1.Application value of dynamic enhanced CT technology in diagnosis of hepatic space occupying lesions

Objective To explore the application value of dynamic enhanced CT technology in diagnosis of hepatic space occupying lesions.Methods A total of 54 cases of patients with hepatic space occupying lesions from January 2015 to October who were treated in our hospital were selected as the research objects,who were carried out CT plain scan and dynamic contrast-enhanced scan.The lesion display rates,and the lesions,blood vessels and tumor characters in the arterial phase,portal phase and delayed phase were recorded and analyzed,respectively.Results The display rate of plain scan was 64.16%,significantly lower than that in the arterial phase of 98.33%,in the portal phase of 81.67%,and in the delayed phase of 78.33% of the portal phase of dynamic contrast-enhanced scan,and the differences were significant (all P <0.01).In the dynamic contrast-enhanced scan,the display rate of the arterial lesions in the arterial phase was higher than those in the portal phase and delayed phase,while that in the portal phase was higher than that in the delayed phase (all P < 0.05).The total detection rate of the lesions of the plain scan was significantly lower than those of the pathological examination and the dynamic enhanced the third phase scan (both P <0.01),and the difference of detection rates between the dynamic enhanced the third phase scan and the pathological examination was not significant (P > 0.05).The differences of the definition of edge sharpness,vascular clarity,and density of the lesions between the second phase scan with the third phase scan were significant (all P < 0.05).The diagnosis rates of malignant and benign liver tumors in the third phase scan were significantly higher than those in the second phase scan,and the tumor characters were significantly higher than those in the second phase scan (all P < 0.05).Conclusions Dynamic enhanced CT scan can effectively improve the detection rate of hepatic space occupying lesions and the effects of qualitative diagnosis and differential diagnosis,and reduce the occurrence of missed diagnosis and misdiagnosis.
3.The clinic effect and safety of inhaled nitric oxide for the neonates with hypoxic respiratory failure

Objective To study the clinic effect and safety of inhaled nitric oxide for the neonates with hypoxic respiratory failure. Methods A total of 21 neonates with hypoxic respiratory failure were trea-ted with nitric oxide inhalation after ineffective treatment of conventional mechanical ventilation. The values of mean airway pressure( MAP) ,oxygenation index( OI) ,mean blood pressure and saturation of blood oxy-gen percutem( TcSaO2 ) were detected before and after nitric oxide inhalation. Meanwhile,the values of coag-ulation function,nitrogen dioxide concentration and methaemoglobin were monitored. Results The lung oxy-genation of 21 cases were significantly improved in 48 hours after nitric oxide inhalation,OI declined from 26. 600 ±4. 169 to 7. 500 ±1. 716,MAP decreased from(15. 100 ±2. 132)cmH2O(1 cmH2O=0. 098 kPa) to (8. 000 ± 0. 816) cmH2O,which all had statistical differences(P <0. 01). The monitoring of coagulation function was in the normal range. The levels of methaemoglobin were less than 3% and nitrogen dioxide less than 1 × 10 -6 . In this study,18 neonates survived,2 neonates died and 1 neonate gave up treatment. Conclu-sion Inhaled nitric oxide early can improve oxygenation effectively for neonates with hypoxic respiratory failure without obvious side-effect. However,it is not yet clear for its long-term prognosis and whether there is influence development of the nervous system.
4.Inhibitory effects of cytotoxin IL-2-PE40 on corneal allograft rejection

