Objective: To analyze the efficiency of ureteroscope and biopsy in the diagnosis of tumor grade, muscle-invasiveness and multifocality in suspected upper tract urinary carcinoma (UTUC) patients in order to find out whether it can be used in the risk stratification of UTUC patients.Methods: A retrospective study of 76 UTUC patients who underwent preoperative ureteroscope and/or biopsy and received radical nephroureterectomy in Peking University Third Hospital during January 2014 to December 2016 was undertaken.Results: In this study, 76 patients were included.There were 31 males (40.8%), and 45 females (59.2%).The median age was 64.5 years (31-88), and 51 patients had the symptom of hematuresis.The tumor was located in renal pelvis in 39 patients, and in ureter in 37 patients.Post-operative pathology confirmed that all the 76 patients included in this study suffered from UTUC, of whom 21 (21.6%) were of low-grade, 51 (67.1%) were of high-grade, 4 (5.3%) were undetermined, and 47 (61.9%) patients were muscle-invasive, and 27 (35.5%) were not, and 2 (2.6%) were undetermined.Among the 50 patients, in whom the grade of the tumor could be diagnosed by biopsy, the sensitivity, specificity, accuracy, positive predictive value and negative predictive value for low-grade tumor was 88.2%, 69.7%, 76.0%, 60.0% and 92.0%, respectively.Among the 27 patients, in whom the muscle-invasiveness could be diagnosed by biopsy, 5 patients were diagnosed with muscle-invasiveness, all confirmed by pathology after surgery and 22 patients were diagnosed with none muscle-invasiveness, turned out to be 50% muscle-invasive and 50% none-muscle invasive after surgery.The accuracy was 59.3%.The accuracy of ureteroscopic biopsy to diagnosis multifocality was 61.0%.On univariate ana-lysis, biopsy grade was associated with postoperative pathology (P=0.001), while gender, age, side, body mass index (BMI), hematuresis, preoperative estimated glomerular filtration rate (eGFR), hydronephrosis, tumor size, location, multifocality and sessile were not associated with postoperative pathology grade.Biopsy grade (P=0.02), preoperative eGFR<90 mL/(min·1.73 m2)(P=0.025) and tumor located in pelvis (P=0.049) were associated with muscle invasiveness.Gender, age, side, BMI, hematuresis, hydronephrosis, tumor size, multifocality and sessile were not significantly associated with muscle invasiveness.Conclusion: Ureteroscope and biopsy can assist risk stratification in upper tract urothelial carcinoma patients.

In the uniform design, all level number of experimental relevant factors are equally distributed in the range of experiment, each experimental point shows representation better. The times of experiment are reduced and the data obtained can be treated with computer. Uniform design is a potent tool in seeking for the optimal experimental condition or the optimal proportion of experiments. So, it is often used to study extraction and preparation form technology of TCM. In recent years, the uniform design has been used in the domain of traditional Chinese recipe and the compatibility of its component drugs. The study showed that the analytical results of TCM theory and that of uniform design are extremely consistent. As an optimal screening means in modernizing study on Chinese herbs, using uniform design in studying classical recipe, the "syndrome" corresponding recipe whereas it is helpful to deeply recognize the scientific connotation of "recipe syndrome".

OBJECTIVETo investigate the mechanism of Compound Qidan Liquid (CQD) for intervening ventricular remodeling (VR) after acute myocardial infarction (AMI) in Chinese mini-pigs from hemodynamic and collagen metabolic views.

METHODSAMI model of Chinese mini-pigs was established by left anterior descending (LAD) coronary artery ligation. The model pigs were then randomly divided into the sham-operative group, the model group, the captopril group, the high and low dose of CQD (hCQD and lCQD) groups, the former two were treated with normal saline and the latter three treated with corresponding drugs by gastrogavage for 4 weeks after modeling. Blood pressure (BP), left ventricular pressure (LVP), maximum ascending velocity of left ventricular pressure (dp/dtmax), myocardial renin (MCR), angiotensin (Ang II), total collagen (TC), procollagen type III (PC III), collagen type IV (CIV), laminin (LM), serum hyaluronic acid (HyA) as well as pathologic changes in myocardium were observed.

RESULTSAs compared with in the model group, levels of BP, LVP and dp/dtmax were significantly higher, LM and Ang II were lower in the hCQD group (P < 0.05 or P < 0.01); LVP was higher in the lCQD group (P <0.05); LVP and dp/dtmax were higher, Ang II was lower in the captopril group (P <0.05). Besides, levels of HyA and TC were lower in all the three medicated groups (P <0.01), while the differences of PC III and CIV among groups were insignificant.

