Objective To explore the optimal immunohistochemical (IHC)diagnostic index for supraclavicular lymph node me-tastasis in lung cancer.Methods In the premise of conforming to the evidence-based medicine,the biopsy of lymph node in 135 pa-tients with supraclavicular lymph node metastasis was performed and 7 antibodies were selected to be detected.135 cases were di-vided into two classes:class A (110 cases)and B (25 cases).The class A included 14 cases of primary lesion inside the lung by the histological analysis,43 cases of primary lesion inside the lung by the cytological detection and 53.cases of primary lesion inside the lung by the radiographic analysis.The class B included 25 cases of lesion outside the lung by the histological analysis.The detecting antibodies included CK7,CK20,EMA,CEA,TTF-1,SPB and vimentin.Results The sensitivity and specificity of single antibody for deducing the primary lesion inside the lung were 90.0% and 56.0% for CK7,98.0% and 40.0% for CK20,90.9% and 4.0%for EMA,80.9% and 36.0% for CEA,62.7% and 100.0% for TTF-1,65.6% and 100.0% for SPB,and 60.9% and 60.0% for vimentin.The quadruple antibodies (CK7,CK20,TTF-1 and SPB)showed the highest sensitivity(85.0%)and highest specificity (100.0%),which made the relatively high detection rate (80.0%)of lung cancer in clinic.Conclusion Analyzing and researching the IMC results is of great significance to guide the surgical pathological practice based on the evidence-based medicine.

Objective To investigate the expression and significances of DPP-4 and IL-6 in febrile seizures(FS).Methods FS were induced in Sprague-Dawley(SD) rats at P14 in a hot water bath by using classical model of hyperthermia-induced seizures.A genome-wide microarray experiment was generated in the rats.The relationship between the differentially expressed genes were analyzed by the method of bioinformatics, and the gene and protein levels of DPP-4 and IL-6 were detected by QPCR,WB and ELISA.Selected 50 children with FS(FS group) and 25 healthy children(control group), and to compare the gene and protein levels of DPP-4 and IL-6 between the two groups.Results Interaction network diagram of differential gene expression showed that there may be interactions between DPP-4 and IL-6.Animal and clinical experiments showed that the DPP-4 and IL-6 gene and protein levels were significantly higher in FS group compared with control group(P<0.05).Conclusion There were high gene and protein expressions of DPP-4 and IL-6 in the FS group compared with the control group.These results indicated that immune and inflammations may play an important role in the FS, and it has provided an attractive pharmacological target for the treatment of FS in clinic.

Objective To explore the effect of Jiawei Wendan decoction on whole regulating function of CaMK/MAPK signal pathway in hippocampus of depression model rats.Methods Isolated depression model rats as research object were living unpredictable chronic stress.Based on the examination of mRNA/protein expression which was the key factor of the signal pathway of CaMK/MAPK and the intersection CREB1 mRNA expression,the relationship between CaMK Ⅱ/RSK protein expression and the CREB1 mRNA expression were analyzed with relation analysis method.Results ① Bidirectional correlation analysis:the respective coefficient (r value) of CaMK Ⅱ protein expression and the CREB1mRNA expression in hippocampus of the model group,Chinese medicine treatment group and western medicine treatnent group were-0.502 (P ＜ 0.01),-0.643 (P ＜ 0.01),-0.408 (P＜ 0.05) ;the respective coefficient(r value) of RSK protein expression and the CREB1 mRNA expression in hippocampus of the model group,Chinese medicine treatment group and western medicine treatment group were 0.550 (P ＜ 0.01),0.687 (P ＜ 0.0 l),0.407 (P ＜ 0.01).②Regression analysis:the respective regression coefficient of CaMK Ⅱ protein (positive direction),RSK protein (negative direction) and CREB1 mRNA expression in the model group,Chinese nedicine treatment group and western medicine treatment group were R 2 =0.472,F =12.983(P＜0.01),R2=0.666,F=28.961(P＜0.01),R2=0.356,F=8.004(P＜0.01).Conclusion The anti-depressant effect mechanism of Jiawei Wendan decoction is to regulate the whole function of the CaMK and MAPK signal pathway and then further up-regulate CREB1 mRNA expression.