Objective To examine the inhibitory effect of IL-2-PE40 on the mouse corneal allograft rejection. Methods A mouse corneal graft model was set up by using C57BL/6 mice as donors and Balb/c mice as recipients. In the treatment group, IL-2-PE40 (0.6 μg/g body weight) was intraperitoneally injected from the day of surgery every 12 h until rejection happened. In the control group, the equal volume of PS was injected intraperitoneally at the corresponding time points. The transplanted cornea was observed under slit-lamp twice a week and the transplanted corneal opacity and neovascularization were rated according to Horis grading standards. It led to the determination of rejection response. The survival of transplanted cornea was regarded to be stopped when the rejection occurred. The operated eyes were observed historically on the 10th, 15th, 25th and 35th day after the surgery, and the peripheral blood was collected for measurement of T cell subgroups and T lymphocyte colonies. Results The survival time of cornea in the treatment and control groups was (30.2±2.9) days and (15.1±2.1) days respectively. In the control group, rejection occurred on the 15th day after the surgery, CD4~+ cells started rise right after the surgery and increased most obviously on the 15th day [(63.9±4.0)%] and decreased afterwards. CD4~+ cells in the treatment group were increased slightly [(42.6±4.0)%] on the 15th day. The number of CD4~+ cells in the treatment group was obviously less than in the control group (P<0.01). No significant changes in CD8~+ cells were observed in both groups. The number of T lymphocyte colonies in the control group was increased at the beginning and dropped then. No obvious change was found in the treatment group. The number of T lymphocyte colonies in the treatment group was significantly less than in the control group (P<0.01). Conclusion IL-2-PE40 is a highly specific immunosuppressive agent. It can delay the development of corneal graft rejection and reduce the percentage of T-helper cells significantly. It also works to weaken the forming capacity of the peripheral T lymphocyte colonies.
5.Treatment strategies for relapsed/refractory multiple myeloma

Multiple myeloma is a malignancy of plasma cells,which is still incurable now.Despite dramatic improvement of the response rate and overall survival have been made due to the application of target therapy and stem cell transplantation,the treatment effect is difficult to maintain,nearly all patients will eventually relapse and become refractory.At relapse,the challenge is to select the appropriate treatment for each patient.The decision will be both disease-dependent and patient-dependent.
6.Observations on the Efficacy of Acupuncture plus Huayu Paishi Decoction in Treating Nephrolithiasis of Qi and Blood Stagnation Type

Objective To investigate the clinical efficacy of acupuncture plus Huayu Paishi Decoction (stagnation- eliminating and stone-removing decoction) in treating nephrolithiasis of qi and blood stagnation type.Methods Sixty patients with nephrolithiasis of qi and blood stagnation type were randomly allocated to treatment and control groups, 30 cases each. The treatment group received acupuncture plus oral administration of Huayu Paishi Decoction and the control group, oral administration of Huayu Paishi Decoction alone. The symptoms and signs were scored and renal hydrops was measured in the two groups before and after treatment. The clinical therapeutic effects were compared between the two groups. Results The total efficacy rate was 76.7% in the treatment group and 56.7% in the control group; there was a statistically significant difference between the two groups (P<0.05). There were statistically significant pre-/post-treatment differences in the symptom and sign scores in the two groups (P<0.05). There were statistically significant differences in pre-/post-treatment symptom and sign score difference values between the treatment and control groups (P<0.05). There was a statistically significant pre-/post-treatment difference in renal hydrops volume in the treatment group (P<0.05). There was a statistically significant difference in pre-/post-treatment renal hydrops volume difference value between the treatment and control groups (P<0.05).Conclusions Acupuncture plus Huayu Paishi Decoction is an effective way to treat nephrolithiasisi of qi and blood stagnation type. It can markedly improve the clinical symptoms and reduce renal hydrops in the patients.
8.Evaluation of tramadol for patient-controlled intravenious analgesia after abdominal total hysterectomy

Objective To compare the effects and side effects of tramadol and morphine used for patient-controlled analgesia (PCA) delivery system after abdominal total hysterectomy Methods A single centre, randomized double-blind study of ASAⅠ~Ⅱ 80 patients undergoing elective abdominal total hysterectomy with inhalational anesthesia was performed. The patients were divided into four groups: tramadol group (group T, n=20), morphine+droperidol group (group M+D, n=20), morphine group (group M, n=20), and tramadol +droperidol group (group T+D, n=20). The study lasted for 24 h and the following data were collected: heart rate, blood pressure, respiration, VAS, sedation score (SS), Prince Henry score (PHS) as well as side effects. The consumption of analgesics within 24 hours were also recorded. Results VAS and PHS showed no differences between the four groups.