CONCLUSIONCQD has a beneficial effect in Chinese mini-pigs after AMI for increasing LVP and dp/dtmax, improving myocardial contractility and hemodynamic condition, decreasing myocardial Ang II contents, decreasing deposition of collagen so as to alleviate the pathological process of VR after AMI.

OBJECTIVETo study the inhibition effect of CyclinD1 specific small interfering RNA(siRNA) on CyclinD1 gene expression in human keloid fibroblast, investigating the effect of CyclinD1 specific siRNA (siRNA-CyclinD1) on the cell cycle, multiplication and apoptosis.

METHODSAccording to the principle of siRNA design, siRNA-CyclinD1 was designed and the keloid fibroblast were transfected. RT-PCR was used to examine CyclinD1 mRNA expression. Flow cytometry was used to examine cell cycle. The apoptotic rate was analyzed by using Annexin V-FITC/PI kit. The DNA gragmentation were measured by DNA ladder analysis.

RESULTSAfter transfection, the expression of CyclinD1 mRNA decreased remarkably. Twenty-four, forty-eight and seventy-two hours after transfection, the radio of G1 stage cell was (59.80 +/- 3.06)%, (66.01 +/- 4.03)% and (67.43 +/- 5.35)%, all significantly higher than in the control group (54.50 +/- 5.35)%; the radio of S stage cell was (18.40 +/- 1.42)%, (17.21 +/- 1.76)% and (11.07 +/- 1.00)%, significantly lower than in the control group (22.33 +/- 1.49)%; the proportion of the cells in G1 stage increased and those in the S stage decreased in the keloid fibroblast transfected with siRNA-CyclinD1. The apoptotic rate of the siRNA-CyclinD1 group was (7.82 +/- 0.45)%, (15.71 +/- 1.06)%, (18.32 +/- 1.08)%, all significantly higher than in the control group (0.68 +/- 0.12)%, and the DNA gragmentation can be seen remarkably.

CONCLUSIONSChemically synthesized siRNA- CyclinD1 effectively inhibits. The expression of CyclinD1 in keloid fibroblast thus arresting the cell cycle at G1 stage and enhancing cell apoptosis. Our study provided a preliminary results in seaching of a RNAi therapy of keloid.

OBJECTIVETo investigate how to select an appropriate surgical approach in the management of fracture and dislocation of lower cervical spine combined spinal cord injury.

METHODSThe clinical data of 54 patients of lower cervical spine fracture and dislocation were retrospectively analyzed. There were 29 cases with vertebral body compressive fracture and dislocation, 7 cases with vertebral body bursting fracture and dislocation, 3 cases with unilateral facet dislocation, 15 cases with bilateral facet dislocation. All cases were associated with spinal cord injury. According to American Spinal Injury Association (ASIA) grades, 21 cases were in A grade, 5 cases in B grade, 22 cases in C grade and 6 cases in D grade. All patients had surgical reduction, decompression, stabilization and fusion, 43 cases in anterior approach and 11 cases in posterior approach.

RESULTSAll patients were followed up in 12 to 36 months, the mean follow-up time was 18 months. There were no great vessels, trachea, esophagus or spinal cord iatrogenic injury. There were no pull-out and breakage of screws or plates. Fusion was achieved in all patients at an average of 12 weeks postoperatively. There were no pseudarthrosis or bone nonunion. Of all the patients, 96.3% were acquired completely reduction and the normal intervertebral height and lordosis were maintained. Patients with complete spinal cord had no neurologic recovery, but they felt relief from upper limb pain or numb. Incomplete spinal cord lesions improved on average 1-2 Frankel grade after surgery.

CONCLUSIONSFor lower cervical spine fracture and dislocation, an ideal anatomy reduction can be obtained with either anterior or posterior approach surgery. It is important to select a suitable surgical approach according to different types of cervical fracture and dislocation.

OBJECTIVETo study the risk factors of secondary kyphotic angle increment after bone cement vertebroplasty for osteoporotic vertebral compression fractures.

METHODSFrom October 2005 to May 2006, 32 (45 vertebrae) bone cement vertebroplasty procedures were performed. The operation time, injected cement volume, bone mineral density, visual analog scale (VAS) pain score, vertebral height, and kyphotic angle were recorded. The secondary increment of the kyphotic angle was calculated, and correlation analysis and linear regression analysis were performed.

