2.Interventional therapy of traumatic pseudoaneurysms in internal carotid artery siphon

Objective To determine the methods and results of treating traumatic pseudoaneurysms in siphon segment of internal carotid artery(ICA)by interventional therapy.Methods Twelve cases of traumatic pseudoaneurysms in siphon segment of internal carotid artery were treated.The collateral circulation of Willis circle was observed after DSA.Different methods of treatment were applied according to the collateral circulation of Willis circle.Ten cases were treated by occlusion of ICA completely,1 case was embolized by guglielmi detachable coil(GDC)only.Results Nine of 12 treated by occlusion of ICA were cured.In the 3 cases who had poor collateral of Willis circle,one was cured by GDC embolization alone;one died 48 hours later after ICA occlusion though his consciousness and the activity of extremities were normal during the temporary balloon test occlusion(BTO)of ICA.One died during the training to improve the collateral of the Willis circle.Conclusion ICA embolization is feasible for treatment of traumatic pseudoaneurysms in siphon segment of internal carotid artery after evaluating the collateral circulation of Willis circle.

This paper presents the results of pharmacological studies on Zhi Tan Ling (ZTL). The results showed that ZTL could decrease the ischemic damage of the brain tissue in case of acute cerebral ischemia and increase synthetic metabolism, promote restoration of Cerebral thrombosis, increase cerebral blood flow, improve hemorheologic indexes and meningeal microcirculation in experimental animals. It is indicated that ZTL is effective for ischemic apoplexy through various mechanisms.
4.The correlation between intraplaque neovascularization detected by contrast-enhanced ultrasound with the level of serum homocysteine

Objective To explore the relationship between the degree of intraplaque neovascularization in carotid artery and the level of serum homocysteine.Methods Contrast-enhanced ultrasound were performed on 72 carotid atherosclerotic plaques of 48 patients.Contrast-enhancement within the plaque was categorizde as grade 1 to grade 3.The level of serum homocysteine were detected in the fasting state during the same period.Results According to the degree of contrast enhancement(grade 1 to 3),patients were divided into 3 groups.The more new vessels in plaque,the higher the level of homocysteine.The levels of homocysteine in three groups increased in turn.There were distinct differences among the three groups(F =18.49,P <0.05),and there was significant difference between every two groups (P <0.05).The linear correlation analysis showed that the level of homocysteine was positively correlated with the degree of carotid plaque enhancement (r =0.66,P < 0.01).Conclusions Contrastenhanced ultrasonography could semi-quantitate new vessles in plaque.There was positive correlation between the degree of intraplaque neovascularization with the level of serum homocysteine.Combine with the level of serum homocysteine based on intraplaque neovascularization detected by contrast-enhanced ultrasound,the plaque stability could be more accurately evaluated.
5.Detection of atherosclerotic plaque progression in the abdominal aorta of rabbits with 3T magnetic resonance imaging.

BACKGROUNDWith features of high tissue contrast, MRI can be used for the qualitative and quantitative evaluation of atherosclerosis plaques. In this study we investigated the development of atherosclerosis plaque with high resolution 3T MRI in a rabbit model and compared the findings with the histopathological results.
METHODTwenty male New Zealand white rabbits were randomly allocated into an experimental group (n = 16) and a control group (n = 4). Atherosclerotic lesions were induced in the abdominal aorta by balloon injury and cholesterol feeding. Multiple sequences MRI examination (ToF, T1WI, T2WI, and CE T1WI) were performed at the 2nd, 3rd, and 4th months after aortic denudation. Vessel wall thickness, total vessel area, lumen area, and vessel wall area were recorded. Plaque components were analyzed using histological results as a standard reference.
RESULTSSeventeen rabbits (14 in the experimental group and 3 in the control group) received all three MR examinations. Gradually, from 2 months to 4 months, vessel wall thickness and area in the experimental group increased significantly compared with the control group (P < 0.01). In the lumen area progressive stenosis was not found, even a slight dilation had developed in the experimental group. Lipid, fibrotic and calcified plaques can be differentiated by MR image. According to histological results, MRI had good performance in detection of lipid plaque.
CONCLUSIONMRI can be used to monitor progression of atherosclerosis and differentiate plaque components.

