2.Reconstruction of upper lip defect.

OBJECTIVETo summarize the clinical experience for the reconstruction of upper lip defects in different degrees.
METHODSDifferent treatment methods were selected to reconstruct the upper lip defects according to the subunit, size or location of defects on the upper lip. The therapeutic results were analyzed and compared.
RESULTSFrom Jan. 1998 to Apr. 2012, 243 cases with upper lip defects were treated, including 85 cases of traumatic defects, 110 cases of defects secondary to lip cleft and 48 cases of defects after tumor resection. 48 cases were treated with direct closure, 69 cases with cross-lip flaps, 5 Cases with orbicularis oris muscle flaps, 53 cases with unilateral labial flaps, 42 cases with bilateral labial flaps, 22 cases with lower lip flaps, and 1 case with forehead flap, 1 case with forearm flap and 2 with nasolabial fold flaps. The patients were followed up for 3-18 months with satisfactory results.
CONCLUSIONSSatisfactory results could be achieved for upper lip defects with appropriate treatment methods according to the defects degree and characters.

3.High-frequency oscillatory ventilation in children with hematologic neoplasms and other causes induced acute hypoxic respiratory failure

Objective To evaluate the significance of high-frequency oscillatory ventilation(HFOV) used in acute hypoxic respiratory failure(AHRF) children,failing to conventional ventilation.Methods This was a retrospective study of AHRF children ventilated by HFOV from January 2011 to September,2013.All patients were initially treated by conventional mechanical ventilation (CMV),and changed to be treated by HFOV if the patient met to one of the following criteria after the CMV parameters of PIP > 30 mmH2O(1cmH2O =0.098 kPa) or PEEP > 10 cmH2O with FiO2 100% ∶ (1) SpO2 < 90% or PaO2 < 60 mmHg (1 mmHg =0.133 kPa) ; (2) severe respiratory acidosis (PaCO2 > 80 mmHg) ; (3) serious air leakage (mediastinal emphysema or pneumothorax).The following parameters were recorded:patient's gender,age,living PICU time,CMV ventilation time,HFOV ventilation time.We reviewed ventilation parameter settings (MAP,△P,F,FiO2),oxygenation index(PaO2/FiO2,OI),arterial blood gas,heart rate,blood pressure at different time points including late CMV(H0),2 h after HFOV(H2),6 h after HFOV(H6),12 h after HFOV(H12),24 h after HFOV (H24) and 48 h after HFOV (H48),respectively.Various indexes at different time points were compared between survival group and death group,oncology group and no-oncology group.Results PaO2 at H2 compared with H0 had significant improvement[76.9(61.9 ~ 128.0) mmHg vs 50.1 (49.5 ~68.0) mmHg,P =0.006] . PaO2/FiO2 at H2,H48 had significant improvement compared with those at H0,H24 [94.9(66.8 ~ 138.9) mmHg vs 68.0(49.5 ~86.8) mmHg,P=0.039; 135.0(77.6~240.0) mmHg vs 90.7 (54.6 ~161.7) mmHg,P =0.023)].All children's systolic pressure,diastolic blood pressure,heart rate at various time points had no difference (P >0.05).Compared to death group(n =14),PaO2/FiO2,OI at H6,H12,H24,H48 in survival group (n =9) had significant improvement(P < 0.05).Compared to oncology group (n =10),OI at H2,H6 in no-oncology group(n =10) had significant improvement [(19.2 (13.9 ~ 26.6) vs 33.8 (19.7 ~ 48.3),P =0.049 ; 16.0(8.4 ~27.1) vs 28.9(20.9 ~38.9),P =0.027)],and mean airway pressure between two groups at H2,H6,H12 had significant improvement(P < 0.05).Mortality had no significant differcence between two groups (4/10 vs 10/13,P =0.086).Conelusion HFOV used in children with AHRF which had failed with CMV ventilation can improve the patient's PaO2 and OI.Heart rate and blood pressure are stable during HFOV treatment.Oncology group patients needed higher initial MAP to improve oxygenation than no-oncology group patients when changed to HFOV treatment,but the mortality showed no difference between two groups.
4.Comparison of therapeutic efficacy among 3 different regimens of neoadjuvant chemotherapy for FIGO stage Ⅰb2-Ⅱb cervical squamous carcinoma before operation

