1.Protective effects of remifentanil preconditioning on myocardium against ischemia-reperfusion injury in rats

Objective To investigate the role of KATP channel in cardioprotective effects of remifentanil preconditioning (RFC) against ischemia-reperfusion (I/R) injury.Methods Forty-eight male SD rats weighing 200-250 g were killed and the hearts were immediately removed and perfused retrogradely at 100 cm H2 O with Krebs- Ringer's solution aerated with 95 % O2 and 5 % CO2 in an Langendorff apparatus. Myocardial ischemia was achieved by tightening the snare which was placed around anterior descending branch of left coronary artery and confirmed by the appearance of a regional cyanosis, decrease in coronary flow (CF) and S-T segment changes on ECG. After 15 min stabilization all hearts were subjected to 30 min ischemia followed by 120 min reperfusion. The 48 isolated hearts were randomly divided into 6 group ( n = 8 each): ( I ) I/R group; ( II ) RPC group received 3 episodes of 5 min remifentanil (100 ?g ? L-1 ) perfusion at 5 min interval before ischemia; ( III ) HMR group; (IV ) 5-HD group; ( V ) HMR + RPC group and ( VI) 5-HD + RPC group. Group III and IV received HMR-1098, a selective sarcolemmal KATP channel blocker (1? 10-4 mol?L-1 ) or 5-HD, a selective mitochondrial KATP channel blocker (1 ? 10 3 mol?L-1 ) perfusion for 45 min before ischemia. In group V and \1 HMR-1098 or 5-HD perfusion was started 10 min before RPC. Infarct size (IS) was determined by 2, 3, 5-triphenyl-tetrazolium staining. Coronary outflow was collected and recorded and lactate dehydrogenase (LDH) activity in coronary outflow was measured. Results The infarct size was significantly smaller and significantly less LDH was released in group II (RPC) than in I/R group. The protective effects of RPC was abolished by pretreatrnent with 5-HD but not HMR-1098. RPC and HMR-1098 caused a slight but significant increase in CF, however there was no significant difference in CF among ail groups during ischemia and reperfusion. Conclusion Mitochondrisl KATP channel activation is involved in the protective effect of RPC on myocardium against I/R injury.
2.Effect of remifentanil on electrically induced intracellular Ca~(2+) transients in rat ventricular cardiomyocytes

Objective To elucidate the direct effect of remifentanil on intracellular Ca2+ ( [Ca2+ ]) transients using electrically stimulated individual rat ventricular cardiomyocytes. Methods Male SD rats weighing 190-210 g were used. The animals were decapitated and their hearts were immediately removed. Fresh ventricular cardiomyocytes were enzymatically isolated with collagenase. For measurement of the electrically induced [ Ca2 + ] transients, cells were first loaded with fura-2/AM as a Ca2+ indicator and [Ca2+ ] transient was determined by spectrofluorometric method. The fluorescence ratio of 340/380 run was used as an index of [ Ca2+]. Myocytes were placed in a chamber and perfused with a bicarbonate Kreb's solution at room temperature. Myocytes were randomly divided into 10 groups according to the different concentrations of remifentanil contained in Kreb's solution: control group (no remifentanil n = 8) and 9 remifentanil groups (0.1, 0.3, 1.0, 3.0, 10, 30, 100, 300 and 1 000 ng ?ml-1) ( n = 8 each) .Results Remifentanil caused decrease in [Ca2+ ] transients in a dose-dependent manner. 5.Fast limit test of trace aconitine in Xiaohuoluo Pills

