1.Therapeutic effect of Xiaoke Huayu tablet on impaired glucose tolerance

Objective To evaluate the therapeutic effect of Xiaoke Huayu tablet on impaired glucose tolerance (IGT). Methods 168 patients who were proved to have IGT were randomly divided into two groups: treatment group (n=86) and control group (n=82). All the patients were asked to take abstinent diet with proper athletic exercises. The patients in the treatment group were treated with Xiaoke Huayu tablet. No drug was given to patients of the control group. The therapeutic effect and data of laboratory findings before and after treatment in both groups were analyzed and compared. OBJECTIVE Epidemic diseases have been recorded in traditional chinese medicine(TCM) for a long time,this paper describes the cognition of TCM on treatment and prevention for epidemic diseases.METHODS This topic was discussed from etiology,pathogenesis,and the process of epidemic diseases in TCM.TCM holds that the occurrence of epidemic diseases is the result of the combined effects of the seasonal and environmental factors,and the internal factor of human body and the epidemic pathogenic factors play the same important role in the process of epidemic diseases.The diagnosis patterns of syndrome differentiation were six meridians,differentiation of Wei,Qi,Ying,and blood,and syndrome differentiation of Sanjiao.RESULTS TCM emphasizes the harmful effects of pathogenic factors on human body,while it pays more attention to the sufficiency of vital Qi defeating the pathogenic factors and the maintenance of human body positive vital Qi.Beside the treatment method,also have some preventive methods been described in TCM,such as different sterilization methods.CONCLUSIONS TCM has its unique system in struggle against epidemic diseases.It is useful and effective in preventing and treating the epidemic diseases.
4.Experimental study of immunoloregulation induced by tripterygium glycosides on IgA nephropathy in rats

ObjectiveTo observe the effect of immunoloregulation induced by tripterygium glycosides on experimental IgA nephropathy in rats.MethodsRat model of IgA nephropathy was established with bovine serum albumin and staphylococcal enterotoxins compound immunization.The urine erythrocyte,urine protein in 24 h,renal function,erythrocyte C3b receptor rosette rate and kidney constitution pathology alteration among the normal control group,model group and tripterygium glycosides group were compared.ResultsCompared with the model group,urine erythrocyte,urine protein,serum Cr and BUN reduced( P<0.05),erythrocyte C3b receptor rosette rate was higher(P<0.05),IgA depositing on glomerular mesangial region was less and renal function improved in rats of the tripterygium glycosides group.ConclusionTripterygium glycosides have better therapeutic effect and adjusting action for immune disturbance.
5.Knowledge-based semi-automated optimization of intensity-modulated radiotherapy plans using a volume modulated arc therapy-configured model

Objective To evaluate the feasibility and dosimetric features of a volume modulated arc therapy (VMAT)-configured model in knowledge-based optimization of fixed-field dynamic intensitymodulated radiotherapy (IMRT) plans based on the Varian RapidPlan system.Methods ① A dose-volume histogram prediction model was trained with 81 qualified preoperative VMAT plans for rectal cancer and then statistically verified.② For clinically approved 10 IMRT plans with the same dose prescription,the above model was used to automatically generate new optimization parameters and dynamic muhileaf collimator (MLC) sequences with field geometry and beam energy unchanged.③ In order to rule out the disparities between different versions,a single algorithm was used to calculate the absolute doses of the original and new plans.④ Statistical analyses were performed on dosimetric parameters after comparable target dose coverage was achieved in the two plans by appropriate normalization.Results On the basis of similar target dose homogeneity and coverage,RapidPlan significantly reduced the doses to the urinary bladder (D50% by 9.01 Gy,P =0.000;Dmean by 8.08 Gy,P =0.005) and the femoral head (D50% by 4.20 Gy,P =0.000;Dmean by 3.84 Gy,P=0.005) but significantly elevated the mean total number of monitor units (1211±99 vs.771±79,P=0.000) and the number of fields with multiple MLC carriage groups.The knowledge-based semi-automated optimization caused a significantly larger number of high-dose hotspots but a similar D2% (52.54 vs.52.71 Gy,P=0.239).Conclusions The VMAT model can be used for the knowledge-based semi-automated optimization of IMRT plans to enhance the efficiency and OAR protection.However,the resulting high-dose hotspots need further manual intervention.

7.Clinical research of needle-pricking therapy for functional retrograde ejaculatioin.

OBJECTIVETo observe the clinical effects of needle-pricking therapy, a newly medical and minimally invasive technique, for functional retrograde ejaculation and to explore its mechanism. Methods Thirty-six patients with functional retrograde ejaculation were randomly divided into an observation group(19 cases) and a control group(17 cases) In the observation group,needle-pricking therapy was used at Guanyuan(CV 4) and bilateral sacral plexus nerve,lumbar 1 nerve and greater occipital nerve stimulating points,once a week. In the control group, midodrine tablets were prescribed orally,three times a day. All the treatment was given for 9 weeks. The clinical effects of the two groups were observed, and the levels of luteinizing hormone(LH), testosterone(Tes) and estra4 diol(E2) were compared between the two groups.
RESULTSThe total effective rate of the observation group was, 89. 47%(17/19), which was better than 47.06% (8/17) of the control group(P<0. 05). The LH and Tes were obviously increased and E2 was decreased compared with those before treatment in the observation group(all P< 0. 01). Tes was raised(P<0. 05) and E2 was apparently declined in the control group(P<0. 01). After treatment, the differences of serum LH and Tes were statistically significant between the two groups(both P<0. 01).
CONCLUSIONNeedle-pricking therapy has advantages for functional retrograde ejaculation probably in that stimulating lumbosacral nerves can strengthen the function of pelvic floor muscles and urethral expansion muscle and regulate sexual gland axis.

