1.Biomechanical analysis of mandibular bilateral sagittal split ramus osteotomy with bicortical screws internal fixation

Objective: To study the mandibular bilateral sagittal split ramus osteotomy (BSSRO) with bicortical screws rigid internal fixation(RIF) with the three-dimensional finite element method, and supply directions for clinic practice.Methods:CT scanned technology and the finite element software (Ansys) were used to establish the three-dimensional finite element model of BSSRO with bicortical screws RIF. The stress distribution of the mandible and the RIF and the displacement of split mandible were calculated under three kind of occlusion situation.Results:Under the same kind of occlusion situation, the stress and displacement of the split mandible with single upper screws fixation was higher than that with the reverse "L" screws fixation, the stress and displacement of the split mandible with 2.0 mm diameter screws fixation was higher than that with 2.7 mm diameter screws fixation. With the same kind of fixation method, the stress and the displacemen of the mandible under the incisor occlusion was the highest.Conclusion:The fixation distance, position, distribution angle of the bicortical screws all have effects on the fixation stability. The patient should avoid the incisor occlusion after the surgery.
2.Localization of sentinel lymph node with indirect lymphography and the application in head and neck tumor.

Whether the sentinel lymph node (SLN) metastasis has important clinical significance for the therapy and prognosis of tumor. Sentinel lymph node biopsy (SLNB) has became the most accurate clinical method to confirm the status of sentinel lymph node. And the key of the success of SLNB is the localization of SLN. The methods used to locate SLN mainly are blue dye tracer method, radionuclide tracer technique, blue dye tracer method combined with radionuclide tracer technique, indirect lymphography, NIR imaging, and photoacoustic imaging. This article demonstrates the principle, application method and contrast agents of the indirect lymphography and the application in head and neck tumor.

4.Infection Rate of HBV among High School Graduates in Jingjiang City of Jiangsu Province

Objective To investigate the positive rates of HBsAg and abnormal rates of Alanine aminotransferase among high school graduates in Jingjiang city.Methods HBsAg were detected by ELISA and ALT was detected by rate assay among 20 988 high school graduates during 2004—2007 in Jingjiang city.Results The HBsAg positive students were 1754.The total positive rate of HBsAg was 8.36%.The positive rate of HBsAg of male(9.46%)was higher than that of female(6.95%).The HBsAg positive rate of rural students(9.44%)was higher than that of urban students(7.28%).HBsAg positive rates of urban students showed a decreasing trend during 2004—2007,the rates were 9.31%,8.08%,6.74% and 5.24%,respectively(P0.05).There were 155 cases(8.4%)with increased ALT among 1754 HBsAg positive students.Conclusion The total positive rate of HBsAg among high school graduates in Jingjiang city from 2004—2007 was 8.36%.The HBsAg positive rate of male students was higher than that of female.The HBsAg positive rate of rural students was higher than that of urban students.8.4% students were found ALT abnormal.We should intensify Hepatitis B vaccine in time to prevent the spreading of HBV.

6.Discussion on Cell Apoptosis and Its Related Molecular Mechanisms of Human Gastric Carcinoma Cell Line BGC-823 Induced by Cinnamaldehyde

This article was aimed to investigate the cell proliferation , cell apoptosis and its related molecular mechanisms of the human gastric carcinoma cell line BGC-823 in v itro after treatment with cinnamaldehyde . The MTT Assay demonstrated the inhibitory effect of cinnamaldehyde . And the Flow Cytometry was used to determine its induction of cell apoptosis. The Hoechst 33342 was used to observe morphological changes during apoptosis . Moreover , quantitative real time PCR and western blot analysis were used to detect the effect of cinnamaldehyde on human gastric carcinoma cell line BGC-823 . The results showed that compared with the control group , cinnamaldehyde had inhibitory effect on human gastric carcinoma cell line BGC-823 ( P <0 . 01 ) . It showed that cinnamaldehyde induced apoptosis through the downregulation of Bcl-2 , Bcl-xL and Survivin expression , upregulation of Bax and Bak expression , downregulation of Bcl-2 and Procaspase-3 , and upregulation of BAX . It was concluded that cinnamaldehyde had inhibitory effect on the proliferation of human gastric carcinoma cell line BGC-823 and induced apoptosis . It may be related to the activation of the endogenous apoptosis pathway .
7.Application of continuous cardiac index monitoring in acute heart failure model