Objective To compare the short-time effect of 3 different regimens of neoadjuvant chemotherapy (NACT) in patients with FIGO stage Ⅰb2-Ⅱb cervical squamous carcinoma.Methods A total of 50 patients with FIGO stage Ⅰb2-Ⅱb cervical squamous carcinoma were divided into 3 groups:systemic chemotherapy group (n=13) ,trans-arterial chemotherapy group (TAC,n=19) ,trans-arterial chemoembolization group (TACE,n=18) .After 1-3 periods of NACT,all patients received surgical operation.Tumor response and reduction ratio after NACT,side effects,hemorrhage volume in surgery and bad prognostic factors (including intraluminal tumor thrombi,pelvic lymph node metastasis,parametrial involvernent,positive surgical margin,ovary metastasis) of operation sample were statistically analyzed.Results Tumor response,reduction ratio and hemorrhage volume in surgery were significantly better in TAC group and TACE group than those in systemic chemotherapy group (P<0.05) ,but no significant difference was found between TAC group and TACE group (P>0.05) .The incidence rate of intraluminal tumor thrombi and lymph node metastasis was lower in TACE group than in systemic chemotherapy group and TAC group,but there was no statistical difference.No difference of parametrial involvement,positive surgical margin,ovary metastasis and side effects was found among 3 groups.Conclusion For stage Ⅰb2-Ⅱb cervical squamous carcinoma,preoperative TAC and TACE have more advantages than systemic chemotherapy.In comparison with TAC,TACE is expected to reduce the incidence rate of bad prognostic factors such as intraluminal tumor thrombi and pelvic lymph node metastasis.

Myopia is one of the most popular eye diseases all over the world. The development of the current understanding of its mechanism is still limited. Many studies indicated that the growth factors closely related to eye development and myopia. Some growth factors with biological activity, such as transforming growth factor ( TGF ), fibroblast growth factor ( FGF ) and epidermal growth factor ( EGF ), have an impact on scleral thickness variation, the regulation of the development of myopia and so on, which plays a non-negligible role in the pathogenesis of myopia. In this paper, the function of various growth factors in myopia will be reviewed.

Objective:To investigate the influence of unilateral periacetabular osteotomy (PAO) on the bony birth canal (BBC) in female patients with developmental dysplasia of the hip (DDH) by using pelvic 3D-CT maximum-inscribed-sphere (MIS) method.Methods:A total of 62 female DDH patients of childbearing age were included in the present study. 9.Spatial-temporal distribution of pulmonary tuberculosis registered in Fuling District of Chongqing during the 13th Five-Year Plan period

Abstract: Objective To analyze the spatial-temporal characteristics of the active pulmonary tuberculosis (PTB) and the pathogenic positive PTB in Fuling District of Chongqing during the 13th Five-Year Plan period, so as to explore the clustering areas, and provide scientific basis for the precise prevention and control of tuberculosis in Fuling District. Methods The PTB registration data of 27 townships in Fuling District from 2016 to 2020 were collected. The descriptive analysis were used to describe the temporal and spatial distribution characteristics of patients, SaTScan9.0 and ArcGis10.6 was used for spatial-temporal scanning analysis and local auto-correlation analysis. The results were visualized by ArcGis10.6. Results A total of 4 038 case of active PTB patients were registered and a downward trend was observed in PTB during the 13th Five-Year Plan period in Fuling District. The average annual registration rate of PTB was 70.17/100 000, and the annual PTB registration rate declined by 8.21%. The peak of active PTB and etiological positive PTB were mainly concentrated in March and June respectively. The top five streets of cumulative active PTB patients registered were Lizhi street, Dunren street, Chongyi street, Ma 'an street and Jiangdong street, accounting for 60.18% of the total registered PTB patients during the 13th Five-Year Plan period. The top three average annual registration rates were Dunren street (101.35/100 000), Chongyi street (101.34/100 000) and Wulingshan Township (99.21/100 000). The registered PTB from 2016 to 2020 showed a global auto-correlation (Moran's I=0.64, P<0.0001). The "high-high" area of active PTB and the etiological positive PTB all covered Lizhi street, Jiangdong street and Longqiao street. By scanning analysis of spatial-temporal, the primary cluster of active PTB concentrated in the main urban area south of the Yangtze River in Fuling during January 2016 to December 2017, and the primary cluster of pathogenic positive PTB concentrated in the main urban area south of the Yangtze River in Fuling and Jiangdong street during January 2019 to December 2020. Conclusions During the 13th Five-Year Plan period, there was the spatial-temporal clustering of PTB in Fuling District, which mainly gathered in the main urban area south of the Yangtze River in Fuling district and surrounding streets centered on Lizhi street.
10.Effect of Underwater Treadmill Training on Patients with Incomplete Spinal Cord Injury

Objective To observe the effect of underwater treadmill training on patients with incomplete spinal cord injury (SCI).Methods 11 patients with incomplete SCI were treated with underwater treadmill training, besides other comprehensive rehabilitation. Motor and sense scores were assessed using ASIA2000 standard and recorded before and after the underwater treadmill training. Walking distances, maximal walking velocity when training and changes of activities of daily living (ADL) before and after training were also recorded.Results After underwater treadmill training, the sense score of ASIA increased ( P<0.05), motor score of ASIA, and scores of walking distance, maximal walking velocity and ADL increased significantly ( P<0.01).Conclusion The underwater treadmill training along with other routine rehabilitation treatment may be helpful to improve motor and sensory functions and ADL of patients with incomplete SCI.
