Objective To investigate the efficacy of emergency endoscopy for the treatment of patients with acute obstructive suppurative cholangitis(AOSC)and the difference compared with therapy of routine operation.Methods After ERCP were performed on 36 patients with AOSC,EST and/or ENBD were executed subsequently.To those asthenic patients complicated with diseases or tumors,ENBD was used to alleviate symptoms and then decided whether to take stones secondly or drain by ERBD.Biochemical indicators,physical sysmtoms and signs were monitored so as to compare with those of 32 patients with AOSC treated by routine operation.Results The clinical effect was satisfactory,and the relief of pain,incidence of complicated diseases,hospital stay,level of serum TBIL after treatment had significant difference between those in study group and in control group(all P＜0.05).Conclusion Endoscopic therapy is safe,easy and minimally invasive,which is suitable for the patients with AOSC.

Objective To explore the effect of fluticasone propionate nasal spray(Fu Shu-liang) in improving children's symptoms of allergic rhinitis and recurrent asthma.Methods 100 children with allergic rhinitis and asthma syndrome were randomly divided into control group and experimental group.The control group was treated with loratadine.The experimental group was treated with water-soluble auxiliary Fu Shu-liang nasal spay on the basis of the control group.The treatment effects were observed.Results The total effective rate of the observation group was 94％,which was significantly higher than 76％ of the control group (x2 =6.35,P ＜ 0.05).After 10-12 weeks,the rhinitis symptom score and asthma symptom score between the two groups had statistically significant differences (t =2.47,2.64,3.41 ; 2.30,3.17,2.47,all P ＜ 0.05).The incidence rate of adverse reactions such as nasal dryness,epistaxis between the two groups had statistically significant differences (x2 =7.11,7.53,all P ＜ 0.05).Conclusion Fu Shu-liang(Eph propionate and fluticasone propionate nasal spray) has good effect in improving symptoms of children with allergic rhinitis and asthma,it is worthy of clinical vigorously promoted.

The treatment of relapsed and refractory multiple myeloma has always been a problem among hematologic malignant tumor treatment.Compared with the initial therapy,there are worse therapeutic effects and more affecting factors of selecting treatment since the existence of drug resistance in relapsed myeloma.In this review,the therapeutic effects of single and combinational use of several novel agents such as thalidomide,lenalidomide and bortezomib were evaluated in treating relapsed and refractory multiple myeloma.The clinical results of some new anti-myeloma agents were also reported.

Objective To examine whether diabetes mellitus increases the risk of pneumonia mortality among seniors in the U.S. general popula-tion. Methods&Results The NHANES III follow-up study data were used. After excluding individuals from other minorities, being hos-pitalized with pneumonia in the previous year at baseline, or death of pneumonia during the first year of follow-up, a total of 3,707 subjects aged 65 years or older (1,794 men and 1,913 women) who had no missing information on variables for the analysis were included. Approxi-mately 16% of seniors at baseline were diabetics, which was defined as either having been diagnosed by a physician, currently taking pills/insulin lowering blood glucose, or HbA1c higher than 6.4%. During an average 11 years of follow-up, a total of 98 deaths due to pneu-monia were recorded (ICD-10:J12-J18). Cox-regression models were used to estimate the risk association between pneumonia mortality and diabetes mellitus. After adjustment for the covariates at baseline, the hazard ratios of pneumonia death were 1.30 (95%CI:0.64-2.70) for pre-diabetics and 2.28 (95%CI:1.18-4.39) for diabetics, respectively. Among those covariates, only age (HR (95%CI);1.16 (1.13-1.20)), gender as female (0.35 (0.22-0.61)) and physical fitness measured as having no problem walking 1+mile during the previous month (0.38 (0.20-0.67)) reached statistical significance. Conclusions The results suggest that diabetes mellitus is a strong risk predictor of pneumonia mortality and the evaluation of physical fitness may also be useful in the risk prediction of pneumonia mortality for seniors.

Objective To explore the relapse prevention effect of community independent skills in patients with schizophrenia.Methods 100 patients with schizophrenia from May 2014 to February 2015 were analyzed and divided into the observation group and the control group by using the computer random grouping.The standardization of community rehabilitation training was taken in the control group patients,whike independent skill training was taken in the observation group patients.The clinical curative effect of the study for a year were evaluated through SAPS, BPRS,SDSS,SANS and ITAQ for all the object.Results There was no statistical significance of ITAQ,SDSS,SAPS and SANS score difference in all the research objects before the rehabilitation treatment has (P >0.05 ).ITAQ, SDSS,SAPS and SANS score of the observation group were better than that of the control group after 8 weeks, 26 weeks and after 1 year,the difference was statistically significant(P <0.05).Recurrence rates of the observation group and control group were 16% and 40% respectively,and hospital admission rates were 10% and 32% respec-tively,the difference was statistically significant(P <0.05).The adherence were significant between the two groups (χ2 =6.92,P <0.05).Medication adherence to drug management ability were better in the observation group than the control group(χ2 =41.558,P <0.05).Conclusion The community independent skill training have an remarka-ble effect on the rehabilitation of patients with schizophrenia,and can improve the residual symptoms of schizophrenia after rehabilitation.It can help people with schizophrenia to maintain a good long-term positive effect,which is worth further clinical research and extension.

Although respiratory distress syndrome( RDS) ,one of the most common cause of respira-tory failure in the very or extremely preterm infant,has not been yet the major disorder of death with advances of respiratory support;In fact,RDS often complicated with BPD which developed with the maturation of de-pendence,the outcome of BPD and its development step into the focus of modern NICU. Powerful evidence-based consensus and suggestions have been stated in early respiratory support for very and extremely preterm infant:Early nasal CPAP( nCPAP) should be the best alternative to intubation for very or extremely preterm infant with high-risk of RDS and RDS; Early administration of Pulmonary surfactant ( PS ) with suitable methods( InSurE,Intubation Surfactant Extubation or LISA,Less invasive surfactant administration);Individ-ual threshould for nCPAP or other noninvasive ventilation failure should be established,but early PS adminis-tration and gentle ventilation will decrease the incidence of ventilator-induced injury on failuring noninvasive ventilation;Noninvasive ventilation with the use of caffeine will short the duration of ventilation and improve the success of weaning off. All the strategies in respiratory support to prevent BPD for very preterm infant still need more practice sustain improvement since multiple causes account for the development of BPD.

Objective To explore the prevalence and risk factors of rheumatoid arthritis in the elderly population in Luohe District of Henan Province.Methods A total of 8 610 residents were recruited for a two-stage cluster sampling survey through a questionnaire of RA according to 2010 ACR/EULAR classification criteria.Physical examinations and blood tests were performed.Results The crude prevalence rate of rheumatoid arthritis was 0.76％ and 0.71％ for standardized prevalence in the middle-aged and elderly population in Luohe city.In the age distribution,of the 65-69 years old and 60-64 years old prevalence were 1.13％ and 0.84 ％,and 1.10 ％ and 0.77 ％ for standardized prevalence respectively.Sexual comparisons showed a significantly higher prevalence in females than in males (1.02 ％ vs.0.39 ％,x2 =10.513,P =0.001).The risk factors of RA were:sex (female),living floor (first floor),smoking and genetic factors,risk were 2.595 (95％CI:1.407-4.784),2.967 (95％CI:1.749-6.307) 1.682 (95％ CI:1.206-4.039) and 1.934 (95 ％ CI:1.286-4.130).Conclusion The prevalence of rheumatoid arthritis was 0.71％ in the middle-aged and elderly population in Luohe city.The risk factors involve women,smoking and hereditary factors