AIM:To establish a capillary electrophoresis with field-amplified sample stacking method for the separation and determination of aconitine alkaloid in Xiaohuoluo Pills. METHODS: An uncoated fused-silica capi-llary column(50 ?m i.d.?43 cm,effective length 35 cm) was used.The running buffer was 60 mmol/L phosphate electrolyte solution(pH=6.0)-acetonitrile(8(∶)2).The ruuning voltage was 10 kV and the capillary inlet was dipped in water for 5 s with 3.5 kPa prior to electrokinetic injection(12 kV,30 s).The detection wavelength was 235 nm. RESULTS: This method allowed 400 fold enrichment of aconitine alkaloid.A good linear relation was obtained in the range of 37.5~2.4?10~3 ng/mL (r=0.999 5),with the detection limit of 9.4 ng/mL.The average recovery was 97.6% with the RSD of 1.9%. CONCLUSION: The method is simple,rapid,specific and selective with high stacking efficiency;it provides a new reliable means for production and quality control of Xiaohuoluo Pills.
6.Expression and significance of NgR mRNA in adult rats′ optic nerve

Objective To observe and evaluate the expression and significance of Nogo-66 receptor (NgR) mRNA in adult rats′ optic nerve. Methods Optic and sciatic nerves of 8 adult rats were used to make the sections, which were divided into 3 groups: optic-nerve experimental group, sciatic-nerve control group, and optic-nerve negative control group. In situ hybridization was used to observe the expression of NgR mRNA in optic nerve and sciatic nerve. Results The expression of NgR mRNA in the 8 rats was positive in optic nerve and negative in sciatic nerve. The positive signals were arranged along the long axis of optic nerve. Conclusion The expression of NgR mRNA is positive in optic nerve while negative in sciatic nerve in adult rats, which suggests that the positive expression and distribution of NgR may be related to the poor regenerate ability of optic nerves.
7.Safety and efficiency of unidirectional barbed suture for laparoscopic partial nephrectomy

Objective To evaluate whether the unidirectional barbed suture is safety and efficiency enough for laparoscopic partial nephrectomy.Method From March 2013 to March 2014,27 patients with renal mass have been involved into this study and divided into 2 groups wsing a lottery.Group A used Coated Vicryl in renorrhaphy after LPN,and group B used unidirectional barbed suture.All patients underwent LPN preformed by single group of surgeons.Perioperative and postoperative indicators were compared in two groups.Result It is identical in age,BMI,tumor size,R.E.N.A.L nephrometry scoring system between 2 groups.However,using barbed suture has significantly shortened in warm ischemia time (WIT).The postoperative indicators,such as blood loss,hospital stay and surgery complication has no significantly difference in two groups.Conclusion The unidirectional barbed suture is safety and efficiency using in LPN which can shorten WIT significantly.
8.Five years experience of Fournier's gangrene emergency treatment of 12 cases in single centre

Objecitve To conclude the clinical manifestation of Fournier's gangrene (FG) and its emergency treatment.Methods From 2006 to 201 1,12 patient were involved in the study,all had received debridement,wide spectrum antibiotics and support therapy,the clinical manifestation,treatment choice and prognosis were analyzed retrospectively.Results All patients were male with 67.7 year-old mean age.Initial lesions were all in scrotal skin,an average procedures of each patient was 3.5,mean hospitalization days was 29.7 days.Surgical wound was healed by left scrotal skin in all cases,2 cases with huge urethra defect were received perineostomy.Ten cases were survived with 57-months follow-up and mortality was 16.7%.Conclusions Fournier is one of emergency disease of Urology.Its mortality is still high.Intensive care and appropriate anti-biotic therapy combined with surgical treatment is the key of FG's management.
9.The role of opioid receptors in the protection of remifentanil preconditioning against heart post-ischemic injury in rats