8.Experience of using anterior windowing of the femur in revision total hip arthroplasty.

OBJECTIVETo discuss the clinical results of anterior windowing of the femur to remove the femoral component and cement in revision total hip arthroplasty.
METHODSFrom September 1999 to May 2011, 31 revision cases received anterior windowing of the femur in operation. There were 12 male and 19 female, with the average age in operation was 61.8 years (from 40 to 83 years). The reason for revision included aseptic loosening in 12 cases, infection in 4 cases, breakage of femoral stem in 11 cases and acetabular liner wear in 4 hemi-arthroplasties. Nine cases were cemented and 22 were non-cemented for the primary stem. The position of the window located in the anterior femur with 6 cases of complete windowing from the proximal to the end of the stem. Another 25 cases received regional windowing just around the tip of the stem. In revisions, non-cemented rectangular revision stem were used for 27 cases and two-stage surgery were used for 4 infection cases.
RESULTSIn all revisions, femoral stems and cement fragments were removed successfully and safely without any complications of fracture and perforation of new stems. All femur windows showed successfully union for average 14 weeks (12-18 weeks). There were some postoperative complications. One recurrent dislocation was treated using plaster external fixation for 8 weeks. One case with dislocation and fracture along with the window was revised by cerclage fixation. One periprosthetic fracture due to trauma was treated by cerclage fixation. The average Harris score of the hip improved to 83 points (75 to 90 points) at an average 5.5 years follow-up (0.5 to 12.0 years). Twenty six cases were completely followed up. Revision femoral stems were well-fixed without any subsidence or loosening.
CONCLUSIONSAnterior windowing of the femur is a proven technique which can be used to remove femoral stem and cement conveniently and safely without any fractures in revision total hip arthroplasty.

9.Expression and correlation of Lewis y antigen and mucin 1 in ovarian epithelial tumor

Objective To investigate the expression and the clinical significance of Lewis y antigen and Mucin 1 (MUC1),as well as to evaluate the correlation between them in epithelial ovarian tumor.Methods The expression of Lewis y antigen and MUC1 in 60 cases of epithelial ovarian malignant tumors,30 cases of borderline ovarian tumors,30 cases of benign ovarian tumors and 20 cases of normal ovarian tissues were detected by immunohistochemical staining.The relationship between Lewis y antigen and MUC1,and their relationship with biology characteristic of ovarian carcinoma were analyzed.An immunofluorescence double labeling methods was performed to detect the correlation between Lewis y antigen and MUC1.Results In malignant epithelial ovarian tumors,the positive rates of Lewis y antigen was 88.33%,which was significantly higher than the positive rates in borderline(60.00%,x2 =9.6405,P <0.01) and benign ovarian tumors(33.33%,x2 =28.8095,P <0.01) and normal ovarian samples (0,x2 =52.3457,P < 0.01).The positive rates of Lewis y antigen had nothing to do with the clinical pathological parameters of ovarian tumor,but the expression intensity of Lewis yantigen was increased with the development of the malignant degree(P < 0.05).The positive rates of MUC1 in malignant epithelial ovarian tumors was also significantly higher than that in borderline,benign ovarian tumors and normal ovarian samples (86.67% vs 53.33%,30.00%,25.00%,x2 =12.0321,29.4064,27.8464 ; P <0.01).And the expression intensity of MUC1 also increased with the development of clinical stage(P <0.01),but had nothing to do with the lymph node metastasis and histological grade(P > 0.05).In ovarian cancer,both Lewis y antigen and MUC1 were highly expressed,and their expression levels were positively correlated (r =0.707,P <0.01),and Lewis y antigen colocalized with MUC1.Conclusion Both Lewis y antigen and MUC1 are associated with the occurrence and development of ovarian cancer.Lewis y antigen and MUC1 might be a sigh of biological behavior in ovarian cancers,and this study provides theoretical evidence of ovarian cancer biological treatment.
10.Discussing tuina for atlantoaxial subluxation in adults based on the theory of 'tendon-bone balance'

According to Chinese medicine, the atlantoaxial joint is a composite joint composed of tendons and bones, and the stability of the joint depends on the 'tendon-bone balance' involving tendons, ligaments, atlas and axis. Multiple causes of 'tendon off-position, joint subluxation' will lead to joint 'tendon-bone imbalance', which will evolve into atlantoaxial subluxation (AAS), endangering human health. Chinese therapeutic massage (tuina) is a very effective treatment for AAS in adults, but conventional manipulations are prone to ineffectiveness or accidents due to neglect of the causal relationship of the 'tendon-bone imbalance' and inappropriate manipulations. Compared with conventional manipulations, the rational choice of modified manipulations under the guidance of 'tendon-bone balance' theory is more effective and less risky, and more worthy of clinical promotion. From the 'tendon-bone balance' theory, we considered the shortcomings of conventional manipulations, and introduced several modified manipulations that have their own strengths in 'tendon smoothing' and 'bone setting', in order to provide new ideas for treatment of AAS in adults.
