5.Treatment of chronic mallet finger deformity with minor bone anchors and palmaris longus tendon graft.

OBJECTIVETo explore the clinical effects of minor bone anchors and palmaris longus tendon graft in treating chronic mallet fingers deformity.
METHODSFrom January 2008 to June 2013, 26 patients with chronic mallet fingers deformity were treated with minor bone anchors and palmaris longus tendon graft. There were 18 males and 8 females, aged from 18 to 52 years old with an average of (32.0±1.3) years. Among them, 8 cases caused by machine injury, 6 cases by fall injury, 6 cases by sprain from fight, 4 cases by tendon spontaneous rupture, 2 cases by knife trauma. There was no tendon attachment of extensor tendon check in 16 cases, and with 0.3 to 0.5 cm tendon attachment in 10 cases. All patients had the flexion deformity and the disability of dorsiflexion activity. During operation, the distal interphalangeal joint was fixed in 10° to 20° dorsiflexion by a Kirshner wire, the minor bone anchor was used to reconstruct the extensor tendon insertion, the palmaris longus tendon slice was transplanted the decayed area of extensor tendon insertion. Four weeks postoperatively, the Kirshner wire was removed and the plaster external fixation was used, and the patient began function exercises. Postoperative complications were observed and fingers functions were assessed according to Dargan standard.
RESULTSThe patients were followed up from 6 to 14 months with an average of (5.0±0.3) months. Wound superficial infection occurred in 2 cases, the skin pressure ulcer in 2 cases, joint activities disability in 1 case; these symptoms got improvement after symptomatic treatment. Traumatic arthritis occurred in 2 cases, 1 case was improved after treatment, and 1 case had chronic pain for a long time. No internal fixation loosening or breakage and tendon rupture were found. According to Dargan standard to evaluate the finger function, 17 cases got excellent results, 8 good, and 1 poor.
CONCLUSIONIt is an effective way to treat the chronic mallet finger deformity using minor bone anchors and palmaris longus tendon graft, and the method has advantages of reliable fixation, easy operation, satisfactory effect and less complication.

6.Mediating effect of rumination on relationship between loneliness and attachment in college students

Objective:To explore the relationship between loneliness and attachment in college students and the mediating effect of rumination.Methods:Totally 545 students [239 males,306 females;average age (20 ± 3) years] were assessed with the Russell UCLA Loneliness Scale (UCLA),Experiences in Close Relationships Inventory (ECR,including attachment avoidance and attachment anxiety) and Nolen-Hoeksema Ruminative Response Scale (RRS).The structural equation model was used to research the mediating effect of rumination on relationship between loneliness and attachment.Results:The scores of UCLA,ECR and RRS were positively correlated each other (r =0.30,0.20,0.44,Ps < 0.01) in college students.The inspection of the mediating effect found fit indices for hypothetical model were acceptable (x2 =16.69,df=5,NFI =0.98,IFI =0.99,TLI =0.96,CFI =0.99,AGFI =0.96,RMSEA =0.07).Path analysis revealed that attachment avoidance and attachment anxiety predicted directly loneliness (β =0.10,0.06) and rumination (β =0.05,0.16),and predicted loneliness by rumination (β =0.18).Conclusion:The rumination may be associated with loneliness and individual attachment and play a partial mediating effect between loneliness and attachment in college students.

Cyclodextrin(CD) is gradually applied in the nonviral gene vector system,due to its biocompatibility and flexibility of tailing via structural modification,polymerization or supramolecular combination.The ideas and research progress of the CD,its low molecular derivatives,CD polymers and CD supramolecular combination in the field of norviral gene vectros were reviewed,and their "structure-safety-transfection efficiency" relationships were discussed.
8.Application of platysma myocutaneous flap with apron incision in the restoration of oral and maxillofacial defects

Objective:To introduce a new style of platysma myocutaneous flap and to evaluate its application value in the restoration of oral-maxiofacial defects.Methods:Platysma myocutaneous flap with apron incision was used for the restoration of oral-maxiofacial defects after oral lesion ablation in 15 cases from January,2014 to September,2015.The platysma muocutaneous flap was harvested with a U shaped apron incision.The skin above the flap pedicle was preserved.The platysma pedicle was widened to ensure the flap survival.All the patients were followed up form 4 to 33 months.Results:Of the 15 platysma myocutaneous flaps,12 survived completely and 3 had partial flap necrosis.The total survival rate of the flaps at the recipient site of buccal mucosa,tongue and alveolar was 8/9,1/3 and 3/3 respectively.Additionally,there were 2 cases of disturbed wound healing in the neck.Except for 1 case of carcinoma of gingiva which had apparent cervical scar and 1 case of tongue carcinoma which had limited tongue mobility,the other cases showed a satisfactory recovery of oral-maxillofacial contour and fucnction.Conclusion:Compared with the traditional platysma myocutaneous flap,the platysma myocutaneous flap with apron incision can provide a larger skin paddle,and is suitable for the restoration of small and medium sized buccal mucosa and alveloar defects,but not for tongue defect.
9.Vascular endothelial growth factor expression in superficial transitional cell bladder carcinoma

AIM: To study the expression of the VEGF in single and multiple superficial transitional cell bladder carcinoma and their clinical significance. METHODS: Immunohistochemical method was used to study the VEGF in 60 cases of superficial transitional cell bladder carcinoma and in 10 cases of normal bladder tissue as control. RESULTS: High expression of VEGF in bladder carcinoma cell was observed. The expression level of VEGF in multiple superficial transitional cell bladder carcinoma was higher than that in single superficial transitional cell bladder carcinoma. The recurrent rate in the patient with VEGF high expression was more than that in the patient with VEGF low expression. CONCLUSION: The expression level of VEGF was correlated to the biological behavior of superficial transitional cell bladder carcinoma.
10.Appealing for Freer Academic Atmosphere in Bioethics

Bioethics, one of the focus subjects in 21 century,attracts the attention in the worldwide.Originated from western developed countries,it has been branded with distinct western civilization,which is disadvantageous to the development of the subject.Some suggestions are put forward as following: 1.Communication between the East and the West should be promoted in order to form a solid multi-cultural base for bioethics. For instance, in America ethics bears the imprint of religious conventionality,while in China politics is infiltrated into traditional ethical ideas. Both of them are not perfect. So they cannot form the base of bioethics. 2.Research on the "base line" of the bioethics should be emphasized in order to provide freer academic atmosphere. Transplanting the western bioethical ideas to the developing countries will never work. As for the bioethical"base line",different countries should establish feasible ethical codes according to their own actual situations. 3.Scientists should be encouraged to adopt various technical ways to explore the problems in the field of bioethics.Such methods as tissue-engineering,antologous stem-cell replanting and cell diffrenttation inducing may bring about earlier breakthrough than fetus stem-cell.Equal attention,encouragement and support should be given to bioethics.
