi-yin deficiency with blood-stasis syndrome were randomly divided into two groups, all were treated with basal hypoglycemic treatment but with QYYHR given to the treatment group additionally for 3 months. The symptoms were observed, blood glucose, urinary albumin excretion rate (UAER), blood urea nitrogen (BUN), creatinine (Cr) and hemorheological parameters were detected before and after treatment.
RESULTSThe total effective rate was higher in the treatment group than that in the control group (89.6% vs. 68.1%, P < 0.01), and the improvement of UAER, BUN, Cr and hemorheological parameters was also significantly better in the former than in the latter (P < 0.05).
CONCLUSIONQYYHR has favorable effects on type 2 DN of qi-yin deficiency with blood-stasis syndrome.

8.Application of embolic protection device in renal angioplasty and stenting

Objective To assess the effectiveness and safety of embolic protection device in renal angioplasty and stenting.Methods From March 2003 through Feb 2005,renal angioplasty and stenting (RAS)were performed in 73 patients with severe renal artery stenosis,14 of them were done with use of distal embolic protection device(EPD,in 17 arteries ).Technical success included not only the stent placement but also the successful use of EPD.Results The EPD and stents were delivered and deployed successfully in all target arteries.The average percentage of renal artery stenosis before and after stent placement were 80.1%?9.0%,and 6.0%?4.2% respectively.The cholesterol particles were found in the EPD grossly in 2 and microscopically in 9 cases.Conclusion The use of embolic protection device during renal angioplasty and stenting is technically feasible and appears to be effective in preventing procedure-related embolization complications.
9.Effect of psoralen on TopoIIα expression in breast cancer stem cells

Objective To explore the effect of psoralen on topoisomerase IIα(TopoIIα) expression of breast cancer stem cells.Methods CD44+CD24-/low breast cancer stem cells were sorted from MCF-7/ADR by magnetic-activated cell sorting(MACS).We observed the growth characteristics of these stem cells through optical microscope and detected the growth-inhibitory effects of psoralen on breast cancer stem cells by CCK-8 assay and IC50 of adriamycin and adriamycin combined with psoralen to calculate the reversal index.The mRNA and protein expressions of Topo IIα were detected using RT-PCR and Western blot, respectively.Results Under the optical microscope, breast cancer stem cells presented spheres.IC10 and IC20 of psoralen on breast cancer stem cells were (6.77±0.23)μg/mL and (10.36±0.21)μg/mL.IC50 of adriamycin and adriamycin combined with psoralen on breast cancer stem cells was (90.03±3.56)μg/mL and (21.47±0.82)μg/mL, the reversal index was 4.19.Psoralen significantly raised the expressions of Topo Ⅱα at mRNA and protein levels.Conclusion Psoralen reversed the resistance of adriamycin by increasing the gene and protein expressions of breast cancer stem cells Topo Ⅱα and the drug targets.
10.Mechanism study on low dose tunicamycin inducing myeloma cells differentiation via unfolded protein response.

OBJECTIVETo explore the molecular mechanism of myeloma cell differentiation induced by low dose tunicamycin.
METHODU266 and RPMI8226 cells were incubated with low dose tunicamycin for 72h. Surface CD49e expression was assayed by flow cytometer (FCM), light chain protein in the cell culture supernatant by ELISA, the unfolded protein response (UPR) related gene GRP78 and GRP94 by real time PCR, and XBP1u and XBP1s transcription and translation changes by real time PCR and Western blot. After XBP1u gene was interfered with small RNA, and constructed plasmid was transfected into myeloma cells to up-regulated gene XBP-1u and XBP-1s reseparately, the differentiation of myeloma cells was observed again.
RESULTSSmall dose tunicamycin could induce both U266 and RPMI8226 myeloma cells differentiation. Compared with the control group, cell morphology changed to mature feature, the nucleo- cytoplasm ratio decreased and nucleolus reduced or disappearance, CD49e expression increased the light chain protein concentration of cell culture supernatant was up-regulated and UPR related gene GRP78 and GRP94 were up-regulated during the differentiation. XBP-1u was up-regulated at both transcription and translation level, while XBP-1s down-regulated. After XBP1u gene expression interfered with small RNA, cell differentiation was disturbed. Cell differentiation was induced while XBP-1u gene was up-regulated by plasmid transfection.
CONCLUSIONLow dosage of tunicamycin could induce myeloma cell UPR and differentiation, while XBP-1u a key role during the process.