6.Application of FICTION technique to the detection of genetic aberrations in multiple myeloma.

OBJECTIVETo investigate the diagnostic value of FICTION (Fluorescence Immunophenotyping and Interphase Cytogenetics as a Tool for the Investigation of Neoplasms) technique, combining immunofluorescence and fluorescence in situ hybridization (FISH), to detect genetic aberrations in multiple myeloma (MM).
METHODSBone marrow samples were collected from 18 MM and 2 plasma cell leukemia (PCL) patients. Probes targeting IgH and MMSET were prepared using a Nick Translation Kit from Bacterial artificial chromosome (BAC) clones. The immunophenotyping was achieved via the CD138 tyramide signal amplification (TSA)-mediated immunofluorescence, followed by FISH with the prepared probes \[t(4;14), t(11;14), t(14;16)\] and the commercial deletion probes (13q and p53) to detect common genetic aberrations in MM.
RESULTSAll the 20 samples were assayed with the probes mentioned above, and revealed 4 cases with t(4;14), 6 with t(11;14), 1 with t(14;16), 3 with p53 deletion; and 8 with 13q deletion. The remaining 4 cases had none of the 5 aberrations.
CONCLUSIONFICTION technique facilitates the detection of genetic abnormalities of MM in situ; enhances both efficiency and sensitivity of positive detection, thus, could be used as the screening test of molecular diagnosis of MM to guide coming-up risk-adapted therapy and evaluate prognosis.

7.Epididymal cyst in von Hippel-Lindau syndrome: clinicopathologic analysis of 3 cases.

OBJECTIVETo investigate the clinicopathologic features of epididymal cyst in von Hippel-Lindau (VHL) syndrome.
METHODSWe reviewed the clinical data of 3 epididymal cyst patients treated by surgery, and detected the expressions of HIF-1alpha, VEGF, alpha-SMA and CD34 in the epididymal tissue samples by the immunohistochemistry SP method.
RESULTSAll the 3 patients underwent surgical removal of the epididymal cyst. Immunohistochemistry of the epididymal tissues showed HIF-1alpha, VEGF, alpha-SMA and CD34 to be positive. All the 3 cases were confirmed to be VHL syndrome, 1 right after surgery, and the other 2 within 8 years postoperatively.
CONCLUSIONEpididymal cyst is a usual benign disease, which may occur independently of or be complicated by VHL syndrome. If immunohistochemistry of epididymal tissues shows HIF-1alpha, VEGF, alpha-SMA and CD34 to be positive, VHL syndrome should be considered, and further clinical examinations and post-operation follow-up are necessitated.

8.Efficacy of combination therapy of tamsulosin and solifenacin for mild and moderate benign prostatic hyperplasia with overactive bladder.