RESULTSThe bone mineral density, the postoperative kyphotic angle and the vertebral midline height were significantly correlated to the secondary increment of the kyphotic angle.

CONCLUSIONLarge postoperative kyphotic angle, poor postoperative recovery of the vertebral midline height, and low bone mineral density are all risk factors of secondary increment of the kyphotic angle.

OBJECTIVETo observe the effects of combination therapy with glycyrrhizin (GL) and triptolide (TP) on collagen-induced arthritis (CIA) rats.

METHODSixty male SD rats were randomly divided into 6 groups: the model group, the TP group, the GL group, and combination 1, 2, 3 groups. The models were induced by collagen type II. The arthritis index (AI) and the edema rate were detected as curative effect, and the level of antibodies to collagen, TNF-alpha and IL-10 were measured by ELISA.

RESULTThe combination therapy with GL and TP significantly reduced the paw edema and arthritis index of CIA rats (P <0. 01 ), and the combination therapy can increase the level of IL-10, while decrease the level of TNF-alpha, and the level of antibodies to collagen decreased too (P <0.05, P <0.01).

CONCLUSIONCombine 26.78 mg x kg(-1) GL with 13.40 microg x kg(-1) TP can significantly inhibited the CIA, and the effect equal to the dosage of 17. 86 microg x kg(-1) TP. It supports the possible of GL in combination with TP to reduce the dose and side effects related to TP.

OBJECTIVETo improve the level of clinical diagnosis and differential diagnosis of benign and malignant prostate lesions.

METHODSOne hundred and nine cases of prostate cancer and prostate hyperplasia were evaluated by the expression of high molecular weight cytokeratin (CK34BE12), prostate specific antigen (PSA) and protein P53 gene using the immunohistochemical technique.

RESULTSThe basal-cells in all of the benign lesions were stained with the CK34BE12 and PSA, while it had not immunoreactivity with P53. In contrast, the prostate carcinoma were not stained or partly stained with the CK34BE12 and PSA, but P53 show significant immunoreactivity with the tissue.

CONCLUSIONBased on the routine histological studies with the expression of CK34BE12 and PSA together, they can indicate the existence of basal-cell distinctly and show indirectly whether the basal-cell is integrated. Combining the expression of P53 to determine the existence of cancer gene, it can help to distinguish benign and malignant prostate lesions.

OBJECTIVETo extract tumor interstitial fluid (TIF) from MKN-45 gastric cancer which is similar to "muddy phlegm" in Chinese medicine and observe influences of MKN-45 tumor interstitial fluid (MKN-45 TIF) intervention on metastasis of gastric cancer and on the expressions of vascular endothelial growth factor (VEGF), kinase insert domain containing receptor (KDR), epithelial-cadherin (E-cad), cyclooxygenase-2 (COX-2), intercellular adhesion molecule-1 (ICAM-1) and telomerase genes and proteins in primary tumor tissue.

METHODSAn MKN-45 tumor-bearing model was established in 50 nude mice. The modeled animals were equally randomized to 5 groups: the simple tumor-bearing group (model group), the normal saline (NS) via tail vein injection (i.v.) group (NS i.v. group), MKN-45 TIF i.v. group (TIF i.v. group), NS intraperitoneal injection (i.p.) group (NS i.p. group), and MKN-45 TIF i.p. group (TIF i.p. group). The TIF and NS intervention groups received injection (i.p. or i.v.) of MKN-45 TIF or NS twice a week, 0.2 mL at a time. After 8 weeks, the primary tumors were removed, weighed and HE stained to observe tumor metastasis. The primary tumor tissues were analyzed by immunohistochemistry and real-time quantitative PCR to detect expressions of VEGF, KDR, E-cad, COX-2, ICAM-1, and telomerase genes and proteins in different groups.

RESULTSThere were significant differences in tumor weight between TIF intervention groups and the model and NS intervention groups. Tumor metastasis was observed in all 5 groups, but the tumor metastasis rate in TIF intervention groups was significantly higher than those in the model and NS intervention groups. The gene and protein expressions of gastric cancer-related factors VEGF, KDR, COX-2, ICAM-1 and telomerase were unregulated while the gene and protein expressions of E-cad were downregulated in TIF intervention groups.

CONCLUSIONSTIF promotes tumor growth, invasion and metastasis of gastric cancer. These findings provide preliminary experimental clues for verifying the hypothesis of "tumor-phlegm microenvironment".