Objective To establish the swine acute heart failure model for surgical experiment, and evaluate the heart function by continuous cardiac index (CCI). Methods Swine heart failure model was attempted to establish by coronary ligation in six swines. CCI was obtained by Swan-Canz catheters and Vigilance monitor, and hemodynamic, biochemical and ultrasonocardiographic results were utilized to evaluate the changes of heart function. Results Five swines accomplished the experiment. Compared with basic status, there were significant differences in mean arterial pressure (MAP) , pulmonary artery systolic pressure (PASP), mixed venous oxygen saturation ( SVO_2) and CCI for swines with heart failure ( P < 0.05) , there was no significant change in biochemical parameters, while left ventricle ejection fraction ( LVEF) significantly decreased (P<0.05). Conclusion CCI is feasible in monitoring and evaluating heart function of animal model. The swine acute heart failure model established by coronary ligations can meet the needs of surgical experiment in principle.

9.Biomechanical study of rigid internal fixation for maxillary LeFort Ⅰ fracture

Objective To biomechanically study the fixation stability of different numbers and shapes of the titanium miniplates (L-shaped and straight four-hole miniplates) in the treatment of maxillary LeFortⅠ fracture by using three-dimensional finite element method so as to provide reference for clinical treatment of the fractures. Methods Three-dimensional finite element model of maxillary LeFortⅠ fracture was established with four kinds of rigid internal fixation (RIF) methods to calculate the stress of the maxilla and the RIF as well as the displacement of the fracture segment under three kinds of occlusion.Then, the fixation stability of different methods was compared. Results Under the same occlusion condition, the decreasing order of the displacement of the fracture segment was the L-shaped plate fixation at both buttress of the maxillary and nasal maxillary zygomatic, the straight four-hole miniplates fixation at both buttress of the maxillary and nasal maxillary zygomatic, the L-shaped plate fixation at the zygomatic maxillary buttress and the L-shaped plate fixation at naso-maxillary buttress. Under the same fixation method, the decreasing order of the displacement of the fracture segment was molar occlusion, premolar oeclusion and incisor occlusion. Conclusions The fixation stability of the L-shaped plate fixation is better than the straight four-hole miniplate fixation for the treatment of LeFortⅠ fracture. Fixation at the zygomaticmaxillary buttress is better than at the naso-maxillary buttress. Use of only two miniplates to fix the LeFort Ⅰ fracture may not be stable. Molar occlusion is not good for fracture healing.
10.Expression and outcome of BRCA1 protein in patients with triple negative breast cancer

Purpose To investigate the protein expression of breast cancer susceptibility gene (BRCA1) in triple negative breast canc-er ( TNBC) and to analyze the prognostic impact on outcome of TNBC by BRCA1 protein. Methods The expression of BRCA1 and p53 in 95 cases TNBCs was detected by immunohistochemistry. The correlation analysis and evaluation of prognosis were conducted in consideration of patients clinical and pathological characteristics. Results The positive rate of BRCA1 protein expression in TNBCs was 31. 6%. Compared with BRCA1 negative expression, those patients with BRCA1 positive expression were associated with younger age (P=0. 047) and higher expression of p53 (P=0. 001). There was no significant difference in outcome between BRCA1 positive and negative expression (HR=1. 10,95%CI=0. 552~2. 235, P=0. 769). Conclusion The expression of BRCA1 protein may have no impact on the outcome of TNBC. There is possible correlationship between p53 pathway and BRCA1 in inhibiting tumor growth.