OBJECTIVETo evaluate the efficacy and safety of the combination therapy of tamsulosin and solifenacin for mild and moderate benign prostatic hyperplasia (BPH) with overactive bladder (OAB).
METHODSWe randomly divided 166 patients with BPH and concomitant OAB into a mild obstruction symptom group (n = 88) and a moderate obstruction symptom group (n =78), 48 of the former group treated with 0. 2 mg tamsulosin + 5 mg solifenacin and the other 40 with 0. 2 mg tamsulosin; 36 of the latter group treated with 0. 2 mg tamsulosin + 5 mg solifenacin and the other 42 with 0. 2 mg tamsulosin, all administered once daily for 12 weeks. We obtained the International Prostate Symptom Score (IPSS), urine storage period symptom score (USPSS), voiding symptom score (VSS), Qmax, residual urine volume, OAB symptom score (OABSS) and adverse reactions, and compared them among different
RESULTSAmong the patients with mild obstruction symptoms, the combination of tamsulosin and solifenacin achieved remark-groups. able improvement in IPSS, USPSS, Qmax and OABSS as compared with the baseline (P < 0.05), but made no significant difference in the residual urine volume (P > 0. 05) , while tamsulosin improved IPSS only (P < 0.05). The combination therapy exhibited an obvious superiority over tamsulosin alone in improving IPSS (9.7 micro 3.0 vs 15.8 micro 3.3), USPSS (8. 1 micro 1.7 vs 12.3 micro 3.1), Qmax ([18.6 micro 2.3] ml/s vs [14.2 micro 2.3] ml/s ), and OABSS (5.3micro 1.3 vs 9.7 micro 2.7) (P < 0.05), but there were no obvious differences in residual urine, urine routine test results and adverse events between the two therapies ( P > 0. 05). In those with moderate obstruction symptoms, the combination therapy significantly improved IPSS, VSS, Qmax and OABSS (P < 0.05) but not the residual urine (P > 0. 05) in comparison with the baseline. The tamsulosin therapy achieved obvious improvement in IPSS, VSS, Qmax, OABSS and residual urine. The combination therapy showed a better effect than tamsulosin only in OABSS (4. 8 +/-1.5 vs 6.5 +/-2.5, P < 0.05), but no significant differences from the latter in IPSS, Qmax, VSS, routine urine test results, and adverse

9.Protective effects of Shuangshen Ningxin capsule on miniature swine after myocardial ischemia by intervention.

OBJECTIVETo study the therapeutical effects of Shuangshen Ningxin capsule on miniature swine after myocardial ischemia by intervention.
METHODMyocardial ischemic model miniature swine induced by self-thrombus via cardiac catheter in left anteriar descending coronary artery (LAD), were administrated Shuangshen Ningxin capsule for 6 days. The changes of coronary arteriography, hemodynamics, biochemistry and pathohistology were observed.
RESULT6 days after modeling, LAD in myocardial ischemic miniature swine was basically embolized, cardiac output (CO), cardiac index (CI), left cardiac work (LCW) and left cardiac work index (LCWI) obviously lowed, and pathohistological analysis revealed myocardial degeneration, necrosis, fibrosis and inflammatory cell infiltration. After being administered with shuangshen Ningxin capsule 6 days, the degree of self-thrombus blocked LAD reduced, hemodynamic indexes of CO, CI, LCW, LCWI and blood plasm superoxide dismutase (SOD) activity increased, and systemic vascular resistance (SVR), systemic vascular resistance index (SVRI) and malondialdehyde (MDA) content were lowed. on the same time, pathohistological degeneration and necrosis reduced.
CONCLUSIONShuangshen Ningxin capsule has anti-myocardial ischemia effect by improving cardiac muscle systolic function, increasing left cardiac work, inhibiting cardiac muscle cellular membrane lipid peroxidation.

OBJECTIVETo study the chemical constituents from the roots of Cudrania cochinchinensis.
METHODThe chemical constituents of C. cochinchinensis were isolated and purified by silica gel, polyamide column chromatography, Their structures were identified on the basis of spectroscopic data.
RESULTSix compounds were isolated and identified. Their structures were established as 3, 5, 7, 4"-tetrahydroxyflavanone-7-O-(6"-acetyl)-glucoside (1), 3, 5, 7, 4'-tetra hydroxyflavanone-7-O-glucoside (2), 2', 4', 5, 7-tetrahydroxy-6-prenyldihydroflavanone (3), 5, 7, 4'-trihysdroxy-6-prenylisoflavanone (4), 1, 3, 5, 6-tetrahydroxyxanthone (5), stilbene-2, 4, 3', 5'-tetraol (6).
CONCLUSIONCompound 1 was isolated from this genus, while compounds 4 and 5 were isolated from this plant for the first time.
