2.Thinking on Several Ethical Problems in the Clinical Research

This study was conducted in the First Affiliated Hospital of Zhejiang Chinese Medical University for the project of Establishment and Assessment of Ethics Review Platform for TCM Clinical Research. The Ethics Review Platform for Clinical Research is a complete system, which is a Participant Protection System. It includes four as-pects, which are the medical and health institution, ethics committee, ethics committee office, research departments and researchers. In this system, the establishment of research departments and researchers platform is the most cru-cial part. All treatment, protection, risk control, adverse event supervision, which are closely related to the Partici-pant Protection System are completed by researchers. However, during the assessment process, a series of problems were found, such as the current weak ethical consciousness of researchers, less familiar with ethical review process and do not pay equal attention to the participant protection as clinical research. This kind of problem was also found in other hospital assessments in China, which has become a short board of the Participant Protection System.Therefore, from the aspect of the ethics committee, this article presented several ethics problems which should be paid more attention to by researchers such as participant protection, plan design and conduction during the clinical research and also gave some useful advices to change the situation, according to the Assessment Standard of Ethics Review Platform for TCM Clinical Research. In this way, researchers will develop their ethical consciousness and be more familiar with the ethics review process in order to complete key task of participant protection.
4.MRI study on the surface area of occipital lobe in children with ametropic amblyopia

Objective Using MRI techniques and the FreeSurfer software package to research the occipital lobe's surface area in children with ametropic amblyopia.Design Prospective,nonrandomized clinical trial.Participants The amblyopic group included 10 children with ametropic amblyopia.The control group included 10 children with normal sight.Methods The brain MRI scan was per- formed with SIEMENS AVANTO 1.5 Tesla scanner.For the surface area analysis,3-demensional MPRAGE images were analyzed with FreeSurfer software package.Main Outcome Measures Occipital lobe related surface area(cuneus,later occipital,lingual,perical- carine)of bilateral hemisphere.Results No difference was between the ages of the two groups.The surface areas of the lingual,pefical- carine,cuneus of bilateral hemisphere and lateraloccipital on the left hemisphere in amblyopic patients were smaller than those of nor- real children(P
5.The Advancement about Baculovirus Surface Display System

At present, microbe surface display system mainly involves phage surface display system, bacte- rial surface display system, yeast surface display system and virus surface display system. Baculovirus sur- face display system is a new type of eukaryote surface display system which developed based on deeply un- derstanding of construction and function of virus genome in recent years. Through fused expression with vi- ral capsid or membrane proteins exogenous peptides can be displayed on the surface of the virus and formed hedgehog-shape "fake virus". Baculovirus surface display system was characterized by safeness and high performance, furthermore, this system can complete post-translation processing and modification of protein to enhance the bioactivity of exogenous product. Combined with the author’s experimental work, this paper briefly introduces the mechanism and traits of this system and summarizes the newest research development on its application in the field of monoclonal antibody preparation, new-type vaccine development, genes transduction and genes therapy. It is believed that the system above may show extensive application through further improvement and optimization.
6.Diagnosis and scr eening for antimalarial retinopathy:two cases report and literature review

Objective To analyze antimalarial assosiated retinopathy detected by different methods with of modern ocular fundus examination, and explore screening strategy and the diagnosis of hydroxych-loroquine assosiatedretinopathy according to clinical experience and international guidelines. Methods Full fundus examination was performed in two patients with antimalarial retinopathy. The related literature were reviewed. Results Two patients had bull's eyes maculopathy and abnormal visual fields. Conclusion Clin-icians' alertness to hydroxychloroquine related retinopathy may improve early diagnosis of hydroxychloroquine toxicity. New objective tests are more sensitive than visual fields examination. Visible bull's-eye maculopathy is a late change, and the goal of screening is to recognize toxicity at early stage.

8.The training needs analysis and improvement strategies of general practitioners' communica-tion skills

Objective To point out the weak link of general medical education and explore the training mode and evaluation mechanism for communication. Methods Totally 100 general practi-tioners from four Shanghai hospitals took a questionnaire survey to know the present situation and the training needs of doctor-patient communication skills. Results Among all the subjects, there were 73%who thought strengthening the doctor communication ability was the key to improving the doctor-patient relationship, 71%who chose doctor-patient communication practice course as the training con-tent, 78%who tended to personalized analysis and guide, 57%who accepted experienced GP in class. Conclusion For GPS communication skills training requirements, curriculum integration, clinical practice teaching should be strengthened to set up the corresponding training evaluation system.
9.Preliminary study on induction of mouse embryonic stem cells into neural stem cells in vitro

AIM:To explore the feasibility of inducing mouse embryonic stem cells into neural stem cells in vitro. METHODS: Embryonic body induced by retinoic acid and retinal m?ller cells were selected in neural stem cell-defined medium for 7 days, and the morphological changes were observed. The selected cells were stained immunocytochemically with anti-nestin, anti-BrdU antibodies, and their ability of expansion and differentiation were analyzed. RESULTS:Large amounts of neurospheres were derived from embryonic body in the selected medium on the 7th day, which could be passaged and differentiated, stained positive with nestin and BrdU, and expressed nestin, glutaminase and Brn-3 genes. CONCLUSION:Neural stem cells could be derived from embryonic body induced by RA and m?ller cells in the selected medium, which would offer an alternative to treat neuropathy such as glaucoma and retinal degeneration in the future.
10.Retrospective and follow-up study on autologous haematopoietic stem cell transplantation in ten patients with refractory inflammatory bowel disease

Objective To investigate the safety and efficacy of autologous haematopoietic stem cell transplantation (HSCT) in treatment of patients with refractory inflammatory bowel disease (IBD). Methods Ten patients with active moderate-severe IBD [nine with Crohn's disease (CD) and one with ulcerative disease (UC)] were treated with HSCT from January 2004 to August 2006.Among 9 CD patients,the CD active inedx(CDAl) of 2 patients were above 450 (severe),6 patients were 150-450 (active).One patient was suffered from severe UC(whole colon).The stem cells were collected from the patients who intravenously received cycloptlosphamide (2.0 g/m2 ) and granulocyte colony-stimulating factor (5 μg · kg-1 · d-1 ).The CD34+ cells were isolated and cryo-preserved.After 2 weeks,the HSCT was carried out.Results In 9 patients with CD,clinical complete remission (CDAI<150) was achieved in 5 and 1 patients at 3 and 12 months after HSCT.The CDAI of other 2 patients decreased but not reached clinical complete remission.The patients were followed up of 16.1 months (ranged 10-33 months).The disease relapsed in 4 patients and complete remission in 5 patients.However,no improvement was observed under repeated eolonoscopy in 1 patient with UC who had not relapsed in 10 months.HSCT-related side effects included neutropenia caused fever,infection,etc.One HBV-carrier developed asymptomatie increase of liver enzymes and HBV-DNA copies after HSCT.Conclusions Autologous HSCT can be conducted safely and is well tolerated in patients with refractory IBD.It can induce clinical remission in most of the patients,although endoscopic remission may not be achieved,and relapses can not be avoided in some patients.