Objective: To study the amount of fat, cholesterol and fatty acid intake in China and provide the basic material for dietary guidance. Methods: Two areas were selected both in North and South China and each area included 3 provinces, or municipality or autonomous region. Three representative survey sites were selected in each province, or municipality or autonomous region. Dietary survey was conducted by the method of weighing and recording and cooking method was also recorded. The amount of food consumption was calculated as standard person (adult male, light physical activity). All foods were gathered as 12 kinds of foods, and each kind of food was cooked and then mixed. The content of fat and fatty acid was analyzed for 8 kinds of foods and the content of cholesterol was analyzed for 4 kinds of foods. The intake of fat, fatty acid and cholesterol per capita was calculated. Results: The amount of fat intake among North I, North II, South I, South II was 70.5 g, 46.5 g, 58.7 g, 71.0 g respectively and the amount of cholesterol intake was 329.6 mg, 128.5 mg, 400.9 mg, 306.0 mg respectively. The main source of dietary fat was from meat and vegetables. Egg was the main source of dietary cholesterol and meat and egg were both the main source of dietary cholesterol in south II area. About 90% of saturated fatty acid was palmitic acid and stearic acid and 90% of monounsaturated fatty acid was oleic acid. Linoleic acid was the principal n-6 polyunsaturated fatty acid and linolenic acid was the principal n-3 polyunsaturated fatty acid. The ratio of S∶M∶P in North I was 1∶1.1∶1, in North II was 1∶1.6∶1.3, in South I was 1∶1.6∶1.3 and in South II was 1∶1.5∶1. Conclusion: The amount of fat intake and fatty acid profile was quite different among different areas and the dietary guidance should be more pertinent The current cholesterol intake was more than dietary guidance in most areas. In addition to egg, meat was also an important source of cholesterol.

Objective To investigate the factors and rate of the forming of the collateral feeding arteries from inferior phrenic artery(IPA) in hepatocellular carinoma(HCC).Methods IPA was demonstrated on angiographs in 137 patients with HCC during transcatheter arterial chemoembolization(TACE),TACE was performed through diaphragmatic inferior artery(DIA) super setectively.Results Of 137 cases,21 cases underwent TACE through DIA(15.3%),of them 2 cases were recurrent after surgical operation(9.5%),3 cases had the tumor receive blood supply from IPA at first TACE(14.3%) and 16 cases were occured after TACE tow and more times(76.2%).Conclusion Collateral branches originated from IPA are important feeding arteries in HCC,TACE of IPA can be performed with a high success rate without major complications.

Objective To evaluate the clinical value of MR spectroscopy in intracranial neoplasm. Methods 52 cases with brain neoplasm or recurrent neoplasm or non-tumor lesion confirmed by pathology were undergone proton MRS before stereotactic biopsy, or operation. Results Of 52 cases, 38 were neoplasm and 14 were nonneoplastic lesion, 34 of 38 brain tumor, 11 of 14 nonneoplastic lesion were diagnosed correctly. The characteristics on MRS of glioma were remarkable Cho increase and NAA decrease, Lac in the tumors and the central necrotic area was elevated. Metastasis: Cho increased,NAA disappeared or obviously decreased and Lac increased, but the peritumoral region was normal. Lymphoma: Cho increased, NAA decreased moderately, Lip presented, but the peritumoral region was normal. Demyelination lesion: slight or median decreased NAA and normal Cr, Lac increased slightly. All metabolism decreased or disappeared in radiation necrosis, Cho was relative high compared with NAA. Brain abscess: Lac increased in the centre of abscess and perilesion was normal.Conclusion MRS has significant value in detecting and differentiating brain lesions.

Objective To explore the relationship between the levels of plasma coagulation factors (F) and acute myocardial infarction (AMI) in low age period (<60 years old) and their diagnostic value in diagnosing AMI in low age period.Methods One hundred and sixty inpatients with low age AMI in the cardiology department of the Heze Municipal Hospital were selected as the case group,and contemporaneous 160 cases of low age non-AMI served as the control group.F Ⅱ,FⅦ,FⅧ,fibrinogen (Fg) and von willebrand (vWF) were measured with enzyme-linked immunoabsorbent anti-sandwich assay.The relationship between coagulation factors and low age AMI was analyzed with univariate and multivariate analysis,and their value for diagnosing low age AMI was evaluated with diagnostic test and receiver operating characteristics (ROC) curve.Results The univariate analysis showed that FⅡ,FⅦ,FⅧ and Fg levels had significantly statistical difference between the case group and control group(P<0.05),and the vWF level had no statistically difference(P>0.05).The multivariate analysis indicated that the FⅡ level≥ 14.27 μg/L and FⅦ level ≥22.99 μg/L were the independent risk factors for low age AMI.The value of FⅡ for diagnosing low age AMI was lower,and the optimal cut off value of Fg for diagnosing low age AMI was 22.99 μg/L,its area under ROC curve was 0.709 with a moderate diagnostic value,and the sensitivity (91.88%) and negative predictive value (86.02%) were higher,the false negative rate (13.98%) was lower,and the accuracy (70.94%) was moderate.Conclusion The FⅡ level ≥14.27 μg/L and Fg level ≥22.99 μg/L are the independent risk factors for low age AMI,and detecting the Fg level could have hint significance in diagnosing low age AMI.

