Objective To explore the change of the serum leptin and tumor necrosis factor-α levels in children with obstructive sleep apnea-hypopnea syndrome and their relationship.Methods The level of serum leptin and TNF-α were determined in 68 children with obstructive sleep apnea-hypopnea syndrome and 30 healthy children by radio-immunity assay,and the results from patients treated with continuous positive airway pressure(CPAP)for six months were compared.The correlation between leptin and the levels of tumor necrosis factor-α,and apnea hypopnea index(AHI)or the minimum saturation of bloed oxygen were analyzed.Results The level of leptin and tumor necrosis factor-α in OSAHS patients were hisher than those in control(P＜0.05).The levels of leptin and tumor necrosis factor-αwere positive correlated with the AHI(r=0.451,r=0.460,P＜0.05)and negative correlated with the minimum saturation of blood oxygen(r=-0.489.r=-0.478,P＜0.05).The levels of leptin and tumor necrosis factor-α were decreased after treatment.Conclusion The levels of leptin and tumor necrosis factor-α are increased in OSAHS children.The increase of leptin and tumor necrosis factor-α is correlated with hypoxemia,and the levels of leptin and tumor necrosis factor-α can be improved by CPAP in OSAHS children.

Objective To investigate the imaging features in neonates with hypoglycemic brain injury by early and serial MRI.Methods Neonates who were admitted to neonatal department from May 2005 to Jul 2013 received MRI and diffusion-weighted imaging(DWI) scan within 7 days after hypoglycemia.Fortynine neonates were diagnosed with hypoglycemic brain injury.Thirty-four neonates received second MRI scan between 2 ～3 weeks after hypoglycemia.Seven neonates received third MRI scan.Results All the 49 neonates showed hyperintensity on DWI in the regions of occipital and parietal lobe for the first MRI scan(mainly involved 33 cases).Six cases combined frontal lobe and temporal lobe.Ten cases presented widespread cortex and white matter involvement.All the cases presented hypointensity on DWI for the affected area,T1 and T2 weighted image signal changes were not obvious.Some cases presented deep white matter and gray matter injury.Thirty-four cases received second MRI scan between 2 ～3 weeks after hypoglycemia,20 presented hypointensity on DWI,hypointensity on T1 weighted image and hyperintensity on T2 weighted image.Eleven cases with normal signals,and the other 3 were absorbing.Seven cases received third MRI scan,appeared encephalomalacia,myelin retardation,white matter volume decreased,hypoplasia of corpus callosum.The more severe the clinical symptoms was,the more severe the degree of brain injury showed.Conclusion Cerebral occipital and parietal regions are the most vulnerable in neonatal hypoglycemic brain injury.Early DWI for the imaging diagnosis of hypoglycemic brain injury should be taken within one week after hypoglycemia.Mild injury is recoverable,but severe would chang into necrosis and encephalomalacia.Some combined deep white and gray matter injury might related to hypoxia and ischemia.

The study is to explore the effect of paeoniflorin on the level of glucocorticoid receptor, including glucocorticoid receptor-alpha (GCRalpha) and glucocorticoid receptor-beta (GCRbeta), of peripheral blood mononuclear cells (PBMCs) in rats of collagen-induced arthritis (CIA). CIA is induced in Wistar rats by an intradermal injection of bovine type II collagen emulsified with complete adjuvant. From the 14th day after primary immunization, the CIA rats were intragastrically administered paeoniflorin 25, 50 and 100 mg x kg(-1) or triptolde 20 microg x kg(-1) or paeoniflorin 50 mg x kg(-1) + RU486 15 mg x kg(-1), once a day, for 28 consecutive days. After administration, apart from PF + RU486 group all experimental rats were took blood by removalling eyeball, then separated PBMCs. The level of GCRalpha, GCRbeta in PBMCs were examined by ELISA, and the mRNA expression of GCRalpha, GCRbeta was detected by RT-PCR. All rats were sacrificed and took the joint with no immunization. The expression of IL-1beta, NF-kappaB p65, TNF-alpha, PGE2 of synovial tissue was detected by immunohistochemistry. Paeoniflorin was able to inhibit the expression of IL-1beta, NF-kappaB p65, TNF-alpha, PGE2 of synovial tissue in CIA rats. While RU486, glucocorticoid receptor's blocker, could weaken the fuction of paeoniflorin. Meanwhile, paeoniflorin obviously induced the expression of GCRalpha and GCRalpha mRNA, while obviously inhibited the expression of GCRbeta and GCRbeta mRNA. These results indicat paeoniflorine suppresses inflammatory mediator production may be relating with it regulating GCR in PBMCs of CIA rats.

