Journal of Clinical Surgery 2015;(2):145-146,147
Since the instability of the spine caused by trauma, tumor, tuberculosis, deformity cor-rection and other reasons requires reconstruction, many scholars have conducted a large number of thorough researches, but the treatment options for unstable cervical spine still plague the clinicians.Considering the factors of stability and safety, more and more clinicians choose cervical pedicle screw placement as the treatment for spinal instability. In this work, the most common methods and the latest technologies of cervi-cal pedicle screw placement at home and abroad are summarized and reviewed. It provides references for clinical practices and presents the significance of high-tech assisted screw placement in the future.

Background:Upon inhibition of STAT3 signaling pathway,cucurbitacin-I elicits anticancer effect in various malignancies. However, the anticancer effect and underlying mechanism of cucurbitacin-I in gastric cancer is still elusive. Aims:To explore the effect of low nanomolar cucurbitacin-I on cell proliferation, cell cycle and apoptosis in human gastric cancer cells and the underlying mechanism in vitro. Methods:Human gastric adenocarcinoma cell lines AGS and HGC-27 were treated with cucurbitacin-I at low nanomolar concentration. The anti-proliferative effect of cucurbitacin-I was detected by CCK-8 assay and colony formation assay. Flow cytometry was used to assess the cell cycle and apoptosis. Expressions of cell cycle-related proteins,as well as activation of related pathways such as caspase-3 / PARP apoptotic pathway,STAT3, GADD45α and JNK/ p38 MAPK signaling pathways were determined by Western blotting. Results:Cucurbitacin-I markedly inhibited the growth of gastric cancer cells at low nanomolar concentration by inducing G2 / M phase arrest and apoptosis via a STAT3-independent manner. Furthermore,it was revealed that the anticancer effect of cucurbitacin-I was associated with up-regulation of GADD45α,activation of JNK/ p38 MAPK signaling pathway and the subsequent apoptotic events. Conclusions:The present study provides new insights into the mechanism of anticancer effect of cucurbitacin-I, supporting cucurbitacin-I as an attractive therapeutic drug in gastric cancer.

Objective To explore the plasma ghrelin levels in children and adolescents with short stature and the role of ghrelin in growth hormone-releasing hormone-growth hormone(GHRH-GH) axis. Methods One hundred and fifty-seven children(115 male,42 female) with short stature were selected.Fasting plasma sample was extracted from 10 mL vemous blood of the children with short stature.Insulin tolerance test and arginine stimulation test was performed initially to differential diagnosis. And blood samples was divided into 3 ca-tegories:37 cases of complete growth hormone deficiency (CGHD),52 cases of partial growth hormone deficiency(PGHD) and 68 cases of idiopathic short stature(ISS) during these two growth hormone(GH)provocative tests.Controls consisted of age and gender-match 20 health children.Plasma ghrelin levels were measured by radioimmunoassay.Serum GH was detected by chemiluminescence method, and serum insulin-like growth factor-1 (IGF-1) was measured by using enzyme linked immunosorbent assay.Fasting glucose,insulin,testosterone,estra-diol,luteinizing hormone,follicle-stimulating hormone were measured.Statistical analysis were conducted by using SPSS 11.5 software. Results The fasting ghrelin levels of CGHD group were significantly lower than that of ISS group and control group(Pa0.05).The ghrelin levels were positive correlated with the stimulated GH peak(r=0.176 P0.05).Conclusion Ghrelin has an important role on GH secretion and abnormal secretion of ghrelin might be a reason of growth hormone deficiency which due to hypothalamic abnormality.

Dihydroartemisinin is an important derivative of artemisinin. We used dihydroartemisinin as the starting material, through esterification, amination and acylation, a series of novel piperazine-sulfonamide contained dihydroartemisinin derivatives were firstly synthesized and their chemical structures were confirmed by IR, 1H NMR, 13C NMR and HR-MS. X-diffraction was used to determine the final configuration of the compound 3c. And the in vitro anti-HeLa activities of compounds 3 were analyzed with CCK-8 method. The preliminary bioassay test shows that compound 3 showed the best inhibition activities against HeLa with IC50 values of 0.14 micromol x L(-1).

Objective To develop a visible detection system prepared by protein microarrays which was based on GoldMag particles-labeled technique and compare the results of detection using the protein which was labeled with GoldMag particles and colloidal gold. Methods Human IgG was printed on the glass slides modified with epoxy groups, and goat anti-human IgG conjugated with GoldMag particles and colloidal gold respectlively was then added on the slide. The glass slides were incubated, and then the black images of microarray spots were produced by immuno-gold-silver staining method and observed by naked eyes and recorded with common flatbed scanner. Results The high signal-to-noise ratio could be obtained when the optimized procedures of GoldMag particles-labeling probe were introduced to the protein chip. The conditions of optimum assay were as follows:the spotting concentration of human IgG was 0.2 mg/ml,the glass slides were incubated at 37 ℃ for 2 hour to immobilize human IgG,and the silver enhancement time was 10 min-15 min.Parallelly, the optimized conditions for colloidal gold were as follows:the spotting concentration of human IgG was 0.1 mg/ml,the slides were incubated at 37 ℃ for 1 hour to immobilize human IgG,and the silver enhancement time was 15 min-20 min. Conclusions GoldMag particles-labeled protein technique is comparable to colloidal gold in applying to protein microarrays. The method is considerably simple and practical, and the protein labeled by GoldMag particles could be quantitative.

Objective To determine the risk factors related to venous thromboembolism after knee arthroscopy. Methods A retrospective study including patients from Feb 2012 to Mar 2016 was carried out to analyze the risk factors of venous thromboembolism after knee arthroscopy. A 1 : 2 matched control group was generated according to the surgeon and the date. Preoperative and perioperative data were collected with respect to age, gender, body mass index, smoking, blood fat, surgical procedure, the time length of ligating tourniquet, anesthesia methods, surgery time and Caprini evaluation. Univariate and multivariate analyses were performed. Results 29 cases of venous thromboembolic events (VTEs) occurred, resulting in an incidence rate of 1.17%. Factors associated with an elevated risk of postoperative VTEs included age OR^ =1.09, 95%CI (1.03, 1.16) and Caprini evaluation OR^ =2.97, 95% CI (1.39, 6.32). Conclusions In this study, VTEs occurred infrequently. Considering the serious result of VTEs, it is important to prevent it. Age and Caprini are associated with elevated risk of postoperative VTEs. It is essential to target those at high risk for VTEs and appropriately treat those patients.