Objective To increase the ultimate yield of periplocin in Periploca sepium adventitious root cultures by a two-stage culture based on nitrogen source.Methods Firstly,the effects of nitrogen source(NH<'+><,4>-NO<'-><,3>-)at different ratios and different total initial nitrogen amounts on the accumulation of biomass and secondary metabolites in adventitious root cultures of P sepium were investigated,and growth and production media for the two-stage culture based on the above results were established.Results The highest biomass and periplocin content were obtained in the culture medium of 15 mmol/L total nitrogen amount with NH<'+><,4>-NO<,3>(1:2)and 30 mmol/L total nitrogen amount with nitrate as the sole nitrogen source.By adopting a fed-batch cultivation strategy,the dry weight adventitious root,periplocin content and yield were increased by 136%,108%,and 389%,respectively when compared with those of the control,reaching up to 8.13 g/L,157.15 μg/g,and 1277.63 μg/L,respectively.Furthermore,it was found that in the process of two-stage culture,the adventitious roots grew thicker significantly after they were transferred into production medium directly.Conclusion The ultimate yield of periplocin in P.sepium adventitious root cultures could be significantly increased by a two-stage culture based on nitrogen source.

Objective To understand oral submucous fibrous (OSF) because the hair cause of disease and the quantitative corresponding measures,do a good job in OSF prevention and control work.Methods The quantitative cluster sampling,according to the diagnostic criteria of the development of the Pindborg,yuhu in Xiangtan city of different types of 57 units of 11 046 people to chew areca cause OSF epidemiological investigation.Results OSF 335 diagnosis example,the prevalence rate of 30.33‰,4 cases were oral cancer,oral cancer coexist rate was 11.94‰; All OSF patients had a history of betel nut,no chewing betel nut (containing the cigarette,wine,and spicy aficionados) were not found in patients with OSF; Of OSF prevalence in the chewing hobby was no differences in sex and age in the crowd; OSF prevalence of high and low with length of fixed number of year of average chewing betel nut dose and chewing betel nuts were closely related(r =0.28828,P ＜ 0.01) ; OSF prevalence was different from eating betel nut additive that had a very significant difference.Different hobbies compatibility with standardized test,7 incidence group had 6 group without significant difference,but people only eat chili can (in the control group,1 329) and outsiders no OSF patients (control group,698).Conclusions Survey results confirm that chewing betel nut is the main factor of Xiangtan people of OSF,and OSF carcinoma prevalence is lower than abroad.

Objective:To observe the impact of rehabilitation training,acupuncture-moxibustion combined with Danshen Chuanxiongqin acupoint injection on the patients with shoulder-hand syndrome (SHS) after cerebral infarction (CI).Methods:82 patients with SHS after CI from March 2014 to March 2016 in our hospital were randomly divided into the group A and the group B (n=41).Patients in the group A received comprehensive drug therapy,acupuncture and rehabilitation training,the group B was given Danshen Chuanxiongqin acupoint injection therapy based on the group A.Before and after treatment,the upper limb movement function score (FMA),digital pain score (NRS),the degree of swelling in the shoulder joint,shoulder joint of motion (ROM),and neural function defect scale (CNFDS),daily life activities ability (MBI) and quality of life scores (WHOQOL) of patients in two groups were compared and analyzed.Results:After treatment,the scores of NRS,shoulder swelling,upper extremity CNFDS were significantly higher than those before treatment,the ROM,while the upper extremity FMA,MBI and WHOQOL of both groups were significantly decreased (P＜0.05),these index improved more significantly in group A than those of group B (P＜0.05).Conclusion:Rehabilitation training,acupuncture-moxibustion combined with Danshen Chuanxiongqin acupoint injection was more effective in the treatment of SHS after cerebral infarction than rehabilitation training and acupuncture-moxibustion.