Aim To determine whether remifentanil preconditioning has cardioprotection against ischemia and reperfusion induced injury via opioid receptor(OR) in rats.Method Male Sprague-Dawley rats of 300~350 g were anaesthetized and the chests were opened and hearts exposed via a left thoracotomy.They were randomly assigned to 12 groups:Control(CON,saline vehicle),naltrindole(NTD,5 mg?kg~(-1) iv.10 min before 30 min ischemia);CTOP(CTOP,1 mg?kg~(-1) iv.10 min before ischemia),nor-binaltorphimine(nor-BNI,5 mg?kg~(-1),iv.15 min before 30 min ischemia);remifentanil preconditioning(RPC,6 ?g?kg~(-1)?min~(-1)),ischemic preconditioning(IPC),NTD+RPC or NTD+IPC(naltrindole 5 mg?kg~(-1) iv.10 min before RPC or IPC),nor-BNI+RPC or nor-BNI+IPC(nor-binaltorphimine 5 mg?kg~(-1),iv.15 min before RPC or IPC),CTOP+RPC or CTOP+IPC(CTOP 1 mg?kg~(-1) iv.before RPC or IPC).Infarct size(IS),a percentage of the area at risk(AAR),was determined by triphenyltetrazolium(TTC) staining.Results IPC and RPC markedly reduced IS/AAR from(52.7?1.8)% to(12.9?2.7)% and(16.2?2.4)%,respectively. CTOP,a selective ?-OR antagonist,or NTD,a selective ?-OR antagonist,administered 10 min before remifentanil PC completely abolished,while nor-BNI,a selective ?-OR antagonist,administered 15 min before RPC attenuated the cardioprotective effect of RPC.In the group preconditioned with ischemia,blockade of ?-OR with NTD abolished,while blockade of ?-OR with nor-BNI attenuated the protection.Blockade of ?-OR with CTOP did not alter the cardioprotective effect of IPC.Conclusion The cardioprotective effect of RPC was abolished by all the three OR antagonist,CTOP,naltrindole and nor-binaltorphimine,suggesting that this effect is mediated via ?-,?-and ?-ORs.Part of the effect of RPC may be produced by ?-OR agonist activity outside the heart.
10.Elevated serum interleukin-17 level but not Th17 cell percentage reduced in patients with rheumatoid arthritis and ankylosing spondylitis after 40 weeks tumor necrosis factor-α blockade therapy

Objective To explore the effect of tumor necrosis factor-alpha(TNF-α) blockade therapy on circulating Th17 cell percentage and serum interleukin (IL)-17 level in patients with rheumatoid arthritis (RA) and ankylosing spondylitis (AS). Methods Twenty-seven RA and 22 AS patients were recruited, of which 14 cases from both diseases received 40 weeks TNF blockade therapy. Twenty-four healthy blood donors were used as controls. The frequencies of circulating Th17 cells were determined by flowcytometry, and serum IL-17 level were measured by enzyme linked immunosorbent assay(ELISA). Results Significantly higher baseline circulating Th17 cells were observed in active RA and AS patients compared with the healthy controls[RA 1.03%(0.66%,1.78%) vs controls 0.50%(0.43%,0.67%), Z=-3.236, P<0.01; AS(1.16±0.09)%vs controls (0.59 ±0.061)% , t =5.226, P <0.01]. Similarly, serum IL-17 level were significantly elevated in patients with both diseases compared with controls[RA(32.3±2.5) pg/ml vs controls(14.3±2.5) pg/ml, t=5.070, P<0.01; AS 28.98(23.84,36.14) pg/ml vs controls 11.84(5.33,22.12) pg/ml, Z=-4.103, P<0.01]. After TNF-α blockade therapy, serum IL-17 was significantly decreased in both diseases groups[RA △(-13.5± 5.0) pg/ml and AS △(-16.0±1.9) pg/ml]. In contrast, no significant differences were found in the frequencies of circulating Th17 cells[RA △(0.104 5±0.212 6)% and AS △(0.002 5±0.183 8)%]. Conclusion Th17 cells and IL-17 have been implicated in the pathogenesis of RA and AS. TNF-α blockade can partially inhibit the function of Th17 cells. However, it is unable to reduce the frequencies of these cells in the circulation after 40 weeks therapy, which may explain the reasons for the relapse.