Objective To explore the effect of health education path on care and household rehabilitation in children with cerebral palsy (CP). Methods 100 CP children from June to December, 2012 were divided into experimental group (n=50) and control group (n=50). The control group was given conventional health education, while the experimental group along with the health education path. Mastery rates of disease knowledge and family rehabilitation methods, cooperation rate, and care satisfaction rate of parents was assessed before discharge.Results Mastery rates of disease knowledge, family rehabilitation methods, cooperation rate, and care satisfaction rate of parents were higher in the experimental group than in the control group (P<0.05). Conclusion Application of health education path can rule the health education of nurses, and improve the CP patient's satisfaction and compliance.

Objective To evaluate the safety and efficacy of transcatheter closure of perimembranous ventricular septal defects(VSD) using domestic symmetric double-disk occluders.Methods From March 2003 to July 2007,689 VSD patients were treated with transcatheter intervention using domestic symmetric double-disk occluders and received clinical follow up at 1 month,3 months,6months after the procedure and annually thereafter.Complications including residual shunt,heart rhythm changing,shape of the occluder and the presence of tricuspid/aortic regurgitation were observed thoroughly.Results The overall technical success rate was 98.8%.One patient died of cerebral bleeding 7 days after intervention.One patient suffered complete A-V block and required permanent pacemaker implantation and another patient conversed to open surgery because of moderate tricuspid regurgitation.Asymmetric Amplatzer occluders were used in 6 patients but the intervention failed in 2 patients whom required an open conversion.Comorbidities such as atrial septum defect,patent foramen ovale were treated concomitantly.No other complications were observed during the follow-up period.Conclusion Complication associated with transcatheter closure of perimembranous VSD using domestic symmetric double-disk occluders was relatively low.The procedure was safe and applicable.Meanwhile,it is necessary to stick to the procedural indications and avoid complications such as tricuspid regurgitation etc.

Objective To compare the effect of tension-free vaginal tape (TVT) and vaginal reconstructive surgery (VR) on women with severe genital prolapse and stress urinary incontinence.Methods 43 patients with stage I or II vaginal anterior defect and stress urinary incontinence were assigned to TVT group(21 patients) and VR group (22 patients) . Results Maximum quantity of urine, pressure of detrusor at maximum quantity of urine, and abdominal leakage point pressure occured significantly in TVT group after operation(P<0.05). Postoperative satisfaction rate were higher in TVT group(P<0.05). Conclusion TVT is more effective to improve the quality of life in women with stress incontinence.

Objective By analyzing the clinical and pathological characteristics of small intestinal neoplasms of patients presenting at our hospital,this study was to improve our cognition of this disease and the prognosis.Methods We collected and reviewed the medical records of 121 patients suffering from small intestinal neoplasms,who underwent surgery at Ruijin hospital from January 2003 to June 2009.Diagnosis was confirmed by pathological examination,and patients were followed-up.Results Intestinal hemorrhage,anemia and abdominal pain were the three main symptoms for all patients.CT,and gastrointestinal endoscopy were valuable for the diagnosis of small intestine neoplasms.Compared with open surgery,laparoscopic procedures can shorten the operation time and the postoperative length of hospital stay.Conclusions Surgical procedure is the key treatment for patients with small intestinal neoplasms.Long term follow-up plays important role in the detection of other synchronous or metachronous gastrointestinal tumors and improves the prognosis.

A simple and effective method for the duplex PCR detection was developed by using sequences of exogenous gene(RDV MP-)and endogenous gene(Zein)as templates for PCR amplification.The results of routine PCR amplification for RDV MP-gene in transgenic maize suggested that RDV MP-gene can stably inheritate in transgenic plants and their progenies;The duplex PCR detection of all negative and part positive samples that obtained by routine PCR amplification confirmed that above negative results were exact,also showed that the quality of extracted DNA can meet the need of PCR amplification.The error ratio of negative samples was 1.4%.The method used in this study was simple and credible and can be used to detect transgenic plants and their products.
